Obesity has long been recognized as a global epidemic. One of the most effective treatments is bariatric surgery. Since the first modern procedure was reported, it has evolved over time, and multiple techniques have emerged. More than 20 years ago, one of the most widely used techniques was the non-adjustable gastric band (NAGB), which showed very promising short-term results. However, over time, it became apparent that it was not as effective in the long term. Associated gastrointestinal symptoms, such as reflux and constant vomiting, along with considerable weight regain, caused this technique to fall out of favor and be replaced by other procedures like the gastric sleeve (GS). Although the technique has fallen into disuse and is no longer recommended in the literature, there are still patients with associated complications. Few recent cases associated with these complications have been reported. Most undergo band removal, and whether to perform another procedure remains with limited evidence. We present the case of a patient who underwent an NAGB procedure 10 years ago and later experienced symptoms (reflux) and weight regain. She successfully underwent band removal and conversion to a GS at our institute in Mexico.

Pattern-Triggered Immunity (PTI) in plants is activated upon recognition by Pattern Recognition Receptors (PRRs) of Damage- and Microbe-Associated Molecular Patterns (DAMPs and MAMPs) from plants or microorganisms, respectively. An increasing number of identified DAMPs/MAMPs are carbohydrates from plant cell walls and microbial extracellular layers, which are perceived by plant PRRs, such as LysM and Leucine Rich Repeat-Malectin (LRR-MAL) receptor kinases (RKs). LysM-RKs (e.g. CERK1, LYK4 and LYK5) are needed for recognition of fungal MAMP chitohexaose (ß-1,4-D-(GlcNAc)6, CHI6), whereas IGP1/CORK1, IGP3 and IGP4 LRR-MAL RKs are required for perception of ß-glucans, like cellotriose (ß-1,4-D-(Glc)3, CEL3) and mixed-linked glucans. We have explored the diversity of carbohydrates perceived by Arabidopsis thaliana seedlings by determining PTI responses upon treatment with different oligosaccharides and polysaccharides. These analyses revealed that plant oligosaccharides from xylans [ß-1,4-D-(xylose)4 (XYL4)], glucuronoxylans and α-1,4-glucans, and polysaccharides from plants and seaweeds activate PTI. Cross-elicitation experiments of XYL4 with other glycans showed that the mechanism of recognition of XYL4 and the DAMP 33-α-L-arabinofuranosyl-xylotetraose (XA3XX) shares some features with that of CEL3 but differs from that of CHI6. Notably, XYL4 and XA3XX perception is impaired in igp1/cork1, igp3 and igp4 mutants, and almost not affected in cerk1 lyk4 lyk5 triple mutant. XYL4 perception is conserved in different plant species since XYL4 pre-treatment triggers enhanced disease resistance in tomato to Pseudomonas syringae pv tomato DC3000 and PTI responses in wheat. These results expand the number of glycans triggering plant immunity and support IGP1/CORK1, IGP3 and IGP4 relevance in Arabidopsis thaliana glycans perception and PTI activation. Significance Statement: The characterization of plant immune mechanisms involved in the perception of carbohydrate-based structures recognized as DAMPs/MAMPs is needed to further understand plant disease resistance modulation. We show here that IGP1/CORK1, IGP3 and IGP4 LRR-MAL RKs are required for the perception of carbohydrate-based DAMPs ß-1,4-D-(xylose)4 (XYL4) and 33-α-L-arabinofuranosyl-xylotetraose (XA3XX), further expanding the function of these LRR-MAL RKs in plant glycan perception and immune activation.

