RESUMO

Recommendations for acute and long-term oxygen therapy (needs assessment, implementation criteria, prescription practices, and follow-up) in children were produced by the Groupe de Recherche sur les Avancées en Pneumo-Pédiatrie (GRAPP) under the auspices of the French Paediatric Pulmonology and Allergology Society (SP2A). The Haute Autorité de Santé (HAS) methodology, based on the Formalized Consensus, was used. A first panel of experts analyzed the English and French literature to provide a second panel of experts with recommendations to validate. Only the recommendations are presented here, but the full text (arguments+recommendations) is available at the website of the French Paediatric Society: www.sfpediatrie.com.

RESUMO

Pulmonary function tests remain the best method to assess underlying bronchial inflammation, particularly distal airway flow. However, these tests are limited in their ability to differentiate treatment-reversible small airway obstruction from definitive remodeling related to a viral infection. The increased availability of the exhaled fraction of nitric oxide (FeNO) measurement means that this method is accessible and attractive. In spite of a number of studies, mainly in adults, its use in the management of asthma needs more investigation. Threshold values that permit optimum management are poorly defined, and strategies based on variations of FeNO do not result in reduction of the number of exacerbations, although they do permit better control of drug use and better control of inflammation. Sputum eosinophil counts are of interest for the prevention of exacerbations, although while not well validated they may be useful in some asthmatic children. Also, they require an experienced cytology laboratory. Finally, recent data on bronchial biopsies in children with difficult-to-treat asthma will be summarized.

RESUMO

The adverse events of the inhaled steroids on growth and bone mineralization concern the pediatricians. From the available current data, we exposed the following conclusions: 1) Inhaled steroids must be preferred to oral steroids; 2) Short, middle and long-term adverse effects of inhaled steroids have to be differentiated. Only the long-term effects reflect possible sequela; 3) The adverse effects are specific from the considered inhaled steroids, the most recent available drugs having less adverse effects on bone and growth; 4) Recent data did not show consequences on the adult height and bone mineralization; 5) Recommendations in the use of inhaled steroids must be known by the practitioners in order to limit the adverse events of these drugs. Growth need to be surveyed in children with inhaled steroids and more studies are still necessary. In addition, we enhanced that local adverse effects should be sought by the physicians. Cough after inhalation was frequent and might be a negative factor of compliance.

UNLABELLED: BACKGROUND AND METHODS. The usefulness of hematological parameters provided by the Technicon H*1 was investigated for the diagnosis of acute lymphoblastic leukemias (ALL) and for the identification of blast cells in peripheral blood samples. RESULTS AND CONCLUSIONS: When automated blast detection was compared with microscopic examination in 62 samples with 4-98% morphologically recognizable lymphoblasts on peripheral blood smears, a false-negative result occurred in 28% (17/62) of the ALL cases. However, in 14 of these 17 false-negative cases the H*1 generated an output listing that would require microscopic evaluation in any event. The H*1 output listing interpretation (based on abnormal leukograms, blast flags and bicytopenia) led us to suspect the presence of ALL in 76% (47/62) of the cases. Three ANLL were recognized as ALL. In three out the 62 ALL, the H*1 output listing interpretation missed identify a leukemic process. In most cases (77%) we noted a characteristic deformation of the mononuclear population profile in the baso/lobularity display, and the cloud of analytical points was shifted toward to the left, in keeping with the presence of a large number of hypodense nuclei.

RESUMO

The psycho-motor sedative action of tiapride, which has been demonstrated in internal medicine, was studied in anesthesiology. A group of 253 patients hospitalized for urological (203 cases) or gynaecological (50 cases) operations were given standard premedication together with 100 to 300 mg of tiapride. Preoperative sedation and reduction of anxiety was excellent in 138 cases, moderate in 97 cases, and nil in 18 cases, under the influence of tiapride. Tiapride was perfectly well tolerated even in patients with debility, obesity, alcoholism and cardiac or respiratory insufficiency.
