RESUMO

At the dawn of of Artificial General Intelligence (AGI), the emergence of large language models such as ChatGPT show promise in revolutionizing healthcare by improving patient care, expanding medical access, and optimizing clinical processes. However, their integration into healthcare systems requires careful consideration of potential risks, such as inaccurate medical advice, patient privacy violations, the creation of falsified documents or images, overreliance on AGI in medical education, and the perpetuation of biases. It is crucial to implement proper oversight and regulation to address these risks, ensuring the safe and effective incorporation of AGI technologies into healthcare systems. By acknowledging and mitigating these challenges, AGI can be harnessed to enhance patient care, medical knowledge, and healthcare processes, ultimately benefiting society as a whole.

RESUMO

The emergence of artificial general intelligence (AGI) is transforming radiation oncology. As prominent vanguards of AGI, large language models (LLMs) such as GPT-4 and PaLM 2 can process extensive texts and large vision models (LVMs) such as the Segment Anything Model (SAM) can process extensive imaging data to enhance the efficiency and precision of radiation therapy. This paper explores full-spectrum applications of AGI across radiation oncology including initial consultation, simulation, treatment planning, treatment delivery, treatment verification, and patient follow-up. The fusion of vision data with LLMs also creates powerful multimodal models that elucidate nuanced clinical patterns. Together, AGI promises to catalyze a shift towards data-driven, personalized radiation therapy. However, these models should complement human expertise and care. This paper provides an overview of how AGI can transform radiation oncology to elevate the standard of patient care in radiation oncology, with the key insight being AGI's ability to exploit multimodal clinical data at scale.

Explainable machine learning attracts increasing attention as it improves the transparency of models, which is helpful for machine learning to be trusted in real applications. However, explanation methods have recently been demonstrated to be vulnerable to manipulation, where we can easily change a model's explanation while keeping its prediction constant. To tackle this problem, some efforts have been paid to use more stable explanation methods or to change model configurations. In this work, we tackle the problem from the training perspective, and propose a new training scheme called Adversarial Training on EXplanations (ATEX) to improve the internal explanation stability of a model regardless of the specific explanation method being applied. Instead of directly specifying explanation values over data instances, ATEX only puts constraints on model predictions which avoids involving second-order derivatives in optimization. As a further discussion, we also find that explanation stability is closely related to another property of the model, i.e., the risk of being exposed to adversarial attack. Through experiments, besides showing that ATEX improves model robustness against manipulation targeting explanation, it also brings additional benefits including smoothing explanations and improving the efficacy of adversarial training if applied to the model.

Over the last decade, deep neural networks (DNNs) are regarded as black-box methods, and their decisions are criticized for the lack of explainability. Existing attempts based on local explanations offer each input a visual saliency map, where the supporting features that contribute to the decision are emphasized with high relevance scores. In this paper, we improve the saliency map based on differentiated explanations, of which the saliency map not only distinguishes the supporting features from backgrounds but also shows the different degrees of importance of the various parts within the supporting features. To do this, we propose to learn a differentiated relevance estimator called DRE, where a carefully-designed distribution controller is introduced to guide the relevance scores towards right-skewed distributions. DRE can be directly optimized under pure classification losses, enabling higher faithfulness of explanations and avoiding non-trivial hyper-parameter tuning. The experimental results on three real-world datasets demonstrate that our differentiated explanations significantly improve the faithfulness with high explainability. BACKGROUND: Emotions after surviving cancer can be complicated. The survivors may have gained new strength to continue life, but some of them may begin to deal with complicated feelings and emotional stress due to trauma and fear of cancer recurrence. The widespread use of Twitter for socializing has been the alternative medium for data collection compared to traditional studies of mental health, which primarily depend on information taken from medical staff with their consent. These social media data, to a certain extent, reflect the users' psychological state. However, Twitter also contains a mix of noisy and genuine tweets. The process of manually identifying genuine tweets is expensive and time-consuming. METHODS: We stream the data using cancer as a keyword to filter the tweets with cancer-free and use post-traumatic stress disorder (PTSD) related keywords to reduce the time spent on the annotation task. Convolutional Neural Network (CNN) learns the representations of the input to identify cancer survivors with PTSD. RESULTS: The results present that the proposed CNN can effectively identify cancer survivors with PTSD. The experiments on real-world datasets show that our model outperforms the baselines and correctly classifies the new tweets. CONCLUSIONS: PTSD is one of the severe anxiety disorders that could affect individuals who are exposed to traumatic events, including cancer. Cancer survivors are at risk of short-term or long-term effects on physical and psycho-social well-being. Therefore, the evaluation and treatment of PTSD are essential parts of cancer survivorship care. It will act as an alarming system by detecting the PTSD presence based on users' postings on Twitter.

