RESUMO

Three cases of a recently established entity named "ossifying fibromyxoid tumor of soft parts" (OFMT) were studied histologically, ultrastructurally and by immunohistochemistry. All three tumors were stated in women from 48 to 54 years of age. One tumor was situated superficially in the skin and subcutaneous tissue. Other two tumors were deep-seated, with more diffuse and abundant ossification also in tumor free parts. In one of cases radiological and histological signs of chondroosteosarcomatous features in the ossifying parts of the tumor were stated. In two cases histologically and ultrastructurally multinucleated, vimentin and S-100 protein positive "fibroblastoid" cells were found. The histological and electron microscopic investigations did not rule out the possibility that the tumor cells have also potential properties of osteoblasts, but on the basis of histological, ultrastructural and immunohistochemical data it seems that these tumors are of mesenchymal multipotential derivatives of fibroblastoid origins.

RESUMO

Familial occurrence of dilated cardiomyopathy is estimated by 2-20%. We present a family with dilated cardiomyopathy inherited in an autosomal dominant way. We examined 9 members of the family, most of them are asymptomatic.

The aim of the study was to investigate the mechanisms of myocardial lesions induced by stress in conscious, intact pigs. The animals were subjected to 24 h immobilization, controls were kept in normal conditions. The pigs were killed by electric shock and exsanguination. Lipid peroxidation products: malondialdehyde (MDA), conjugated dienes (CDB), fluorescent end products (RF) and adenine nucleotides (ATP, ADP) were measured in the left ventricular myocardium which was also subjected to histoenzymatic and electron microscopic examination. In stressed animals as opposed to the control group, alterations in ultrastructure and diminution of mitochondrial ATP have been found, together with augmented formation of MDA and CDB reflecting increased free radical generation. These changes may be the component of stress-induced myocardial injury.

The prognostic significance of certain factors in the outcome of the haemolytic-uraemic syndrome (HUS) was evaluated. Seventy-three children with HUS were observed; 35 recovered without residual abnormalities, 6 had normal renal function with hypertension (HT) and/or urinary abnormalities, 5 had chronic renal failure, 1 child required chronic dialysis treatment, 4 children died of end-stage renal failure, and 22 died of acute renal failure (ARF). To identify the risk factors which influence prognosis, a logistic discrimination method was used to analyse the clinical and laboratory data. The logistic discrimination method established the role of independent factors which were linked together. The grouping of the patients in three different categories of favourable and unfavourable prognosis yielded conclusions concerning: (1) outcome of ARF survivors (2) recovery without sequelae or all other outcomes including death; (3) survival or death. The most important unfavourable factors for recovery of ARF survivors were HT and to a lesser degree cardiovascular (CV) disturbances; for recovery of the entire group, and for survival, the presence of CV disturbances was the worst prognostic factor. Among other factors were seasonal incidence, central nervous system disturbances, prodromal symptoms and age, which varied in their prognostic significance in the various groups.

Cyclophosphamide caused changes in Paneth cells. The relationship of these changes to the proliferative reaction of the intestinal epithelium requires a more complete knowledge of various physiological functions of Paneth cells.

From among 1464 children with ALL 167 (11.4%) at diagnosis had two or less than two years, 53 of them were infants. Most of them had a great tumor bulk, 15 had initial CNS infiltration and 31 WBCc greater than or equal to 100000/ML. 66 were treated according to St. Jude or LSA2L2 programs (the I group), 101 according to BFM programs (the II group). Complete remission was obtained in 85% of patients. In 63 children relapses occurred in the course of treatment whereas in 5 after the therapy cessation. In majority of cases, there were isolated relapses, mostly, they involved bone marrow. CNS involvement was found, in both mixed and isolated relapses, in 28 children (20%). 64 children are alive, in 43 of them the therapy was stopped. Kapla-Meier estimates for event free survival (EFS) after 8 years were 19 +/- 4.7% in the first group vs 37.38 +/- 6.3% in the second group. Due to therapy intensification the treatment results have been improved (p = 0.05) but still they are not satisfactory, particularly in case of infants and children with WBC greater than or equal to 100000/ML. Analysis of the lot of children showed that apart from age at ALL diagnosis and the initial WBC the methods of treatment are also the prognostic factors.