The main objective of this study was to investigate the influence of carbohydrate intake (cyclodextrin) on performance during the performance of two consecutive workouts of the day (WODs) lasting 20 min each. Twenty-one male CrossFit (CF) athletes (29.5 ± 4.3 years; 72.81 ± 12.85 kg; 1.74 ± 0.06 m; 3.41 ± 1.21 years of experiences) participated in a crossover, randomized, and double-blind study. The effect of supplementation with 30 g of cyclodextrin (SG) (Cluster Dextrin®) or placebo (PG) (Bolero Advanced Hydration®) was evaluated on the performance of two specific WOD. Additionally, the effect on handgrip maximum strength, countermovement jump (CMJ), Wingate test, and 1 RM bench press test was evaluated. The effect on blood glucose and lactate was also evaluated. No differences were found in time, height, and power (W/Kg) in CMJ. However, there was a percentage improvement in CMJ jump power (W) (p < 0.05) between the groups, assuming an improvement in performance due to the intervention. Moreover, both conditions experimented differences in execution speed between sets (p < 0.05) in pre-WOD, and differences in post-WOD only in the placebo group, as well as decreases in this variable per repetition across the set (p < 0.01) in both conditions. However, no differences were found in the rest of the variables. Supplementation with 30 g of cyclodextrin did not have any metabolic or performance effects in CF tests. Although some differences between groups were observed in CMJ and power tests for bench press, the data are not conclusive and further research is needed in this regard.

In this paper, I critically assess two recent proposals for an interpretation-independent understanding of non-relativistic quantum mechanics: the overlap strategy (Fraser & Vickers, 2022) and the textbook account (Egg, 2021). My argument has three steps. I first argue that they presume a Quinean-Carnapian meta-ontological framework that yields flat, structureless ontologies. Second, such ontologies are unable to solve the problems that quantum ontologists want to solve. Finally, only structured ontologies are capable of solving the problems that quantum ontologists want to solve. But they require some dose of speculation. In the end, I defend the conservative way to do quantum ontology, which is (and must be) speculative and non-neutral.

Zymoseptoria tritici, the causal agent of Septoria tritici blotch, is one of Europe's most damaging wheat pathogens, causing significant economic losses. Genetic resistance is a common strategy to control the disease, Stb6 being a resistance gene used for more than 100 years in Europe. This study investigates the molecular mechanisms underlying Stb6-mediated resistance. Utilizing confocal microscopy imaging, we determined that Z. tritici epiphytic hyphae mainly accumulate the corresponding avirulence factor AvrStb6 in close proximity to stomata. Consequently, the progression of AvrStb6-expressing avirulent strains is hampered during penetration. The fungal growth inhibition co-occurs with a transcriptional reprogramming in wheat characterized by an induction of immune responses, genes involved in stomatal regulation, and cell wall-related genes. Overall, we shed light on the gene-for-gene resistance mechanisms in the wheat-Z. tritici pathosystem at the cytological and transcriptomic level, and our results highlight that stomatal penetration is a critical process for pathogenicity and resistance. This study combines volcanic gas compositions, SO2 flux and satellite thermal data collected at Nevado del Ruiz between 2018 and 2021. We find the Nevado del Ruiz plume to have exhibited relatively steady, high CO2 compositions (avg. CO2/ST ratios of 5.4 ± 1.9) throughout. Our degassing models support that the CO2/ST ratio variability derives from volatile exsolution from andesitic magma stored in the 1-4 km depth range. Separate ascent of CO2-rich gas bubbles through shallow (< 1 km depth), viscous, conduit resident magma causes the observed excess degassing. We infer that degassing of ~ 974 mm3 of shallow (1-4 km) stored magma has sourced the elevated SO2 degassing recorded during 2018-2021 (average flux ~ 1548 t/d). Of this, only < 1 mm3 of magma have been erupted through dome extrusion, highlighting a large imbalance between erupted and degassed magma. Escalating deep CO2 gas flushing, combined with the disruption of passive degassing, through sudden accumulation and pressurization of bubbles due to lithostatic pressure, may accelerate volcanic unrest and eventually lead to a major eruption.

