RESUMO

The parameters for survival prediction of esophageal squamous cell carcinoma (ESCC) patients treated with neoadjuvant chemoradiotherapy (NCRT) combined with surgery are unclear. Here, we aimed to construct a nomogram for survival prediction of ESCC patients treated with NCRT combined with surgery based on pretreatment serological hepatic and renal function tests. A total of 174 patients diagnosed as ESCC were enrolled as a training cohort from July 2007 to June 2019, and approximately 50% of the cases (n = 88) were randomly selected as an internal validation cohort. Univariate and multivariate Cox survival analyses were performed to identify independent prognostic factors to establish a nomogram. Predictive accuracy of the nomogram was evaluated by Harrell's concordance index (C-index) and calibration curve. ALT, ALP, TBA, TP, AST, TBIL and CREA were identified as independent prognostic factors and incorporated into the construction of the hepatic and renal function test nomogram (HRFTNomogram). The C-index of the HRFTNomogram for overall survival (OS) was 0.764 (95% CI 0.701-0.827) in the training cohort, which was higher than that of the TNM staging system (0.507 (95% CI 0.429-0.585), P < 0.001). The 5-year OS calibration curve of the training cohort demonstrated that the predictive accuracy of the HRFTNomogram was satisfactory. Moreover, patients in the high-risk group stratified by the HRFTNomogram had poorer 5-year OS than those in the low-risk group in the training cohort (27.4% vs. 80.3%, P < 0.001). Similar results were observed in the internal validation cohort. A novel HRFTNomogram might help predict the survival of locally advanced ESCC patients treated with NCRT followed by esophagectomy.

Given that protein peptide powders (PPPs) from different biological sources were inherited with diverse healthcare functions, which aroused adulteration of PPPs. A high-throughput and rapid methodology, united multi-molecular infrared (MM-IR) spectroscopy with data fusion, could determine the types and component content of PPPs from seven sources as examples. The chemical fingerprints of PPPs were thoroughly interpreted by tri-step infrared (IR) spectroscopy, and the defined spectral fingerprint region of protein peptide, total sugar, and fat was 3600-950 cm-1, which constituted MIR finger-print region. Moreover, the mid-level data fusion model was of great applicability in qualitative analysis, in which the F1-score reached 1 and the total accuracy was 100%, and a robust quantitative model was established with excellent predictive capacity (Rp: 0.9935, RMSEP: 1.288, and RPD: 7.97). MM-IR coordinated data fusion strategies to achieve high-throughput, multi-dimensional analysis of PPPs with better accuracy and robustness which meant a significant potential for the comprehensive analysis of other powders in food as well.

Objective: Several guidelines highlight the beneficial impact of exercise on the management of symptoms and health-related quality of life (HRQOL) in patients with fibromyalgia syndrome (FMS). However, few analyses have compared different types of exercise. We, therefore, intent to compare the effects of different exercise types on improving the overall HRQOL and typical symptoms in patients with FMS. Methods: Medline, Embase, the Cochrane Register of Clinical Trials, and NIH ClinicalTrials.gov were searched from inception to April 21, 2022. Randomized clinical trials (RCTs) were included to assess the impact of exercise intervention on health parameters in adult FMS patients. Data were extracted independently and a frequentist network meta-analyses (NMA) was performed to rank the effects of interventions according to P-scores. The NMA evidence certainty was assessed using the method recommended by Grading of Recommendations Assessment, Development, and Evaluation Working Group. Results: A total of 57 RCTs were identified, including 3319 participants, involving 9 interventions (7 types of exercise, 2 controls). Of all treatments compared with usual care in efficacy outcomes, Mind-body exercise was associated with the best HRQOL (SMD, -12.12; 95% CI, -15.79 to -8.45). On the other characteristic symptom dimensions, based on moderate quality evidence, sensorimotor training was associated with minimal pain scores compared with usual care (SMD, -1.81; 95% CI, -2.81 to -0.82), whole body vibration therapy was most promising for improving sleep quality (SMD, -6.95; 95% CI, -10.03 to -3.87), pool-based aerobic exercise was most likely to ease anxiety (SMD, -4.83; 95% CI, -7.47 to -2.19), and whole body vibration was most likely to improve depression (SMD, -10.44; 95% CI, -22.00 to 1.12). Conclusion: Mind-body exercise seems to be the most effective exercise to improve the overall HRQOL of patients with FMS. But at the same time, clinicians still need to develop individualized exercise plans for patients according to their symptoms and accessibility.

