Contextualização: O selênio é um componente fundamental em uma série de reações orgânicas que protegem contra o estresse oxidativo e com potencial para tratar e prevenir doenças, segundo a literatura. Objetivos: Sumarizar as evidências de revisões sistemáticas da Cochrane, referentes à efetividade da suplementação de selênio para tratamento e prevenção de doenças. Métodos: Trata-se de overview de revisões sistemáticas Cochrane. Procedeu-se à busca na Cochrane Library (2024), sendo utilizado o descritor "SELENIUM". Todas as revisões sistemáticas de ensaios clínicos foram incluídas. O desfecho primário de análise foi a melhora clínica, a redução dos sintomas ou a prevenção da doença. Resultados: Sete estudos foram incluídos, totalizando 122 ensaios clínicos e 50.267 participantes. Discussão: Há evidência com alto grau de certeza, segundo as revisões sistemáticas realizadas pela Cochrane, de que a suplementação de selênio não reduz o risco de câncer. Pode haver algum benefício para redução de episódios de sepse em crianças com prematuridade extrema, mas o nível de evidência é bastante limitado nesse caso, devido a reduzida amostragem e alta heterogeneidade nos estudos.

Contexto e objetivo: A transmissão de doenças por mosquitos afeta a população e a economia de todo o mundo. Há um número considerável de doenças que podem ser transmitidas por mosquitos, com destaque para a malária e a dengue, endêmica em regiões tropicais. Evidentemente, medidas preventivas são imprescindíveis para a redução da transmissão. Avaliar as evidências de efetividade das telas de proteção com e sem inseticida para prevenção de doenças transmitidas por mosquitos. Métodos: Trata-se de sinopse baseada em evidências. Procedeu-se à busca por estudos que associavam o uso de telas de proteção contra mosquitos à redução do contágio de doenças transmitidas por mosquitos em três bases de dados: PubMed (1966-2024), Portal BVS (1982-2024) e Epistemonikos (2024) e também no metabuscador de evidências TRIP DATABASE (2024). O desfecho de análise envolveu a efetividade das telas de proteção na redução de doenças transmitidas por mosquitos. Resultados: Foram encontradas 307 citações. Seis estudos (1 revisão sistemática e 5 ensaios clínicos) foram incluídos. Discussão: A maioria dos estudos envolveu a colocação de telas de proteção com inseticida, havendo evidência de alta certeza para redução de mortalidade por malária e redução na entrada de mosquitos nas habitações, mesmo com redes sem inseticida. Conclusões: Embora não haja robustez na evidência da efetividade das telas de proteção sem inseticidas contra mosquitos transmissores de doenças, o que demanda a necessidade de realização de novos estudos prospectivos, parece lícita e benéfica a utilização de telas de proteção em regiões endêmicas para doenças transmitidas por esses vetores.

BACKGROUND: This study investigates myocardial structural changes in stable coronary artery disease (CAD) patients with type 2 diabetes (T2D) using cardiac magnetic resonance (CMR) strain and T1 mapping. METHODS: A total of 155 stable CAD patients underwent CMR examination, including left ventricular (LV) morphology and function assessment, late gadolinium enhancement (LGE), and feature tracking (CMR-FT) for LV global longitudinal, circumferential, and radial strain. T1 mapping with extracellular volume (ECV) evaluation was also performed. RESULTS: Among the enrolled patients, 67 had T2D. Diabetic patients exhibited impaired LV strain and higher ECV compared to non-diabetics. Multivariate analysis identified T2D as an independent predictor of increased ECV and decreased strain. CONCLUSIONS: CMR-based strain and T1 mapping highlighted impaired myocardial contractility, elevated ECV, and potential interstitial fibrosis in diabetic patients with stable CAD. This suggests a significant impact of diabetes on myocardial health beyond CAD, emphasizing the importance of a comprehensive assessment in these individuals. Oceanic oil spills present significant ecological risks that have the potential to contaminate extensive areas, including coastal regions. The occurrence of the 2019 oil spill event in Brazil resulted in over 3000 km of contaminated beaches and shorelines. While assessing the impact on benthic and beach ecosystems is relatively straightforward due to direct accessibility, evaluating the ecotoxicological effects of open ocean oil spills on the pelagic community is a complex task. Difficulties are associated with the logistical challenges of responding promptly and, in case of the Brazilian mysterious oil spill, to the subsurface propagation of the oil that impeded remote visual detection. An oceanographic expedition was conducted in order to detect and evaluate the impact of this oil spill event along the north-eastern Brazilian continental shelf. The pursuit of dissolved and dispersed oil compounds was accomplished by standard oceanographic methods including seawater polycyclic aromatic hydrocarbons (PAHs) analysis, biomass stable carbon isotope (δ13C), particulate organic carbon to particulate organic nitrogen (POC:PON) ratios, nutrient analysis and ecotoxicological bioassays using the naupliar phase of the copepod Tisbe biminiensis. Significant ecotoxicological effects, reducing naupliar development by 20-40 %, were indicated to be caused by the presence of dispersed oil in the open ocean. The heterogeneous distribution of oil droplets aggravated the direct detection and biochemical indicators for oil are presented and discussed. Our findings serve as a case study for identifying and tracing subsurface propagation of oil, demonstrating the feasibility of utilizing standard oceanographic and ecotoxicological methods to assess the impacts of oil spill events in the open ocean. Ultimately, it encourages the establishment of appropriate measures and responses regarding the liability and regulation of entities to be held accountable for oil spills in the marine environment.

