RESUMO

Pulmonary blastoma (PB) is a rare, highly malignant tumor prone to distant metastasis and recurrence, and the prognosis of these patients is often poor. We report a case of metastatic PB with a good prognosis with the aim of providing data to support a clinical diagnosis and treatment. In December 2015, a 43-year-old male patient was admitted to our hospital because of a cough and blood-stained sputum. Positron emission-computed tomography showed massive high-density imaging in the lower lobe of the right lung, with a maximum cross-section of 76 × 58 mm. Thoracoscopic-assisted right lower lobectomy with lymph node dissection was performed. After 1 month, computed tomography showed a high possibility of metastasis. The patient then received docetaxel and cisplatin chemotherapy for a total of six courses. After chemotherapy, enhanced computed tomography showed considerable absorption of pleural effusion, and a left lobe pulmonary nodule was not detected. The postoperative pathological diagnosis was PB, and epithelial and mesenchymal differentiation components were observed. The patient continued to visit the hospital regularly for re-examination and imaging examinations. Currently, no signs of recurrence or distant metastasis have been detected.

CONTEXT: Membranous glomerulonephritis (MGN) is a leading cause of nephrotic syndrome in adults. Diosgenin (DG) has been reported to exert antioxidative and anti-inflammatory effects. OBJECTIVE: To investigate the renoprotective activity of DG in a cationic bovine serum albumin-induced rat model of MGN. MATERIALS AND METHODS: Fourty male Sprague-Dawley rats were randomized into four groups. The MGN model was established and treated with a DG dose (10 mg/kg) and a positive control (TPCA1, 10 mg/kg), while normal control and MGN groups received distilled water by gavage for four consecutive weeks. At the end of the experiment, 24 h urinary protein, biochemical indices, oxidation and antioxidant levels, inflammatory parameters, histopathological examination, immunohistochemistry and immunoblotting were evaluated. RESULTS: DG significantly ameliorated kidney dysfunction by decreasing urinary protein (0.56-fold), serum creatinine (SCr) (0.78-fold), BUN (0.71-fold), TC (0.66-fold) and TG (0.73-fold) levels, and increasing ALB (1.44-fold). DG also reduced MDA (0.82-fold) and NO (0.83-fold) levels while increasing the activity of SOD (1.56-fold), CAT (1.25-fold), glutathione peroxidase (GPx) (1.55-fold) and GSH (1.81-fold). Furthermore, DG reduced Keap1 (0.76-fold) expression, Nrf2 nuclear translocation (0.79-fold), and induced NQO1 (1.25-fold) and HO-1 (1.46-fold) expression. Additionally, DG decreased IL-2 (0.55-fold), TNF-α (0.80-fold) and IL-6 (0.75-fold) levels, and reduced protein expression of NF-κB p65 (0.80-fold), IKKß (0.93-fold), p-IKKß (0.89-fold), ICAM-1 (0.88-fold), VCAM-1 (0.91-fold), MCP-1 (0.88-fold) and E-selectin (0.87-fold), and also inhibited the nuclear translocation of NF-κB p65 (0.64-fold). DISCUSSION AND CONCLUSIONS: The results suggest a potential therapeutic benefit of DG against MGN due to the inhibition of the NF-κB pathway, supporting the need for further clinical trials.