OBJECTIVE: It is known that noxious stimuli from inflamed tissue may increase the excitability of spinal dorsal horn neurons (a process known as central sensitization), which can signal back and contribute to peripheral inflammation. However, the underlying mechanisms have yet to be fully defined. A number of recent studies have indicated that spinal NF-κB/p65 is involved in central sensitization, as well as pain-related behavior. Thus, the aim of this study was to determine whether NF-κB/p65 can facilitate a peripheral inflammatory response in rat adjuvant-induced arthritis (AIA). METHODS: Lentiviral vectors encoding short hairpin RNAs that target NF-κB/p65 (LV-shNF-κB/p65) were constructed for gene silencing. The spines of rats with AIA were injected with LV-shNF-κB/p65 on day 3 or day 10 after treatment with Freund's complete adjuvant (CFA). During an observation period of 20 days, pain-related behavior, paw swelling, and joint histopathologic changes were evaluated. Moreover, the expression levels of spinal tumor necrosis factor α (TNFα), interleukin-1ß (IL-1ß), and cyclooxygenase 2 (COX-2) were assessed on day 14 after CFA treatment. RESULTS: The presence of peripheral inflammation in rats with AIA induced an increase in NF-κB/p65 expression in the spinal cord, mainly in the dorsal horn neurons and astrocytes. Delivery of LV-shNF-κB/p65 to the spinal cord knocked down the expression of NF-κB/p65 and significantly attenuated hyperalgesia, paw edema, and joint destruction. In addition, spinal delivery of LV-shNF-κB/p65 reduced the overexpression of spinal TNFα, IL-1ß, and COX-2. CONCLUSION: These findings indicate that spinal NF-κB/p65 plays an important role in the initiation and development of both peripheral inflammation and hyperalgesia. Thus, inhibition of spinal NF-κB/p65 expression may provide a potential treatment to manage painful inflammatory disorders.

OBJECTIVE: To investigate clinical value of three-dimensional transvaginal sonography (3D-TVS) in the diagnosis of septate uterus and the relationship between its parameters and adverse pregnancy. METHODS: From Mar. 2010 to Sept. 2011, 73 patients (aged 23-35 years) with septate uterus who were diagnosed by 3D-TVS in Fujian Province Maternal and Child Health Hospital were enrolled in this retrospective study. The septum width, septum angle, septum length and the length of remaining uterine cavity were measured among the patients with subseptate uterus, and then, the distortion rate was calculated. The hysteroscopic surgery was used as the gold standard, and the diagnostic titer of 3D-TVS in the diagnosis of septate uterus was determined.Receiver operating characteristic curve (ROC) were plotted to evaluate the diagnostic titer of uterine parameters measured by 3D-TVS in predicting the adverse pregnancy outcome among patients with subseptate uterus.Univariate logistic regression was used to analyze the effectiveness of uterine parameters on adverse pregnancy outcome. RESULTS: Using hysteroscopic surgery as the gold standard, the coincidence rate of diagnosis of septate uterus by 3D-TVS was 94% (69/73) . Among the patients with septate uterus, 25% (17/69) were complete septate uterus, 75% (52/69) were subseptate uterus. Among patients with subseptate uterus, the septum length[(2.2 ± 0.6) cm] and distortion rate in patients with adverse pregnancy (0.60 ± 0.10) were significantly higher than those without adverse pregnancy [(1.5 ± 0.6) cm,0.43 ± 0.13, both P < 0.05]. However, no significant difference in the width, angle and length of septum were observed between the two groups (P > 0.05). The area under ROC curve (AUC) of septum length and distortion rate in determining adverse pregnancy were 0.833 (95%CI: 0.721-0.944) and 0.800 (95%CI: 0.671-0.929), respectively. The optimal cutoff point of septum length was 1.94 cm, with the sensitivity was 74.3% and the specificity was 76.5%; the optimal cutoff point of distortion rate was 0.48, with the sensitivity was 77.1% and the specificity was 76.5%. The expectation morbidity ratio of adverse pregnancy was 2.717, 3.067 and 0.514 by every adding level of septum length, distortion rate, and length of remaining uterine cavity, respectively. CONCLUSIONS: 3D-TVS showed high accuracy in diagnosis of septate uterus. The septum length and distortion rate may predict the risk of adverse pregnancy, and the value of them can be used for screening adverse pregnancy in clinical practice.

