RESUMO

Adaptor chimeric antigen receptor (CAR) T-cell therapy offers solutions for improved safety and antigen escape, which represent main obstacles for the clinical translation of CAR T-cell therapy in myeloid malignancies. The adaptor CAR T-cell platform 'UniCAR' is currently under early clinical investigation. Recently, the first proof of concept of a well-tolerated, rapidly switchable, CD123-directed UniCAR T-cell product treating patients with acute myeloid leukaemia (AML) was reported. Relapsed and refractory AML is prone to high plasticity under therapy pressure targeting one single tumour antigen. Thus, targeting of multiple tumour antigens seems to be required to achieve durable anti-tumour responses, underlining the need to further design alternative AML-specific target modules (TM) for the UniCAR platform. We here present the preclinical development of a novel FMS-like tyrosine kinase 3 (FLT3)-directed UniCAR T-cell therapy, which is highly effective for in vitro killing of both AML cell lines and primary AML samples. Furthermore, we show in vivo functionality in a murine xenograft model. PET analyses further demonstrate a short serum half-life of FLT3 TMs, which will enable a rapid on/off switch of UniCAR T cells. Overall, the presented preclinical data encourage the further development and clinical translation of FLT3-specific UniCAR T cells for the therapy of AML.

PURPOSE: Simultaneous pancreas-kidney transplantation (SPKT) remains the best treatment option in patients with type 1 diabetes and chronic kidney failure. There are only a few studies addressing the potential ischemic deterioration of peripheral arterial disease (PAD) due to blood diverting from the iliac artery to the kidney graft. We aimed to evaluate diabetic foot lesions and PAD evolution in SPKT recipients and investigate if they are more frequent in ipsilateral lower limb of kidney graft. METHODS: We developed a retrospective cohort, including patients submitted to SPKT in our tertiary center, between 2000 and 2017. Diabetic foot lesions and PAD frequencies were compared in the period before and after transplantation. RESULTS: Two hundred and eleven patients were included, 50.2% (n = 106) female, with a median age at transplantation of 35 years (IQR 9). After a median follow-up period of 10 years (IQR 7), patient, kidney, and pancreatic graft survival were 90.5% (n = 191), 83.4% (n = 176), and 74.9% (n = 158), respectively. Before transplant, 2.8% (n = 6) had PAD and 5.3% (n = 11) had history of foot lesions. In post-transplant period, 17.1% (n = 36) patients presented PAD and 25.6% (n = 54) developed diabetic foot ulcers, 47.6% (n = 35) of which in the ipsilateral and 53.3% (n = 40) in the contralateral lower limb of the kidney graft (p = 0.48). Nine patients (4.3%) underwent major lower limb amputation, 3 (30%) ipsilateral and 7 (70%) contralateral to the kidney graft (p = 0.29). CONCLUSIONS: Diabetic foot lesions were not more frequent in the ipsilateral lower limb of the kidney graft, therefore downgrading the 'steal syndrome' role in these patients.

The aim of this systematic review with meta-analysis was to determine the effect of caffeine gum (Caff-gum) on exercise performance-related outcomes. Several databases were searched for studies assessing the effect of Caff-gum in placebo-controlled protocols involving healthy adults. Random-effects meta-analyses using standardized mean differences (SMD) were performed to determine the effect of Caff-gum on exercise outcomes with several sub-analyses (training status, exercise type, timing and dose) for potential modifying factors. 14 studies were included, totalling 200 participants. There was a significant overall effect of Caff-gum compared to placebo (SMD = 0.21, 95%CI: 0.10-0.32; p = 0.001). Subgroup analysis showed improved performance for trained (SMD = 0.23, 95%CI: 0.08-0.37; p = 0.004), but not for untrained (SMD = 0.14, 95%CI: -0.02-0.29; p = 0.07) individuals. Caff-gum improved both endurance (SMD = 0.27, 95%CI: 0.12-0.42; p = 0.002) and strength/power (SMD = 0.20, 95%CI: 0.03-0.37; p = 0.03) performance outcomes. Caff-gum was ergogenic when consumed within 15 min prior to initiating exercise (SMD = 0.27, 95%CI: 0.07-0.4; p = 0.01), but not when provided >15 min prior (SMD = -0.48, 95%CI = -1.7-0.82; p = 0.25). There was no significant effect of Caff-gum with doses <3 mg/kg body mass (BM) (SMD = 0.20, 95%CI: -0.03-0.43; p = 0.07), but there was a significant effect when the dose was ≥3 mg/kg BM (SMD = 0.22, 95%CI: 0.07-0.37; p = 0.01). Caff-gum supplementation may be an effective ergogenic strategy for trained athletes involved in both endurance and strength/power exercise, using a recommended dose of ≥3 mg/kg BM consumed within 15 min of initiating exercise.HighlightsThis study determined the effect of Caff-gum on exercise performance, using a systematic review and meta-analysis. Fourteen studies, totalling 200 participants performing a variety of endurance and strength/power exercise tests were included. The relative Caff-gum dose ranged from 1.27-4.26 mg/kg BM and timing ranged from 120 min prior to exercise up to intra-test application.Caff-gum was shown to be an effective ergogenic aid for trained individuals involved in both endurance and strength/power exercise.Supplement dose and timing modified the efficacy of Caff-gum. Supplementation with Caff-gum was effective when provided in doses ≥3 mg/kg BM and within 15 min prior to initiating exercise.Trained endurance or strength/power athletes seeking to benefit from caffeine in the form of chewing gum should supplement within 15 min prior to initiating an exercise task, in doses ≥3 mg/kg BM.

