RESUMO

To assess the pharmacotherapy of elderly patients with chronic heart failure (CHF), by studying the effect of perindopril, lozartan potassium and their combination on the severity of clinical symptoms, hemodynamics, exercise capacity and heart morphofunctional parameters, 78 patients with chronic heart failure II-IV FC (for classification NYHA), complicating the course of coronary heart disease (CHD) with left ventricular ejection fraction ≤45 % were examined. Men were 42 (53,8 %), women - 36 (46,2 %), mean age 64,8±3,7 years. The study revealed that the background as long-term monotherapy with perindopril and lozartan potassium in patients with CHF complicating CHD, CHF FC decreased significantly and significantly increased left ventricular ejection fraction (p < 0,05). The combined treatment with perindopril and lozartan potassium showed no advantage over monotherapy with these agents. These patients showed a decrease in the frequency of angina attacks by 28,5 and 27 % respectively (p < 0,05) only after a long course of treatment with perindopril and the combination of perindopril and lozartan potassium.

RESUMO

This pharmacoepidemiological study at the regional level permitted to reveal the real strategy of medical prescriptions and the choice of medications for the treatment of bronchial asthma and chronic obstructive respiratory disease. Regional variations in the combination of preparations actually used by patients with these diseases are investigated for the first time. The main causes of non-compliance with medical prescriptions are analysed.

The study was designed to estimate quantitative and qualitative characteristics of drug consumption for therapy of bronchial asthma and chronic obstructive pulmonary disease in outpatient and inpatient facilities. The frequency and intensity of the use of selected drugs are considered in terms of their conformity to principles of evidence-based medicine. Analysis of prescriptions revealed different degree of awareness of district physicians and other specialists about rational drug therapy. The drawbacks of the treatment of bronchial asthma and chronic obstructive pulmonary disease are first and foremost attributable to poor compliance with the existing standards and inefficient use of medications. It was shown that each outpatient and inpatient facility has its own policy of fund allocation for pharmacotherapy. The results of the study can be useful for rational organization of the treatment of patients with bronchial asthma and chronic obstructive pulmonary disease and for optimization of medical aid provision with a view to saving healthcare resources.

RESUMO

A pharmacoepidemiological study of drug consumption was performed on 352 patients with III to IV NYHA functional class chronic heart failure (CHF) in different hospitals of Kursk region. The study found that only 51% of expenses related to pharmacological providence of CHF patients were associated with the drugs used directly for the treatment of CHF. Undesirable drug combinations are used more often in city hospitals and hospitals belonging to certain organizations than in the regional clinical hospital.
