RESUMO

Online pharmacy services and online ordering of pharmaceutical products are in demand, especially in the context of the COVID-19 pandemic. The problems of the purchase of pharmacy products through online orders in the Far Eastern Federal District were analyzed. Sociological survey of the population (n = 3789) was carried out. Spearman's Rank correlation coefficient was used. Drugs, dietary supplements, hygiene, and care items are most often purchased in online pharmacies. There is a correlation with the female sex, which may be related to care about family/loved people. The purchase of prescription drugs through e-pharmacies is also more often in demand among women and older patients. Among prescription drugs, the order of drugs used in the treatment of coronavirus infection and its complications (antibiotics, nonsteroidal anti-inflammatory, gastroenterological, and hormonal) is widespread. The problems of e-pharmacies identified due to the study are the lack of proper control over prescription drugs, low role of the pharmaceutical worker in compliance of the patient, and frequent refusals of Internet orders. It is necessary clarify the regulatory status of e-pharmacies and the role of pharmaceutical specialists in it, to reduce the possibility of error in the preparation and/or assembly of orders, to reduce the risk of the presence of defective goods. These ways can increase customer loyalty to this service.

RESUMO

BACKGROUND: Diseases characterized by elevated blood pressure are the most frequently diagnosed pathology among socially significant diseases in the Russian Federation. According to clinical recommendations of the Russian Medical Society for Arterial Hypertension 2010-2019, the use of two and more component drugs improves the compliance of patients to treatment. The purpose of this study was a comparative analysis of preferences in the usage of combined antihypertensive drugs in ten regions of the Far Eastern Federal District (FSD). MATERIALS AND METHODS: Intra-group analysis of data on the sale of antihypertensive drugs in pharmacy organizations (162 drugstores) for the period 2018 was carried out in the 10 regions of the FSD. The sample is uneven due to the difference in total number of drugstores in the regions. The sample was random. Pharmacies were in urban and rural areas. The total purchase of all defined daily doses of all antihypertensive drugs was taken as 100%. Then, fixed-dose combinations were selected. The share of each drug in the sales structure was then calculated. Kruskal-Wallis criterion was used for determining the differences in the structure of acquisition of drugs between the regions. The Spearman's rank correlation and factor analysis were used for determining the correlation between the drug's acquisition structure and the FSD region. RESULTS: Two-component drugs are most in demand in the Amur region - 15.2% and Zabaykalsky Krai - 12.3%, and the lowest demand in the Sakhalin region - 5.25% and Magadan region - 6.5%. The largest sales indicators of three-component drugs were observed in the Zabaykalsky Krai - 1.2% and the Republic of Buryatia - 1.0%. The most acquired combinations were atenolol+chlorthalidone, valsartan+amlodipine, losartan+hydrochlorothiazide, perindopril/lisinopril/ramipril+amlodipine, perindopril+indapamide. CONCLUSION: Group leaders may vary depending on the region, which is due to the preferences of medical specialists. However, the differences are not significant, which proves the unity of approaches to combined drug therapy of arterial hypertension.

RESUMO

Objective: The purpose of this study is to develop and validate the psychometric properties of a scale for measuring the quality of patient medication counseling by using the Rasch model. Methods: In this study, the scale was developed based on the literature review. It consisted of 31 items across five subscales: introduction, problem identification, content, behavior, and conclusion. A convenient sample of community pharmacists was recruited from four major cities in Vietnam: Hanoi, Da Nang, Ho Chi Minh, and Can Tho. Data collection was conducted from June 10 to October 30, 2017. A Rasch analysis for polytomous data was performed to assess the suitability of the item and the reliability of the scale. Results: The research results showed that all items had a positive point-measure correlation coefficient between 0.47 and 0.77. All items had infit and outfit values in the optimal range between 0.5 and 1.5 except for D5, but its value was within acceptable range. Differential item function analysis indicated that all items had no DIF, except for items B4 and E4 containing moderate magnitude of DIF. Response category statistics found that there was a gradual increase in difficulty level from category 1 to 5 and no presence of reversal. Infit and outfit statistics of these categories were also considered good, with their values close to 1. The test result of the item characteristic curve and the person-item map showed that there were some overlapping items. Their appearance, however, might play an important role in measuring different aspects of construct. The overall scale reliability index (0.97) was high and the overall scale separation index (6.11) was good. Conclusions: The developed scale satisfied the requirements of the Rasch model. The scale is a useful tool that could be used to measure the quality of patient medication counseling among community pharmacists
