BACKGROUND: Bovine Genital Campylobacteriosis (BGC), a worldwide distributed venereal disease caused by Campylobacter fetus subsp. venerealis (Cfv), has a relevant negative economic impact in cattle herds. The control of BGC is hampered by the inexistence of globally available effective vaccines. The present in silico study aimed to develop a multi-epitope vaccine candidate against Cfv through reverse vaccinology. RESULTS: The analysis of Cfv strain NCTC 10354 proteome allowed the identification of 9 proteins suitable for vaccine development. From these, an outer membrane protein, OmpA, and a flagellar protein, FliK, were selected for prediction of B-cell and T-cell epitopes. The top-ranked epitopes conservancy was assessed in 31 Cfv strains. The selected epitopes were integrated to form a multi-epitope fragment of 241 amino acids, which included 2 epitopes from OmpA and 13 epitopes from FliK linked by GPGPG linkers and connected to the cholera toxin subunit B by an EAAAK linker. The vaccine candidate was predicted to be antigenic, non-toxic, non-allergenic, and soluble upon overexpression. The protein structure was predicted and optimized, and the sequence was successfully cloned in silico into a plasmid vector. Additionally, immunological simulations demonstrated the vaccine candidate's ability to stimulate an immune response. CONCLUSIONS: This study developed a novel vaccine candidate suitable for further in vitro and in vivo experimental validation, which may become a useful tool for the control of BGC.

This study aimed to evaluate the timing and frequency of spontaneous apneas during breathing and swallowing by using cineradiography on mouse models of early/initial or late/advanced Parkinson's disease (PD). C57BL/6 J mice received either 6-OHDA or vehicle injections into their right striatum, followed by respiratory movement recordings during spontaneous breathing and swallowing, and a stress challenge, two weeks later. Experimental group animals showed a significantly lower respiratory rate (158.66 ± 32.88 breaths/minute in late PD, 173.16 ± 25.19 in early PD versus 185.27 ± 25.36 in controls; p<0.001) and a significantly higher frequency of apneas (median 1 apnea/minute in both groups versus 0 in controls; p<0.001). Other changes included reduced food intake and the absence of swallow apneas in experimental mice. 6-OHDA-induced nigrostriatal degeneration in mice disrupted respiratory control, swallowing, stress responsiveness, and feeding behaviors, potentially hindering airway protection and elevating the risk of aspiration.

PURPOSE: To describe the use of work process modelling to optimise the organisation of the demand for oral health treatment in primary care units in Brazil. MATERIALS AND METHODS: The oral health care routine was at first described as the "AS IS" model, which was evaluated by the oral team professionals, rearranged, and further described as the "TO BE" model described using a business process management modelling tool. The significant increase in the demand of patients due to restrictions offered by the dental service in addition to non-urgent treatments being avoided by patients during COVID-19 pandemic was also considered. RESULTS: Structuring the work processes in a visual way using modelling tools was useful to picture the entire treatment process and adjust when needed. The use of the managerial tool was useful to understand and reorganise the workflow of organising the demand and ultimately improve the efficiency of the resources. The use of such managerial tools helped oral health professionals to efficiently rearrange their tasks and set priorities to meet their needs. CONCLUSIONS: With the use of management tools, each unit can readjust its structures and ways of working, aiming to improve the quality of public health care services provided to patients.