Cellulose modified hydrogels can be produced directly from raw biopolymers in novel cellulose solvents such as NaOH/urea aqueous solution. The effect of cellulose characteristics on the synthesis of a cellulose-graft-(net-poly(acrylamide-co-acrylic acid)) and its performance as water absorbent/methylene blue dye removal material is analyzed. Three cellulose samples, one analytical grade and two obtained from teak wood sawdust with different pretreatments (one alkaline and the other, a novel one known as (gas phase) acid pretreatment) were compared. The starting raw celluloses were characterized by scanning electron microscopy (SEM), Fourier transform infrared spectroscopy (FTIR), thermogravimetric analysis (TGA), X-ray diffraction (XRD) and viscosity in cupri ethylenediamine hydroxide (CED) solution, whereas the chemically modified materials were characterized by SEM, FTIR, and TGA. The pretreatment used influences composition, crystallinity index and degree of polymerization (DP) of the cellulose obtained. The modified material produced with cellulose from alkaline pretreatment showed the highest swelling ratio in water absorption tests at room temperature (12,714 %); in contrast, the one with cellulose from acid pretreatment showed the lowest swelling ratio (7,470 %). However, this difference is not so significative in dye removal tests, where absorption capacity is 139 and 140 mg/g, respectively. The results indicate that cellulose composition, particularly structures with significant hemicellulose and lignin remaining content, has a major effect on the performance of modified materials for water absorption, and degree of polymerization has a major effect on adsorption capacity of methylene blue.

This study evaluates the performance of large intelligent surface (LIS) technology in the context of a multi-user MIMO mobile communication system (Mu-MIMO) proposed for the sixth generation (6G). LIS employs digitally controlled reflectors to enhance Signal-to-Interference plus Noise Ratio (SINR) and establish line of sight (LoS) connectivity in non-LoS environments, improving transmission security. Analytical expressions are derived to assess LIS performance metrics, including distribution parameters, bit error probability, and secrecy outage probability, considering the presence of eavesdroppers and environmental fading. The study highlights the potential of LIS technology to enhance the confidentiality and reliability of digital communication systems in next-generation networks.

PURPOSE: To present the content and response process validity evidence of the Speaking in Public Coping of Scale (ECOFAP). METHODS: A methodological study to develop and validate the instrument. It followed the instrument development method with theoretical, empirical, and analytical procedures, based on the validity criteria of the Standards for Educational and Psychological Testing (SEPT). The process of obtaining content validity evidence had two stages: 1) conceptual definition of the construct, based on theoretical precepts of speaking in public and the Motivational Theory of Coping (MTC); 2) developing items and response keys, structuring the instrument, assessment by a committee with 10 specialists, restructuring scale items, and developing the ECOFAP pilot version. Item representativity was analyzed through the item content validity index. The response process was conducted in a single stage with a convenience sample of 30 people with and without difficulties speaking in public, from the campus of a Brazilian university, belonging to various social and professional strata. In this process, the respondents' verbal and nonverbal reactions were qualitatively analyzed. RESULTS: The initial version of ECOFAP, consisting of 46 items, was evaluated by judges and later reformulated, resulting in a second version with 60 items. This second version was again submitted for expert analysis, and the content validity index per item was calculated. 18 items were excluded, resulting in a third version of 42 items. The validity evidence based on the response processes of the 42-item version was applied to a sample of 30 individuals, resulting in the rewriting of one item and the inclusion of six more items, resulting in the pilot version of ECOFAP with 48 items. CONCLUSION: ECOFAP pilot version has items with well-structured semantics and syntactic, representing strategies to cope with speaking in public.