BACKGROUND: IgA nephropathy (IgAN) is the most prevalent primary glomerulonephritis worldwide and a leading cause of chronic kidney failure. There are currently no definitive therapeutic regimens to treat or prevent the progression of IgAN. However, biologic agents offer novel therapeutic approaches that target immunological mechanisms to slow or halt disease progression. The objective of this study is to evaluate the efficacy and safety of biologic agents in patients with IgA nephropathy. METHODS: We will systematically search PubMed, EMbase, Web of Science, Cochrane Library, and www.clinicaltrials.gov for randomized controlled trials of biologic agents for the treatment of IgA nephropathy. The search period will span from the establishment of each database until October 2023. The quality assessment of included studies will be performed individually using the revised Cochrane risk-of-bias tool for randomized trials (RoB 2), and meta-analysis will be conducted using Revman 5.4.1 software. CONCLUSIONS: The results of this study will provide evidence-based medical evidence for the clinical application of biologic agents in patients with IgA nephropathy. BACKGROUND: Atrial cardiopathy(AC) and patent foramen ovale (PFO) are two etiologies of embolic strokes of undetermined source (ESUS). We aimed to explore the relationship between them in ESUS. METHODS: A total of 1146 participants were included from January 2019 to June 2022, which included the ESUS group and non-embolic stroke which includes LAA(large arterial atherosclerosis) + SAO(small artery occlusion) group. AC was defined as the presence of at least one of the following: PTFV1(P-wave terminal force in lead V1) > 4000 µV*ms in the electrocardiograms, NT-proBNP(N-terminal probrain natriuretic peptide) > 250 pg/mL in laboratory tests or LAD(left atrial diameter) > 3.8 cm for women and > 4.0 cm for men in cardiac ultrasound. The presence of PFO was assessed by transthoracic echocardiography, transcranial Doppler ultrasound, transesophageal echocardiography or cardiac MRI. PFO was considered pathogenic if the RoPE score was 7 to 10. RESULTS: The prevalence of AC and PFO was higher in the ESUS group than the LAA + SAO group. The prevalence of AC was lower in ESUS patients with pathogenic PFO (37.9%) than those without PFO (68.4%) and with incidental PFO (64.0%) (p = 0.006). The prevalence of pathogenic PFO was lower in ESUS patients with AC than those without AC (6.0% vs. 17.8%, p = 0.006). The AUC(area under the curve) of PTFV1 for predicting ESUS was 0.724 [95%CI (0.686-0.762), p < 0.05)], indicating that PTFV1 the most valuable AC biomarker. CONCLUSIONS: The prevalence of AC is inversely related to the prevalence of pathogenic PFO in ESUS patients. PTFV1 was the most valuable index to predict ESUS among the AC biomarkers.

To improve the effectiveness of marketed drugs related to active ingredients, it is necessary to designate a more unified quality evaluation standard. Taking Nvjin Pills as an example, this study reported the development of a novel principle of analysis in traditional Chinese medicine. The core of the experiment is to prepare three batches of traditional Chinese medicine reference drugs by high-quality Chinese materia medica. The active ingredients identified in the herbal formula including glycyrrhizic acid, cinnamaldehyde, paeonol, baicalin, hesperidin, paeoniflorin, and ferulic acid were analyzed in traditional Chinese medicine reference drugs by the high-performance liquid chromatography method combined with wavelength switching. The simple prediction results of network pharmacological analysis verified the feasibility and reliability of the established quantitative analysis method for seven target-focused compounds in Nvjin Pills, which were recommended as candidate indicators for quality evaluation ultimately. Using the seven target-focused compounds as the scientific ruler, quality grade specifications of Nvjin Pills were proposed by comprehensive analysis. Accordingly, 16, 47, and 13 batches of samples were primarily graded as first grade, second grade, and unqualified grade, respectively. This study will provide a chemical basis for quality control of Nvjin Pills, which is necessary for the production process of pharmaceutical development.

This study aimed to systematically evaluate the efficacy and safety of different Chinese patent medicines in the treatment of idiopathic membranous nephropathy. The relevant randomized controlled trial(RCT) was retrieved from PubMed, EMbase, Cochrane Library, CNKI, SinoMed, Wanfang, and VIP with the time interval from database inception to December 2022. The Cochrane risk of bias assessment tool was employed to evaluate the quality of the included RCT, and Stata 15.0 and GEMTC to perform the Bayesian network Meta-analysis. Finally, 51 RCTs were included, involving 9 Chinese patent medicines and 3 591 patients. The results of network Meta-analysis showed that in terms of the total effective rate and the increase in plasma albumin, the top three interventions were Zhengqing Fengtongning Sustained Release Tablets + conventional western medicine, Bailing Capsules + conventional western medicine, and Tripterygium Glycosides Tablets + conventional western medicine. In terms of reducing 24-hour urine total protein, the top three interventions were Zhengqing Fengtongning Sustained Release Tablets + conventional western medicine, Shenfukang Capsules +conventional western medicine, and Huangkui Capsules + conventional western medicine. In terms of reducing serum creatinine, the top three interventions were Shenfukang Capsules + conventional western medicine, Bailing Capsules + conventional western medicine, and Zhengqing Fengtongning Sustained Release Tablets + conventional western medicine. In terms of safety, Chinese patent medicines combined with conventional western medicine had fewer adverse reactions than the control group. The results suggest that Chinese patent medicines combined with conventional western medicine can improve the therapeutic effect on idiopathic membranous nephropathy, and differentiated medications can be adopted according to the specific symptoms of patients in clinical treatment. Further validation needs to be carried out in the future with multi-center, large-sample, and high-quality RCT.

