PROBLEM: Child abuse is an important independent risk factor for suicide. The purpose of the project was to evaluate the implementation of a universal suicide risk screening and assessment tool in an outpatient child abuse evaluation clinic. METHODS: Participants consisted of all patients between the ages of 10 and 18 years who received services at the study site within a 3-month period (n = 162). The study site implemented a tool created for the sole purpose of detecting suicide risk, the Ask Suicide-Screening Questions (ASQ) tool. Data was retrospectively collected using electronic health records; rates of compliance with screening and screen positive rates were calculated. FINDINGS: Suicide risk screening with the ASQ was conducted on 90.7% (147) of the patients. Forty-three (29.2%) patients screened positive for suicide risk. This is a significant increase in detection rates compared to baseline preimplementation data. Of the 67 preteens, 10-12 years of age, that were screened, 14.9% (10/67) screened positive for suicide risk, making up 23% of the positive screens. CONCLUSIONS: Implementing a universal suicide risk screening tool was a successful strategy for detecting suicide risk. A quarter of the youth who screened positive on the ASQ were preteens, highlighting the importance of screening the 10- to 12-year olds for suicide risk.

RESUMO

As difficult as it may be for someone to decide to seek mental health care, following up with repeat appointments is more difficult. A common route to that first appointment is discharge from an emergency department or a first hospitalization following a mental health crisis. Best practice in discharge planning is establishing an appointment with an outpatient therapist. Key to establishing care with a mental health professional is building a relationship based on trust and compassion. Establishing that relationship takes time, yet the beginning of that process occurs in the initial appointment and negotiation of a plan of care that the client can commit to. This article focuses on the process of negotiating a plan of care that is collaboratively agreed upon and renegotiated as appropriate. When the plan of care is collaboratively developed, psychotherapy and pharmacotherapy adherence become non-issues because the client and provider have established trust and a clear path to achieve the desired goals. RESUMO

The COVID-19 pandemic has aroused a level of anxiety and uncertainty that spans demographics and diagnoses. Because the coronavirus is a novel variant, the expert scientific community presents to the public as uncertain and inconsistent in information and ways to deal with potential infection, which creates disbelief and uncertainty. The most consistent recommendation is physical distancing and self-imposed isolation to reduce the spread. However, such isolation also adds to individual and community distress. This column focuses on the neurobiological explanations for anxiety, complications to pre-existing psychiatric disorders, sub diagnostic states of stress and anxiety generally and with health care providers, and pharmacological and nonpharmacological ways of addressing this anxiety. RESUMO

Lumateperone (Caplyta®) is a drug recently approved by the U.S. Food and Drug Administration for the treatment of schizophrenia. But is it a new drug with promise, or a similar drug with new wrappings? This drug, similar to other second- and third-generation serotonin dopamine antagonists, is a potent antagonist at higher serotonin 2A receptors as well as brief binding to dopamine 1 and dopamine 2 (D2) receptors, but also has partial agonism at presynaptic D2 and indirect modulation of N-Methyl-D-aspartic acid (NMDA) and alpha-amino-3-hydroxy-5-methyl-isoxazolepropionic acid (AMPA) of the glutamine receptors. The current article reviews the putative effects of this novel mechanism of action on symptoms of schizophrenia as discussed in Phase II and III trials. Attention-deficit/hyperactivity disorder (ADHD) is the most commonly diagnosed mental disorder in children and a small proportion retain the disorder into adulthood. The primary feature of ADHD is inattentiveness that contributes to restlessness, and it is considered a neurodevelopmental disorder characterized by a 2- to 3-year delay in cortical maturation in children. Neurophysiological studies identify functional connectivity changes in the dorsolateral prefrontal cortex, insula, and anterior cingulate cortex. The sympathomimetics that act as dopamine agonists are the mainstay in pharmacological treatment. The current article addresses the definition and manifestations of attention-deficit disorder, comorbidities in children and adults, epidemiology, neurophysiology, and pharmacological treatment strategies. Many clients do not take their medications as prescribed. One of the reasons may be the common adverse drug effects and drug-drug interactions of certain medications. This article reviews adverse drug effects (including less serious side effects), the pharmacokinetics and pharmacodynamics involved in adverse effects, and the pharmacokinetics of drug interactions. For medications to be effective in treating mental disorders, nurses need to carefully assess clients and their motivations for taking medications, routinely inquire about when and how they are taking their prescriptions, any adverse effects they are experiencing, and how they are managing common, less severe adverse effects. Treatment resistance continues to challenge and frustrate mental health clinicians and provoke psychiatric researchers to seek additional explanatory theories for psychopathology. Because the inflammatory process activates symptoms of depression, anxiety, and psychosis, it is a reasonable route to follow for primary and/or indirect contribution to mental disorders. The current article reviews the research literature regarding the role the inflammatory process and immune system play in mental disorders as well as novel treatments under investigation for resistant depression, anxiety, substance use, and psychotic disorders. Pregnancy, a sensitive time when two bodies are changing and developing simultaneously, demands careful consideration in assessing and treating mental health conditions. Add to that the restrictions on researching such a vulnerable population, psychiatric nurses face a challenge in providing evidence-based care. The current article focuses on the epidemiology of postpartum depression and long-term consequences, neurobiology of postpartum depression that guides medication selection, and treatment options for supporting postpartum women and their families. RESUMO