OBJETIVO: Apresentar as evidências de validade baseadas no conteúdo e nos processos de resposta da Escala de Coping para a Fala em Público (ECOFAP). MÉTODO: Estudo metodológico de elaboração e validação de instrumento. Seguiu-se o modelo de elaboração de instrumentos com procedimentos teóricos, empíricos e analíticos, baseados nos critérios de validade do Standards for Educational and Psychological Testing (SEPT). O processo de obtenção das evidências de validade baseadas no conteúdo foi realizado em duas etapas: 1) definição conceitual do construto, elaborado com base nos preceitos teóricos da fala em público e da Teoria Motivacional do Coping (TMC); 2) elaboração dos itens e chave de respostas, estruturação do instrumento, avaliação por comitê de dez especialistas, reestruturação dos itens da escala, realizada em três momentos, até a elaboração da versão piloto da ECOFAP. O processo de resposta foi realizado com amostra de conveniência de 30 indivíduos, com e sem dificuldades de fala em público, no campus de uma universidade brasileira, pertencentes a diferentes extratos sociais e profissões. Nesse processo, foram analisadas qualitativamente as reações verbais e não verbais dos respondentes. RESULTADOS: A primeira versão da ECOFAP, composta por 46 itens, foi avaliada pelos juízes e posteriormente reformulada, resultando em uma segunda versão com 60 itens. Essa segunda versão foi novamente submetida à análise de especialistas e calculado o índice de validade de conteúdo por item. Foram excluídos 18 itens, originando uma terceira versão de 42 itens. As evidências de validade com base nos processos de resposta da versão de 42 itens foram aplicadas em uma amostra de 30 indivíduos, resultando na reescrita de um item e inclusão de mais seis itens, originando a versão piloto da ECOFAP de 48 itens. CONCLUSÃO: A versão piloto da ECOFAP apresenta itens bem estruturados semântica e sintaticamente que representam estratégias de enfrentamento para a fala em público.

Rattlesnakes belonging to the genus Crotalus are widely distributed throughout the Americas. In Brazil, symptoms commonly associated with envenomation by Crotalus durissus collilineatus include myalgia, rhabdomyolysis, renal failure, neurotoxicity, and progressive paralysis, which are related to the protein composition of this venom. Snake venom composition exhibits compositional variability that may reflect geographic distribution, age, captivity, diet, sex, and even individual genetics. Although seasonality is also considered a possible source of variation, there are few reports of such variability in snake venom. In this work, venoms of the same eight C. durissus collilineatus were extracted every three months for two years, to analyze seasonal changes in composition and activities. To this end, venom composition was analyzed by protein quantification, SDS-PAGE, and HPLC, and the LAAO, PLA2 and coagulant activities were measured. Venoms of these C. d. collilineatus showed minor seasonal differences in venom activities and no composition differences were found. LAAO and coagulant activities displayed a pattern of seasonal change, while PLA2 activity seemed to have no seasonality tendency. Also, there are sexual differences, in which males seem to be more stable than females in regard to some activities. Individual variability occurs even in seasonal variation of activities, highlighting the importance of controlling circumstances of venom extraction before comparing results between groups of snakes.