Aims: The differential diagnosis between ALK-negative anaplastic large cell lymphoma (ALK- ALCL) and peripheral T-cell lymphoma, not otherwise specified (PTCL, NOS) with high expression of CD30 (CD30high) are essential. However, no reliable biomarker is available in daily practice except CD30. STAT3 is characteristically activated in ALCL. We aimed to investigate whether the status of STAT3 phosphorylation could help the differential diagnosis. Methods: The status of phosphorylation of STAT3 was examined using two antibodies against pSTAT3-Y705 and pSTAT3-S727 by immunohistochemistry in ALK+ ALCL (n=33), ALK- ALCL (n=22) and PTCL, NOS (n=34). Ten PTCL, NOS with diffuse CD30 expression were defined as CD30high PTCL, NOS. Flowcytometric analysis were performed to evaluate the expression of pSTAT3-Y705/S727 in PTCL, NOS (n=3). Results: The median H-scores of pSTAT3-Y705 and S727 were 280 and 260 in ALK+ ALCL, 250 and 240 in ALK- ALCL, and 45 and 75 in CD30high subgroup, respectively. Using H score of 145 as the cutoff value, pSTAT3-S727 alone distinguished between ALK- ALCL and CD30high PTCL, NOS with a sensitivity of 100% and specificity of 83%. Additionally, pSTAT3-S727, but not pSTAT3-Y705, was also expressed by background tumor-infiltrating lymphocytes (S727TILs) in PTCL, NOS. PTCL, NOS patients with high S727TILs H score had a favorable prognosis than those with no TILs (3-year OS rate: 43% vs. 0, p=0.013) or low S727TILs (3-year OS rate: 43% vs. 0, p=0.099). Flowcytometric analysis revealed that of the three patients investigated, two had enhanced pSTAT-S727 signals in neoplastic cell populations, and all three patients were negative for pSTAT3-Y705 expression in both tumor cells and background lymphocytes. Conclusions: pSTAT3-Y705/S727 can be used to help distinguish ALK- ALCL from CD30high PTCL, NOS and pSTAT3-S727 expression by TILs predicts the prognosis of a subset of PTCL, NOS.

Recovering lithium from lithium batteries (LIBs) is a promising approach for sustainable ternary lithium battery (T-LIB) development. Current lithium recovery methods from spent T-LIBs mainly concentrated on chemical leaching methods. However, chemical leaching relying on the additional acid seriously threatens the global environment and nonselective leaching also leads to low Li recovery purity. Here, we first reported a direct electro-oxidation method for lithium leaching from spent T-LIBs (Li0.8Ni0.6Co0.2Mn0.2O2); 95.02% of Li in the spent T-LIBs was leached under 2.5 V in 3 h. Meanwhile, nearly 100% Li recovery purity was also achieved, attributed to no other metal leaching and additional agents. We also clarified the relationship between lithium leaching and other metals during the electro-oxidation of spent T-LIBs. Under the optimized voltage, Ni and O maintain the electroneutrality in the structure assisting Li leaching, while Co and Mn maintain their valence states. A direct electro-oxidation Li leaching approach achieves high Li recovery purity and meanwhile overcomes the secondary pollution problem.