Objetivo: Analisar o impacto orçamentário da adoção de dispositivos contraceptivos reversíveis de longa duração em uma operadora de plano de saúde localizada no Sul do Brasil. Especificamente, analisamos a incorporação do implante subdérmico de etonogestrel (Implanon®) como alternativa ao sistema intrauterino de levonorgestrel (DIU Mirena® ou DIU Kyleena®), ao longo de um período de 15 anos. Métodos: Realizamos uma análise do impacto orçamentário incremental, considerando a inclusão gradual do implante subdérmico de etonogestrel. Foram considerados dados de uma operadora de planos de saúde com mais de 600.000 beneficiários. O horizonte temporal de 15 anos permitiu uma avaliação abrangente dos efeitos financeiros. Resultados: Identificamos 5.345 pacientes elegíveis para a utilização de contraceptivos reversíveis de longa duração. No cenário em que somente o sistema intrauterino de levonorgestrel era adotado, projetou-se um impacto orçamentário total de R$ 746.379.857,80 ao longo de 15 anos. No cenário alternativo, com a incorporação gradual do implante subdérmico, o impacto orçamentário total foi calculado em R$ 689.800.196,83. Isso resultou em um impacto orçamentário incremental negativo de -R$ 56.579.660,97 ao longo do período. Conclusão: A análise de impacto orçamentário realizada indica um potencial benefício financeiro ao adotar o implante subdérmico de etonogestrel como alternativa ao sistema intrauterino de levonorgestrel para contracepção. Esse achado sugere possíveis reduções de custos na área de saúde suplementar no Brasil, reforçando a importância de avaliar opções economicamente viáveis.


Objective: To analyze the budgetary impact of the adoption of long-acting reversible contraceptive devices in a health plan operator located in southern Brazil. Specifically, we analyzed the incorporation of the etonogestrel subdermal implant (Implanon®) as an alternative to the levonorgestrel intrauterine system (Mirena® IUD or Kyleena® IUD), over a period of 15 years. Methods: We performed an analysis of the incremental budgetary impact, considering the gradual inclusion of the etonogestrel subdermal implant. Data from a health plan operator with more than 600,000 beneficiaries were considered. The 15-year time horizon allowed for a comprehensive assessment of the financial effects. Results: We identified 5,345 patients eligible for the use of long-acting reversible contraceptives. In the scenario where only the levonorgestrel intrauterine system was adopted, a total budget impact of BRL 746,379,857.80 was projected over 15 years. In the alternative scenario, with the gradual incorporation of the subdermal implant, the total budgetary impact was calculated at BRL 689,800,196.83. This resulted in a negative incremental budgetary impact of -R$56,579,660.97 over the period. Conclusion: The budget impact analysis carried out indicates a potential financial benefit in adopting the etonogestrel subdermal implant as an alternative to the levonorgestrel intrauterine system for contraception. This finding suggests possible cost reductions in the supplementary healthcare area in Brazil, reinforcing the importance of evaluating economically viable options.

OBJECTIVE: To evaluate survival and direct medical costs of patients admitted in private hospitals with COVID-19 during the first wave. METHODS: A retrospective, observational study analyzing survival and the economic data retrieved on hospitalized patients with COVID-19. Data from March 2020 to December 2020. The direct cost of hospitalization was estimated using the microcosting method with each individual hospitalization. RESULTS: 342 cases were evaluated. Median age of 61.0 (95% CI 57.0‒65.0). 194 (56.7%) were men. The mortality rate was higher in the female sex (p = 0.0037), ICU (p < 0.001), mechanical ventilation (p<0.001) and elderly groups. 143 (41.8%) patients were admitted to the ICU (95% CI 36.6%-47.1%), of which 60 (41.9%) required MV (95% CI 34.0%-50.0%). Global LOS presented median of 6.7 days (95% CI 6.0-7.2). Mean costs were US$ 7,060,00 (95% CI 5,300.94-8,819,00) for each patient. Mean cost for patients discharged alive and patients deceased was US$ 5,475.53 (95% CI 3,692.91-7,258.14) and US$ 12,955.19 (95% CI 8,106.61-17,803.76), respectively (p < 0.001). CONCLUSIONS: Patients admitted with COVID-19 in these private hospitals point to great economic impact, mainly in the elderly and high-risk patients. It is key to better understand such costs in order to be prepared to make wise decisions during the current and future global health emergencies.