This study aimed to investigate seven outbreaks of A. marginale infection in two regions of Brazil, affecting taurine, zebu, and crossbred cattle. We assessed the possible causes, treatment measures, and genetic diversity of A. marginale. These outbreaks occurred in two states (Goiás: outbreaks 1-7; Mato Grosso do Sul: outbreak 3), breeds (Holstein, Nellore, and crossbreed), age groups (beef cattle: 18-25 days old and 7-8 months; dairy cattle: 18-25 days old, 13-14 months, and cow after the first birth) and rearing systems (feedlot, pasture, pen in a wood shaving bedding system and compost bedded-pack barns). Metaphylactic or prophylactic treatments varied according to outbreak (imidocarb dipropionate: outbreaks 1-4 and 6; enrofloxacin: outbreaks 5 and 7; diminazene diaceturate: outbreak 5). In outbreaks 6 and 7, the packed cell volume was monitored. In all outbreaks, the practice of needle/syringe sharing was discontinued. For outbreaks 1-3, clinical signs and mortality (range, 4.8-13.3%) occurred 36-45 days after entry into the feedlot. In outbreak 4, A. marginale was diagnosed in 66.2% of the calves (bacteremia, 0-4.5%), with a mortality of 8.6%. Among nursing calves aged 60 days during outbreak 5, 53.8% were infected with A. marginale, with average bacteremia of 2.7% (range, 0-21.3%), and a mortality of 13.8%. In dairy heifers aged 14 months, raised in paddocks lacking vegetation cover and infested with R. microplus, then transitioned to a rotational grazing system also infested with R. microplus, the A. marginale bacteremia ranged from 3.2 to 6.7%, with a mortality of 20%. Before monitoring during outbreak 7, the mortality was 17.9%, but no further deaths were observed after monitoring initiation. In conclusion, possible causes triggering the outbreaks included primary tick infestation, needle/syringe sharing, and stress factors which may have affected the immunological statues of animals in the feedlots. Control measures performed in all outbreaks were effective. The partial msp4 gene sequences of A. marginale generated herein belonged to two haplotypes, but further research would be needed to investigate if this finding has any clinical significance.

Aiming to study the performance, carcass characteristics, nutrient digestibility, blood parameters, salivary cortisol levels, and economic viability of pigs administered aspartic protease, a total of 135 pigs were housed in pens in a randomized block design, divided into five treatments with nine replications. The experimental diets were positive control (PC), basic diet with a 5.0% reduction in protein and amino acid requirements; negative control (NC) with a 7.5% reduction in protein and amino acid requirements; NC + 100 g/mT of aspartic protease (NC100); NC + 150 g/mT of aspartic protease (NC150); and NC + 200 g/mT of neutral serine protease (NC200). The inclusion of protease, independently of the source and amount, increased the average daily weight gain (P < 0.05) of animals compared with the control treatments (PC and NC), improved feed conversion (P < 0.05) in early stages, and improved diet digestibility (P < 0.05) compared with the PC. Treatment with NC150 and NC200 resulted in greater carcass weights (P < 0.05) than treatment with the PC. NC100 led to a greater carcass yield than PC (P < 0.05), and NC150 resulted in a greater loin eye area than PC (P < 0.05). No differences (P > 0.05) in the blood parameters or salivary cortisol levels were found. Regarding economic viability, proteases increased the profitability, with NC150 leading to the best results. Thus, the use of aspartic proteases is recommended to improve performance and further facilitate pork production.

BACKGROUND AND AIMS: A healthy diet is one of the pillars of familial hypercholesterolemia (FH) treatment. However, the best dietary pattern and indication for specific supplementation have not been established. Our aim is to conduct a pilot study to assess the effect of an adapted cardioprotective diet with or without phytosterol and/or krill oil supplement in participants with a probable or definitive diagnosis of FH, treated with moderate/high potency statins. METHODS: A national, multicenter, factorial, and parallel placebocontrolled randomized clinical trial with a superiority design and 1:1:1:1 allocation rate will be conducted. The participants will undergo whole exome sequencing and be allocated into four treatment groups: 1) a cardioprotective diet adapted for FH (DICAFH) þ phytosterol placebo þ krill oil placebo; 2) DICA-FH þ phytosterol 2 g/day þ krill oil placebo; 3) DICA-FH þ phytosterol placebo þ krill oil 2 g/day; or 4) DICA-FH þ phytosterol 2 g/day þ krill oil 2 g/day. The primary outcomes will be low-density lipoprotein (LDL)-cholesterol and lipoprotein (a) levels and adherence to treatment after a 120-day follow-up. LDL- and high-density lipoprotein (HDL)-cholesterol subclasses, untargeted lipidomics analysis, adverse events, and protocol implementation components will also be assessed. RESULTS: A total of 58 participants were enrolled between May e August 2023. After the end of the follow-up period, the efficacy and feasibility results of this pilot study will form the basis of the design of a large-scale randomized clinical trial. CONCLUSIONS: This study's overall goal is to recommend dietary treatment strategies in the context of FH.