BACKGROUND: Toll-like receptors (TLRs) play a pivotal role in the defense against invading pathogens by detecting pathogen-associated molecular patterns (PAMPs). TLR4 recognizes lipopolysaccharides (LPS) in the cell walls of Gram-negative bacteria, resulting in the induction and secretion of proinflammatory cytokines such as TNF-α and IL-6. The WW domain containing E3 ubiquitin protein ligase 1 (WWP1) regulates a variety of cellular biological processes. Here, we investigated whether WWP1 acts as an E3 ubiquitin ligase in TLR-mediated inflammation. METHODOLOGY/RESULTS: Knocking down WWP1 enhanced the TNF-α and IL-6 production induced by LPS, and over-expression of WWP1 inhibited the TNF-α and IL-6 production induced by LPS, but not by TNF-α. WWP1 also inhibited the IκB-α, NF-κB, and MAPK activation stimulated by LPS. Additionally, WWP1 could degrade TRAF6, but not IRAK1, in the proteasome pathway, and knocking down WWP1 reduced the LPS-induced K48-linked, but not K63-linked, polyubiquitination of endogenous TRAF6. CONCLUSIONS/SIGNIFICANCE: We identified WWP1 as an important negative regulator of TLR4-mediated TNF-α and IL-6 production. We also showed that WWP1 functions as an E3 ligase when cells are stimulated with LPS by binding to TRAF6 and promoting K48-linked polyubiquitination. This results in the proteasomal degradation of TRAF6.

RESUMO

We introduce Talbot self-imaging into photoalignment technique to record Talbot carpet into an LC cell. Through the design of the setup, different images are presented on a single sample. By taking a simple 1D grating mask as an example, an LC cell with complex alignment structures applicable as Mach-Zehnder interferometer arrays is demonstrated. Further mask design permits feasibility of various structures which are practicable for many applications. This method may facilitate the fabrication of photonic applications such as optical communication, computing and sensing, etc.

Blue phase liquid crystals (BPLCs) are promising candidates for next generation display thanks to their fast response and quasi-isotropic optical properties. By taking these advantages, we propose to introduce the material into fiber-optic applications. As an example, a BPLC based variable optical attenuator (VOA) is demonstrated with a polarization independent design. The device shows normally-off feature when no field is applied. Response time down to submillisecond scale is achieved in switching between two arbitrary attenuation states. The attenuation range is also measured from 1480 to 1550 nm, which cover the whole telecomm S-band and part of the C-band. The overall performances reach the requirements for practical use; while still have room for further improvement. Through this example, the applicability of BPLC in fiber-optic devices is presented, which may impel the development of many other photonic applications from infrared to even microwave regions.

A dual-frequency liquid crystal based optical switch with orthogonally photo-patterned alignments is designed and fabricated. The cell gap is theoretically optimized for high switching performance. The measured extinction ratio of first diffraction order is over 20 dB with a low electric field of 3 V/µm. The switch On-Off time are measured to be 350 µs and 600 µs, respectively, both of which have reached submillisecond scale. Moreover, the switch is polarization independent, which has been predicted theoretically and further proved experimentally. This design is suitable in wide applications requiring fast response and polarizer free properties.

