RESUMO

Previous studies suggest that flood-rich and flood-poor periods are present in many flood peak discharge series around the globe. Understanding the occurrence of these periods and their driving mechanisms is important for reliably estimating future flood probabilities. We propose a method for detecting flood-rich and flood-poor periods in peak-over-threshold series based on scan-statistics and combine it with a flood typology in order to attribute the periods to their flood-generating mechanisms. The method is applied to 164 observed flood series in southern Germany from 1930 to 2018. The results reveal significant flood-rich periods of heavy-rainfall floods, especially in the Danube river basin in the most recent decades. These are consistent with trend analyses from the literature. Additionally, significant flood-poor periods of snowmelt-floods in the immediate past were detected, especially for low-elevation catchments in the alpine foreland and the uplands. The occurrence of flood-rich and flood-poor periods is interpreted in terms of increases in the frequency of heavy rainfall in the alpine foreland and decreases of both soil moisture and snow cover in the midlands.

OBJECTIVE: To observe the efficacy of the circumferential decompression with posterior transpedicular osteotomy and segmental instrumentation with interbody fusion for thoracic ossification of posterior Iongitudinal ligament (T-OPLL). METHODS: From May 2012 to June 2015, 16 consecutive patients underwent posterior transpedicular osteotomy and segmental instrumentation with interbody fusion.Osteotomy range was depended by length and types of OPLL.Patient's data included level, clinical presentation, blood loss, length of surgery, complications, VAS, JOA, and Frankel grading system before and after the surgery. All data were collected, retrospectively. RESULTS: The follow-up period was (30±19) months (range from 12 to 50 months). The operation time was (261.6±51.3) min (range from 190 to 310 min). The blood loss was (980.3±370.5) ml (range from 600 to 2 100 ml). All patients were well treated with posterior compression and segmental instrumentation with interbody fusion.The VAS score was (4.2±0.2) in all patients at a week, improving to (2.7±0.1) points at 3 months, (2.4±0.2) at 1 year, and (2.0±0.1) at last fellow-up.The statistical analysis of the results showed a significant improvement of pain at 3 months (P<0.05) when compared to the preoperative status.The preoperative JOA score was (4.2±1.7) in all patients, improving to (7.8±2.5) points at 3 months, (8.5±2.7) at 1 year, and (9.0±1.0) at last fellow-up.The mean recovery rate for the total JOA score was (72%±8%). Differences in the overall JOA Scores showed significant postoperative improvement.Frankel grade improved by either 1 or 2 grades in 16 patients at the last follow-up.None of the patients showed any signs of instrument migration or failure during follow-up. CONCLUSION: The results suggested that the procedure achieved a total resection of the ossified posterior longitudinal ligament.The treatment method with posterior transpedicular osteotomy and circumferential decompression was found to be safe, effective, reliable, and technically feasible.

AIM: To determine the half life (T1/2), time taken to reach maximum plasma concentration (Tmax) and maximum plasma concentration (Cmax) of thalidomide in sheep following I/V, oral and topical treatment with a single dose of thalidomide. METHOD: Three groups of 4-6-month-old ram lambs were treated with thalidomide dissolved in dimethylsulphoxide (DMSO). The first group (n=10) was treated I/V with 100 mg thalidomide in 2 mL DMSO; the second group (n=8) received 400 mg thalidomide in 2 mL DMSO orally, and the third group (n=8) had 400 mg thalidomide in 4 mL DMSO applied topically. Plasma samples were collected up to 36 hours after treatment, snap-frozen at -80°C and analysed for concentrations of thalidomide using high performance liquid chromatography. RESULTS: Following I/V administration, T1/2 was 5.0 (SEM 0.4) hours, volume of distribution was 3,372.0 (SEM 244.3) mL/kg and clearance was 487.1 (SEM 46.1) mL/hour.kg. Topical application of 400 mg thalidomide did not increase plasma concentrations. Following oral administration, thalidomide bioavailability was 89%, with T1/2, Tmax, and Cmax being 7.2 (SEM 0.8) hours, 3.0 (SEM 0.4) hours and 1,767.3 (SEM 178.1) ng/mL, respectively. CONCLUSION: Topical administration using DMSO as a solvent did not increase concentrations of thalidomide in plasma. The mean pharmacokinetic parameters determined following oral treatment with 400 mg of thalidomide were similar to those reported in humans receiving a single 400 mg oral dose (T1/2 7.3 hours; Tmax 4.3 hours and Cmax 2,820 ng/mL). There is potential for thalidomide to be used as a model for the treatment of chronic inflammatory conditions in sheep, such as Johne's disease, where tumour necrosis factor alpha plays a pathogenic role.

