RESUMO

Providing patient centered care is a crucial element of high quality care. It can be defined as a responsive way of caring for and empowering patients, embodying compassion, empathy, and responsiveness to the patient's needs. The aim of this study was to assess the potential of using EHRs as information source in the development of tools for assessing PCC. An annotation guide following the Person-centred Practice Framework proposed by McCance and McCormack was developed for the purpose of this study. Twenty patients' documents were manually annotated, resulting in 539 expressions. All dimensions of the framework were covered in the documents, with 61.3% of expressions describing the activity of engaging authentically with the patient. The results of this study indicate that electronic health records are one potential source of information in automated evaluation of patient centered care, however more information is still needed on how to interpret this information.

Introducción: para las universidades públicas, uno de los indicadores fundamentales es consolidar los grupos de investigación en Cuerpos Académicos (CA). Sin embargo, no existe una guía que apoye la transición. El propósito es exponer algunas recomendaciones para conformar un CA con éxito, con base en las experiencias de trabajo del Cuerpo Académico Intervenciones para el Cuidado de la Salud. Desarrollo: los CA están conformados por docentes-investigadores de tiempo completo que comparten y desarrollan una línea de investigación en temas disciplinares o multidisciplinares, enfocados en objetivos académicos comunes. La reflexión que plantea este manuscrito contribuye a identificar las fortalezas y áreas de oportunidad de los grupos de investigación, así mismo se describen algunas estrategias para transitar y consolidar un CA: 1. Selección de los integrantes, 2. Línea de investigación, 3. Proyectos, 4. Servicio a la sociedad, 5. Trabajo colegiado, 6. Redes de investigación, 7. Integración de estudiantes y 8. Liderazgo transformacional. Conclusión: la creación de CA es fundamental en el desarrollo de las instituciones de educación superior en México, para alcanzar este reto es necesario integrar estrategias innovador... (AU)


Abstract Introduction: For public universities, a fundamental indicator is to consolidate research groups in Academic Bodies (AB). However, there is no guide to support the transition. Purpose: to present some recommendations to form AB successfully based on the work experiences of the Academic Body Interventions for Health Care. Development: The academic bodies are made up of full-time professors-researchers who share and develop a line of research on disciplinary or multidisciplinary topics and which entail a set of common academic objectives and goals. The reflection proposed by this manuscript contributes to identifying the strengths and areas of opportunity of the research groups, as well as describing some strategies to consolidate as an academic body: 1. The selection of the members, 2. The line of research, 3. Projects, 4. Service to society, 5. Collegiate work, 6. Research networks, 7. Integration of students and 8. Transformational leadership. Conclusion: Creating Academic Bodies is essential in the development of higher education institutions in Mexico, to meet this challenge it is necessary to integrate innovative strategies to generate and disseminate knowledge and the disposition of the members for collegiate work.

OBJECTIVE: To evaluate the representation of environmental concepts associated with health impacts in standardized clinical terminologies. METHODS: This study used a descriptive approach with methods informed by a procedural framework for standardized clinical terminology mapping. The United Nations Global Indicator Framework for the Sustainable Development Goals and Targets was used as the source document for concept extraction. The target terminologies were the Systematized Nomenclature of Medicine Clinical Terms (SNOMED CT) and the International Classification for Nursing Practice (ICNP). Manual and automated mapping methods were utilized. The lists of candidate matches were reviewed and iterated until a final mapping match list was achieved. RESULTS: A total of 119 concepts with 133 mapping matches were added to the final SNOMED CT list. Fifty-three (39.8%) were direct matches, 37 (27.8%) were narrower than matches, 35 (26.3%) were broader than matches, and 8 (6%) had no matches. A total of 26 concepts with 27 matches were added to the final ICNP list. Eight (29.6%) were direct matches, 4 (14.8%) were narrower than, 7 (25.9%) were broader than, and 8 (29.6%) were no matches. CONCLUSION: Following this evaluation, both strengths and gaps were identified. Gaps in terminology representation included concepts related to cost expenditures, affordability, community engagement, water, air and sanitation. The inclusion of these concepts is necessary to advance the clinical reporting of these environmental and sustainability indicators. As environmental concepts encoded in standardized terminologies expand, additional insights into data and health conditions, research, education, and policy-level decision-making will be identified.

