PURPOSE: To assess the safety and efficacy of transzonular moxifloxacin and dexamethasone versus standard postoperative topical drug regimen in phacoemulsification. DESIGN: Nonrandomized prospective study. METHODS: The study included 100 eyes of 100 age and gender-matched individuals with senile cataract undergoing routine phacoemulsification. The patients were consecutively divided into transzonular (TZ = 50) and topical (TP = 50) groups. Both the groups were followed up for 4 weeks and assessed for intraocular inflammation, visual acuity, changes in intraocular pressure (IOP), and any adverse events. RESULTS: The grades of inflammation were significantly lower in TZ as compared to the TP group ( P < 0.001). The IOP remained normal and comparable in both the groups. Most of the patients in the two groups attained a visual acuity of 0.2 or better at the end of the follow-up. No adverse effects and increased rate of endophthalmitis were noted in TZ group. CONCLUSION: A one-time peroperative TZ moxifloxacin and dexamethasone combination is a safe and effective method to control postoperative inflammation after cataract surgery. A word of caution though, due precautions to be exercised to prevent the risk of inflammation and endophthalmitis.

The rare earth metal, samarium (Sm3+) doped bismuth tungstate (Bi2WO6) nanoparticles were prepared by a one-pot hydrothermal method. The powder X-ray diffraction (XRD) analysis confirmed the formation of Bi2WO6 with an orthorhombic crystal structure. The crystallite size of Bi2WO6 decreased from 20.73 to 9.25 nm as the Sm substitution in the W lattice increased. The vibrational modes of W-O, Bi-O, and Sm-O were identified in the range of 500-900 cm-1. The optical bandgap of Sm3+ doped Bi2WO6 nanoparticles increased from 2.86 to 2.95 eV with higher Sm doping levels. The surface morphology revealed the formation of flower-like sheets in the Sm3+ doped bismuth tungstate. The energy dispersive X-ray (EDX) spectrum of Sm3+ doped Bi2WO6 nanoparticles confirmed the presence of Sm, Bi, W, and O without any other impurities. The small peak detected at 1082.14 eV in the survey scan of Sm3+ doped Bi2WO6 nanoparticles belonged to Sm3d. In the photocatalytic degradation of direct blue 15 (DB15) under visible light irradiation, the efficiency of the nanoparticles increased with higher Sm3+ concentration. The obtained results demonstrated that the Sm-Bi2WO6 nanosheets could provide an effective and sustainable solution for treating the wastewater containing direct blue 15 dye.

Fillers or particulate fillers find a growing utilization as reinforcement material in polymer composites due to their ability to enhance the properties of the ensuing composites. The discarded seed in sapodilla fruit is available in abundant and the shell of the seed can be used as a reinforcing filler. The primary goal of this study is to extract and characterize the sapodilla seed shell powder (SSS) physically and chemically in order to assess its potential for reinforcement as a particulate filler in polymer composites. The sapodilla seed shell filler was characterized experimentally by Physio-chemical analysis, X-ray diffraction (XRD), Fourier transform infrared spectroscopy (FTIR) and Energy dispersive X-ray analysis (EDAX). The morphology and the filler size were determined by Scanning electron microscopy (SEM) and Particle size analysis. The thermal degradation behaviour was evaluated by Thermogravimetric analysis (TGA).

Purpose: To describe the long-term outcomes of transcutaneous retrobulbar amphotericin B (TRAMB) in COVID-19-associated mucormycosis. Methods: In total, 18 cases of COVID-19-associated mucormycosis were reviewed. In addition to the recommended treatment protocol, all patients were to be given 3.5 mg/ml/day of TRAMB for five days. Results: Of the 18 patients, 2 presented with stage 3a disease, 13 had stage 3c disease, and 3 patients had central nervous system (CNS) involvement (stage 4a and 4c). In addition to planned retrobulbar doses, five patients were given more while two patients received fewer injections (i.e., <5). At the last mean follow-up of 34.67 (±8.88) weeks, 11 patients were in radiological regression and 4 had stable disease while 2 patients had to undergo exenteration; one mortality was observed because of disease progression. Clinical regression in terms of visual and ptosis improvement was seen in seven and nine patients, respectively. Conclusion: Rhino-orbito-cerebral mucormycosis is a serious condition which warrants an aggressive treatment strategy. In unprecedented situations witnessed recently, TRAMB turned out to be an effective and economical alternative. Though large randomized studies are needed to establish its efficacy, TRAMB still manages to halt progression and salvage the globe in significant number of patients, and hence its use should be encouraged on a case-to-case basis especially in developing countries with limited resources.