Graphene has shown great potential in various application fields due to its excellent carrier transportation, ultra-high specific surface area, good mechanical properties, and light transmittance. However, pure graphene still exhibits some insurmountable defects, such as difficulty in simple and large-scale preparation, and limitations in application. The electrochemical method is a simple, clean, and environmentally friendly method. The rapid and simple preparation of graphene and its derivatives by electrochemical methods has important environmental significance. Moreover, rGO-based nanohybrids can be prepared by convenient and quick electrodeposition or cyclic voltammetry (CV), or to change the morphology and structure of graphene and its derivatives to achieve the purpose of improving material properties. This work mainly summarizes electrochemically related graphene from four aspects: (i) the method of electrochemical exfoliation of graphene; (ii) types of electrodeposition rGO-based nanohybrids; (iii) electrochemical regulation of the structure of rGO-based mixtures; (iv) environmental applications of rGO-based nanohybrids prepared by electrodeposition. This article critically discusses the advantages and disadvantages of electrochemical-related graphene, outlines future challenges, and provides insightful views and references for other researchers.

BACKGROUND: Liver fibrosis is a common process resulting from various etiologies. Sustained progression of liver fibrosis leads to cirrhosis, even hepatocellular carcinoma. Thus, noninvasive staging of liver fibrosis is of clinical importance. Radiomics is an emerging approach for staging liver fibrosis. However, the feature selection methods and classifier models are complicated, and may result in a discrepancy of diagnostic performance owing to different radiomics models. PURPOSE: To identify the optimal feature selection and classifier methods for predicting liver fibrosis by using nonenhanced T1 -weighted imaging. STUDY TYPE: Prospective. ANIMAL MODEL: Wistar rats, total 97. FIELD STRENGTH/SEQUENCE: 3T, 3D T1 -weighted images with fast-spoiled gradient echo (FSPGR). ASSESSMENT: Liver fibrosis rats were induced via subcutaneous injection of a mixture of carbon tetrachloride. Rats in the control group were injected with saline. Segmentation and feature extraction were performed by 3D slicer and the image biomarker explorer (IBEX) software package. Data preprocessing, feature selection, model building, and model comparative evaluation were conducted with Python. The liver fibrosis stage was determined by pathological examination. STATISTICAL TESTS: Receiver operating characteristic curve, fuzzy comprehensive evaluation. RESULTS: For discriminating between F0 and F1-2, F0 and F3-4, F0 and F1-4, F0-1 and F2-4, F0-2 and F3-4, and F0-3 and F4, the accuracies of 12 radiomics models were 77.27-90.91%, 73.33-86.67%, 80.56-91.67%, 74.07-88.89%, 76.47-88.24%, and 79.49-92.31%, respectively. The AUCs of the radiomics models were 0.86-0.97, 0.85-0.95, 0.89-0.97, 0.81-0.96, 0.82-0.93, and 0.85-0.96, respectively. The least absolute shrinkage and selection operator / support vector machine (LASSO-SVM) model had high AUCs of 0.93-0.97. For discriminating between F0 and F1-2, F0 and F3-4, F0 and F1-4, F0-1 and F2-4, and F0-2 and F3-4, the fuzzy comprehensive evaluation showed that the LASSO-SVM model had a high fuzzy score/order of 0.087-0.091/1. DATA CONCLUSION: LASSO-SVM appears to be the optimal model for predicting liver fibrosis by using nonenhanced T1 -weighted imaging in a rodent model of liver fibrosis. LEVEL OF EVIDENCE: 2. PURPOSE: To investigate the relationship between gut microbiota and liver fibrosis and establish a microbiota biomarker for detecting and staging liver fibrosis. METHODS: 131 Wistar rats were used in our study, and liver fibrosis was induced by carbon tetrachloride. Stool samples were collected within 72 hours after the last administration. The V4 regions of 16S rRNA gene were amplified. The sequencing data was processed using the Quantitative Insights Into Microbial Ecology (QIIME version 1.9). The diversity, principal coordinate analysis (PCoA), nonmetric multidimensional scaling (NMDS), and linear discriminant analysis (LDA) effect size (LEfSe) were performed. Random-Forest classification was performed for discriminating the samples from different groups. Microbial function was assessed using the PICRUST. RESULTS: The Simpson in the control group was lower than that in the liver fibrosis group (p = 0.048) and differed significantly among different fibrosis stages (p = 0.047). The Chao1 index in the control group was higher than that in the liver fibrosis group (p < 0.001). NMDS analysis showed a marked difference between the control and liver fibrosis groups (p < 0.001). PCoA analysis indicated the different community composition between the control and liver fibrosis groups with variances of PC1 13.76% and PC2 5.89% and between different liver fibrosis stages with variances of PC1 10.51% and PC2 7.78%. LEfSe analysis showed alteration of gut microbiota in the liver fibrosis group. Biomarkers obtained from Random-Forest classification showed excellent diagnostic accuracy in prediction of liver fibrosis with AUROCs of 0.99. The AUROCs were 0.77~0.84 in prediction of stage F4. There were six increased and 17 decreased metabolic functions in the liver fibrosis group and 6 metabolic functions significantly differed among four liver fibrosis stages. CONCLUSION: Gut microbiota is a potential biomarker for detecting and staging liver fibrosis with high diagnostic accuracies.