Resumo A fim de analisar a produção científica sobre bioética e psiquiatria no Brasil publicada nos últimos dez anos, realizou-se revisão integrativa da literatura disponível em bases de dados, utilizando descritores e operadores de pesquisa para relacionar os termos "ética or bioética or ética médica and psiquiatria". O processo de busca dos estudos culminou em seleção de treze artigos, dos quais apenas dois foram elegíveis para a revisão integrativa de acordo com os critérios de inclusão. A pesquisa mostrou que apenas os temas da espiritualidade e da pedofilia foram abordados nos estudos analisados, ambos com metodologia de revisão de literatura. Apesar de os campos de bioética e psiquiatria terem grande relevância na contemporaneidade, há escassez de trabalhos científicos no Brasil sobre o tema, pouco explorado e incentivado.


Abstract Our integrative literature review sought to analyze the scientific production on bioethics and psychiatry published in Brazil in the last ten years. Bibliographic search used descriptors and search operators to relates the terms " ética or bioética or ética médica and psiquiatria, " resulting in thirteen articles selected, of which only two were eligible for review according to the inclusion criteria. Results show that only spirituality and pedophilia were themes addressed by the analyzed studies, both literature reviews. Despite the contemporary relevancy of bioethics and psychiatry, research on these themes is little explored and encouraged in Brazil.


Resumen Con el fin de analizar la producción científica sobre bioética y psiquiatría en Brasil publicada en los últimos diez años, se realizó una revisión integradora de la literatura disponible en bases de datos, utilizando descriptores y operadores de investigación para relacionar los términos "ética or bioética or ética médica and psiquiatría". La búsqueda culminó con la selección de trece artículos, de los cuales solo dos fueron elegibles para la revisión integradora según los criterios de inclusión. Los resultados mostraron que solo se abordaron los temas de espiritualidad y pedofilia en los estudios analizados, ambos siguieron la metodología de revisión de literatura. Aunque la bioética y la psiquiatría tienen gran relevancia en la contemporaneidad, los estudios científicos en Brasil sobre el tema son escasos, poco explorados y fomentados.

There are many challenges in teaching veterinary anatomy, such as available classroom time, costs, and difficulties accessing animal cadavers, mainly due to animal welfare concerns. Furthermore, veterinary surgeons and radiologists complain that recent graduates lack anatomical knowledge. On the other hand, the current limitations of face-to-face teaching due to the COVID-19 pandemic suggest that the development of online distance education tools is necessary, mainly in specialties that lack this type of material. Teaching platforms promoting the integration of anatomy with other applied disciplines such as imaging and surgery in the horse were not found in the consulted literature. Therefore, this work aimed to develop an online distance education platform for studying the surgical anatomy of a horse's locomotor apparatus as a complementary tool for training students enrolled in undergraduate courses in veterinary surgery. The locomotor apparatus was chosen as the focus as it is the most commonly found in equine surgeries. Anatomical pieces referring to the locomotor apparatus were prepared. These were complemented with material related to diagnostic imaging, surgery videos, theoretical explanations, and an interactive radiological anatomy tool. Finally, all the material was uploaded to a virtual platform accessible via the Internet. The platform is expected to be a tool that helps students in surgical training and prepares them with a better understanding of anatomy and its application in surgery.

Objetivo: compreender as percepções de profissionais da Estratégia Saúde da Família (ESF) que atuam em territórios rurais de municípios do Nordeste brasileiro acerca das inter-relações entre a saúde e ambiente no processo saúde-doença-cuidado da população.Método: pesquisa de abordagem qualitativa, em que se realizaram 29 entrevistas de sete categorias profissionais. Adotou-se a análise temática para o processamento e interpretação do material empírico. Resultados: foram discutidas a necessidade de informação acerca de questões ambientais e seu impacto no processo saúde-doença e a consciência ambiental na dimensão do território. Há compreensão do ambiente como determinante de saúde, porém sob uma perspectiva embrionária, além de uma articulação insuficiente entre determinação social e práticas emancipatórias no contexto de saúde e ambiente. Conclusões: ressalta-se a importância de fortalecer as práticas na ESF a partir dos pressupostos explicitados na Política Nacional de Saúde Integral da População de Campo Florestas e Águas. Reforça-se a necessidade de se desenvolver processos formativos, envolvendo a equipe para contribuir na transformação da realidade em saúde da população de campo e das águas.