RESUMO

The trauma of cancer often leaves survivors with PTSD. Tweets posted on Twitter usually reflect the users' psychological state, which is convenient for data collection. However, Twitter also contains a mix of noisy and genuine tweets. The process of manually identifying genuine tweets is expensive and time-consuming. Thus, we propose a knowledge transfer technique to filter out unrelated tweets. Our experiments show that our model outperforms the baselines.

Social network analysis is an important problem in data mining. A fundamental step for analyzing social networks is to encode network data into low-dimensional representations, i.e., network embeddings, so that the network topology structure and other attribute information can be effectively preserved. Network representation leaning facilitates further applications such as classification, link prediction, anomaly detection, and clustering. In addition, techniques based on deep neural networks have attracted great interests over the past a few years. In this survey, we conduct a comprehensive review of the current literature in network representation learning, utilizing neural network models. First, we introduce the basic models for learning node representations in homogeneous networks. We will also introduce some extensions of the base models, tackling more complex scenarios such as analyzing attributed networks, heterogeneous networks, and dynamic networks. We then introduce techniques for embedding subgraphs and also present the applications of network representation learning. Finally, we discuss some promising research directions for future work.

Online healthcare forums (OHFs) have become increasingly popular for patients to share their health-related experiences. The healthcare-related texts posted in OHFs could help doctors and patients better understand specific diseases and the situations of other patients. To extract the meaning of a post, a commonly used way is to classify the sentences into several predefined categories of different semantics. However, the unstructured form of online posts brings challenges to existing classification algorithms. In addition, though many sophisticated classification models such as deep neural networks may have good predictive power, it is hard to interpret the models and the prediction results, which is, however, critical in healthcare applications. To tackle the challenges above, we propose an effective and interpretable OHF post classification framework. Specifically, we classify sentences into three classes: medication, symptom, and background. Each sentence is projected into an interpretable feature space consisting of labeled sequential patterns, UMLS semantic types, and other heuristic features. A forest-based model is developed for categorizing OHF posts. An interpretation method is also developed, where the decision rules can be explicitly extracted to gain an insight of useful information in texts. Experimental results on real-world OHF data demonstrate the effectiveness of our proposed computational framework.

OBJECTIVES: Prediabetes is a major epidemic and is associated with adverse cardio-cerebrovascular outcomes. Early identification of patients who will develop rapid progression of atherosclerosis could be beneficial for improved risk stratification. In this paper, we investigate important factors impacting the prediction, using several machine learning methods, of rapid progression of carotid intima-media thickness in impaired glucose tolerance (IGT) participants. METHODS: In the Actos Now for Prevention of Diabetes (ACT NOW) study, 382 participants with IGT underwent carotid intima-media thickness (CIMT) ultrasound evaluation at baseline and at 15-18 months, and were divided into rapid progressors (RP, n = 39, 58 ± 17.5 µM change) and non-rapid progressors (NRP, n = 343, 5.8 ± 20 µM change, p < 0.001 versus RP). To deal with complex multi-modal data consisting of demographic, clinical, and laboratory variables, we propose a general data-driven framework to investigate the ACT NOW dataset. In particular, we first employed a Fisher Score-based feature selection method to identify the most effective variables and then proposed a probabilistic Bayes-based learning method for the prediction. Comparison of the methods and factors was conducted using area under the receiver operating characteristic curve (AUC) analyses and Brier score. RESULTS: The experimental results show that the proposed learning methods performed well in identifying or predicting RP. Among the methods, the performance of Naïve Bayes was the best (AUC 0.797, Brier score 0.085) compared to multilayer perceptron (0.729, 0.086) and random forest (0.642, 0.10). The results also show that feature selection has a significant positive impact on the data prediction performance. CONCLUSIONS: By dealing with multi-modal data, the proposed learning methods show effectiveness in predicting prediabetics at risk for rapid atherosclerosis progression. The proposed framework demonstrated utility in outcome prediction in a typical multidimensional clinical dataset with a relatively small number of subjects, extending the potential utility of machine learning approaches beyond extremely large-scale datasets.