Streptococcus pneumoniae (pneumococcus) is touted to be the generally found pathogen in patients with respiratory issues and there is an epidemiologic linkage present between Respiratory syncytial virus (RSV). This study aim at investigating the interaction between RSV and two serotypes of S. pneumoniae using a distinct animal model and a well-established colonizing pneumococcal strain. Phase variants phenotype of each strain was determined under oblique light. Co infection model was developed using BALB/c mice housed in a BSL-2 facility. Coinfection experiments were performed and number of bacterial colonies was quantified and phase determination was evaluated. RSV was detected in sample through real-time quantitative PCR. Adherence assays were performed to determine adherence of Spn strains and its knock out ΔNanA to nasopharyngeal carcinoma (NPC) epithelial CNE3 cell line. The biofilm viability was determined and phase composition was counted using plate count. Neuraminidase activity was measured in fluorometircassessed using 2'-(4-methylumbelliferyl)-α-D-N-acetylneuraminic acid (MUAN) as substrate as described in earlier literature. The GraphPad Software version 5.01 i.e., GraphPad Prism was used to conduct the statistical analysis. The extent of bacterial colonization was increased significantly (p < 0.05), when the mice were co infected. Nasal epithelium remained intact in mock sample with features of a thick mucociliary border. A small percentage of pneumococci exhibit phase variation between opaque phase and transparent phase. The percentage adherent of both phase were not found to be varying significantly within serotype but it was seen that nonpathogenic type 27 was more adherent. Biofilm formation was selectively more for transparent phase from a mixed-phase inoculum. Adherence of both phase variant of S. pneumoniae to nasopharyngeal epithelial cells 2 h post infection expressed as the percentage of adherent bacteria relative to the inoculum. In absence of viral infection, the nasal colonization of the opaque and the transparent variant was increased many folds, which was a significant differences. The extent of nasal colonization by the ΔNanA mutant strain were significantly reduced post-bacterial infection for both type of wild-type (P < 0.05). The findings explore insights into the interactions occurring between S. pneumoniae and RSV during respiratory infections and pneumococcal acquisition, indicate that pneumococcal serotypes have different ability to cause infection as well as co infections and potentially follow an unappreciated mechanism. Much more research work is needed to further understand the minutiae of this interaction within co-infection process.