Objective: To understand the perceptions of Family Health Strategy (ESF, in Portuguese) professionals working in rural territories in municipalities of the Brazilian Northeast on the interrelationships between health and the environment in the health-disease-care process of the population. Method: qualitative research, in which 29 interviews in seven professional categories were carried out. Thematic analysis was adopted for the processing and interpretation of the empirical material. Results: it was discussed the need for information about environmental issues and their impact on the health-disease process, as well as environmental awareness in the dimension of the territory. From an embryonic perspective, there is an understanding of the environment as a determinant of health, with insufficient articulation between social determinants and emancipatory practices in the context of health and the environment. Conclusion: We emphasize the importance of strengthening ESF practices based on the assumptions outlined in the National Policy for the Integral Health of the Populations of the Field, Forest, and Water. We reinforce the need to develop training processes involving the team to contribute to the transformation of the health reality of the countryside and water populations.

BACKGROUND: Non-human primates are the closest animal models to humans regarding genetics, physiology and behaviour. Marmoset monkeys in particular are one of the most versatile species for biomedical research. OBJECTIVE: To assess the craniofacial growth and development of the masticatory function in the common marmoset (Callithrix jacchus), from birth to the fourth month of life through minimally invasive cineradiographic imaging. METHODS: Ten individuals were followed-up from 0 to 4 months of age regarding craniofacial growth and masticatory function assessed by cineradiography. For the experimental procedure, we used a microfocal X-ray source apparatus and a beryllium fast-response image-intensifier. RESULTS: The duration of the masticatory cycles was stable across age groups. Chewing a very soft Castella cake or the slightly harder Marshmallow did not change the masticatory cycle in the time domain. On the other hand, linear and angular measurements of the jaw-opening movement showed a tendency for bigger movements at the latter stages of craniofacial growth. Qualitative analysis showed that marmosets had a small preference for Castella over Marshmallow, that they most often bit off pieces of food to chew with their posterior teeth, that they manipulated the food with their hands, and that they chewed the food continuously. CONCLUSION: We observed critical developmental events during the first 3 months of life in marmosets. Cineradiographic imaging in marmosets may provide valuable information on craniofacial form and function for basic and preclinical research models.

RESUMO Este estudo caracteriza condições de vida e situação de saúde das famílias que vivem da agricultura familiar e da pesca artesanal no sertão cearense. Trata-se de pesquisa descritiva quantitativa e delineamento transversal realizada mediante entrevistas com 152 agricultores(as) familiares e/ou pescadores(as) artesanais em Novo Oriente, Ceará, Brasil. A análise estatística foi realizada com frequência absoluta e relativa, média aritmética e desvio-padrão. Observaram-se algumas vulnerabilidades vivenciadas pelas famílias: 19,5% (n=29) com deficiência no tratamento da água para consumo, 25,7% (n=39) relataram existência de problemas ambientais, 26,6% (n=40) apresentando insatisfatória produção familiar, 34,6% (n=47) com baixa renda familiar, 75% (n=114) com inexistência na localidade de escolas e 88,2% (n=134) de creches, 51,3% (n=78) com carência de opções de lazer e 37,5% (n=57) em insegurança alimentar. Na análise da situação de saúde, destaca-se prevalência de doenças crônicas, como cardiovasculares e mentais. Essa realidade é contraditória com as metas propostas pelos Objetivos de Desenvolvimento Sustentável: fome zero; boa saúde e bem-estar; educação de qualidade; água limpa e saneamento; emprego digno e crescimento econômico; redução das desigualdades. Dessa forma, as ações de saúde não podem se desvincular dessas iniquidades, devendo-se buscar uma articulação entre governos, empresas e cidadãos para alcançar as metas da Agenda 2030.