Objetivo: Describir las fases de un Ensayo Clínico Pragmático como una metodología viable para la transformación de políticas de salud. Desarrollo: El Ensayo Clínico Pragmático (ECP) no solo considera escenarios clínicos, también implica contextos reales como las comunidades y los hogares. También implica gestión administrativa, factibilidad cultural, estudio socioeconómico y propuesta política para mejorar los servicios de salud. Las fases de un ECP son: (1) Descripción del fenómeno, (2) Estudio de factibilidad, (3) Implementación, (4) Utilidad clínica, (5) Política pública y (6) Evaluación. Consideraciones finales: La contribución de las intervenciones de enfermería a la sociedad (derivado de la evidencia científica), podría ser considerada por los actores políticos, quienes pueden valorar la validez externa de un ECP, porque los datos son traducibles y utilizables en la vida real a más bajo costo y con inversiones menos costosas.(AU)


Objective: To describe the phases of a Pragmatic Clinical Trial (PCT) as a methodology for the transformation of health policies. Development: The Pragmatic Clinical Trial (PCT) not only considers clinical scenarios, it involves visits in real contexts such as communities and homes. It also implies administrative management, cultural feasibility, socioeconomic study and political proposal to improve health services. The phases of a PCT are: (1) Description of the phenomenon, (2) Feasibility study, (3) Implementation, (4) Clinical utility, (5) Public policy and (6) Evaluation. Final considerations: The contribution of nursing interventions to society (derived from scientific evidence) could be considered by political actors, who can assess the external validity of a PCT, because the data is translatable and usable in real life, at a lower cost and with less expensive interventions.(AU)

OBJECTIVE: Integration of environmentally sustainable digital health interventions requires robust evaluation of their carbon emission life-cycle before implementation in healthcare. This scoping review surveys the evidence on available environmental assessment frameworks, methods, and tools to evaluate the carbon footprint of digital health interventions for environmentally sustainable healthcare. MATERIALS AND METHODS: Medline (Ovid), Embase (Ovid). PsycINFO (Ovid), CINAHL, Web of Science, Scopus (which indexes IEEE Xplore, Springer Lecture Notes in Computer Science and ACM databases), Compendex, and Inspec databases were searched with no time or language constraints. The Systematic Reviews and Meta-analyses Extension for Scoping Reviews (PRISMA_SCR), Joanna Briggs Scoping Review Framework, and template for intervention description and replication (TiDiER) checklist were used to structure and report the findings. RESULTS: From 3299 studies screened, data was extracted from 13 full-text studies. No standardised methods or validated tools were identified to systematically determine the environmental sustainability of a digital health intervention over its full life-cycle from conception to realisation. Most studies (n = 8) adapted publicly available carbon calculators to estimate telehealth travel-related emissions. Others adapted these tools to examine the environmental impact of electronic health records (n = 2), e-prescriptions and e-referrals (n = 1), and robotic surgery (n = 1). One study explored optimising the information system electricity consumption of telemedicine. No validated systems-based approach to evaluation and validation of digital health interventions could be identified. CONCLUSION: There is a need to develop standardised, validated methods and tools for healthcare environments to assist stakeholders to make informed decisions about reduction of carbon emissions from digital health interventions.

RESUMO

Nurses need to have sufficient competencies in nursing informatics to be able to provide safe and efficient care. The Self-Assessment of Nursing Informatics Competencies Scale (SANICS) has been developed and validated as a self-report measure of informatics competencies in Western settings. In this work, we describe the ongoing study that aims to validate and translate SANICS into the Mexican setting.

RESUMO

This follow-up survey on trends in Nursing Informatics (NI) was conducted by the International Medical Informatics Association (IMIA) Student and Emerging Professionals (SEP) group as a cross-sectional study in 2019. There were 455 responses from 24 countries. Based on the findings NI research is evolving rapidly. Current ten most common trends include: clinical quality measures, clinical decision support, big data, artificial intelligence, care coordination, education and competencies, patient safety, mobile health, description of nursing practices and evaluation of patient outcomes. The findings help support the efforts to efficiently use resources in the promotion of health care activities, to support the development of informatics education and to grow NI as a profession.

Diabetic foot affects nearly 6% of people living with Type 2 diabetes (T2D). It is also the most common complication of T2D and is associated with infection, ulceration. eHealth facilitation of Non-communicable diseases management has the potential to increase self-care and engage patients. The aim of this study is to develop an eHealth intervention to prevent diabetic foot.