Objective: Saturation biopsy is more sensitive than transrectal biopsy in the detection of prostate cancer but is an invasive method and has a risk of overdiagnosis. Multiparametric magnetic resonance imaging (mpMRI) provides imaging and working information of the prostate. The purpose of the study was to compare the performance of pelvic phased-array mpMRI against saturation biopsies in men with suspected advanced prostate cancer considering pathology of the surgical specimen as the reference standard.Materials and methods: Data of men (n = 81) with prostate-specific antigen >10 ng/mL, low free-to-total ratio <0.1, and/or prostate-specific antigen density >0.15 who underwent mpMRI and saturation biopsy prior to radical prostatectomy were reviewed. The mpMRI was characterized as per Prostate Imaging Reporting and Data System v2.1. Gleason scores ≥3 + 4 were considered as prostate cancer. The beneficial score was evaluated for each diagnostic method for the decision-making of prostatectomy.Results: mpMRI was positive in 72 men, while saturated biopsies reported 57 men with positive prostate cancer. The histopathology of the surgical specimen reported prostate cancer in 76 men. mpMRI and saturated biopsies reported 0.934 and 0.737 sensitivities and 0.926 and 0.741 specificities, respectively. mpMRI had cancer detectability between 0.55 and 0.965 diagnostic confidence and saturation biopsies had cancer detectability between 0.85 and 0.952 diagnostic confidence. Above 0.965 and 0.952 diagnostic confidence, mpMRI and saturation biopsies had the risk of overdiagnosis.Conclusions: mpMRI can provide additional information for the detection of prostate cancer before saturation biopsies.Level of Evidence: III.

Highly pathogenic porcine reproductive and respiratory syndrome virus (HP-PRRSV) is characterized by high fever and high mortality in pigs of all ages and has severely affected the pork industry of China in the last decades. This study evaluated the differences in protection conferred by of three MLV PRRS vaccines derived from classical PRRSV (C-PRRSV, VR2332) and HP-PRRSV (TJM-F92 and JXA1-R) against the field challenge of HP-PRRSV TP strain (JXA1-R like). Compared to the experiment pigs in control group which were vaccinated with normal saline, the MLV PRRS vaccinated pigs had milder clinical symptoms, fewer pathological changes in the lung, and higher body weight gain at the end of the study. However, piglets vaccinated with VR2332 had higher body temperature, higher viral loads and lower body weight gain when compared with piglets vaccinated with TJM-F92 or JXA1-R vaccines at the end of the study. The results demonstrated that VR2332 vaccine provided a limited cross-protection against the HP-PRRSV TP strain infection, while in contrast the TJM-F92 and JXA1-R vaccines provided more efficacious protection. The findings of this study could serve as a valuable reference guide for the pig producers and veterinarians when considering the choice of which type of MLV PRRS vaccines to protect their pig herds against field challenge by HP-PRRSV TP strain.

OBJECTIVES: To assess the quality of life (QOL) and factors affecting QOL in hemodialysis patients so as to improve QOL of dialysis patients and provide the basis for better clinical care. METHODS: A retrospective study was performed to assess the QOL and factors affecting QOL in hemodialysis patients. We recruited 125 patients who had been receiving hemodialysis for at least 2 years in the dialysis units of nine hospitals in Shanxi Province, China, and conducted a multi-center questionnaire survey between 1 May 2015 and 1 July 2016. We investigated the patients' general condition and clinical data and used the Short Form-36 (SF-36) scale to measure QOL in these patients. RESULTS: The overall SF-36 score was 107.55 ± 14.50 in patients who had received hemodialysis for more than 2 years. Age (p < .05, F = 4.972) and gender (p < .01, t = 3.993) significantly affected the overall QOL score in these patients. Education level was also an influencing factor (p < .05, Z= -0.838), especially on the mental health of these patients. In addition, residual urine volume (p < .05, Z= -2.465) and diabetic nephropathy (p < .05, Z= -2.062) were important factors that affected the physical strength and QOL score in these patients. However, sources of medical expenses, marital status and different methods of dialysis, had no effect on the QOL score. CONCLUSION: The overall score of QOL in patients who have received maintenance hemodialysis for more than 2 years is higher in Shanxi Province than that in other provinces of China. Only a few factors influenced the QOL of these patients.

