STUDY DESIGN: Retrospective review of hospital charts. OBJECTIVE: (1) To determine the microbiological profile of patients with surgical site infections following posterior spinal fusion surgery (PSF) for Adolescent Idiopathic scoliosis (AIS). (2) To study the treatment outcome of patients with surgical site infections (SSI) following surgery for AIS. (3) To identify the key differences in presentation and management of acute and delayed SSI following AIS surgery. There has been increasing evidence of the role of P. acnes in deep surgical site infections. Literature related to this is abundant in relation to shoulder arthroplasty; however, it is sparse in relation to spine surgery. METHODS: We conducted a retrospective review of all patients treated for AIS during a 5-year period (2010-2014) at our institution, with a minimum of 2-year follow-up after the index surgery. Patients with a postoperative infection following their index surgery were included. Charts of AIS patients with post-op infections were reviewed for details of the index surgery, time to presentation of the infection, presenting signs/symptoms, microbiology details, details of surgical and antibiotic treatment, and outcomes. RESULTS: Nine (2.8%) post-op infections were identified out of 315 cases for AIS during this period. Seven (2.2%) involved P. acnes. Two (0.6%) involved MSSA. The average time for cultures to show growth was 6.1 days (range 5-8 days) in P. acnes group and 2-3 days in MSSA group. Patients with P. acnes infections were treated with implant removal, debridement and antibiotics. All patients achieved solid fusion except two patients from the P. acnes group had pseudoarthrosis and had to undergo revision fusion. CONCLUSION: Propionibacterium acnes was the single most common bacteria isolated from delayed surgical site infection following PSF in AIS patients. Optimal treatment consists of debridement, implant removal and antibiotics. These patients have high incidence of pseudoarthrosis. LEVEL OF EVIDENCE: Level IV.

BACKGROUND: Congenital longitudinal deficiency of the fibula is the most common lower extremity congenital deficiency, with a broad spectrum of severity and subsequent reconstructive treatment. Published classification schemes do not accurately predict reconstructive treatment currently in practice. METHODS: We reviewed all medical records of patients with a dominant deformity of congenital fibular deficiency who were managed at our institution between 1971 and 2005. We assessed the impact of limb-length inequality, foot deformity, bilateral extremity involvement, and extent of fibular preservation on the treatment of the limb deficiency. RESULTS: One hundred and four patients (including twenty-two with bilateral congenital fibular deficiency) with 126 affected extremities had adequate radiographs to be included in the study. Femoral shortening was noted in seventy (85.4%) of eighty-two patients with unilateral limb involvement. Limb-length discrepancy prior to any treatment remained proportional in forty-seven (82.5%) of fifty-seven patients during an average duration of follow-up of ten years and ten months (range, two years to fifteen years and six months). Limb salvage with foot preservation was deemed feasible in thirty-eight (97.4%) of thirty-nine five-rayed feet, thirty (81.1%) of thirty-seven four-rayed feet, twenty (48.8%) of forty-one three-rayed feet, and one of nine feet having fewer than three rays. Twenty-two (41.5%) of fifty-three limbs with an absent or vestigial fibula were not treated with amputation. Of the twenty-two patients with bilateral fibular deficiency, twelve (54.5%) had preservation of both feet, three (13.6%) had unilateral amputation, and seven (31.8%) had bilateral amputation. CONCLUSIONS: We propose a simplified classification for congenital fibular deficiency based on the clinical status of the foot and the magnitude of limb shortening as a percentage of the contralateral limb on radiographs. This classification may be effectively applied in infancy to allow the physician and family to anticipate the extent of deformity at maturity and to estimate the amount of treatment required to reconstruct this limb deformity. This system more accurately predicted the management of patients with fibular deficiency who were managed at our institution over the past three decades .

RESUMO

This review of infantile idiopathic scoliosis highlights the clinical features, etiology, epidemiology, and treatment considerations that clearly distinguish this entity from the more common diagnosis of juvenile and adolescent idiopathic scoliosis. A comprehensive understanding of infantile idiopathic scoliosis provides the basis for reliable prediction of those curves that are likely to spontaneously resolve and those that will relentlessly progress if left untreated.

METHODS: The authors conducted a survey of 296 competitive swimmers to assess the incidence and importance of interfering groin pain in breaststroke swimmers. RESULTS: Breaststroke swimmers were more likely to have current groin pain (6.92%) than individual medley swimmers who did not compete in pure breaststroke events (0, P = 0.015). Breaststroke swimmers (42.7%) were also more likely than individual medley swimmers (21.5%, P = 0.000622) or nonbreaststroke, nonindividual medley swimmers (5.8%, P = 0.00000311) to have been unable to train breaststroke in practice during the past year due to groin injury. CONCLUSION: Results indicate that breaststroke swimmers are at significant risk of groin injury, groin injury is positively correlated with increased magnitude of breaststroke training, and groin injury may prevent participation in practices and competitions.

RESUMO

Most rotational variations in young children, such as in-toeing, out-toeing, and torticollis, are benign and resolve spontaneously. Understanding the normal variations in otherwise healthy children is vital to identifying true structural abnormalities that require intervention. A deliberate assessment of the rotational profile is necessary when evaluating children who in-toe or out-toe. In-toeing is usually attributable to metatarsus adductus in the infant, internal tibial torsion in the toddler, and femoral anteversion in children younger than 10 years. Out-toeing patterns largely result from external rotation hip contracture, external tibial torsion, and external femoral torsion. Although congenital muscular torticollis is the most common explanation for the atypical head posture in children, more serious disorders, including osseous malformations, inflammation, and neurogenic disorders, should be excluded.

RESUMO

This study demonstrates the feasibility and advantages of near real-time, multiplanar, dynamic magnetic resonance image-assisted treatment of patients with developmental dysplasia of the hip. Pathoanatomy and dynamic blocks to reduction are visualized with anatomic clarity not otherwise possible. Continuous imaging allows accurate assessment and maintenance of optimum positioning throughout the casting procedure. Patient charges for this new technique are less than standard methods of treatment, and the child receives no ionizing radiation.