A diffraction grating is proposed by periodically defining the liquid-crystal director distribution to form alternate parallel aligned and twist nematic regions in a cell placed between two crossed polarizers. Based on the combined phase and amplitude modulation, both 1D and 2D tunable gratings are demonstrated. Low voltage ON/OFF switching of 1st order diffracted light with extinction ratio over 80 is achieved within a small voltage interval of 0.15 Vrms. Unique four-state feature of the cell is obtained and their applications in optical logic devices are discussed.

A transflective nano-wire grid polarizer is fabricated on a single mode fiber tip by focused ion beam machining. In contrast to conventional absorptive in-line polarizers, the wire grids reflect TE-mode, while transmitting TM-mode light so that no light power is discarded. A reflection contrast of 13.7 dB and a transmission contrast of 4.9 dB are achieved in the 1,550 nm telecom band using a 200-nm wire grid fiber polarizer. With the help of an optic circulator, the polarization states of both the transmissive and reflective lights in the fiber may be monitored simultaneously. A kind of robust fiber optic sensor is thus proposed that could withstand light power variations. To verify the idea, a fiber pressure sensor with the sensitivity of 0.24 rad/N is demonstrated. The corresponding stress-optic coefficient of the fiber is measured. In addition to pressure sensing, this technology could be applied in detecting any polarization state change induced by magnetic fields, electric currents and so on.

We theoretically propose a broadband optical isolator based on second-harmonic generation in a one-dimensional quadratic nonlinear photonic crystal (NPC) with an embedded defect. An external electric field along the z axis is applied to modulate the NPC refractive index periodically. Complete optical isolation always could be reached with the help of an external field. Influences of the defect position and thickness are discussed. The spectral and power tolerances of the isolator also have been investigated and show high contrast within a wide wavelength range at different power levels.

OBJECTIVE: To explore suppression of allograft vasculopathy (AV) by transfer of the calcitonin gene-related peptide (CGRP). METHODS: The descending thoracic aortas from 39 Lewis rats were grafted to the abdominal aortas of 39 F344 rats, and then the recipient rats were randomized into 3 groups: Group A transfected with Ad5-CGRP-EGFP, a gene construct containing sequences from the adenoviral oncoprotein, CGRP, and enhanced green fluorescent protein, Group B, transfected with Ad5-EGFP containing sequences from the adenoviral oncoprotein and enhanced green fluorescent protein, and Group C without transfection. Four and 8 weeks later the abdominal aortas of 6 recipient rats from each were collected respectively. The degree of vascular obstruction was observed by microscopy. Frozen tissue sections were made and the inverted phase contrast fluorescence microscopy was used to observe the green fluorescence showing the expression of CGRP and enhanced green fluorescent protein (EGFP). RT-PCR was used to detect the expression of CGRP. Immunohistochemistry was used to examine the expression of inducible nitric oxide synthase (iNOS) and vascular cell adhesion molecule-1 (VCAM-1). The apoptosis index (AI) was detected by TUNEL method. RESULTS: CGRP expression was positive in both Group A and B 4 weeks later and negative in both groups 8 weeks later. Group A is approximately normal in pathological morphology 4 weeks later. The vascular luminal occlusion score of Group A was lower than Groups B and C 4 weeks later, and significantly higher than that I the same group (P < 0.05), however, still lower than those of Groups B and C 8 weeks later. The AI 4 weeks later of Group A was significantly lower than those of Groups B and group C, however, there was no significance in AI among the 3 groups. Four weeks later the VCAM-1 expression levels in the tunicae intima, media, and adventitia of Group A were all significantly lower than those of Group B C 4 weeks later, however, the iNOS expression level of Group A was only significantly lower than those of Groups B and C in the tunica intima 4 weeks later. CONCLUSION: The expression of CGRP effectively suppresses the development of AV 4 weeks after the operation.