ABSTRACT This study characterizes the living conditions and health situation of families who live from family farming and artisanal fishing in hinterlands of Ceará. This is a descriptive quantitative research and cross-sectional design conducted through interviews with 152 family farmers and / or artisanal fishermen in Novo Oriente, Ceará, Brazil. Statistical analysis was performed with absolute and relative frequency, arithmetic mean and standard deviation. Some vulnerabilities experienced by the families were observed: 19.5% (n=29) with a deficiency in the treatment of drinking water, 25.7% (n=39) reported the existence of environmental problems, 26.6% (n=40) showing unsatisfactory family production, 34.6% (n=47) with low family income, 75% (n=114) with no schools in the locality and 88.2% (n=134) of day care centers, 51.3% ( n=78) with a lack of leisure options, and 37.5% (n=57) with food insecurity. Prevalence of chronic diseases, such as cardiovascular and mental, stands out. This reality is contradictory with the goals proposed by the SDG: zero hunger; good health and welfare; quality education; clean water and sanitation; decent employment and economic growth; reduction of inequalities. Thus, health actions cannot be separated from these inequities, and a link between governments, companies, and citizens must be sought to achieve the goals of the 2030 Agenda.

Objetivo: Identificar na literatura disponível as sequelas de saúde desenvolvidas pela população adoecida pela COVID-19. Métodos: Revisão integrativa com publicações recuperadas a partir do acesso ao Medline via PubMed e ao Portal Regional da Biblioteca Virtual em Saúde (BVS) em abril de 2021 que responderam à questão norteadora: Quais as principais sequelas de saúde desenvolvidas pela população adoecida pela COVID-19?. A estratégia de busca incluiu os descritores: Coronavirus Infections, COVID-19, SARS-COV-2, Complications, Disease, Adult, Long COVID. Identificaram-se 348 publicações, 196 da BVS e 152 no PubMed. Após análise dos critérios de inclusão/exclusão, permaneceram 66 publicações para leitura na íntegra, restando 27 artigos que compuseram a amostra. Resultados: As evidências selecionadas dos 27 artigos permitiram identificação das principais sequelas: neurológicas, saúde mental, cardíacas, no olfato e paladar, vasculares, cutâneas, respiratórias e gastrointestinais. A maior prevalência de sequelas neurológicas ocorreu em mulheres, bem como as cardíacas nos homens; não havendo distinção da prevalência de sintomas de saúde mental entre homens e mulheres. Conclusão: Com base nesses achados, evidenciou-se a importância do acompanhamento em longo prazo das pessoas que tiveram COVID-19, uma vez que os sintomas desenvolvidos como sequelas não são exclusivos dessa doença e podem impactar na qualidade de vida.


Objective: To identify, in the literature available, the health sequelae developed by the population affected by COVID-19. Methods: This is an integrative review of publications retrieved from Medline via PubMed and the Regional Portal of the Virtual Health Library (Biblioteca Virtual em Saúde ­ BVS) in April 2021 that answered the research question: What are the main health sequelae developed by the population affected by COVID-19? The search strategy included the descriptors: Coronavirus Infections, COVID-19, SARS-COV-2, Complications, Disease, Adult, Long COVID. 348 publications were identified: 196 on BVS and 152 on PubMed. After analyzing the inclusion/exclusion criteria, 66 publications remained for full reading, with a final 27 articles included in the sample. Results: The selected evidence from the 27 articles allowed the identification of the main sequelae: neurological, mental health, cardiac, smell and taste, vascular, cutaneous, respiratory and gastrointestinal sequelae. The highest prevalence of neurological sequelae was found in women, whereas cardiac sequelae were found mostly in men; there was no difference in the prevalence of mental health symptoms between men and women. Conclusion: Based on these findings, the importance of long-term follow-up of people who had COVID-19 was highlighted since the symptoms developed as sequelae are not exclusive to this disease and can impact quality of life.

