The multisystem impairment promoted by COVID-19 may be associated with a reduction in exercise capacity. Cardiopulmonary abnormalities can change across the acute disease severity spectrum. We aimed to verify exercise physiology differences between COVID-19 survivors and SARS-CoV-2-naïve controls and how illness severity influences exercise limitation. A single-centre cross-sectional analysis of prospectively collected data from COVID-19 survivors who underwent cardiopulmonary exercise testing (CPET) in their recovery phase (x = 50[36;72] days). Patients with COVID-19 were stratified according to severity as mild [M-Cov (outpatient)] vs severe/critical [SC-Cov(inpatients)] and were compared with SARS-CoV-2-naïve controls (N-Cov). Collected information included demographics, anthropometrics, previous physical exercise, comorbidities, lung function test and CPET parameters. A multivariate logistic regression analysis was performed to identify low aerobic capacity (LAC) predictors post COVID-19. Of the 702 included patients, 310 (44.2%), 305 (43.4%) and 87 (12.4%) were N-Cov, M-Cov and SC-Cov, respectively. LAC was identified in 115 (37.1%), 102 (33.4%), and 66 (75.9%) of N-CoV, M-CoV and SC-CoV, respectively (p < 0.001). SC-Cov were older, heavier with higher body fat, more sedentary lifestyle, more hypertension and diabetes, lower forced vital capacity, higher prevalence of early anaerobiosis, ventilatory inefficiency and exercise-induced hypoxia than N-Cov. M-Cov had lower weight, fat mass, and coronary disease prevalence and did not demonstrate more CEPT abnormalities than N-Cov. After adjustment for covariates, SC-Cov was an independent predictor of LAC (OR = 2.7; 95% CI, 1.3-5.6). Almost two months after disease onset, SC-CoV presented several exercise abnormalities of oxygen uptake, ventilatory adaptation and gas exchange, including a high prevalence of LAC.Highlights Weeks after the acute disease phase, one-third of mild and three-quarters of severe and critical patients with COVID-19 presented a reduced aerobic capacity. Previous studies including SARS-CoV-1 survivors observed much lower values.A severe or critical COVID-19 case was an independent predictor for low aerobic capacity.In our sample, pre-COVID-19 exercise significantly reduced the odds of post-COVID-19 low aerobic capacity. Even severe or critical patients who exercised regularly had a prevalence of low aerobic capacity 2.5 times lower than those who did not have this routine before sickening.

Altos níveis de nitrito (NO2-) podem ocorrer em sistemas de cultivo com alta densidade de estocagem, mas análises sobre os valores de concentração letal e o efeito do NO2 - em parâmetros metabólicos e no crescimento são escassos. Neste estudo foi analisada a concentração letal em 96 h (CL50-96h) para nitrito (NO2 -) em juvenis de jundiá, Rhamdia quelen, e o efeito de quatro níveis de nitrito (0,06; 0,46; 1,19 e 1,52 mg.L-1) no crescimento e no lactato, glicose, glicogênio e proteína hepática e muscular. A CL50-96h para NO2 - foi 20,46 (intervalo de confiança: 16,10-23,68) mg.L-1. No experimento de crescimento, a exposição ao NO2 não afetou o peso, comprimento ou taxa de crescimento específico, mas devido à mortalidade (66,7% e 100% após 20 e 40 dias, respectivamente), a biomassa dos juvenis expostos a 1,52 mg.L-1. NO2 - foi significativamente mais baixa que a biomassa dos juvenis expostos aos outros tratamentos. Deste modo, o nível seguro de NO2 - para o crescimento do jundiá é abaixo de 1,19 mg.L-1 (2% da CL50-96h). A exposição do jundiá ao NO2 - por 40 dias diminuiu os níveis de lactato no músculo, mas esses níveis aumentaram nos exemplares mantidos em 1,19 mg.L-1 NO2 -. Além disso, os níveis de glicose no músculo e fígado foram significativamente mais baixos nos jundiás expostos à concentração mais elevada de NO2 . Estes resultados indicam que a exposição crônica ao NO2 provoca uma oxidação anaeróbica do substrato para obtenção de energia.


High nitrite (NO2 -) levels may develop in aquaculture systems due to high fish density, but studies of lethal concentration values and the effect of NO2 - on metabolic parameters and growth are scarce. Consequently, in this study was verified the lethal concentration at 96 h (LC50-96h) for (NO2 -) in juvenile silver catfish, Rhamdia quelen and the effect of four waterborne NO2 - concentrations (0.06, 0.46, 1.19, and 1.52 mg.L-1) on growth, and hepatic and muscular lactate, glucose, glycogen and protein. Nitrite LC50-96h was 20.46 (confidence interval: 16.10-23.68) mg.L-1. In the growth experiment, exposure to NO2 - did not affect weight, length or specific growth rate, but due to mortality (66.7% and 100% after 20 and 40 days, respectively), biomass of juveniles exposed to 1.52 mg.L-1 NO2 - was significantly lower than the biomass of juveniles exposed to other treatments. Therefore, the safe level of nitrite for growth of silver catfish juveniles is below 1.19 mg.L-1 (2% of LC50-96h). Exposure of silver catfish to NO2 - for 40 days reduced lactate levels in muscle, but lactate levels increased in liver tissue of fish maintained at 1.19 mg.L-1 NO2 -. In addition, glucose levels in muscle and liver tissues were significantly lower in silver catfish exposed to the highest NO2 - level. These results indicate that chronic NO2 - exposure causes anaerobic substrate oxidation to meet energy demand.

