[Diagnostics and surgical treatment of biliary ileal obstruction].

7 patients with biliary ileal obstruction were operated on during the past 2 years. The article focuses on the features of the clinical presentation and the efficacy of diagnostic means. The surgical tactics and volume of the intervention is suggested. Recommendations on the prevention of the biliary ileus were worked out.

[Prognosis and daily evaluation of changes in the condition of patients with peritonitis]. / Prognoz i ezhednevnaia otsenka dinamiki sostoianiia bol'nykh s peritonitom.

The authors examined 159 patients with peritonitis (87 with the generalized form and 72 patients with the diffuse form) and conducted a statistical analysis of the findings. Quantitative and some qualitative parameters of the patients were studied. The Student and Wilcoxon criteria and the correlation and discriminant analysis were used in construction of the determinative rules. Two groups of discriminant functions were constructed to obtain the prognosis of the outcome of peritonitis in a patient with daily appraisal of the dynamics of changes in his condition. The first group of prognostic indices was obtained only from the qualitative data. The second group of "simplified" prognostic indices was constructed with the use of parameters which are used most frequently in the wide clinical practice. The quality of both groups of discriminant functions proved to be comparable, i. e. both sets of determinative rules may be used in medical practice to evaluate the condition of a patient with peritonitis and construct the prognosis of the outcome of the disease.

In 49 patients with general peritonitis and acute ileus, the authors used the method of direct electromyography. On the basis of analysis of the results, the functional state of the intestine, dynamics of its motor activity, effectiveness of the treatment performed were assessed and outcome of the disease was predicted.

Methods of determination of general toxicity are thought to be necessary to obtain higher efficiency of treatment of patients with peritonitis. They facilitate the establishment of indications for choice of methods of the complex detoxicating therapy. Using the method of determination of the concentration of toxic middle mass metabolites gives an objective assessment of the degree of the endogenic intoxication syndrome.
