RESUMO

Erythrocytes deformability, plasma cAMP, 6-keto-PGF1 alpha, TXB2 levels and fibrinolytic activity in patients suffering from arteriosclerosis obliterans before and after Trental were studied. Trental improved the deformability of red cells and resulted in an enhancement of diminished cAMP level in plasma and increased the fibrinolytic activity. The improvement of deformability observed after Trental could depend upon the influence of a higher cAMP level in plasma on the erythrocyte membrane. The increase of cAMP level seems to be the result of the inhibiting effect of Trental on phosphodiesterase and of higher fibrinolytic activity.

In patients with venous thrombosis the behaviour of leukocyte random locomotion, migration, phagocytosis, blast transformation and NBT reduction properties were investigated. During the course of venous thrombosis the leukocyte adherence activity and their NBT reduction properties increased whereas their random locomotion decreased. The lymphocytes evaluated by the migration and blast transformation tests showed no signs of stimulation nor was their immunologic response changed. The thermolabile factor present in the clot lysis products affected only the adherence activity and the NBT reduction properties.

In patients with thrombophlebitis receiving intravenously a nicotinic acid derivative, pentoxifylline and streptokinase the fibrinolytic activity and leukocyte adhesiveness were studied. The euglobulin lysis time was significantly shortened in all cases; at the same time the adhesiveness of leukocytes and the leukocyte number were reduced. The addition of the drugs to blood in vitro resulted in a significant increase in leukocyte adhesiveness; the fibrinolytic activity was enhanced only upon streptokinase. Leukocytes are believed to promote the thrombolytic effect of fibrinolytic agents.

RESUMO

The influence of anabolic steroids plus phenformin on the course of disease was studied in patients suffering from occlusive vascular diseases. In arteriosclerosis obliterans of the lower limbs the combined therapy led to a higher percentage of improvement than the control group showed. In coronary heart disease there was also a beneficial effect of this treatment. The drugs were found to exert beneficial clinical effects especially in patients with thrombophlebitis migrans and superficial thrombophlebitis. From the results of this study it is concluded that this therapy may be of clinical value in the treatment of patients with occlusive vascular diseases.

Deformability of red cells and plasma levels of cAMP and cGMP were studied in patients with arteriosclerosis obliterans of lower limbs. Deformability of red cells and plasma levels of cAMP were found to be decreased in these patients. Venous injection of HR (Venoruton) in one dose (1,000 mg) to patients, led to an increase of red cell deformability and normalisation of cAMP plasma level. The plasma level of cGMP was not changed. The beneficial effect of HR in arteriosclerosis obliterans depends, first of all, on improving the deformability of red cells. The decreased deformability observed in arteriosclerosis seems to be a result of disturbances in the rate of spectrin phosphorylation of the erythrocyte membrane. By normalising the plasma level of cAMP HR had a considerable influence on the appropriate state of elasticity of the red cell membrane.

RESUMO

On the basis of standardized protocols of the therapeutic results of acute non-lymphoblastic leukaemias in adults sent to the Institute of Haematology in Warsaw from 8 haematological centres in Poland it was demonstrated that complete remission occurred in 34.4% of patients (129 out of 375 cases). The mean survival time of the patients treated intensively according to programmes I, II, III and IV 8.6 months, those of patients with complete remission - 13.5 months, patients without complete remission - 3.7 months. The most frequent cause of death (82.5%) were infections and/or haemarrhagic diathesis.

In three groups of patients with hypochromic anemia, chronic myelocytic leukemia and chronic lymphatic leukemia, the fibrinolytic activity, kininogen level and kininase activity in the peripheral blood and bone marrow punctates were analyzed. In patients with anemia and myelocytic leukemia similar, unchanged kininase activity, significantly higher fibrinolytic activity and lower kininogen content were found in bone marrow as compared to the peripheral blood samples. In cases of lymphatic leukemia the fibrinolytic activity and kininogen levels did not show significant difference in the blood and bone marrow, while a significantly decreased blood kininase activity was followed by a similarly low activity of the bone marrow punctate. The differences found may result from an enzymatic effect of juvenile forms of granulocytes and of the lymphoreticular system.

The effects of 6 months' combined therapy with phenformin and an anabolic steroid were compared in patients with thrombophlebitis migrans (12 patients) and those with superficial thrombophlebitis (15 patients). In both groups of patients an increase in blood fibrinolytic activity, and "capacity" decrease in platelet adhesiveness, plasma fibrinogen, blood lipids, beta lipoproteins as well as serum cholesterol level were found. A statistically significant decrease in frequency of inflammations in patients with thrombophlebitis migrans occurred. In these patients a return of the previously low "fibrinolytic capacity" to normal values was observed. It seems that prolonged activation of fibrinolysis by means of phenformin and an anabolic steriod may be of value in the prophylaxis of venous thrombosis especially thrombophlebitis migrans.
