RESUMO

Cognitive decline in older adults is a major public health problem and can compromise independence and quality of life. Exercise and diet have been studied independently and have shown to be beneficial for cognitive function, however, a combined Tai Chi, resistance training, and diet intervention and its influence on cognitive function has not been undertaken. The current study used a 12-week non-randomized research design with experiment and control groups to examine the effect of a combined Tai Chi, resistance training, and diet intervention on cognitive function in 25 older obese women. Results revealed improvements in domain specific cognitive function in our sample. Baseline cognitive function was correlated with changes in dietary quality. These findings suggest that Tai Chi and resistance training combined with diet intervention might be beneficial for community-based programs aiming to improve cognitive function.

RESUMO

During the last 20 years, the technology advancement of small flexible ureterorenoscopes has dramatically changed the management of renal calculi. Retrograde intrarenal surgery (RIRS) has currently a high impact on active stone treatment, and it is increasingly used worldwide. Nevertheless, kidney stone fragmentation and direct removal of fragments require many passages of the ureteroscope, is often time-consuming, and may be very difficult through anatomical and technical factors. We describe a simple, feasible and efficient technique for small stone fragments retrieval, which are often difficult to remove during RIRS.

Obesity is a major health problem in the USA, especially in minority populations over the age of 60 years, and the aging process can cause adverse effects on physical function. Previous research has shown that Tai Chi, resistance training (RT), and diet result in overall health improvements. However, the combination of these specific interventions has yet to be translated to obese older women in an urban setting. The purpose of this study was to examine a combined intervention on the primary outcomes of physical function and body composition. Using a nonrandomized design, 26 obese women (65.2 ± 8.1 years) completed a 12-week intervention; participants were assigned to an intervention (EXD) group or a control (CON) group. The EXD group (n = 17) participated in Tai Chi, RT, and a dietary session. The CON group (n = 9) was asked to continue their normal lifestyle. Timed up and go (TUG) time was reduced by 0.64 ± 2.1 seconds (P = 0.04) in the EXD group while the CON group saw a borderline significant increase of 0.71 sec (P = 0.051). The combined intervention helped improve performance on TUG time, but there were no significant increases in other body composition or function measures.

RESUMO

AIM: To describe the pharmacotherapy of urinary lithiasis. METHOD: Results of a literature search (PubMed) was summarized by an experts board. RESULTS: The incidence of urolithiasis constantly increases in industrialized countries and its prevalence in France is estimated around 10% in the general population. Due to its high recurrence rate urolithiasis is a public health concern. Significant progresses in both surgery and pharmacotherapy during the last thirty years markedly improve prognostic of high recurrence risk patients. CONCLUSION: Better understanding of the physiologic disturbances involve in stone formation has allowed to identify at lest one factor in every subset of patient in which diet and pharmacotherapy will reduce the recurrence rate. In this review are discussed treatments of the crisis as well as the metabolic treatment.

Plasma lidocaine concentrations were intermittently measured in 8 upper gastrointestinal endoscopy and 12 bronchoscopy patients. The highest individual concentration was 0.98 microgram/ml in the upper gastrointestinal endoscopy patients and 3.79 microgram/ml in the bronchoscopy patients. Highest concentrations were reached at 15 minutes in the gastrointestinal endoscopy patients and at 30 or 60 minutes in the bronchoscopy patients. Thus, since lidocaine does not produce toxic effects at concentrations inferior to 6 microgram/ml, doses of this topical anaesthetic up to 16 mg/kg can be safely given during endoscopic procedures to patients with normal hepatic and cardiovascular functions. However, patients with liver metastases should be considered at high risk even if their liver function tests are normal. Patients at high risk of developing lidocaine toxicity should receive lower doses and be closely watched for at least 60 minutes after the end of the procedure.

Sixteen patients with an established diagnosis of oculopharyngeal muscular dystrophy underwent clinical, radiologic and manometric assessment. Secondary pharyngo-oral and pharyngonasal regurgitations are usually associated with this condition and chronic aspiration with consequent bronchorrhea is common. Such patients may present with marked oropharyngeal dysphagia. Cineradiologic findings correlated well with the mamometric results. The pharynx showed very weak contractions of longer duration than those observed in normal subjects. The proximal esophageal sphincter had a normal resting and closing pressure; however, relaxation and coordination of the sphincter were substantially different from those in a control group. Eleven patients underwent cricopharyngeal myotomy. All had notable improvement of their symptoms. Surgery on the sphincter results in a substantial decrease in its resting pressure; pharyngeal contraction remains unaltered.
