RESUMO

ABSTRACT: Large language models (LLMs) can support nursing education but pose questions of validity, reliability, and ethical use. This article proposes using the five rights of nursing delegation framework by the National Council of State Boards of Nursing to teach nursing students about the appropriate use of LLMs in health care and nursing education. Nursing faculty can teach students how to assess the validity and reliability of the information provided by LLMs, document its use, and reference and cite information appropriately.

Body donation is a prosocial act providing a unique learning experience to students, ultimately impacting on patient care and science. With an increasing number of training professionals, there is an increasing need for body donors, yet little is understood about donors' beliefs and preferences. A four-center study aimed to understand donors' perceptions, 843 responses highlighted a significant relationship between completing a body donor consent form and being both an organ donor and having ever donated blood (p < 0.01). In exploring donor intentions, 69% had been considering donation for fewer than five years, 40% knew another body donor, and 27% had a family member or friend currently registered. Of those who had requested donor information packs, 97% had completed body donation consent forms. Of these, 92% had not selected any time restriction for their donation and 96% had consented to images being taken. Almost all (98%) were aware that their donation might not be accepted. Donors' motives highlighted a wish to: improve education, improve health care, advance science, and contribute to the greater good. A bimodal response was observed with body donation being used to save relatives money and inconvenience. Donors felt comfortable with their bodies being used by medical, dental, and allied health professionals; however, donors were less comfortable with their bodies being used by other groups. Only 57% were comfortable with artists, beauticians and yoga teachers using their remains. Understanding donors' motivations and decision-making process is vital to ensure resources for future and to meet any changing requirements of both donors and those studying them.

BACKGROUND: Screening rates for chronic kidney disease (CKD) in primary care settings remain low. Although primary care providers are well positioned to offer this testing, there are many barriers that contribute to low screening rates. PURPOSE: The purpose of this study was to examine both practice and contextual factors that relate to screening for CKD in a cohort of primary care patients with hypertension. METHODS: A mixed-methods, single-embedded, convergent parallel design was used for this organizational case study. The Chronic Care Model served as the framework. Electronic medical record data, resource walk-through, and provider surveys were collected from selected primary care clinics within one large academic medical center in the Pacific Northwest. Analyses included regression models, descriptive statistics, narrative content analysis, and pattern matching for organizational case creation. RESULTS: Screening rates were low in the cohort, but patients with more risk factors for CKD were more likely to receive orders for screening. Organizational and community support was deemed necessary for primary care providers to effectively translate guidelines into practice. IMPLICATIONS FOR PRACTICE: The results of this study offer support for a wholistic approach to guideline translation into practice. Participants in the study were open to increased screening for CKD but needed support in multiple ways from the organization. Guidelines alone were not deemed the ideal vehicles for translation of research into practice.

RESUMO

Early diagnosis and management of chronic kidney disease (CKD) in primary care settings remains complex. Nephrology nurses have the opportunity to positively impact this care trajectory. The Chronic Care Model has been applied as a means to improve chronic care management. A scoping review was conducted to identify how elements of the Chronic Care Model have been applied to CKD identification and early management in primary care settings. Elements of the Chronic Care Model were applied across studies and quality improvement projects to various degrees. Nurses, especially those with CKD knowledge, were highlighted as integral parts of successful interventions.

Uncertainty is often presented in the literature as a barrier to chronic kidney disease (CKD) management and end stage renal disease (ESRD) planning by individuals, but the definition of uncertainty has not been clarified for individuals with CKD. After reviewing the literature, a conceptual definition for uncertainty in individuals with CKD was developed. To further clarify this concept, exemplars of individuals with CKD who do and do not exhibit uncertainty are offered. This concept clarification may offer nephrology nurses additional insight into assessing and caring for individuals with CKD.
