RESUMO

OBJECTIVE: To investigate the effects of different running intensities on interleukin-6 (IL-6) and tumor necrosis factor-α (TNF-α) in patients with early knee osteoarthritis (KOA). STUDY DESIGN: Clinical comparative study. PLACE AND DURATION OF STUDY: Department of Orthopedics, Longyan First Affiliated Hospital of Fujian Medical University, Fujian, China, from January 2019 to January 2021. METHODOLOGY: A total of 131 patients with early KOA admitted to Longyan First Hospital Affiliated to Fujian Medical University from January 2019 to January 2021 were selected and randomly divided into four groups using the random number table method: Group A (static control group, 34 cases), Group B (walking group, 32 cases), Group C (brisk walking group, 32 cases), and Group D (jogging group, 33 cases), and the training duration of the four groups was 6 months. Moreover, the levels of IL-6 and TNF-α, the activity of daily living (ADL), Fugl-Meyer assessment scale (FMA) scores, and adverse events before and after training were compared between the four groups. RESULTS: No statistical significance was observed in the differences of TNF-α, IL-6, ADL-advanced ability, ADL-basic ability, FMA-upper limb, and FMA-lower limb among the four groups before training (p>0.05). After training, the levels of TNF-α and IL-6 of the four groups were decreased, while the scores of FMA and ADL were increased. Group C had the highest FMA-upper limb and FMA-lower limb scores, ADL- advanced ability and ADL-basic ability after training, which were higher than those in groups A, B, and D (p<0.05). No statistical significance could be observed in the difference in the adverse event rate among the four groups during training (X2=3.339, p>0.05). CONCLUSION: Exercise training of different intensities, such as slow walking, brisk walking, and jogging, is touted to enhance the overall recovery of patients with early KOA. The exercise intensity of brisk walking training is moderate, and the activity of daily living and exercise ability of patients are significantly improved after training, while the levels of TNF-α and IL-6 are obviously decreased. OBJECTIVE: To compare the difference in the clinical efficacy on perennial allergic rhinitis between three nasal points acupuncture therapy and the oral administration of loratadine so as to provide the better acupuncture program in clinical treatment. METHODS: Sixty cases were randomized into an acupuncture group (30 cases) and a medication group (30 cases). In the acupuncture group, acupuncture was applied to three nasal points [Yingxiang (LI 20), Yintang (EX-HN 3), Bitong (Extra)] and acupoints selected by syndrome differentiation. Acupuncture was given once every two days, three times a week, for 4 weeks totally. In the medication group, loratadine was prescribed for oral administration, 10 mg every day, for 4 weeks. The symptom and physical sign scores before and after treatment, as well the short-term and long-term efficacy were compared between the two groups. RESULTS: The total effective rate was 96.7% (29/30) in the acupuncture group and was 93.3% (28/30) in the medication group after treatment. The efficacy was similar between the two groups (P>0.05). In follow-up, the total effective rate was 86.7% (26/30) in the acupuncture group, which was better than 56.7% (17/30, P<0.05) in the medication group. The scores of symptoms and physical signs after treatment and in follow-up were all reduced apparently as compared with those before treatment in the patients of the two groups (all P<0.05). The scores of symptoms and physical signs were reduced more apparently in the acupuncture group as compared with those in the medication group in follow-up (all P<0.05). CONCLUSION: The acupuncture at three nasal points and the acupoints selected by syndrome differentiation achieves the similar short-term efficacy on perennial allergic rhinitis as compared with the oral administration of loratadine. The acupuncture therapy presents the obvious advantages on long-term efficacy.

We performed an extensive quantitative proteomic analysis on the pooled medulla sample of the 11-week-old spontaneously hypertensive rats (SHR) compared to age-matched normotensive Wistar rats, using iTRAQ technology coupled with nano two-dimentional liquid chromatography followed by high resolution mass spectrometric abundance indexes techniques. Many differentially expressed proteins identified were involved in energy metabolism, such as mitochondrial part, pyruvate dehydrogenase complex, and respiratory chain. These proteins were included in citrate cycle (TCA cycle), pyruvate metabolism and oxidative phosphorylation. The proteomic analysis and subsequent Western blotting on two independent cohorts of animials indicated that the dysregulation of energy metabolism existed in the medulla of the SHR rats. The differentially expressed proteins in the dysregulation of energy metabolism in the medulla of SHR rats included down-regulated ATP6V1D, ATP6VOA1, ATP5L, DLD proteins and up-regulated AK1 protein. MAO-A protein also exhibited decreased regulation, as well as the other 3 above-mentioned energy-relative proteins (ATP6V1D, ATP5L and DLD proteins) belonging to the heterocycle metabolic process. A receiver operating characteristic curve (ROC) analysis on 4 of the differentially expressed proteins respectively resulted in an area under the receiver operating characteristic curve (AUC) of 0.95, 0.90, 0.92, and 0.81 for differentiating the SHR rats from the normotensive rats. This dysfunction in energy metabolism localizes to the medulla, the lower part of brain stem, and is, therefore, likely to contribute to the development, as well as to pathophysiological complications of hypertension.
