RESUMO

In order to understand the role of leptin in the formation of secondary amenorrhea (SA) during puberty, 78 girls aged from 13 to 17 years with SA and 74 girls of the same age with regular menstrual cycle have been examined with the estimation of body mass index (BMI) and hormonal/metabolic state. The obtained data show a strong connection between leptin level, BMI and parameters of energetic metabolic state (insulin; HOMA index); regression analysis results indicated the participation of leptin in steroidogenesis. Odds ratio (OR) values indicated an important role of leptin in the formation of SA during body weight deficit and normal BMI. It has been found that various clinical types of SA have different patterns of leptin influence.

Radioimmune assay of serum levels of luteinizing hormone (LH) and follicle-stimulating hormone (FSH) in 216 girls aged 13-18 years with pubertal delay (PD) unrelated to gonadal dysgenesis have shown that LH and FSH levels were normal in 48.1%; one or both gonadotropins were decreased in 17.1%, only LH level was increased in 13.9% and only FSH level in 12.6%. Hypergonadotropinemia was seen in 8.3% of the patients. Bioassay of total gonadotropin excretion was performed in 117 patients. Comparison of the radioimmune and biological assays suggested misproduction of the biologically active gonadotropins in a proportion of girls with PD. Hypoestrogenism in normogonadotropic PD appears to be related to regional blood and lymph flow disturbance.

Based on a study of an x-ray picture of the small pelvis, cranium and bone system in 400 girls aged 10-18, the results of the use of various roentgeno-radiological methods were analyzed, their diagnostic value was shown, and the data were compared with those of ultrasound investigation.

RESUMO

Diurnal excretion of 17-ketosteroids and their fractions was studied in 43 girl athletes with retarded sexual development (RSD) and 22 patients who did not go in for sports. Twenty-five healthy girls with a normal menstrual cycle constituted the control group. Athletes with RSD showed relative hyperandrogeny marked by a higher proportion of more active androgenic fractions (dehydroepiandrosterone and androsterone) in total 17-ketosteroids. Excretion of neutral 17-ketosteroids in athletes examined, as compared with non-athletes with RSD, was within age-associated norms or surpassed them. It is suggested that androgens of the adrenal cortex are involved into the pathogenesis of RSD in female athletes.
