RESUMO

The authors used D. A. Kenny's social relations model to examine J. C. Coyne's interpersonal theory of depression among a clinical sample of well-acquainted prison inmates. Members of 12 therapy groups (N = 142) diagnosed with a substance abuse disorder completed a self-report measure of depression and anxiety and indicated their desire to interact with other group members. There was both consensus about which group members were rejected and individual differences in the participants reported desire for future interaction with other group members. Those reporting high levels of depressive negative affect were most likely to be rejected. Those lowest in positive affect indicated the least desire for future interaction with others.

Because of legal constraints and statistical limitations there has been little research on social influence in actual juries. We used Kenny's (1994) social relations model to examine jurors' perceptions of social influence in the jury. After rendering a verdict in criminal or civil court cases, jurors rated how influential each member of the jury had been and provided self-reports of their personality traits. Perceptions of influence in the jury were mostly in the eye of the beholder, with jurors high in Conscientiousness and low in Openness being most likely to report that they were personally influenced by other jurors. There were small but statistically significant levels of consensus in the ratings of how influential the jurors were. To the extent that they did agree, jurors rated extraverted, tall men as most influential.

RESUMO

Subacromial impingement syndrome is one of the most frequently seen causes of shoulder pain. The symptoms are often vague and may be difficult to differentiate from other shoulder problems. The purpose of this paper is to provide the clinician with a more thorough understanding of this entity by reviewing the pertinent anatomy and underlying etiology. Suggestions are made to aid in establishing the diagnosis, and treatment recommendations are proposed.

RESUMO

We prospectively and randomly compared the outcomes of the Chrisman-Snook and modified-Broström procedures for chronic lateral ankle instability in 40 patients. Both operations provided good or excellent stability in more than 80% of the patients. However, the modified-Broström procedure resulted in higher Sefton scores than the Chrisman-Snook procedure. In addition, a statistically significant greater proportion of complications occurred in patients treated with the Chrisman-Snook procedure.

To understand better the natural history of fractures of the fifth metatarsal, a fracture registry was established consisting of patients who had acute fractures of the fifth metatarsal. The first 100 patients were studied to develop data on the natural history of injuries to this bone, and on the results of standard treatment for those injuries. In this study, 3 fracture subtypes were identified: avulsion, true Jones' fracture, and shaft/neck fracture. Avulsion fractures healed without immobilization, usually in 4 weeks. Shaft/neck fractures healed in plaster casts with weight bearing as tolerated in 4 to 6 weeks. True Jones' fractures required a prolonged time to heal when treated with 8 weeks of nonweight bearing and then weight bearing as tolerated in a cast. This method was, however, successful in 72% of patients (average time until union, 21.2 weeks). For the 7 patients in whom conservative treatment failed, surgical fixation at an average of 25 weeks after injury reliably achieved bony union in half the time required with cast treatment. This low-risk procedure met with higher patient satisfaction than prolonged casting.

Food intake was weighed and recorded daily during one complete menstrual cycle in 18 healthy normally menstruating women. Urinary luteinizing hormone indicated the time of ovulation. Mean daily intakes of energy, macronutrients, and alcohol were calculated for five phases during the menstrual cycle: menses, postmenses, ovulatory, postovulatory, and premenses. Weekly variations were also measured. Energy intake was lowest during the ovulatory phase compared with postovulatory, premenses, and menses phases (p less than 0.05). The maximum difference, 1.36 MJ (324 kcal)/d, occurred between ovulatory and postovulatory phases and was twofold higher than the increase of 0.64 MJ (152 kcal)/d observed at weekends. This reduction of food intake at ovulation has not been previously described in humans. It coincides with the expected peak in circulating estrogen levels and is consistent with the hypothesis in animal models that estrogen is an appetite suppressant.

We compared the effects on the ratio of plasma tryptophan to large neutral amino acids (trp:LNAA) of two different carbohydrate meals (sucrose or starch, 120 g) and a contrasting meal of fat + protein given at breakfast to 10 healthy adults. Plasma glucose and insulin were also measured. The trp:LNAA ratio rose after both carbohydrate meals (p less than 0.001). Glucose and insulin peaks were higher after sucrose than starch, and trp:LNAA rose correspondingly higher (sucrose +34% and starch +20%, p less than 0.05). The ratio declined 45% after the fat + protein meal. At 180 min, absolute ratio values were twofold higher after carbohydrate (sucrose 0.133 and starch 0.127) than after fat + protein (0.057). Similar results were found with the same meals given in the evening. Our results suggest that high-carbohydrate meals have an influence on serotonin synthesis. We predict that carbohydrates with a high glycemic index would have a greater serotoninergic effect than carbohydrates with a low glycemic index.