Efficient energy management in residential environments is a constant challenge, in which Home Energy Management Systems (HEMS) play an essential role in optimizing consumption. Load recognition allows the identification of active appliances, providing robustness to the HEMS. The precise identification of household appliances is an area not completely explored. Gaps like improving classification performance through techniques dedicated to separability between classes and models that achieve enhanced reliability remain open. This work improves several aspects of load recognition in HEMS applications. In this research, we adopt Neighborhood Component Analysis (NCA) to extract relevant characteristics from the data, seeking the separability between classes. We also employ the Regularized Extreme Learning Machine (RELM) to identify household appliances. This pioneering approach achieves performance improvements, presenting higher accuracy and weighted F1-Score values-97.24% and 97.14%, respectively-surpassing state-of-the-art methods and enhanced reliability according to the Kappa index, i.e., 0.9388, outperforming competing classifiers. Such evidence highlights the promising potential of Machine Learning (ML) techniques, specifically NCA and RELM, to contribute to load recognition and energy management in residential environments.

AIMS: To investigate the association between diabetic retinopathy (DR) and coronary artery disease (CAD) using coronary angiotomography (CCTA) and multimodal retinal imaging (MMRI) with ultra-widefield retinography and optical coherence tomography angiography and structural domain. METHODS: Single-center, cross-sectional, single-blind. Patients with diabetes who had undergone CCTA underwent MMRI. Uni and multivariate analysis were used to assess the association between CAD and DR and to identify variables independently associated with DR. RESULTS: We included 171 patients, 87 CAD and 84 non-CAD. Most CAD patients were males (74 % vs 38 %, P < 0.01), insulin users (52 % vs 38 %, p < 0.01) and revascularized (64 %). They had a higher prevalence of DR (48 % vs 22 %, p = 0.01), microaneurysms (25 % vs 13 %, p = 0.04), intraretinal cysts (22 % vs 8 %, p = 0.01) and areas of reduced capillary density (46 % vs 20 %, p < 0.01). CAD patients also had lower mean vascular density (MVD) (15.7 % vs 16.5,%, p = 0.049) and foveal avascular zone (FAZ) circularity (0.64 ± 0.1 vs 0.69 ± 0.1, p = 0.04). There were significant and negative correlations between Duke coronary score and MVD (r = -0.189; p = 0.03) and FAZ circularity (r = -0,206; p = 0.02). CAD, DM duration and insulin use independently associated with DR. CONCLUSIONS: CAD patients had higher prevalence of DR and lower MVD. CAD, DM duration and insulin use were independently associated with DR.

Introduction: Community-acquired pneumonia (CAP) is one of the most common causes of childhood morbidity and mortality, causing about two million deaths per year worldwide. The complicated CAP (CCAP) results from the worsening of CAP. Their incidence has reduced in the last 30 years due to vaccination. However, the coronavirus disease (COVID-19) pandemic reduced vaccination coverage, resulting in increased incidence of CCAP in 2021 and 2022. Objective: To analyze the clinical and epidemiological profile of CAP in children under five years of age in two periods: pre- (2018 to 2019) and during the COVID-19 pandemic (2020 to 2022). Methods: This cross-sectional retrospective study was conducted at the Professor Fernando Figueira Institute of Integral Medicine (IMIP). We analyzed the sociodemographic and clinical variables of children with CAP aged below five years who were admitted to IMIP from 2018 to 2022. Analysis encompassed the Pearson's Chi-square test, Fischer's exact test, and Student's T tests. Results: A total of 468 children were analyzed: 382 in the pre-pandemic period and 86 during the COVID-19 pandemic. Concerning the antibiotic therapy, the most prescribed was Ampicillin (45.00%) in both periods. The combination of Oxacillin and Ceftriaxone was prescribed in 6.86% of cases in the pre-pandemic period; this value increased to 20.90% during the COVID-19 pandemic. Pleural effusion represented 12.10% of cases in the pre-pandemic period and 24.40% during the COVID-19 pandemic. The presence of pleural empyema went from 1.60% to 8.20%, and necrotizing pneumonia from 1.30% to 5.90% in the respective periods. Regarding ICU admission, 5.30% were admitted during the pre-pandemic period and 34.10% during the COVID-19 pandemic. Clinical suspicion of influenza presented a positivity rate of 17.60%. Conclusion: Children with CAP presented a higher frequency of complications during the COVID-19 pandemic. Further research is needed to find the cause of increased CAP complications in this period.

