RESUMO

This article will discuss alternatives to restraint use in the management of patients who have difficult behavior or an increased risk for injury in the acute care, surgical setting. A discussion of the efficacy and adverse effects of restraint use will be followed by review of new regulations established by HCFA and JCAHO and suggestions for care that minimizes restraint use.

RESUMO

The assessment and management of the patient with idiopathic Parkinson's disease is complicated by multisystem involvement and variability in symptom presentation and severity. An imbalance in dopaminergic/ cholinergic activities, related to profound dopamine deficiency, is the basis for disease presentation and is the target of treatment measures. Although the cause of dopamine deficiency in the substantia nigra is not known, several theories of causation have been offered. Understanding the physiologic basis for pharmacological and surgical treatment allows the nurse to provide individualized, effective care to Parkinson's patients.

RESUMO

Research is widely accepted as an integral component of the CNS role. However, nursing literature reveals controversy over the ideal form and extent of CNS involvement in research. Acute care CNSs are in an ideal position to conduct clinical research. A model for support of CNS involvement in research in acute care and three examples of CNS-led research that impacted nursing practice are described in this article.

RESUMO

Patients can become more agitated after the application of restraints. Restraints may be necessary to prevent injury to patients. Diversions, presence of family members, recliner chairs, day rooms, or sitters are alternatives to physical restraints.

RESUMO

Older patients and their physicians are likely to choose surgery, when indicated, despite old age. To facilitate recovery, functional assessment provides data to plan immediate and long-term needs. Functional assessment includes physical, social, and environmental data. It also helps define "normal" functioning after surgery. Cognitive assessment is especially important and can be obtained during the initial interview of the patient. Cognitive assessment tools may be indicated if data are unclear. Postoperative complications occur in all body systems and include: MI, CHF dysrhythmias, pneumonia, atelectasis, hyponatremia, renal failure, confusion, deconditioning, skin tears, incontinence, and constipation. Referral may be needed before dismissal for significant changes in functional ability.