1. The effect of the calcium channel blocker nisoldipine on the myocardial content of lipid peroxidation products (malondialdehyde (MDA), conjugated double bonds (CDB), fluorescent end-products (RF) and mitochondrial adenine nucleotides) was investigated in conscious pigs (n = 14) subjected to 24 h of immobilization stress. Histoenzymatic and electron microscopic studies of the myocardium were also performed. Nisoldipine was given orally in a twice daily dose of 20 mg for 2 days before and on the day of the experiment. Results were compared with those obtained in immobilized untreated pigs (n = 10) and in non-stressed treated controls (n = 8). 2. Pretreatment with nisoldipine significantly attenuated stress-induced increase in myocardial contents of CDB and RF and prevented decline of mitochondrial adenine nucleotides. Stress-induced myocardial histoenzymatic changes (decrease of succinic dehydrogenase, ATPase, acid phosphatase activity) and ultrastructural alterations (mitochondrial damage, lysis of myofibrils, dilatation of sarcoplasmic reticulum and endothelial swelling) were also diminished. 3. It is concluded that treatment with a Ca2(+)-antagonist is beneficial to the heart exposed to environmental stress.

The study was carried out in three groups of rats: group I of 36 rats received methotrexate, group II of 36 rats were given methotrexate, pentagastrin and glucagon, group III of 12 rats served as control. For histological examination the ileum was taken from the animals killed on days 1, 2, 3, 4, 7 and 9 after the last injection of methotrexate. Besides histological examinations the mitotic index and the number of cells in crypt longitudinal-section were determined. Relatively low doses of methotrexate caused characteristic changes in the intestinal mucosa: disappearance of mitotic figures, shortening of crypts particularly on the first day after the last dose of methotrexate. On the following days regeneration processes were noted causing an excessive epithelial proliferation observed up to the 9th day after the last dose of methotrexate. This indicates that the mobilized mechanisms of epithelial proliferation stimulation exert their effect fairly long. In the group receiving methotrexate administration of pentagastrin and glucagon contributed to the increased number of mitoses in crypt epithelium and the number of cells on crypt longitudinal-section. These observations are an evidence of a beneficial effect of the hormones administered to the rats in the regeneration of the ileal epithelium damaged by the cytostatic agent.

Dynamics of changes in the myocardium of rabbits subjected to food hypercholesterolemia lasting from 6 to 16 weeks was investigated. An intensification of coronary atheromatosis was proportional to the duration of the high-cholesterol diet. There were observed focal and growing in time damages of cardiomyocytes. They were sharply outlined in atherosclerotically changed coronary arteries. The following morphological and histochemical changes have been observed: increased acidophilia and fuchsinophilia in the single and grouped muscle fibres, foci of infiltrations by mononuclear cells, contraction bands, and outlines of myocardial fibres, separation of myofibrils, granular disintegration of cardiomyocytes, healed infarcts, depletion and excessive accumulation of glycogen in muscle fibres, presence of neutral fats, cholesterol and its esters in atheromatous plaques of coronary arteries, presence of neutral fats in some cardiomyocytes: focal acid phosphates, loss of activity of Mg- and Ca-dependent ATPases and SDH: oedema of mitochondria with disorganization of cristae, disorganisation of fibres and lysis of myofilaments, margination of chromatin in cardiomyocyte nuclei, increased number of lysosomes, intensified symptoms of egzocytosis, widening of channels of sarcoplasmatic reticulum, oedema of endothelium of capillary vessels and increased number of the collagenous fibres in the interstitial space. The results of histological, histochemical and microscopic-electron investigations correlated with each other. It may be considered that the observed damages of cardiomyocytes precede myocardial infarction and result from progressive and increasing ischemia, hypoxia and accumulation of fat substances and cholesterol and its esters in intima of coronary arterioles as well as in cardiomyocytes.