RESUMO

Type 2 Diabetes (T2D) has been increasing in prevalence over the last decade in Mexico. The glycemic control actions can help to prevent complications. The aimed is to pilot an educational intervention in self- management of adults with T2D through using text messages. Findings show the respondents reported a low level of self-care skills of T2D and there was an increase in the self-care skills after it.

RESUMO

La Covid-19 ha causado millones de muertes, México es el cuarto país con mayor índice de defunciones a nivel mundial. Dicho fenómeno se abordó desde la perspectiva teórica del Modelo de Adaptación de Roy. Objetivo: describir los niveles de estrés agudo en el personal de enfermería de primera línea expuesto y recuperado de Covid-19 un hospital hibrido de segundo nivel del Estado de Puebla, México. Metodología: descriptivo, comparativo, transversal. Muestra (n=126) dividida en dos grupos; personal de enfermería expuesto a Covid-19 (n=68) y personal de enfermería recuperado de Covid-19 (n=58). Resultados principales: el Personal expuesto presenta un menor nivel de estrés agudo en comparación con el personal recuperado (50.78±17.43 vs. 54.88±20.25 respectivamente), sin embargo, no hubo diferencia estadísticamente significativa (t=1.22; p>.05). Conclusión principal: Los puntajes de estrés agudo en la mayoría del personal muestran una respuesta de adaptación ineficaz. (AU)


Covid-19 has caused millions of deaths; Mexico is the fourth country with a higher index of deaths worldwide. Such phenomenon was approached from the theoretical perspective of Roy's Adaptation Model. Objective: To describe acute stress levels of the first line nursing staff both exposed or recovered from Covid-19, at a hybrid hospital in the State of Puebla, Mexico. Methodology. Descriptive, comparative, transversal. Sample (n - 126) divided in two groups: nursing staff exposed to Covid-19 (n =68) and nursing staff recovered from Covid-19 (n= 58). Results: Nursing staff exposed to Covid-19 showed less acute stress levels compared to nursing staff recovered from Covid-19 (50.78±17.43 vs. 54.88±20.25), however, there was no statistically significant difference (t= 1.22; p> .05). Conclusion: The acute stress level scores of most staff show an ineffective adaptation response. (AU)

Los modelos teóricos que explican las conductas de salud son diversos, pero no se tiene un modelo ecléctico que explique cómo es que se adquieren los estilos de vida saludables para ejercicio y nutrición desde el punto de vista cognitivo. Es aquí donde cobra importancia la interacción de la Teoría del procesamiento humano de información (PHI) y la Teoría conexionista y facilitación semántica que deriven en Auto-descripción para conductas saludables representado por el autocuidado. La manera en que se adquiere conocimiento que guía las conductas de salud, parte de una base sensorial bajo el postulado principal del empirismo, todos los estímulos que vienen del medio externo a través de los sentidos se procesan en el cerebro otorgando sentido y significado mediante una interacción simbólica que propicia reaccionar en consecuencia. Pero ¿Cómo se les da significado a estos estímulos del exterior? ¿Cómo es que se conectan estos estímulos y en consecuencia se opera una conducta que afecta a la salud, como las acciones de autocuidado? El propósito de este ensayo es presentar la propuesta de un modelo teórico de interacción simbólica como base de la red semántica que las personas utilizan para el cuidado de su salud, así mismo comprender cómo es que desde el punto de vista cognitivo, la auto-descripción de una persona es fundamental para realizar o no acciones de autocuidado para la salud


The theoretical models that explain health behaviors are diverse, however, there is not an eclectic model that explains how healthy lifestyles for exercise and nutrition are acquired from the cognitive point of view. It is here that the interaction of The Human Information Processing Theory (PHI) and The Connectionist Theory and Semantic Facilitation lead to self-description and healthy behaviors represented by self-care. The way in which knowledge is acquired that guides health behaviors, part of a sensory base under the main postulate of empiricism, all the stimuli that come from the external environment through the senses are processed in the brain giving meaning through a symbolic interaction that favors reacting accordingly. The purpose of this article is to present a proposal of a theoretical model of symbolic interaction as the basis of the semantic network that people use for their health care, as well as to understand how, from the cognitive point of view, self-description of a person is essential to perform or not self-care actions for health

The curriculum associated with nursing informatics (NI) education is not standardized, therefore the perspectives of new and emerging nurse informaticians is important. How these curricula differences affect career opportunities of new nursing informaticians, and in turn influenced current career choices will be explored. Synthesizing opinions with themes extracted from a 2014 international study-Advancing nursi informatics in the next decade: Recommendations from an international survey will be summarized.