There is limited contemporary prospective real-world evidence of patients with chronic arterial disease in Latin America. The Network to control atherothrombosis (NEAT) registry is a national prospective observational study of patients with known coronary (CAD) and/or peripheral arterial disease (PAD) in Brazil. A total of 2,005 patients were enrolled among 25 sites from September 2020 to March 2022. Patient characteristics, medications and laboratorial data were collected. Primary objective was to assess the proportion of patients who, at the initial visit, were in accordance with good medical practices (domains) for reducing cardiovascular risk in atherothrombotic disease. From the total of patients enrolled, 2 were excluded since they did not meet eligibility criteria. Among the 2,003 subjects included in the analysis, 55.6% had isolated CAD, 28.7% exclusive PAD and 15.7% had both diagnoses. Overall mean age was 66.3 (± 10.5) years and 65.7% were male patients. Regarding evidence-based therapies (EBTs), 4% were not using any antithrombotic drug and only 1.5% were using vascular dose of rivaroxaban (2.5 mg bid). Only 0.3% of the patients satisfied all the domains of secondary prevention, including prescription of EBTs and targets of body-mass index, blood pressure, LDL-cholesterol, and adherence of lifestyle recommendations. The main barrier for prescription of EBTs was medical judgement. Our findings highlight that the contemporary practice does not reflect a comprehensive approach for secondary prevention and had very low incorporation of new therapies in Brazil. Large-scale populational interventions addressing these gaps are warranted to improve the use of evidence-based therapies and reduce the burden of atherothrombotic disease.ClinicalTrials.gov NCT04677725.

We hypothesized that cottonseed cake in confined Nellore young bulls' diet as fiber source, could maintain or improve the nutrient intake, and productive and metabolic parameters. It was evaluated the total replacement of whole plant corn silage (WPCS) by cottonseed cake (CSC) as a source of fiber in the diet. A completely randomized design with two treatments and 12 replications was used. The treatments were two experimental diets containing 300 g/kg of WPCS or CSC as roughage. All animals at 0, 15, 30, 60 and 112 days were weighed to monitor productive performance and nutrient intake. Dry matter intakes, organic matter, neutral detergent fiber, physically effective neutral detergent fiber, crude protein and non-fiber carbohydrates of the WPCS treatment cattle were higher (p < 0.05), and the ethereal extract intake of cattle fed CSC diet was higher. The BW of cattle fed WPCS was higher in the two initial periods of confinement (p < 0.05). Animals from both treatments showed linear growth during the confinement period. There was a fiber source × period interaction for total weight gain (TWG), average daily gain (ADG) and feed efficiency (FE) (p < 0.05), with the lowest in the first 15 days and the highest at 30 days, reflecting a compensatory gain in animals fed with CSC. Bulls fed with CSC showed negative TWG and ADG in the first period (p < 0.05), but recovered in the following periods, surpassing that of the SC treatment in the last two periods. There was negative EF of bulls fed with CSC in the first period (P < 0.05), but in the following periods the CSC diet did not differ from the WPCS diet. The animals fed CSC diet showed a higher concentration of cholesterol and glucose in the blood plasma (p < 0.05). The forage-free diet containing cottonseed cake as a source of fiber replacing corn silage promoted better animal performance.

Introduction: Acute liver failure (ALF) is a rapidly progressing, life-threatening syndrome characterized by liver-related coagulopathy and hepatic encephalopathy (HE). Given that higher HE grades correlate with poorer outcomes, clinical management of ALF necessitates close neurological monitoring. The primary objective of this case report is to highlight the diagnostic value of utilizing multimodal neuromonitoring (MNM) in a patient suffering from ALF. Case report: A 56-year-old male patient with a history of chronic alcoholism, without prior chronic liver disease, and recent acetaminophen use was admitted to the hospital due to fatigue and presenting with a mild flapping tremor. The primary hypothesis was an acute hepatic injury caused by acetaminophen intoxication. In the following hours, the patient's condition deteriorated, accompanied by neurological decline and rising ammonia levels. The patient's neurological status was closely monitored using MNM. Bilaterally altered pupillary light reflex assessed by decreasing in the Neurological Pupil Index values, using automated pupillometry, initially suggested severe brain oedema. However, ultrasound measurements of the optic nerve sheath diameter showed normal values in both eyes, P2/P1 noninvasive intracranial pressure waveform assessment was within normal ranges and the cerebral computed tomography-scan revealed no signs of cerebral swelling. Increased middle cerebral artery velocities measured by Transcranial Doppler and the initiation of electroencephalography monitoring yielded the presence of status epilepticus. Discussion: The utilization of MNM facilitated a more comprehensive understanding of the mechanisms underlying the patient's clinical deterioration in the setting of HE. Nonetheless, future studies are needed to show feasibility and to yield valuable insights that can enhance the outcomes for patients with HE using such an approach. Given the absence of specific guidelines in this particular context, it is advisable for physicians to give further consideration to the incorporation of MNM in the management of unconscious patients with ALF.