La estrategia de búsqueda incluyó los descriptores: Coronavirus Infections, COVID-19, SAR-COV-2, Complications, Desease, Adult, Long COVID. Fueron identificadas 348 publicaciones, 196 de la BVS Y 152 en PubMed. Después del análisis de los criterios de inclusión/exclusión, permanecieron 66 publicaciones para lectura completa, restando 27 artículos que compusieron la muestra. Resultados: Las evidencias seleccionadas de los 27 artículos permitieron la identificación de las principales secuelas: neurológicas, salud mental, cardíacas, en el olfato y paladar, vasculares, cutáneas, respiratorias y gastrointestinales. La mayor prevalencia de secuelas neurológicas ocurrió en mujeres, y las cardíacas en los hombres; no habiendo distinción de la prevalencia de síntomas de salud mental entre hombres y mujeres. Conclusión: Con base en estos hallazgos, se evidenció la importancia del acompañamiento a largo plazo de las personas que tuvieron COVID-19, ya que los síntomas desarrollados como secuelas no son exclusivos de esta enfermedad y pueden impactar en la calidad de vida.

BACKGROUND: Enteroaggregative Escherichia coli (EAEC) is an E. coli pathotype that presents aggregative adhesion patterns on in vitro cultivated cells, mainly related to persistent diarrhea cases in children. EAEC virulence factors are important for host colonization and pathogeni-city. Intestinal epithelial cells (IECs) recognize pathogen-associated molecular patterns (PAMPs) and initiate an immune response. Several studies using in vivo and in vitro models emphasize the probiotic activity and immunomodulatory capacity of Lactobacillus and Bifidobacterium species. OBJECTIVE: To evaluate the modulation of cytokine production by probiotics Bifidobacterium animalis and Lactobacillus casei in human intestinal Caco-2 cells exposed to different strains of EAEC. METHODS: Caco-2 cells were incubated with EAEC strains in the presence or absence of probiotics. The production of cytokines IL-8, TNF-α, IL-1ß and IL-10 was evaluated in the supernatants by a sandwich enzyme-linked immunosorbent assay (ELISA). RESULTS: Cytokine production did not change when uninfected and EAEC-infected Caco-2 cells were exposed to probiotics separately. All EAEC induced a significant increase in IL-8 production by Caco-2 cells, but the probiotics, even together, could not reduce its production. On the other hand, the synergic activity of probiotic strains significantly increased TNF-α production but decreased the basal production of IL-1ß. Also, probiotics induced a significant increase in the production of the anti-inflammatory cytokine IL-10 during EAEC infection. CONCLUSION: Our results reinforce the synergistic immunomodulatory activity of probiotics during EAEC infection.

RESUMO CONTEXTO: Escherichia coli enteroagregativa (EAEC) é um patotipo de E. coli que apresenta o padrão de aderência agregativa em células cultivadas in vitro, sendo comumente relacionada a casos de diarreia persistente em crianças. Fatores de virulência presentes em EAEC são importantes para a colonização do hospedeiro e patogenicidade. As células epiteliais intestinais (IECs) reconhecem padrões moleculares associados a patógenos (PAMPs) e iniciam uma resposta imune. Vários estudos usando modelos in vivo e in vitro enfatizam a atividade probiótica e a capacidade imunomoduladora de espécies de Lactobacillus e Bifidobacterium. OBJETIVO: Este estudo avaliou a modulação da produção de citocinas pelos probióticos Bifidobacterium animalis and Lactobacillus casei em células intestinais humanas Caco-2 expostas a diferentes cepas de EAEC. MÉTODOS: As células Caco-2 foram incubadas com as cepas de EAEC na presença ou ausência dos probióticos. A produção das citocinas IL-8, TNF-α, IL-1β e IL-10 foi avaliada nos sobrenadantes por ELISA sanduíche. RESULTADOS: Não houve alteração na produção de citocinas quando as células não infectadas e as células infectadas com EAEC foram expostas aos probióticos separadamente. Todas as cepas de EAEC induziram aumento significativo na produção de IL-8 pelas células Caco-2, mas os probióticos, ainda que em conjunto, não foram capazes de reduzir a produção desta citocina. Por outro lado, as cepas de probióticos aumentaram significativamente a produção de TNF-α mas diminuíram a produção basal de IL-1ß. Além disso, os probióticos induziram um aumento significativo na produção da citocina anti-inflamatória IL-10 durante a infecção por EAEC. CONCLUSÃO: Nossos resultados reforçam a atividade imunomodulatória sinérgica dos probióticos durante a infecção de EAEC.