The Neotropical freshwater cladoceran Bergamina lineolata (Sars, 1901) was found in a small temporal pond in the Magdalena department. Hitherto, it has been reported in Brazil and El Salvador. It was originally described as Alonella lineolata by Sars, 1901 from Brazil and then placed to the genus Bergamina by Elmoor-Loureiro et al. (2013). This is the first record of this species in Colombia. B. lineolata can be identified by a unique combination of characters including: 1) a remarkably large and oblong postabdomen, with three denticles on distal corner; 2) basal spine of the claw very short, length less than the half claw diameter at base; 3) IDL with two setae shorter than ODL seta, armed with fine setules unilaterally in terminal half; 4) endite 1 of trunk limb I with a long smooth seta between endites 1 and 2.

Hemotherapy services play a key role in attracting donors and providing safe blood to the population. The apher-esis platelet collection procedure is a relatively simple, safe and important procedure for increasing the stocks of these services. However, the recruitment and retention of these donors still represent a major challenge. Objec-tive: Evaluating the profile of donors of blood components by apheresis in the Transfusion Unit of Professor Alberto Antunes University Hospital - UFAL, as well as knowing the hematological parameters pre- and post-donation, the occurrence of the main adverse events related to the procedure and the difficulties faced by the donor. Method:This was a cross-sectional observational study. We analyzed a total of 160 forms of apheresis donors from March 2017 to June 2018. The data were tabulated using the Excel program, and then analyzed in order to determine the objectives. Results: Most donors were male (93.13%), aged between 25 and 40 years (48.75%) and brown (25.62%). There was a slight prevalence of singles (49.37%) and 73.75% were from Maceió. The most prevalent ABO and Rh phenotyping was O+ (39.3%). Most of the procedures were simple platelet collection (75.60%) and the occurrence of adverse events during donations was 30.63%. Conclusion: Evaluation of apheresis donor profile and the knowledge of the possible side effects related to the procedure provided a better understanding of this type of donation and may improve the capture and retention processes of these individuals, minimizing the effects of lack of blood for Alagoana population. Os serviços de hemoterapia desempenham um papel fundamental na atração de doadores e no fornecimento de sangue seguro à população. No entanto, o recrutamento e a retenção desses doadores ainda representam um gran-de desafio. Objetivo: Avaliar o perfil dos doadores de hemocomponentes por aférese na Unidade de Transfusão do Hospital Universitário Professor Alberto Antunes - UFAL, bem como conhecer os parâmetros hematológicos pré e pós-doação, a ocorrência dos principais eventos adversos relacionados à procedimento e as dificuldades enfrenta-das pelo doador. Método: Estudo observacional transversal. Foram analisadas 160 fichas de doadores de aférese de março de 2017 a junho de 2018. Os dados foram tabulados no programa Excel e analisados para determinar os objetivos. Resultados: A maioria dos doadores era do sexo masculino (93,13%), com idade entre 25 e 40 anos (48,75%) e parda (25,62%). Houve uma leve prevalência de solteiros (49,37%) e 73,75% eram de Maceió. A feno-tipagem ABO e Rh mais prevalente foi O+ (39,3%). A maioria dos procedimentos foi de coleta simples de plaquetas (75,60%) e a ocorrência de eventos adversos durante as doações foi de 30,63%. Conclusão: A avaliação do perfil do doador de aférese e o conhecimento dos possíveis efeitos colaterais relacionados ao procedimento proporcio-naram uma melhor compreensão sobre esse tipo de doação e podem ajudar a melhorar os processos de captura e retenção desses indivíduos, minimizando os efeitos da falta de sangue para a população Alagoana.(AU)

