RESUMO
For 35 of 50 patients with rheumatoid arthritis traditional drug therapy was a minor success for a long time. Without any modifications of the drug therapy every patient went through a course of hyperbaric oxygenation (HBO): 21 sessions under 1.7 ata for 40 min. Good clinical results both immediate and remote have been obtained. The effect of HBO on the immune system of the patients has intensified the suppressive function of T-lymphocytes (especially with systemic symptoms of the disease), normalized cell-bound immunity and decreased the serum concentration in immune complexes.

Patients with nonspecific ulcerative colitis and Crohn's disease were treated with drug therapy (prednisolone, sulphasalazine, metronidazole per os and hydrocortisone per rectum) and subjected to 12 sessions of HBO. Every 10-14 months the HBO course was repeated. After 7 years of such a treatment a gradual partial recovery of large intestine mucosa and in some cases even its absolute recovery were observed. It was most difficult to get good results when treating lesion of distal colon segments. HBO produced a good effect when the disease was diagnosed at the early stage, when the intestine injury was accompanied by hepatobiliary system disease and in teenagers.

The study was made of HBO effect on changes of biologic fluid toxicity in purulent peritonitis. HBO was introduced in combined therapy of 68 patients (32 with general peritonitis and 36 with peritoneal sepsis). Lethality in peritonitis patients reached 40.6% (19 patients), in sepsis patients 33.3% (12 patients).

Studies of the blood coagulation processes in the patients with atopic dermatitis have revealed an increase in the activities of the blood coagulation system in the acute phase of the disease, the degree of these changes depending on the severity and activity of the pathologic process. Hyperbaric oxygenation, an effective treatment modality for atopic dermatitis, normalized some of the altered parameters of the hemostasis system.

Forty patients with acute neurosensory hypoacusis of vascular genesis were exposed to combined treatment that included hyperbaric oxygenation (HBO) and drug therapy to improve microcirculation, vasodilation and metabolism. In order to control HBO side effects (vasoconstriction), the patients were subdivided into two subgroups of 20 subjects each. Group 1 patients were treated with HBO immediately before drug administration and Group 2 patients were treated with HBO immediately after drug administration. Good effects in Group 2 patients were twice as large. This was achieved by elimination of HBO side effects due to drug administration immediately before hyperbaric exposure.

The paper is concerned with the results of a study of the C-peptide concentration in blood serum of 45 patients with type I diabetes mellitus prior to and during therapy with hyperbaric oxygenation (HBO). Each patient was given 10-12 HBO sessions. A HBO therapeutic course was shown to stimulate residual insulin secretion in type I diabetes mellitus with a period of disease not more than 3 years, a daily dose of insulin not more than 60 units, in the absence of excess body mass over 10%, and retinopathy over stage I. Prognostic criteria make it possible to take into account clinical factors in the course of diabetes mellitus preventing the normalization or stimulation of residual basal insulin secretion which can be used for the determination of indications for HBO therapy aimed at the stimulation of insulin secretion.

The stage-wise character of the course of sepsis is substantiated on the basis of an analysis of the central venous pressure parameters, central and peripheral hematocrite, pulse rate, respiration, ECG data and rheohepatograms in 81 patients with surgical sepsis. The stage-wise course of sepsis is also confirmed by specific hemodynamic reactions of the organism in response to the first session of HBO which allows to differentiate the indications for this kind of treatment.
