RESUMO

OBJECTIVE: To observe the changes of serum γ-glutamyl transpeptidase (GGT) levels before and after the pathogen treatment in patients with subclinical schistosomiasis, and explore its clinical value in the diagnosis and treatment of subclinical schistosomiasis. METHODS: Totally 109 patients with subclinical schistosomiasis, who were found in the endemic investigation of schistosomiasis in Ezhou City, were selected as the investigation subjects, and then they were treated with praziquantel. The serum GGT levels of the subjects before and after the treatment were detected and compared. RESULTS: Before the treatment, the average value of the GGT levels of the 109 patients was (48.1 ± 45.9) IU/L, among which, the GGT levels of 69 cases (63.3%) were normal, and the levels of 40 cases (36.7%) were increased. After the treatment, the average GGT level of the patients was (32.1 ± 23.4) IU/L, which decreased by 33.3% comparing with that before the treatment, and the difference had a statistical significance (U=2.17, P = 0.01). The GGT levels of 65 patients decreased in different degrees. Among the 40 patients whose GGT levels had increased before the treatment, the GGT levels of 31 ones returned to the normal. CONCLUSIONS: The GGT level detection can accurately reflect the liver function in the patients with subclinical schistosomiasis, and also it has certain clinical application value to judge the liver function damage and recovery of the patients before and after the pathogen treatment.

Inpatients in the intensive care unit (ICU) are at high risk for healthcare-associated infections (HAIs). In the current study, a bundle of interventions and measures for preventing and controlling HAIs were developed and implemented in the ICU by trained personnel, and the impact of the bundle was evaluated. The incidence of HAIs, the adjusted daily incidence of HAIs and the incidence of three types of catheter-related infections before and after the bundle implementation were compared. The execution rate of the bundle for preventing and controlling ventilator-associated pneumonia (VAP) was increased from 82.06% in 2012 to 96.88% in 2013. The execution rate was increased from 83.03% in 2012 to 91.33% in 2013 for central line-associated bloodstream infection (CLABSI), from 87.00% to 94.40% for catheter-associated urinary tract infection (CAUTI), and from 82.05% to 98.55% for multidrug-resistant organisms (MDROs), respectively. In total, 136 cases (10.37%) in 2012 and 113 cases (7.72%) in 2013 involved HAIs, respectively. Patients suffered from infection of the lower respiratory tract, the most common site of HAIs, in 134 cases (79.29%) in 2012 and 107 cases (74.30%) in 2013 respectively. The incidence of VAP was 32.72‰ and 24.60‰, the number of strains of pathogens isolated was 198 and 173, and the number of MDROs detected in the ICU was 91 and 74 in 2012 and 2013, respectively. The percentage of MDROs among the pathogens causing HAIs was decreased in each quarter of 2013 as compared with the corresponding percentage in 2012. In 2013, the execution rate of the bundle for preventing and controlling HAIs was increased, whereas the incidence of HAIs and VAP decreased as compared with that in 2012.

Summarize the value of the change of different parts in treadmill exercise test (TET) ECG to coronary heart disease (CHD) diagnosis. Four hundred and forty-five cases have been included in this investigation, which stayed in our hospital from January of 2006 to March of 2011 and underwent TET and coronary arteriography (CAG). The change of different parts in TET ECG in these patients had been retrospective summarized to determine its diagnosis value to CHD. (1) In the 445 cases of TET testers, 200 cases showed positive in TET with 150 cases of positive CAG, and 50 cases of negative CAG; 245 cases showed negative in TET with 206 cases of negative CAG, and 39 cases of positive CAG. The diagnosis sensitivity of CHD was 79.36% (150/189), the specificity was 80.47% (206/256), positive prediction value was 75.00% (150/200), negative prediction value was 84.08% (206/245), and false-positive rate was 25.00% (50/200) with prediction accuracy of 80.00% (356/445). (2) In the 200 cases with positive TET: 51 cases were in the limb lead group; 73 cases were in the chest lead group; and 76 cases were in the limb lead + chest lead group. There were 150 cases showing positive in CAG: 22 cases were in the limb lead group; 58 cases were in the chest lead group; and 70 cases were in the limb lead + chest lead group. The positive diagnosis rate of ST change in the chest lead was obviously higher than that of simple limb lead group (P < 0.05). (3) People with healthy coronary artery will have decreased amplitude of R wave while patients with coronary stenosis have elevated amplitude of R wave. (4) As for the T wave, the positive CAG had no statistical significance between normal T wave group and TET positive group (P > 0.05); CAG results had statistical significance between normal T wave group and TET negative group (P < 0.05). (5) Positive CAG results had no statistical significance between U-wave inversion group and TET positive group (P > 0.05); positive CAG results has statistical significance when TET negative group compared with U-wave inversion group or TET positive group (P < 0.05). TET is a relatively idea invasive diagnosis method for coronary disease, which can be utilized to evaluate the stage of CHD when integrating with the change of TET ECG.
