RESUMO

Literature data and original observations have been used to develop the rationale for the necessity of studying psychic disorders (PD) in patients with immuno-inflammatory rheumatic diseases (RD), such as rheumatoid arthritis (RA) and systemic lupus erythematosus (SLE). Special attention is given to the high frequency of disturbances of the anxiety and depression profile (DADP) in patients having RA and SLE with reference to the common provoking stress and pathogenic factors, clinical manifestations, and RD. Great importance is attributed to the degree of depression associated with inflammatory activity, pain intensity, fatigue, sleep disorders, severe functional insufficiency and low quality of life in the patients with RA and SLE. Special emphasis is laid on the influence of depression and stress factors on the survival and morality of patients with these pathologies, the necessity of their combined treatment with the participation of rheumatologists, psychiatrists and medical psychologists for the improvement of clinical course and prognosis of RD.

Using the method of semiquantitative reverse transcription PCR we studied the expression of hOGG1 gene (exons 1-4 and 1-7) in peripheral blood cells from patients with systemic lupus erythematosus. Expression of this gene was disturbed in 9 of 18 patients (50%). In two patients mRNA of this gene was not expressed. In 5 patients hOGG1 mRNA was practically absent and was detected only after 35 amplification cycles. Suppression of exons 4-7 expression was detected in two of these cases and expression of both mRNA fragments (exons 1-4 and 4-7) was suppressed in 3 cases. In two patients a decrease in the relative content of exons 4-7 mRNA was found against the background of normal level of mRNA for exons 1-4. In none of the patients changes in the sequence of hOGG1 mRNA exons were revealed by parallel DGGE analysis. Our findings attest to disturbances in the expression of hOGG1 gene, primarily alpha-hOGG1 isoforms of the enzyme, in peripheral blood cells in patients with systemic lupus erythematosus.

Blood plasma samples from patients with systemic lupus erythematosus having the anti-phospholipid antibody syndrome were found to contain anti-plasminogen antibodies of the IgG class. The titers of anti-plasminogen autoantibodies of the IgG class were elevated in these patients compared with normal controls. Part of the pool of IgG anti-plasminogen antibodies reacts with an epitope in the lysine-binding sites of plasminogen. Anti-plasminogen IgG isolated from patients' blood plasma is specific only for a native epitope of human plasminogen passively adsorbed on immunosorbent micro-titration plate. As shown by enzyme immunoassay, autoantibodies to plasminogen of the IgG class cross-react with human fibrinogen.

Induction of DNA breaks with 4-NQO and their resynthesis in the cultivated lymphocytes of peripheral blood of 23 patients with systemic lupus erythematosus (SLE) and 12 patients with rheumatoid arthritis (RA) were studied by the method of hydroxyapatite column chromatography of cell lysates. The number of spontaneous DNA breaks was shown to increase in 48% of patients with SLE and in 42% of patients with RA, compared to that in the control. Inhibition of the repair process was observed in 35% of patients with SLE and in 17% of patients with RA. Complete repair of DNA breaks was observed in 67% of patients with RA and only in 26% of patients with SLE.
