The data suggesting the enterosorbent influence on the activity of proteolysis inhibitors and arginase, on the fibrin-fibrinogen and fibronectin decay products content, blood serum toxin fixating activity of injured with the burns were presented. The dependence of expression of mentioned indexes changes from the trauma severity in acute burn disease period was shown. The enterosgel application promotes the lowering of dystrophic degree severity, alike the hepar and fixed macrophage system load, the investigated indexes normalization acceleration, and patients condition improvement.

The effectiveness of the use of a stabilized composition of alpha 2-macroglobulin concentrate obtained from fraction III of plasma proteins according to Cohn in burn disease in the experiment on guinea pigs was studied. Under the influence of intraabdominal administration for 4 days of alpha 2-macroglobulin concentrate with a stabilizer, healing of the burn wounds, normalization of a state of the animals occurred 2 times more rapidly than in control ones, activity of acid proteinases restored by day 5 (in control--by day 14) due to increase in activity of proteolytic enzyme inhibitors.

In severely burned children, depending on severity of injury, the anti-triptic activity of blood serum, alpha 2-macroglobulin level, fibronectin, activity of an arginase decrease at the acute period of toxemia and septic toxemia. The content of the products of fibrin-fibrinogen degradation increases considerably. Alteration of the indices studied is of informative value and is associated with dynamics of the toxic syndrome development. Use of donor antiproteic plasma has a positive therapeutic effect.

In combined use of general and local sorption detoxication in 30 patients with severe burns, the pronounced antiinflammatory action is caused by redistribution of neutrophilic granulocytes with different activity of myeloperoxidase systems, decrease in lesion of a vascular wall and development of the perfect cellular reactions into extravascular phase of inflammation. The area of deep burn wounds reduced 2.8-fold when compared with that in patients, in whom no sorption methods were used.

In patients with pathological scars, especially in children, the activity of proteolytic enzymes (prekallikrein, acid and neutral proteinases) is increased. Summary inhibitory activity and content of the alpha 2-macroglobulin in the serum and scar tissue is also increased. This is indicative of continued inflammation.

The development of inflammatory reaction in 106 patients with severe and extremely severe burns in combined use in the complex treatment of extracorporeal hemosorption, enterosorption, applicative sorption by the powdery PMS-Zn sorbent, passive immunotherapy was studied. The modulative effect of the methods of general and local sorptive detoxication on the development of inflammatory reaction of the area of thermal trauma with reduction of damaging effect of proteolytic enzymes and formation of cellular reaction directed at protection from microorganisms and beginning of the proliferative stage of reparative regeneration is shown. Lethality in severe burns was 17.7%, in extremely severe--50.8%.

The applicative-sorptive method of treatment was used by the authors in 57 patients ranging in age from 18 to 60 years with II-III AB degree burns of 10 to 50% of the body surface. The decrease in activity of proteolytic enzymes in the wound discharge, the increase in the activity of microbicidal systems of neutrophilic leukocytes, and formation of a phagocytic type of local resistance of an organism were noted. The epithelization of II-III A degree burn wounds accelerated, and the area of deep burn wounds reduced in patients.

The use of hemosorption detoxication at early terms after burn trauma against the background of the intensive infusion-transfusion treatment is an effective method of treatment in the acute period of burn disease. In the total it shortens the period of treatment of critically burned patients.

Hydrolysis of various substrates by proteolytic enzymes from human blood leukocytes was studied. Among the protein substrates used protamine was hydrolyzed at the highest rate. Among the synthetic substrates--N-benzoyl-D,L-arginine ethyl ester was the one hydrolyzed most rapidly. Effect of native inhibitors of proteinases and of bivalent metal ions on the proteolytic activity in leukocytes was also studied. The highest proteolytic activity, estimated by hydrolysis of protamine, was found in lysosomal and microsomal fractions.

The nature of protamine splitting proteinase, which was formed after treatment of human blood plasma by kaolin or silicone (aerosile), was studied. The activated enzyme did not exhibite the properties of thrombin and plasmin in reactions with specific substrates. Kyninogenic, TAME (N-tosyl-d-,l-arginine methyl ester)-esterase and protamine splitting activities were inherent in the proteinase; these properties enabled to group the enzyme with blood kallikreins. The purified preparations of human blood plasma kallikrein hydrolyzed protamine sulphate even at 10 mcg/ml concentration. alpha2-macroglobulin inhibited the protamine splitting activity of the plasma kallikrein.