Despite the psychotherapeutic and pharmacotherapeutic advances in the treatment of depressive, anxiety, and psychotic disorders over the past several decades, the suicide rate in the United States has increased over the past 18 years. Medications are safer and more difficult to use in overdosing, but not necessarily more efficacious in bringing about complete remission of symptoms. The current article focuses on the theories and psychosocial and neurobiological correlates of suicide with the intent of formulating intervention strategies to prevent suicide. Particular attention is on medications that may contribute to suicidal ideation and behaviors and those that show evidence of reducing the risk of suicide. Prescribing for individuals with mental health problems requires a thorough initial assessment that includes extensive interviewing of the client and significant others, review of medical records, and physical appraisals, such as vital signs, laboratory tests, and electrocardiography. Arriving at a diagnosis and treatment plan includes ruling out comorbid conditions as well as medical conditions that mimic psychiatric disorders. Because substance use frequently coincides with other psychiatric disorders and has serious health consequences, this potential for misuse of substances must also be considered. The current article focuses on the physiological assessments necessary for prescribing and monitoring psychotropic medications. Tardive dyskinesia (TD), the choreoathetoid movements of fingers, arms, legs, and trunk and irregular stereotypical movements of the mouth, face, and tongue, has been the scourge of antipsychotic medications since the approval of chlorpromazine. TD tends to occur late in treatment and sometimes remains after discontinuation of the antipsychotic medication. With the recent approval of two medications, valbenazine (Ingrezza®) and deutetrabenazine (Austedo®), there are now treatments for this disfiguring consequence of dopamine-blocking medications. The current article distinguishes the movement disorder adverse effects of dopamine antagonists, explains the putative mechanism of action, and describes how best to treat TD with the new vesicular monamine transporter 2 (VMAT2) medications now approved by the U.S. Food and Drug Administration. RESUMO

Pharmacogenetic testing to aid in making decisions about prescribing medications was approved by the U.S. Food and Drug Administration in 2005 and gradually became a common practice. However, an innovation that was thought to help individualize prescribing psychotropic medications with fewer trials and errors soon became a disappointment to clinicians. Current pharmacogenetic testing assesses how the liver metabolizes drugs through the cytochrome p 450 system; however, much of the variability in how a drug affects an individual also relies on the pharmacodynamics of the drug (i.e., the specific ways the drug changes the body). The current article discusses the advantages and disadvantages of pharmacogenetic testing to aid in prescribing psychotropic medications. RESUMO

Long-acting injectable (LAI) antipsychotic medications have been around since the 1960s as alternatives to oral formulations to improve medication adherence. LAIs are similar enough to their corresponding oral formulations to be used interchangeably and have convincing evidence of improving consistency in pharmacotherapy that reduces the rates of relapse and frequency of hospitalization for individuals with psychosis. So why are they not used as often? The current article presents an argument to initiate LAIs early in treatment as a way of establishing consistency in treatment, thereby, potentially improving client outcomes. When dopamine was identified as a primary target for schizophrenia, the dopamine antagonists, now referred to as first-generation antipsychotics, were added to our pharmacopeia. In the 1990s, with the discovery of risperidone and clozapine, the mechanism of dopamine receptor antagonism was paired with serotonin receptor antagonism to give rise to second-generation antipsychotics. A decade later these mechanisms were further refined to selective dopamine receptors antagonism and serotonin receptors antagonism and agonism to create a modulation or stabilization of dopamine nerve firing in differential ways. This new wave may be referred to as the third generation. The current article reviews the pharmacodynamics and pharmacokinetics of these dopamine modulators. Traditionally, schools of nursing teach legal and ethical responsibilities at the pre-licensure and advanced practice levels, with emphasis on legal duties and ethical principles. However, involving clinicians in the process of ethical reasoning, a process similar to the scientific method and nursing process, is much less common. The current article describes and demonstrates the steps in ethical reasoning in common situations faced by prescribing advanced practice RNs and psychiatric-mental health nurses. Treatment-resistant depression (TRD) is a disabling and demoralizing phenomenon for patients and clinicians. For many years, treatment for depression has relied on the monoamine theory, with medications limited in efficacy and requiring prolonged use before any therapeutic effects are evident. The current article reviews novel approaches to pharmacological treatment of TRD, including N-methyl-D-aspartate glutamate receptor antagonists and other receptor agonists and antagonists beyond serotonin and norepinephrine. Treatment-resistant depression (TRD) refers to depression that fails to remit after at least two to four medication and psychotherapy treatment strategies. TRD carries significant personal burden and risk for suicide and poses substantial burden to society. Less than 30% of individuals diagnosed with major depressive disorder achieve complete remission of symptoms. Using the evidence-based predictors of TRD in the initial assessment of patients, clinicians can arrive at a treatment plan to improve outcomes. The current article includes first-line medications based on symptom presentation as well as augmentation and more aggressive treatment recommendations for patients who continue to experience depressive symptoms beyond 6 months of effective dose and time trials of treatment. As a survival strategy, trauma arouses intensive neurobiological mechanisms that, if unchecked, contribute to severe chronic stress responses and disability. Although posttraumatic stress disorder can be considered a distinctive psychiatric disorder, it likely underlies many psychiatric disorders, including depression and mood, anxiety, substance use, and personality disorders. Psychiatric-mental health nurses need to carefully assess for trauma symptomatology in all patients and diverse populations, such as children; adolescents; immigrants; active and inactive military personnel; women; and lesbian, gay, bisexual, transgender, queer individuals. Pharmacotherapy should focus on priority of target symptoms with first-line medications including serotonin reuptake inhibitors, serotonin norepinephrine reuptake inhibitors, and adrenergic blocking agents. Serotonin antagonists (i.e., second-generation antipsychotic agents) and benzodiazepine drugs are contraindicated and should be used sparingly and with caution. Psychotherapy is an effective adjunct to pharmacotherapy in treating trauma-related conditions. [Journal of Psychosocial Nursing and Mental Health Services, 56(8), 7-10.].