Zebrafish (Danio rerio) are widely employed as an experimental model in various scientific fields. The investigation of glucose metabolism dysfunctions has gained recent significant prominence. Considering that certain anesthetics may impact glycemic levels, it is imperative to carefully select an anesthetic that does not induce such side effects, thereby mitigating potential adverse influences on research outcomes. In this sense, this study aimed to evaluate potential glucose alterations and induction and recovery times resulting from the use of eugenol, menthol and lidocaine as anesthetics in zebrafish. A total of 150 adult male and female zebrafish were divided into ten groups, comprising a control group euthanized by rapid chilling, and three groups anesthetized with low (40 mg/L eugenol, 60 mg/L menthol, 100 mg/L lidocaine), intermediate (60 mg/L eugenol, 90 mg/L menthol, 225 mg/L lidocaine), and high (80 mg/L eugenol, 120 mg/L menthol, 350 mg/L lidocaine) anesthetic concentrations. Glucose levels and induction and recovery times were assessed. The findings reveal that eugenol and menthol did not cause glucose level alterations at any of the investigated concentrations, while lidocaine caused a non-concentration-dependent hyperglycemia. Eugenol and menthol also exhibited similar recovery times at different concentrations, while lidocaine recovery times were concentration-dependent. This study, therefore, concludes that eugenol and menthol are safe and satisfactory anesthetics for use in zebrafish research involving glucose analyses, while lidocaine use can cause biases due to altered glucose levels and safety concerns. Researchers should, therefore, carefully consider anesthetic selection to ensure reliable results in zebrafish assessments.

ABSTRACT BACKGROUND AND OBJECTIVES: Low back pain (LBP) is one of the leading causes of disability worldwide. Its etiology is multifactorial and results in losses for individuals and society. The biopsychosocial model of LBP offers a comprehensive approach to assessment and interventions. Researchers investigate the relationship between LBP and Common Mental Disorders (CMD) within this concept. This systematic review aimed to identify and analyze recent studies on the topic. CONTENTS: The study was submitted to the International Prospective Register of Systematic Reviews (registration number CRD 42020210375). The 27 articles in the corpus were selected from the Medlin/PubMed and BVS (Biblioteca Virtual em Saúde) databases from 2015 to 2022. Only those that investigated a possible relationship between CMD (anxiety or depression) and LBP in adult populations were included. Most articles in the corpus confirmed the relationship between LBP and CMD, however, gaps were identified regarding the direction of causality between the two outcomes and why this relationship was established. Methodological limitations in relation to sample selection and LBP assessment instruments used were also identified. CONCLUSION: This review highlights the need for studies on the topic with greater methodological criteria and with a design that allows causality to be assessed. Thus, it will be possible to guide clinical practice based on scientific evidence. Longitudinal studies with an emphasis on the biopsychosocial model can contribute to support specific therapeutic approaches.