In this study, a multifunctional graphene based nanohybrid, termed as GN/Fe3O4/PF127, is engineered via a facile one-pot process consisting of simultaneous reduction of graphene oxide/Fe3O4 and subsequent assembly of Pluronic F127 (PF127) onto graphene nanosheets (GNs). The unique aromatic and planar structure of GNs allows the attachment of multiple functional components including MRI contrast agent (Fe3O4 nanoparticles) and an aromatic anticancer drug like doxorubicin (DOX), as well as PF127 coating which imparts physiological dispersivity and stability to the nanohybrid. The successful assembly process is revealed by TEM observation, size and FITR monitoring. In contrast with the primitive graphene or its oxide derivative, the resulting GN/Fe3O4/PF127 nanohybrids have shown high biological dispersion and MRI effect for diagnosis due to the incorporation of superparamagnetic Fe3O4 nanoparticles without evident cytotoxicity. Moreover, the GN/Fe3O4/PF127 nanohybrid exhibits a photothermal effect due to the considerable optical absorption in the near-infrared region of GNs. The GN/Fe3O4/PF127 nanohybrid could be a further platform for chemophototherapy assisted by the therapeutic DOX. Cellular toxicity assays indicated that the DOX-loaded GN/Fe3O4/PF127 nanohybrid showed a remarkable cytotoxicity to HeLa cells and the cytotoxic effect was intensified when subjected to photoirradiation. Confocal laser scanning microscopy (CLSM) and flow cytometric analysis (FCAS) revealed that the nanohybrid could be easily uptaken into HeLa cells.

BACKGROUND AND OBJECTIVE: It has been proven that telomerase activation correlates with the carcinogenesis, aggressiveness and turnover of lung cancer. Telomerase is one of the improtant molecular biomarkers for diagnosis and targeting therapy in lung cancer. The aim of this study is to investigate the diagnostic value of the combined determination of telomerase activity in induced sputum, pleural effusion and fiberobronchoscopic biopsy in lung cancer patients. METHODS: The technique of TRAP (telomeric repeat amplification protocal)-PCR-ELISA was employed to detect telomease levels of induced sputum, pleural effusion and fiberobronchoscopic biopsy in 80 lung cancer patients with pleural effusion and 50 benign pulmonary disease patients with pleural effusion. RESULTS: Telomemse levels of induced sputum, pleural effusion and fiberobronchoscopic biopsy were all significantly higher in patients with lung cancer than those with benign pulmonary disease (P < 0.001). There was no significant difference in the level of telomerase activity between different pathologic types (P > 0.05). The sensitivity of induced sputum, pleural effusion and fiberobronchoscopic biopsy were 62.5% (50/80), 46.3% (37/80) and 60.0% (48/80), respectively. The specificity were 72.0% (36/50), 66.0% (33/50) and 70.0% (35/50), respectively. The overall accuracy were 66.2% (86/130), 53.8% (70/130) and 63.8% (83/130), respectively. The sensitivity, specificity and overall accuracy of combined induced sputum, pleural effusion and fiberobronchoscopic biopsy were 85.0% (68/80), 78.0% (39/50) and 82.3% (107/130), respectively. The sensitivity of telomease level in combined detection for diagnosis of lung cancer was much higher than that in single sample detection (P < 0.01). CONCLUSION: The sensitivity of telomease activity in combined three samples was the highest. It can further improve the accuracy for the diagnosis of lung cancer with pleural effusion.

OBJECTIVE: To access the therapeutic effect of Tuina for treatment of infants eczema. METHODS: Two hundred and forty children with eczema were randomly divided into a Tuina group and a medication group, 120 cases in each group. The Tuina group was treated with Tuina on ten points using the thumb and middle finger, and the medication group was treated with oral administration of Chlorpheniramine and topical application of zinc oxide ointment or Youzhuoer ointment, etc. The therapeutic effects were evaluated after 3 weeks. RESULTS: The cured-markedly effective rate and total effective rate were 94.2% and 99.2% in the Tuina group and 98.0% and 100.0% in the medication group, respectively, the therapeutic effects were similar in the two groups (both P>0.05); 6 months after treatment, the recurrence rate of 3.8% in the Tuina group was significantly lower than 42.9% in the medication group (P<0.01), and there were no adverse reactions in the whole research process. CONCLUSION: Tuina on ten points for treatment of infants eczema has unequivocal short-term effect, a stable long-term effect, and low recurrence rate.
