[Tubal adenocarcinoma. Apropos of 4 cases and review of the literature]. / Adénocarcinome tubaire. A propos de quatre cas et revue de la littérature.

The authors describe 4 cases of tubal cancer reported in the gynaecology department of the University Hospital in Caen and review the international literature. If all authors are in agreement on the early symptoms (leukorrhea, haemorrhages, pelvic pain), the lack of diagnosis before the procedure, the lack of information regarding the staging and the grading, as well as the surgical and radiation treatment, there is, above all, controversy regarding the use of chemotherapy, since the low number of patients in the different series does not permit reaching a conclusion.

25 cases of extra uterine pregnancy treated by salpingectomy via laparoscopy, representing 33 p. cent of all ectopic pregnancies reported between September 1985 and 1986. The authors describe the techniques, its advantages and its drawbacks. This technique represents an interesting alternative to classical laparotomy in the treatment of extra uterine pregnancy since it allows: a reduced surgical time and shortened hospital stay, a lesser risk of adhesions, an uneventful postoperative course, a good esthetic result.

[Post-traumatic feto-maternal hemorrhage. Apropos of a case]. / Hémorragie foeto-maternelle post-traumatique. A propos d'un cas.

When a sinusoidal fetal heart rate pattern suggestive of severe anaemia is found one always has to think of feto-maternal haemorrhage. The diagnosis is made using the Kleihauer test. The quantity of fetal blood loss as estimated by this test must be suspect from this case history and from the figures that are quoted in the literature. It will probably be advisable to compare the Kleihauer test and electrophoresis of the haemoglobin in trying to estimate the quantity of blood that has passed from the fetus to the mother. In any case of trauma it is important to look for evidence of feto-maternal haemorrhage.

[Rupture of the pregnant uterine horn following salpingectomy with resection of the interstitial portion. Report of 3 cases of which one followed in-vitro fertilization and embryo transfer]. / Rupture de corne utérine gravide après salpingectomie avec résection de la portion interstitielle. Uterine horn pregnancies are rare among ectopic pregnancies. Total salpingectomy with resection of the interstitial portion of the tube is usually recommended in order to prevent implantation in the interstitial stump. Three personal case histories of rupture of the uterine horn early in pregnancy, which all followed resection of the interstitial portion, have led the authors to discuss the physio-pathological mechanisms that lead to intra-mural implantations. The value of total salpingectomy in preventing these accidents is also considered. One of these three cases particularly interested the authors because it followed in vitro fertilisation and embryo transfer. This case has to be added to the twenty series already published of ectopic pregnancies after this treatment for sterility.

[Diabetes and pregnancy: our experience with the insulin pump]. / Diabète et grossesse: notre expérience de la pompe à insuline.

The author describes a retrospective study on the use of the insulin pump or continuous sub-cutaneous insulin injection in 12 insulin-dependent diabetic women during the last six months of pregnancy. Use was found to be easy and the control of glycemia was significantly better (m = 5.81 mmol/l with continuous subcutaneous insulin injection as opposed to 7.66 before continuous sub-cutaneous insulin injection, p less than 0.01) than with the standard treatment of 3 discontinuous insulin injections. Although not significant, the number of episodes of hyperglycemia was found to be lower. However, no reduction in hypoglycemia was noted. Maternal complications were rare and the children did not present any serious abnormalities. The results encourage the use of the insulin pump with all insulin-dependent diabetics right from the onset of pregnancy.

Antiperinuclear factors (APF), studied in 90 patients suffering from psoriasis, were found in psoriasis without arthritis, in psoriasis with psoriatic arthritis and in psoriasis with true rheumatoid arthritis (RA): respectively 57, 36 and 43 p. 100. Patients with APF did not differ from patients without APF apart form rheumatoid factors detected by indirect immunofluorescence. They were much more frequent in psoriasis with APF. These results are compared with those of RA. They lead to look for other auto-antibodies in psoriasis.