Objective: Transoral robotic surgery (TORS) has evolved since its 2009 US Food and Drug Administration approval for use in local stage T1-T2 oropharyngeal carcinoma. The ability to resect increasingly larger and more complex lesions has led to the need to introduce reconstructive techniques through this route, avoiding the classic transmandibular or pull-through approach. Few studies have compared the safety, efficacy, and advantages of TORS versus classic open approaches in oropharyngeal salvage surgery with reconstruction using microanastomosed flaps. Here we retrospectively compare our center's experience with the open approach and TORS and describe the technical variations used. Methods: Between 2013 and 2021, 30 stage III-IV oropharyngeal cancer patients underwent salvage surgery with reconstruction in our center. From 2013 to 2017, 15 patients underwent surgery with the classic open approach, and from 2018 to 2021, an additional 15 patients underwent TORS. We have compared surgical outcomes, post-surgical results, and survival in the two groups. Results: Patient characteristics were similar in the two groups. TORS was associated with shorter surgical time (p < .001), fewer complications (p = .01), shorter hospital stay (p < .001), and lower feeding tube requirements (p = .003). No significant differences were observed between the two groups in the free margin rate or survival. Conclusion: Oropharyngeal salvage surgery with TORS with free flap reconstruction reduced associated morbidity compared to the open approach in a patient cohort with poor prognosis. Introducción y objetivo: El manejo del dolor es esencial en el postoperatorio inmediato de los colgajos microquirúrgicos para reconstrucción de mama: Presentamos un estudio de optimización del manejo postoperatorio basado en la introducción de un catéter de bupivacaína subfascial abdominal y la movilización precoz de las pacientes. Material y método: Incluimos los colgajos DIEP y MS-TRAM libres para reconstrucción mamaria llevados a cabo en nuestro Servicio de enero 2013 a diciembre 2019, divididos en 2 grupos según el protocolo de manejo postoperatorio empleado, estándar o de rápida recuperación, para comparar su efectividad. Resultados: Analizamos un total de 186 pacientes: 68 en el grupo estándar y 118 en el grupo de rápida recuperación. La estancia hospitalaria, la necesidad de mórficos de rescate y los vómitos durante el postoperatorio inmediato disminuyeron de forma estadísticamente significativa en el grupo de rápida recuperación (p = 0.013, p = 0.001 y p = 0.012, respectivamente). El inicio de la deambulación fue más precoz en el grupo de rápida recuperación, también de forma estadísticamente significativa (p = 0.01). No objetivamos más complicaciones abdominales asociadas al cambio del protocolo. Conclusiones: En nuestra experiencia, el protocolo de rápida recuperación permite optimizar el manejo del dolor postoperatorio, iniciar una deambulación más temprana y reducir la estancia hospitalaria en la reconstrucción microquirúrgica mamaria con colgajo abdominal. Background and objective: Pain management is essential during the immediate postoperative period of microsurgical flaps for breast reconstruction. The aim of this study is to assess the effectiveness of a postoperative management protocol based on the use of an abdominal bupivacaine catheter and the early mobilization of patients. Methods: Patients underwent free DIEP and MS-TRAM flaps for breast reconstruction in our Service from January 2013 to December 2019 were included and divided into 2 groups according to the postoperative management protocol used, standard versus fast-track, to compare its effectiveness. Results: A total of 186 patients were included: 68 standard group, 118 fast-track group. The length of hospital stays, the intravenous opiate use and vomiting during the immediate postoperative period, were reduced in the fast-track group, and the results were statistically significant (p = 0.013, p =0.001 and p = 0.012, respectively). The initiation of ambulation was earlier in the fast-track group (p = 0.01). No differences in abdominal complications were found between both protocols. Conclusions: In our experience, the fast-track protocol allows to optimize the management of postoperative pain, to initiate an earlier ambulation and to reduce the length of the hospital stay in breast microsurgical reconstruction with abdominal fap. In this paper, we put forward an alternative interpretation of time-reversal symmetry in philosophy of physics: Humean time-reversal symmetry. According to it, time-reversal symmetry is understood as a heuristic, epistemic virtue of the best system, not as a property of the Humean mosaic. One of the consequences of this view is that one of the main arguments against a primitive direction of time is rendered harmless, which paves the way for primitivism about the direction of time.

