RESUMO
We have reported a case of suppurative pylephlebitis in an elderly, debilitated patient. The infection was caused by toothpick perforation of and migration from the gastrointestinal tract. The patient had risk factors for foreign body ingestion, including poor dentition, aphasia caused by a previous cerebrovascular accident, and alcohol abuse. Investigation for the otherwise obscure cause of suppurative hepatic disease should take into account the possible, yet difficult to define, source related to an ingested foreign body.

We report a case of an unusual variant of megalourethra associated with urethral duplication. The surgical technique for an enlarged and massively deformed phallus is described.

Management of obstructive complications in a renal transplant patient are most efficiently performed using endoscopic and percutaneous techniques where possible. A case of such obstruction managed by placement of an internal stent is described. An unusual complication involving the fracture of the stent was also managed percutaneously.

A case of osteogenic differentiation in a prostate gland associated with high-dose x-irradiation for adenocarcinoma is reported. Heterologous cancerous elements in adults are extremely unusual in the prostate and their occurrence after treatment has rarely been documented. The relationship of this lesion to the primary glandular neoplasm is discussed.

Long-term indwelling catheters constitute a risk factor for the development of bladder malignancy. We assessed prospectively by bladder biopsy and urine cytology 25 consecutive spinal cord injury patients catheterized for a minimum of 10 years. Two cases of squamous cell carcinoma of the bladder were identified. Both patients with cancer had positive cytology findings and hematuria, suggesting that routine bladder biopsy in the asymptomatic patient may not be warranted. The high risk of malignancy (8 per cent) in these patients is yet another compelling reason to minimize the use of long-term indwelling catheters.

An IR remote sensing instrument, similar in principle to some of those on terrestrial meteorological satellites, was flown to Venus on board the Pioneer Venus orbiter. Observations of the atmosphere were made from 5 December 1978 until 14 February 1979, during seventy-two orbits of the planet. The optical techniques employed and the design and implementation of the instrument are described.