AIMS: Surgical treatment of periacetabular tumors remains one of the most challenging problems in musculoskeletal oncology. The purpose of this study was to review the clinical and functional outcomes of resection hip arthroplasty and analyze its feasibility. METHODS: This study assesses twenty-seven patients with periacetabular tumors treated by resection hip arthroplasty between 1999 and 2010. The tumors were excised with wide margins and the residual intact femoral head placed underneath the resected ilium. Clinical, functional and oncological outcomes as well as complications were carefully evaluated. RESULTS: The average follow-up time was 55 months (range, 3-118) and the mean surgical time 170 min (range, 120-350) with an average blood loss 1200 ml (range, 600-2200). Six patients died in 6-33 months postoperatively; no other local recurrences or deaths occurred. The 1-year, 5-year, and 10-year disease-free survival rates were 96.3%, 77.8% and 77.8% respectively. The mean limb-length discrepancy was 5 cm (range, 2-7.5) and all patients required custom-made shoes with their heels heightened by 2-5 cm. At the last follow-up, the mean functional score was 75.6%. Twenty patients recovered normal ambulation function with custom-made shoes and seven had to walk with crutches. Wound healing problems were observed in nine patients and deep or superficial infection in none. CONCLUSIONS: Resection hip arthroplasty is recommended as a feasible surgical protocol for periacetabular tumors because it has few complications, good functional results, short surgical time and little blood loss.

Hyperexpression of the Saccharomyces cerevisiae multidrug ATP-binding cassette (ABC) transporter Pdr5p was driven by the pdr1-3 mutation in the Pdr1p transcriptional regulator in a strain (AD/PDR5(+)) with deletions of five other ABC-type multidrug efflux pumps. The strain had high-level fluconazole (FLC) resistance (MIC, 600 microg ml(-1)), and plasma membrane fractions showed oligomycin-sensitive ATPase activity up to fivefold higher than that shown by fractions from an isogenic PDR5-null mutant (FLC MIC, 0.94 microg ml(-1)). In vitro inhibition of the Pdr5p ATPase activity and chemosensitization of cells to FLC allowed the systematic screening of a 1.8-million-member designer D-octapeptide combinatorial library for surface-active Pdr5p antagonists with modest toxicity against yeast cells. Library deconvolution identified the 4-methoxy-2,3,6-trimethylbenzensulfonyl-substituted D-octapeptide KN20 as a potent Pdr5p ATPase inhibitor (concentration of drug causing 50% inhibition of enzyme activity [IC(50)], 4 microM) which chemosensitized AD/PDR5(+) to FLC, itraconazole, and ketoconazole. It also inhibited the ATPase activity of other ABC transporters, such as Candida albicans Cdr1p (IC(50), 30 microM) and Cdr2p (IC(50), 2 microM), and chemosensitized clinical isolates of pathogenic Candida species and S. cerevisiae strains that heterologously hyperexpressed either ABC-type multidrug efflux pumps, the C. albicans major facilitator superfamily-type drug transporter Ben(R)p, or the FLC drug target lanosterol 14 alpha-demethylase (Erg11p). Although KN20 also inhibited the S. cerevisiae plasma membrane proton pump Pma1p (IC(50), 1 microM), the peptide concentrations required for chemosensitization made yeast cells permeable to rhodamine 6G. KN20 therefore appears to indirectly chemosensitize cells to FLC by a nonlethal permeabilization of the fungal plasma membrane.

When performing dynamic studies using emission tomography the tracer distribution changes during acquisition of a single set of projections. This is particularly true for some positron emission tomography (PET) systems which, like single photon emission computed tomography (SPECT), acquire data over a limited angle at any time, with full projections obtained by rotation of the detectors. In this paper, an approach is proposed for processing data from these systems, applicable to either PET or SPECT. A method of interpolation, based on overlapped parabolas, is used to obtain an estimate of the total counts in each pixel of the projections for each required frame-interval, which is the total time to acquire a single complete set of projections necessary for reconstruction. The resultant projections are reconstructed using traditional filtered backprojection (FBP) and tracer kinetic parameters are estimated using a method which relies on counts integrated over the frame-interval rather than instantaneous values. Simulated data were used to illustrate the technique's capabilities with noise levels typical of those encountered in either PET or SPECT. Dynamic datasets were constructed, based on kinetic parameters for fluoro-deoxy-glucose (FDG) and use of either a full ring detector or rotating detector acquisition. For the rotating detector, use of the interpolation scheme provided reconstructed dynamic images with reduced artefacts compared to unprocessed data or use of linear interpolation. Estimates for the metabolic rate of glucose had similar bias to those obtained from a full ring detector.

In this work, we introduce a deblocking algorithm for Joint Photographic Experts Group (JPEG) decoded images using the wavelet transform modulus maxima (WTMM) representation. Under the WTMM representation, we can characterize the blocking effect of a JPEG decoded image as: 1) small modulus maxima at block boundaries oversmooth regions; 2) noise or irregular structures near strong edges; and 3) corrupted edges across block boundaries. The WTMM representation not only provides characterization of the blocking effect, but also enables simple and local operations to reduce the adverse effect due to this problem. The proposed algorithm first performs a segmentation on a JPEG decoded image to identify the texture regions by noting that their WTMM have small variation in regularity. We do not process the modulus maxima of these regions, to avoid the image texture being "oversmoothed"by the algorithm. Then, the singularities in the remaining regions of the blocky image and the small modulus maxima at block boundaries are removed. We link up the corrupted edges, and regularize the phase of modulus maxima as well as the magnitude of strong edges. Finally,the image is reconstructed using the projection onto convex set (POCS)technique on the processed WTMM of that JPEG decoded image.This simple algorithm improves the quality of a JPEG decoded image inthe senses of signal-to-noise ratio (SNR) as well as visual quality. We also compare the performance of our algorithm to the previous approaches,such as CLS and POCS methods. The most remarkable advantage of the WTMM deblocking algorithm is that we can directly process the edges and texture of an image using its WTMM representation.