Social cheating is the exploitation of public goods that are costly metabolites, like exoproteases. Exoprotease exploitation in Pseudomonas aeruginosa has been studied in reference strains. Experimental evolution with reference strains during continuous growth in casein has demonstrated that nonexoprotease producers that are lasR mutants are selected while they behave as social cheaters. However, noncanonical quorum-sensing systems exist in P. aeruginosa strains, which are diverse. In this work, the exploitation of exoproteases in the environmental strain ID4365 was evaluated; ID4365 has a nonsense mutation that precludes expression of LasR. ID4365 produces exoproteases under the control of RhlR, and harbors an inducible prophage. As expected, rhlR mutants of ID4365 behave as social cheaters, and exoprotease-deficient individuals accumulate upon continuous growth in casein. Moreover, in all continuous cultures, population collapses occur. However, this also sometimes happens before cheaters dominate. Interestingly, during growth in casein, ID4565's native prophage is induced, suggesting that the metabolic costs imposed by social cheating may increase its induction, promoting population collapses. Accordingly, lysogenization of the PAO1 lasR mutant with this prophage accelerated its collapse. These findings highlight the influence of temperate phages in social cheating.

Symmetry-based inferences have permeated many discussions in philosophy of physics and metaphysics of science. It is claimed that symmetries in our physical theories would allow us to draw metaphysical conclusions about the world, a view that I call 'symmetry inferentialism'. This paper is critical to this view. I claim that (a) it assumes a philosophically questionable characterization of the relevant validity domain of physical symmetries, and (b) it overlooks a distinction between two opposing ways through which relevant physical symmetries become established. My conclusion is that symmetry inferentialism loses persuasive force when these two points are taken into consideration.

INTRODUCTION: An important challenge in the secondary prevention of cardiovascular diseases is the optimization of risk factors (RFs) after hospital discharge. These have been shown to be insufficiently controlled in clinical practice. AIM: To evaluate whether secondary prevention goals were met at our institution at 12 months after an acute coronary syndrome (ACS) index event, as well as analyzing if achieving these goals was associated with a lower incidence of major adverse cardiovascular events (MACE). METHODS: Retrospective cohort of patients with a former diagnosis of ACS over a period of 4 years. To evaluate the proportion of patients who met RF control goals at 12 months after the index event, we used two sets of preestablished goals: stringent and lenient. During follow-up we evaluated the occurrence of MACE, defined by the following: ACS, coronary revascularization, stroke, hospitalization because of acute heart failure and cardiovascular death. RESULTS: We included 468 patients during the study period. The mean age of the patients was 60 ± 10.76 years, 20.5% were women, and mean follow-up was 41.8 ± 22.0 months. The proportion of patients that met all secondary prevention stringent and lenient goals accounted for 5.5% and 17.2%, respectively, and 8% did not achieve any target. Overall, 9.6% presented the composite of MACE during follow-up. The number of RFs in control at 12 months was associated with a lower rate of MACE, both with stringent and lenient goals. CONCLUSION: Achieving established goals for modifiable RFs can lower the incidence of MACE during long-term follow-up.

In this paper I argue that the concept of time-reversal invariance in physics suffers from metaphysical underdetermination, that is, that the concept may be understood differently depending on one's metaphysics about time, laws, and a theory's basic properties. This metaphysical under-determinacy also affects subsidiary debates in philosophy of physics that rely on the concept of time-reversal invariance, paradigmatically the problem of the arrow of time. I bring up three cases that, I believe, fairly illustrate my point. I conclude, on the one hand, that any formal representation of time reversal should be explicit about the metaphysical assumptions of the concept that it intends to represent; on the other, that philosophical arguments that rely on time reversal to argue against a direction of time require additional premises.

A proposed low-cost, portable, 32-channel (4 rings of 8-channel) Electrical Impedance Tomography (EIT) system based on the AFE4300 analog front-end for body composition measurement. Each ring allows obtaining the conductivity distribution of 4 cross sections, 4 cm apart; to analyze the behavior of conductivity in a volume. The switching of the 4 rings and the current injection and voltage measurement patterns are done with three Texas Instruments 74HC4067 multiplexers, which are managed by an ESP32 board. The proposed system has an average signal-to-noise ratio of 74.71 dB and a frame rate of 50 fps. The sensitivity tests to impedance and volume changes consisted of introducing 4 tubes of different diameters (2 steel and 2 polyvinyl chloride) into a tank with saline solution; then conductivity distribution images were generated in 4 cross-sections of the tank, using the algorithms Gauss-Newton and Noser. Finally, the global impedance index (GI) is calculated to estimate the volume of each tube inside the tank. The results show that the proposed system is highly sensitive to impedance and volume changes, being a promising system for monitoring tissues, and fluids biological.