Objetivo: Avaliar o custo-efetividade do uso de um painel genético de 21 genes em pacientes adultas diagnosticadas com câncer de mama em estádio inicial em uma operadora de saúde com mais de 500.000 vidas. Métodos: Foi utilizada uma coorte prospectiva seguida de um estudo de custo-efetividade entre os pacientes que utilizaram Oncotype DX® em 2020. Calcularam-se as despesas totais de cada esquema de quimioterapia (QT), somando-se os custos dos produtos e taxas de infusão. Resultados: Das 35 pacientes que utilizaram o teste de 21 genes no período avaliado, 60% (n = 21) não necessitaram de QT. Quando aplicadas simulações, houve custo evitado de R$ -1.945.448,88 (custos incrementais potenciais de R$ -6.488.207,56 até R$ 443.485,26, dependendo do esquema de QT escolhido). Conclusão: A inserção do teste de 21 genes na jornada do tratamento de câncer de mama na saúde suplementar evidenciou significativa relevância, pois contribuiu com o uso adequado da terapêutica, garantindo a sustentabilidade do sistema de saúde. Apresentando-se como uma opção custo-efetiva para a maioria dos esquemas de QT em comparação com a sua não utilização no tratamento, para a saúde suplementar brasileira


Objective: To evaluate the cost-effectiveness of the use of a genetic panel of 21 genes in adult patients diagnosed with early stage breast cancer in a healthcare provider with more than 500,000 lives. Methods: A prospective cohort study was conducted, followed by cost-effectiveness, among patients who used Oncotype DX® , in 2020. The total costs of each chemotherapy scheme (QT) were calculated, adding the costs of the products and infusion fees. Results: Of the 35 patients who used 21 gene tests in the evaluation period, 60% (n = 21) did not require QT. When simulations were applied, there was an avoided cost of R$ -1.945.448,88 (Potentials incremental costs from -R$ 6.488.207,56 to +R$ 443.485,26, depending on the chosen QT scheme). Conclusion: The insertion of 21-Gene recurrence score in the breast cancer treatment journey in supplementary health showed significant relevance, as it contributes to the appropriate use of therapy, guaranteeing the sustainability of the health system. Presenting itself as a cost-effective option for most QT schemes compared to not being used in treatment, for Brazilian supplementary health System

Facial recognition is a method of identifying or authenticating the identity of people through their faces. Nowadays, facial recognition systems that use multispectral images achieve better results than those that use only visible spectral band images. In this work, a novel architecture for facial recognition that uses multiple deep convolutional neural networks and multispectral images is proposed. A domain-specific transfer-learning methodology applied to a deep neural network pre-trained in RGB images is shown to generalize well to the multispectral domain. We also propose a skin detector module for forgery detection. Several experiments were planned to assess the performance of our methods. First, we evaluate the performance of the forgery detection module using face masks and coverings of different materials. A second study was carried out with the objective of tuning the parameters of our domain-specific transfer-learning methodology, in particular which layers of the pre-trained network should be retrained to obtain good adaptation to multispectral images. A third study was conducted to evaluate the performance of support vector machines (SVM) and k-nearest neighbor classifiers using the embeddings obtained from the trained neural network. Finally, we compare the proposed method with other state-of-the-art approaches. The experimental results show performance improvements in the Tufts and CASIA NIR-VIS 2.0 multispectral databases, with a rank-1 score of 99.7% and 99.8%, respectively.