BACKGROUND: Technetium 99m N-DBODC5 is a new myocardial perfusion tracer shown to exhibit high heart uptake and rapid liver clearance in normal rats. The objectives of this canine study were (1) to compare the organ biodistribution and myocardial uptake, washout, and redistribution kinetics of Tc-99m N-DBODC5 with Tc-99m sestamibi over a period of 3 hours in a more clinically relevant large animal species and (2) to compare the myocardial uptake of Tc-99m N-DBODC5 with thallium 201 when co-injected during vasodilator stress in dogs with coronary stenoses. METHODS AND RESULTS: At peak adenosine-induced hyperemia, 10 dogs with critical left anterior descending artery stenoses received either Tc-99m N-DBODC5 (n = 6) or Tc-99m sestamibi (n = 4) and microspheres, followed by serial imaging and blood sampling over a period of 3 hours. Another 14 dogs with either critical (n = 7) or mild (n = 7) left anterior descending artery stenoses underwent simultaneous injection of Tc-99m N-DBODC5, Tl-201, and microspheres during peak vasodilator stress. Like sestamibi, Tc-99m N-DBODC5 showed good myocardial uptake with slow washout and minimal redistribution over a period of 3 hours (P = not significant); however, Tc-99m N-DBODC5 cleared more rapidly from the liver (heart-lung ratio at 30 minutes, 0.92+/-0.11 versus 0.51 +/- 0.05; P < .05). When injected during hyperemic flow, the myocardial extraction plateau for Tc-99m N-DBODC5 was lower than that for Tl-201 and was intermediate between Tc-99m sestamibi and Tc-99m tetrofosmin. CONCLUSIONS: Excellent organ biodistribution and myocardial uptake and clearance kinetic properties, combined with rapid liver clearance and a favorable flow-extraction relationship, make Tc-99m N-DBODC5 a very promising new myocardial perfusion imaging agent.

UNLABELLED: (99m)Tc-[bis (dimethoxypropylphosphinoethyl)-ethoxyethylamine (PNP5)]-[bis (N-ethoxyethyl)-dithiocarbamato (DBODC)] nitride (N-PNP5-DBODC or N-DBODC5) is a new monocationic myocardial perfusion tracer. We sought to compare the myocardial uptake and clearance kinetics and organ biodistribution of (99m)Tc-N-DBODC5 with (99m)Tc-sestamibi and (99m)Tc-tetrofosmin. METHODS: Seventy-five anesthetized Sprague-Dawley rats were injected intravenously with 22.2-29.6 MBq (99m)Tc-N-DBODC5 (n = 25), (99m)Tc-sestamibi (n = 25), or (99m)Tc-tetrofosmin (n = 25). Rats were euthanized at either 2, 10, 20, 30, or 60 min after injection and gamma-well counting was performed on excised organ (heart, lung, and liver) and blood samples. In 3 additional rats, serial in vivo whole-body gamma-camera imaging with each tracer was performed. RESULTS: (99m)Tc-N-DBODC5 cleared rapidly from the blood pool. At 2 min after injection, (99m)Tc-N-DBODC5 blood activity was significantly lower than either (99m)Tc-sestamibi or (99m)Tc-tetrofosmin (P < 0.01) and remained lower over 60 min. Myocardial (99m)Tc-N-DBODC5 uptake was rapid (2.9% +/- 0.1% injected dose/g at 2 min), and there was no significant clearance over 60 min, similar to (99m)Tc-sestamibi and (99m)Tc-tetrofosmin. All 3 tracers exhibited rapid lung clearance. Importantly, (99m)Tc-N-DBODC5 cleared more rapidly from the liver than either (99m)Tc-sestamibi or (99m)Tc-tetrofosmin. As early as 30 min after injection, (99m)Tc-N-DBODC5 heart-to-liver ratio was 5.7 +/- 1.0 versus 1.6 +/- 0.1 and 2.9 +/- 0.3 for (99m)Tc-sestamibi and (99m)Tc-tetrofosmin (P < 0.05). By 60 min, (99m)Tc-N-DBODC5 heart-to-liver ratio further increased to 18.4 +/- 2.0 compared with 2.6 +/- 0.2 and 5.8 +/- 0.7 for (99m)Tc-sestamibi and (99m)Tc-tetrofosmin (P < 0.001). The rapid blood pool, lung, and liver clearance of (99m)Tc-N-DBODC5 resulted in excellent-quality myocardial images within 30 min after injection. CONCLUSION: (99m)Tc-N-DBODC5 is a promising new myocardial perfusion tracer with superior biodistribution properties. The rapid (99m)Tc-N-DBODC5 liver clearance may shorten the duration of imaging protocols by allowing earlier image acquisition and may markedly reduce the problem of photon scatter from the liver into the inferoapical wall on myocardial images.