RESUMO JUSTIFICATIVA E OBJETIVOS: A dor lombar (DL) é uma das principais causas mundiais de incapacidade. Trata-se de um desfecho cuja etiologia é reconhecidamente multifatorial, com prejuízos para os indivíduos e a sociedade. O modelo biopsicos-social da DL apresenta uma abordagem ampla, tanto na avaliação quanto nas intervenções. A relação entre DL e Transtorno Mental Comum (TMC) é objeto de estudo dentro da concepção desse modelo. O presente estudo teve como objetivo identificar e analisar os estudos recentes sobre esse tema. CONTEÚDO: O estudo foi submetido ao International Prospective Register of Systematic Reviews (número de registro CRD 42020210375). Os 27 artigos do corpus foram selecionados das bases de dados Medline/Pubmed e BVS (Biblioteca Virtual em Saúde) no período de 2015 a 2022. Foram incluídos apenas aqueles que investigaram possíveis relações entre TMC (ansiedade ou depressão) e DL em populações adultas. A maioria dos artigos do corpus confirmou a relação entre DL e TMC, no entanto foram identificadas lacunas sobre a direção de causalidade entre os dois desfechos e sobre os motivos pelos quais essa relação se estabeleceu. Também foram identificadas limitações metodológicas em relação à seleção da amostra e aos instrumentos de avaliação da dor lombar utilizados. CONCLUSÃO: Esta revisão destacou a necessidade de estudos sobre o tópico com maiores critérios metodológicos e com um desenho que permita avaliar a causalidade. Assim, será possível orientar a prática clínica baseando-se em evidências científicas. Estudos longitudinais com ênfase no modelo biopsicossocial podem contribuir para embasar abordagens terapêuticas específicas.

Resumo Objetivo Apresentar as evidências de validade baseadas no conteúdo e nos processos de resposta da Escala de Coping para a Fala em Público (ECOFAP). Método Estudo metodológico de elaboração e validação de instrumento. Seguiu-se o modelo de elaboração de instrumentos com procedimentos teóricos, empíricos e analíticos, baseados nos critérios de validade do Standards for Educational and Psychological Testing (SEPT). O processo de obtenção das evidências de validade baseadas no conteúdo foi realizado em duas etapas: 1) definição conceitual do construto, elaborado com base nos preceitos teóricos da fala em público e da Teoria Motivacional do Coping (TMC); 2) elaboração dos itens e chave de respostas, estruturação do instrumento, avaliação por comitê de dez especialistas, reestruturação dos itens da escala, realizada em três momentos, até a elaboração da versão piloto da ECOFAP. O processo de resposta foi realizado com amostra de conveniência de 30 indivíduos, com e sem dificuldades de fala em público, no campus de uma universidade brasileira, pertencentes a diferentes extratos sociais e profissões. Nesse processo, foram analisadas qualitativamente as reações verbais e não verbais dos respondentes. Resultados A primeira versão da ECOFAP, composta por 46 itens, foi avaliada pelos juízes e posteriormente reformulada, resultando em uma segunda versão com 60 itens. Essa segunda versão foi novamente submetida à análise de especialistas e calculado o índice de validade de conteúdo por item. Foram excluídos 18 itens, originando uma terceira versão de 42 itens. As evidências de validade com base nos processos de resposta da versão de 42 itens foram aplicadas em uma amostra de 30 indivíduos, resultando na reescrita de um item e inclusão de mais seis itens, originando a versão piloto da ECOFAP de 48 itens. Conclusão A versão piloto da ECOFAP apresenta itens bem estruturados semântica e sintaticamente que representam estratégias de enfrentamento para a fala em público.


Abstract Purpose To present the content and response process validity evidence of the Speaking in Public Coping of Scale (ECOFAP). Methods A methodological study to develop and validate the instrument. It followed the instrument development method with theoretical, empirical, and analytical procedures, based on the validity criteria of the Standards for Educational and Psychological Testing (SEPT). The process of obtaining content validity evidence had two stages: 1) conceptual definition of the construct, based on theoretical precepts of speaking in public and the Motivational Theory of Coping (MTC); 2) developing items and response keys, structuring the instrument, assessment by a committee with 10 specialists, restructuring scale items, and developing the ECOFAP pilot version. Item representativity was analyzed through the item content validity index. The response process was conducted in a single stage with a convenience sample of 30 people with and without difficulties speaking in public, from the campus of a Brazilian university, belonging to various social and professional strata. In this process, the respondents' verbal and nonverbal reactions were qualitatively analyzed. Results The initial version of ECOFAP, consisting of 46 items, was evaluated by judges and later reformulated, resulting in a second version with 60 items. This second version was again submitted for expert analysis, and the content validity index per item was calculated. 18 items were excluded, resulting in a third version of 42 items. The validity evidence based on the response processes of the 42-item version was applied to a sample of 30 individuals, resulting in the rewriting of one item and the inclusion of six more items, resulting in the pilot version of ECOFAP with 48 items. Conclusion ECOFAP pilot version has items with well-structured semantics and syntactic, representing strategies to cope with speaking in public.
