RESUMO
PURPOSE: To determine the effectiveness of transconjunctival frontalis suspension in patients with blepharoptosis and poor levator function (eyelid excursion less then 5 mm). METHODS: The medical records of 29 patients (50 ptotic eyelids) were reviewed. RESULTS: Four patients (eight eyelids) had blepharophimosis syndrome, ten patients (27 eyelids) had congenital ptosis, seven patients (14 eyelids) had myogenic ptosis, and one patient (one eyelid) had neurogenic ptosis. Surgical results were good and complications were minimal during follow-up intervals ranging from six months to seven years. CONCLUSIONS: Transconjunctival frontalis suspension is technically simpler than traditional external frontalis suspension and yields satisfactory functional and cosmetic results in patients with poor levator function.

Canalicular lacerations need to be correctly identified before surgical repair. Methods to find the medial cut end are numerous. With the use of direct sight or the bubble test or both, the identification and repair of the medial cut end of the lacerated canaliculus should be achievable in the majority of cases.

A 19-year-old woman developed papillary and pigmentary changes of the skin over her entire body with extensive eyelid involvement causing trichiasis. A four-lid blepharoplasty with rotation of the anterior lid lamella led to resolution of the ocular symptoms and secondary cosmetic improvement. Histologic examination of the tissue demonstrated epidermal nevi which are congenital abnormalities of surface or adnexal epithelium. This patient was diagnosed with systematized epidermal nevus as described by Lever's classification. There is a high association of epidermal nevi with other systemic abnormalities; that association is known as "epidermal nevus syndrome" (ENS).

Many techniques have been described for performing successful dacryocystorhinostomies (DCRs). The patency of the passageway from the internal common punctum through the osteotomy site into the nasal cavity is usually the limiting factor in the success of the DCR. We present a modification using a "coffee can lid" stent to keep the osteotomy site open when there is no free space between the osteotomy and the nasal septum (medial wall of the nose).