• Herein we propose a framework for assembling and analyzing Genotype by Sequencing (GBS) data to better understand evolutionary relationships within a group of closely related species using the mastiff bats (Molossus) as our model system. Many species within this genus have low-levels of genetic variation within and between morphologically distinct species, and the relationships among them remain unresolved using traditional Sanger sequencing methods. Given that both de novo and reference genome pipelines can be used to assemble next generation sequences, and that several tree inference methodologies have been proposed for single nucleotide polymorphism (SNP) data, we test whether different alignments and phylogenetic approaches produce similar results. We also examined how the process of SNP identification and mapping can affect the consistency of the analyses. Different alignments and phylogenetic inferences produced consistent results, supporting the GBS approach for answering evolutionary questions on a macroevolutionary scale when the genetic distance among phenotypically identifiable clades is low. We highlight the importance of exploring the relationships among groups using different assembly assumptions and also distinct phylogenetic inference methods, particularly when addressing phylogenetic questions in genetic and morphologically conservative taxa. • The method uses the comparison of several filter settings, alignments, and tree inference approaches on Genotype by Sequencing data. • Consistent results were found among several approaches. • The methodology successfully recovered well supported species boundaries and phylogenetic relationships among species of mastiff bats not hypothesized by previous methods.

Macrophyte stands are a major contributor for this complexity due to their differential architectures. Nevertheless, the influence of distinct aquatic habitat architectures (with different types of macrophytes or without them) on microcrustaceans' taxa composition, usually found in macrophyte colonized water bodies, is underexplored in limnological studies. The main objective of this study was to analyze this influence by comparing the Cladocera composition among four habitat architectures: (1) fluctuant macrophytes, (2) rooted emergent macrophytes, (3) submerged macrophytes and (4) the limnetic zone of oxbow lakes associated to a large subtropical reservoir. Wide compositional variation was observed. Fluctuant macrophytes exhibited the richest Cladocera assemblage, dominated by Chydoridae. Submerged and rooted emergent macrophytes had the most similar assemblages between them. The most distinctive fauna was found in the limnetic zone, dominated by Bosminidae. Probable differences in resource availability in each sampled habitat architecture are considered as the driving factor for the Cladocera composition variation. We concluded that for a complete inventory of a given local fauna, it is imperative to take into account the aquatic habitat architecture, including macrophyte stands, in the data sampling design.

Resumo A complexidade ambiental é considerada um fator chave para o aumento na diversidade de ambientes aquáticos. A presença de bancos de macrófitas é um dos principais contribuintes para tal complexidade devido às suas arquiteturas diferenciadas. Entretanto, a influência de diferentes arquiteturas de habitat (com diferentes macrófitas ou sem elas) sobre a composição taxonômica de microcrustáceos, comumente encontrados em ambientes colonizados por macrófitas, é pouco explorada em estudos limnológicos. O objetivo desse estudo foi avaliar esta influência através da comparação da composição das associações de Cladocera entre quatro arquiteturas de habitat: (1) macrófitas flutuantes, (2) macrófitas enraizadas emersas, (3) macrófitas submersas e (4) a zona limnética em lagoas laterais associadas a um grande reservatório subtropical. Ampla variação composicional foi observada. Macrófitas flutuantes apresentaram as mais ricas assembléias de Cladocera, dominadas por Chydoridae. Macrófitas submersas e emersas enraizadas apresentaram as associações mais similares. A fauna mais distinta foi a da zona limnética, dominada por Bosminidae. Prováveis diferenças na disponibilidade de recursos entre as arquiteturas de habitat amostradas foram consideradas o fator mais relevante levando à variação composicional dos microcrustáceos. Concluímos que para um inventário completo de uma determinada fauna local é imperativo que seja considerada a arquitetura dos habitats aquáticos, incluindo os bancos de macrófitas, no desenho amostral.