Background: Electroacupuncture is one of the most popular physical treatments for clinical pain, but the potential influence of a patient's age on the effectiveness of electroacupuncture treatment has not been clearly established. Objectives: The present study aimed to detect a potential difference in electroacupuncture- induced analgesia between juvenile and adult rats. Methods: In this study, we investigated the effects of electroacupuncture treatment on the nociceptive jaw-opening reflex evoked by tooth-pulp stimulation in juvenile and adult rats. Results: Our results showed there were age differences in electroacupuncture-induced analgesic effects in rats, especially with naloxone antagonization. The ratio of naloxonereversibility against electroacupuncture analgesia was greater in adult rats than in juvenile rats. Conclusion: These results suggest that electroacupuncture analgesia is produced mainly by the non-opioid system in juvenile rats and by the opioid system in adult rats.

The new coronavirus (SARS-CoV-2) pandemic has been wreaking havoc all over the planet. In a precautionary measure, populations have been forced and kept under quarantine to contain the outbreak of the COVID-19 disease. The quarantine primary goal is to avoid the overload to the hospitals, which should be available for the care of COVID-19 patients. However, the virus does not have a uniform spread throughout the planet, and Brazil is no different. Although all the world's attention is now on the COVID-19 pandemic, there is no similar pattern of spread, and other diseases are still a real problem. Given the risks of transmission between patients and healthcare providers, there is a great challenge for healthcare institutions who must balance resources to assure safe care to patients and professionals while they take care of other disease patients, and perform surgical procedures that need to be carried out. Under such circumstances, as COVID-19 can also present pre- or asymptomatic transmission, it can be challenging to identify patients who are carrying and spreading the virus. Studies and information on mandatory testing for who are candidates to undergo elective surgery are scarce. Thus, the authors have reviewed the literature, and discuss the need to test these patients under the current context.

Bamboo species are an alternative for the composition of forest plantations. However, their potential has not been explored due to the hard time in producing large-scale clonal plants. Thus, the aim this work was to evaluate the in vitro establishment, bud multiplication and ex vitro rooting of Bambusa vulgaris. The first experiment tested different systemic and contact fungicide solutions, based on exposure time, during the establishment phase. Established explants were subjected to evaluation of residual fungicide effect on subcultures during the multiplication and elongation phases. The second experiment evaluated the influence of activated carbon on ex vitro survival and on adventitious rooting. Explant immersion in liquid culture medium added with 1.0 mL of fungicide for 120 hours has favored the in vitro establishment and reduced fungal contamination. On the other hand, it favored the shoot emission of shoots per explant during the multiplication phase. Both rooting induction culture medium and mini-incubator system use were effective in enabling adventitious root formation. The presence of activated carbon in the rooting induction culture medium resulted in a higher clonal plant survival rate.


As espécies de bambus são uma alternativa para a composição de plantios florestais. Entretanto, esse potencial não tem sido explorado devido à dificuldade de produção de mudas clonais em larga escala. Assim, objetivo deste trabalho foi avaliar o estabelecimento in vitro, a multiplicação e o enraizamento ex vitro de Bambusa vulgaris. No primeiro experimento foram testadas diferentes soluções de fungicida sistêmico e de contato em relação ao tempo de exposição durante a fase de estabelecimento. Os explantes estabelecidos foram avaliados quanto ao efeito residual do fungicida durante subcultivos nas fases de multiplicação e alongamento. No segundo experimento, foi avaliada a influência do carvão ativado sobre a sobrevivência e enraizamento ex vitro. Durante o estabelecimento in vitro, a imersão de explantes em meio de cultura líquido contendo alíquota de 1,0 mL de fungicida durante 120 horas favoreceu o estabelecimento e reduziu a contaminação fúngica, enquanto na fase de multiplicação, houve o favorecimento da emissão de brotos por explante. O meio de cultura de indução ao enraizamento e uso de sistema de mini-estufim foram efetivos para a formação de raízes adventícias e a presença de carvão ativado resultou em uma maior sobrevivência das mudas clonais.