Background: Bacteriophage therapy is becoming part of mainstream Western medicine since antibiotics of clinical use tend to fail. It involves applying lytic bacteriophages that self-replicate and induce cell lysis, thus killing their hosts. Nevertheless, bacterial killing promotes the selection of resistant clones which sometimes may exhibit a decrease in bacterial virulence or antibiotic resistance. Methods: In this work, we studied the Pseudomonas aeruginosa lytic phage φDCL-PA6 and its variant φDCL-PA6α. Additionally, we characterized and evaluated the production of virulence factors and the virulence in a Galleria mellonella model of resistant mutants against each phage for PA14 and two clinical strains. Results: Phage φDCL-PA6α differs from the original by only two amino acids: one in the baseplate wedge subunit and another in the tail fiber protein. According to genomic data and cross-resistance experiments, these changes may promote the change of the phage receptor from the O-antigen to the core lipopolysaccharide. Interestingly, the host range of the two phages differs as determined against the Pseudomonas aeruginosa reference strains PA14 and PAO1 and against nine multidrug-resistant isolates from ventilator associated pneumonia. Conclusions: We show as well that phage resistance impacts virulence factor production. Specifically, phage resistance led to decreased biofilm formation, swarming, and type III secretion; therefore, the virulence towards Galleria mellonella was dramatically attenuated. Furthermore, antibiotic resistance decreased for one clinical strain. Our study highlights important potential advantages of phage therapy's evolutionary impact that may be exploited to generate robust therapy schemes.

Objetivo principal: Describir la actividad y carga asistencial de una unidad de monitorización de intermedios respiratorios (UMIR) en el contexto de pandemia por Covid-19, así como características de los pacientes ingresados en ella. Metodología: Estudio prospectivo y observacional entre enero y diciembre de 2021. Se evaluaron características del paciente, características de su ingreso y la carga asistencial medida a través de la escala TISS-28. Resultados principales: Ingresaron 127 pacientes. La edad media fue de 69,52 ± 11,61 años, y la gran mayoría eran hombres. Causa principal de ingreso fue insuficiencia respiratoria aguda. La puntuación media de la escala TISS-28 fue de 21,59 ± 2,37. Conclusión principal: La UMIR demostró ser un buen recuso hospitalario para responder a las necesidades de la pandemia por Covid-19, así como reducir la estancia media en otros servicios más especializados. Se debe tener en cuenta la elevada carga asistencial existente.(AU)

Methods: Prospective and observational study between January and December 2021. Characteristics of the patient, characteristics of their admission and the care burden measured through the TISS-28 scale were evaluated. Results: 127 patients were admitted. The mean age was 69.52 ± 11.61 years, and the vast majority were men. Main cause of admission was acute respiratory failure. The mean score on the TISS-28 scale was 21.59 ± 2.37. Conclusion: The UMIR proved to be a good hospital resource to respond to the needs of the Covid-19 pandemic, as well as reduce the average stay in other more specialized services. The existing high care burden must be taken into account.(AU)

Objetivo: conocer la percepción que tenían los pacientes con diagnóstico de COVID-19 sobre su experiencia vivida durante su estancia en una Unidad de Monitorización de Intermedios Respiratoria (UMIR), tras un ingreso previo en la Unidad de Cuidados Intensivos (UCI).Método: estudio cualitativo de enfoque fenomenológico realizado entre enero y junio de 2021. La muestra estaba constituida por pacientes mayores de edad ingresados durante al menos seis días en la UMIR por COVID-19, procedentes de la UCI, del Complexo Hospitalario de Ourense (España). Se llevaron a cabo entrevistas semiestructuradas individuales según un guion preestablecido. Las conversaciones fueron grabadas en audio, con el consentimiento de los participantes, y luego fueron transcritas haciendo un análisis temático. Este estudio fue aprobado por el Comité Ético de Investigación Clínica de Galicia.Resultados: del análisis de los datos surgieron seis categorías temáticas: recuerdos previos UMIR, estancia UMIR, factores motivacionales, afrontamiento del proceso, futuro y familia. A pesar de que las secuelas de la COVID-19 son todavía muy desconocidas, todos los sujetos hacen una valoración positiva de su progreso durante el tiempo en la unidad y se sienten con fortaleza para lograr una recuperación completa.Conclusiones: los pacientes se sentían más seguros y protegidos en la UMIR que en una habitación convencional, siendo conscientes de que estaban monitorizados y vigilados. Además, se sentían tranquilos y aliviados tras abandonar la UCI, una vez superada la situación crítica de la enfermedad.(AU)