The objective of this study was to determine the prevalence of antimicrobial resistance (AMR) exhibited by enterococci isolated from faeces of pets and its underlying risk factors. From September 2009 to May 2012, rectal swabs were collected from 74 dogs and 17 cats, selected from the population of animals visiting the Veterinary Hospital of University of Porto, UPVet, through a systematic random procedure. Animal owners answered a questionnaire about the risk factors that could influence the presence of AMR in faecal enterococci. Enterococci isolation, identification and antimicrobial (AM) susceptibility testing were performed. Data analyses of multilevel, univariable and multivariable generalised linear mixed models were conducted. From all enterococci isolated (n=315), 61 per cent were considered multidrug-resistant, whereas only 9.2 per cent were susceptible to all AMs tested. Highest resistance was found to tetracycline (67.0 per cent), rifampicin (60.3 per cent), azithromycin (58.4 per cent), quinupristin/dalfopristin (54.0 per cent) and erythromycin (53.0 per cent). Previous fluoroquinolone treatments and coprophagic habits were the features more consistently associated with the presence of AMR for three (chloramphenicol, ciprofloxacin and azithromycin) and seven (tetracycline, rifampicin, gentamicin, chloramphenicol, ciprofloxacin, erythromycin and azithromycin), respectively, out of nine AMs assessed. Evaluating risk factors that determine the presence of drug-resistant bacteria in pets, a possible source of resistance determinants to human beings, is crucial for the selection of appropriate treatment guidelines by veterinary practitioners.

An increased number of aortic aneurysms are expected to be diagnosed in chronic renal failure patients awaiting kidney transplantation because of its strong association with atherosclerosis. The development of endovascular aortic aneurysm repair in the 1990s, which is characterized by low morbidity as well as shorter operative time and length of hospital stay, is particularly suited to these patients who are at high risk for conventional surgery. Herein we describe a case of a 58-year-old, man who was diagnosed with a 5.5-cm aortic aneurysm in 2001 and treated with a Gore Excluder (WL Gore and Assoc, Flagstaff, Ariz) aorto-iliac stent-graft. In 2004, he underwent uneventful deceased donor kidney transplantation in the right iliac fossa. With a follow-up of 6 and 9 years, respectively, there is no evidence of an endoleak. The renal function remains normal. This promising new surgical technique has been the subject of only a few published cases. At our center, endovascular aortic aneurysm repair is the first-intention treatment for these patients, as long as it is possible to preserve the iliac arteries for the renal anastomoses and the anatomic conditions are suitable for stent-graft deployment.

RESUMO

Aortic aneurysm, chronic renal failure, and kidney transplantation are common clinical entities that rarely coexist. The increased number of kidney transplant patients, their greater survival, and the sharing of risk factors for aortic aneurysms, suggest the emergence of a larger number of diagnoses of aortic aneurysms among kidney transplant patients. These patients cases present the particularities of immunosuppression that can alter the healing of the wounds or the development of infections. There is a need to protect the kidney during aortic clamping and several co-morbidities that increase the surgical morbidity and mortality. The advent in 1991 of the endovascular aortic aneurysm repair (EVAR), with less physiologic aggression, brought a new therapeutic perspective that minimizes these problems. Our experience with renal transplantation began in 1987 and with EVAR in 2001. During this period we had the opportunity to treat aortic aneurysms in three kidney transplanted patients using EVAR.

La mayoría de los niños infectados con el virus de la inmunodeficiencia humana presentan, dentro de los primeros signos de la enfermedad, manifestaciones orales. Algunas de estas lesiones orales tienen valor pronóstico de cara a la evolución de la infección y la aparición de SIDA.El propósito de este trabajo es determinar el estado de salud oral de los niños uruguayos portadores del VIH que asisten al Centro Nacional de Referencia Obstétrico-Pediátrico del Centro Hospitalario Pereira Rossell. Fueron examinados 76 niños con edades comprendidas entre 1 y 17 años, con una media de 7,5 años. Los resultados obtenidos muestran un índice ceo de 4,52±6,2 para el grupo de dentición caduca, 4,42±3,35 en el de dentición mixta. El índice CPO fue de 1,28±1,72 en el grupo de dentición mixta y 2,8±2,4 en el grupo de dentición permanente. Un 51,8% de la población estudiada presentó algún tipo de maloclusión, mientras un 75% presentaba gingivitis. La prevalencia de candidiasis alcanzó el 16% de la población, 8% de lesiones herpéticas, 11,6% de hipertrofia parotídea, 2,4% de leucoplasia vellosa y un caso de GUNA (0,7%).(AU)