Objective: to understand the perception of patients diagnosed with COVID-19 of their experience during their stay at an Intermediate Respiratory Monitoring Unit (IRMU), after previous admission at the Intensive Care Unit (ICU).Method: a qualitative study with phenomenological approach conducted between January and June 2021. The sample was formed by patients of age admitted during at least six days at the IRMU due to COVID-19, and transferred from the ICU of the Complexo Hospitalario de Ourense (Spain). Individual semistructured interviews were conducted, following a pre-established script. The conversations were audio recorded, with consent by participants, and then transcribed with thematic analysis. This study was approved by the Clinical Research Ethics Committee of Galicia.Results: six thematic categories appeared from data analysis: memories previous to IRMU, stay at the IRMU, motivating factors, process coping, future, and family. Although the consequences of COVID-19 are still largely unknown, all subjects made a positive assessment of their progress during their stay at the unit, and were feeling strong enough to achieve complete recovery.Conclusions: patients felt safer and more protected at the IRMU than in a traditional room, with the awareness of being monitored and watched. Besides, they felt calm and relieved after leaving the Intensive Care Unit once the critical stage of the disease had been overcome.(AU)

Two views on the direction of time can be distinguished-primitivism and non-primitivism. According to the former, time's direction is an in-built, fundamental property of the physical world. According to the latter, time's direction is a derivative property of a fundamentally directionless reality. In the literature, non-primitivism has been widely supported since most (if not all) our fundamental dynamical laws are time-reversal invariant. In this paper, I offer a way out to the primitivist. I argue that we do have good grounds to support a primitive direction of time in the quantum realm. The rationale depends on exploiting the metaphysical and dynamical underdetermination of quantum theories to make a case in favor of primitivism. In particular, primitivism can be grounded in spontaneous collapse theories (e.g., GRW and CSL). The specific sense in which these theories capture a primitive direction of time is that, when the ontology of the theory is seriously taken into account, it does not remain invariant under time reversal. In taking GRW with a matter-density field (GRWm), I will argue that primitivism about the direction of time can be defended in the quantum case.

The prefoldin complex (PFDc) was identified in humans as a co-chaperone of the cytosolic chaperonin T-COMPLEX PROTEIN RING COMPLEX (TRiC)/CHAPERONIN CONTAINING TCP-1 (CCT). PFDc is conserved in eukaryotes and is composed of subunits PFD1-6, and PFDc-TRiC/CCT folds actin and tubulins. PFDs also participate in a wide range of cellular processes, both in the cytoplasm and in the nucleus, and their malfunction causes developmental alterations and disease in animals and altered growth and environmental responses in yeast and plants. Genetic analyses in yeast indicate that not all of their functions require the canonical complex. The lack of systematic genetic analyses in plants and animals, however, makes it difficult to discern whether PFDs participate in a process as the canonical complex or in alternative configurations, which is necessary to understand their mode of action. To tackle this question, and on the premise that the canonical complex cannot be formed if one subunit is missing, we generated an Arabidopsis (Arabidopsis thaliana) mutant deficient in the six PFDs and compared various growth and environmental responses with those of the individual mutants. In this way, we demonstrate that the PFDc is required for seed germination, to delay flowering, or to respond to high salt stress or low temperature, whereas at least two PFDs redundantly attenuate the response to osmotic stress. A coexpression analysis of differentially expressed genes in the sextuple mutant identified several transcription factors, including ABA INSENSITIVE 5 (ABI5) and PHYTOCHROME-INTERACTING FACTOR 4, acting downstream of PFDs. Furthermore, the transcriptomic analysis allowed assigning additional roles for PFDs, for instance, in response to higher temperature.