Purpose@#With advancing endovascular technology and increasing interest in minimally invasive intra-arterial therapies such as stem cell and chemotherapy for cerebral disease, the establishment of a translational model with cerebral circulation accessible to microcatheters is needed. We report our experience catheterizing canine cerebral circulation with microcatheters, present high-resolution angiographic images of the canine vascular anatomy, describe arterial branch flow patterns and provide measurements of canine arterial conduits. @*Materials and Methods@#Angiograms were performed on 10 intact purpose-bred hounds. Angiography, measurements of arterial conduits and catheterization information for intracranial arterial branches were obtained. @*Results@#Selective and superselective cerebral angiography was successful in all subjects. Relevant arterial mean diameters include the femoral (4.64 mm), aorta (9.38 mm), external carotid (3.65 mm), internal carotid arteries (1.6 mm), vertebrobasilar system and Circle of Willis branches. Catheterization of the Circle of Willis was achieved via the posterior circulation in all subjects tested (n=3) and the use of flow directed microcatheters resulted in reduced arterial tree deformation and improved superselection of intracranial vessels. Catheterization of the intracranial circulation was attempted but not achieved via the internal carotid artery (n=7) due to its tortuosity and subsequent catheter related vasospasm. @*Conclusion@#The canine cerebral vasculature is posterior circulation dominant. Anterior circulation angiography is achievable via the internal carotid artery, but direct cerebral arterial access is best achieved via the posterior circulation using flow-directed microcatheters. It is feasible to deliver intra-arterial therapies to selective vascular territories within the canine cerebral circulation, thus making it a viable animal model for testing novel intra-arterial cerebral treatments.

Children with Autism Spectrum Disorder (ASD) have basic motor impairments in balance, gait, and coordination as well as autism-specific impairments in praxis/motor planning and interpersonal synchrony. Majority of the current literature focuses on isolated motor behaviors or domains. Additionally, the relationship between cognition, symptom severity, and motor performance in ASD is unclear. We used a comprehensive set of measures to compare gross and fine motor, praxis/imitation, motor coordination, and interpersonal synchrony skills across three groups of children between 5 and 12 years of age: children with ASD with high IQ (HASD), children with ASD with low IQ (LASD), and typically developing (TD) children. We used the Bruininks-Oseretsky Test of Motor Proficiency and the Bilateral Motor Coordination subtest of the Sensory Integration and Praxis Tests to assess motor performance and praxis skills respectively. Children were also examined while performing simple and complex rhythmic upper and lower limb actions on their own (solo context) and with a social partner (social context). Both ASD groups had lower gross and fine motor scores, greater praxis errors in total and within various error types, lower movement rates, greater movement variability, and weaker interpersonal synchrony compared to the TD group. In addition, the LASD group had lower gross motor scores and greater mirroring errors compared to the HASD group. Overall, a variety of motor impairments are present across the entire spectrum of children with ASD, regardless of their IQ scores. Both, fine and gross motor performance significantly correlated with IQ but not with autism severity; however, praxis errors (mainly, total, overflow, and rhythmicity) strongly correlated with autism severity and not IQ. Our study findings highlight the need for clinicians and therapists to include motor evaluations and interventions in the standard-of-care of children with ASD and for the broader autism community to recognize dyspraxia as an integral part of the definition of ASD.

RESUMO

Several anatomical variables critically influence therapeutic strategizing for anterior choroidal artery (AChA) aneurysms, and specifically, the safety of flow diversion for these lesions. We review the microsurgical anatomy of the AChA, discussing and detailing these considerations in the treatment of AChA aneurysms, theoretically and in the light of our recent findings.

Mobile phone induced electromagnetic field (MPEMF) as well as chanting of Vedic mantra 'OM' has been shown to affect cognition and brain haemodynamics, but findings are still inconclusive. Twenty right-handed healthy teenagers (eight males and 12 females) in the age range of 18.25 ± 0.44 years were randomly divided into four groups: (1) MPONOM (mobile phone 'ON' followed by 'OM' chanting); (2) MPOFOM (mobile phone 'OFF' followed by 'OM' chanting); (3) MPONSS (mobile phone 'ON' followed by 'SS' chanting); and (4) MPOFSS (mobile phone 'OFF' followed by 'SS' chanting). Brain haemodynamics during Stroop task were recorded using a 64-channel fNIRS device at three points of time: (1) baseline, (2) after 30 min of MPON/OF exposure, and (3) after 5 min of OM/SS chanting. RM-ANOVA was applied to perform within- and between-group comparisons, respectively. Between-group analysis revealed that total scores on incongruent Stroop task were significantly better after OM as compared to SS chanting (MPOFOM vs MPOFSS), pre-frontal activation was significantly lesser after OM as compared to SS chanting in channel 13. There was no significant difference between MPON and MPOF conditions for Stroop performance, as well as brain haemodynamics. These findings need confirmation through a larger trial in future.

BACKGROUND: Mindfulness along with breathing is a well-established meditation technique. Breathing is an exquisite tool for exploring subtle awareness of mind and life itself. AIM: This study aimed at measuring changes in the different parameters of electrophotonic imaging (EPI) in anapanasati meditators. MATERIALS AND METHODS: To carry out this study, 51 subjects comprising 32 males and 19 females of age 18 years and above (mean age 45.64 ± 14.43) were recruited voluntarily with informed consent attending Karnataka Dhyana Mahachakra-1 at Pyramid Valley International, Bengaluru, India. The design was a single group pre- post and data collected by EPI device before and after 5 days of intensive meditation. RESULTS: Results show significant changes in EPI parameter integral area with filter (physiological) in both right and left side, which reflects the availability of high functional energy reserve in meditators. The researchers observed similar trends without filter (psycho-physiological) indicating high reserves of energy at psycho-physiological level also. Activation coefficient, another parameter of EPI, reduced showing more relaxed state than earlier, possibly due to parasympathetic dominance. Integral entropy decreased in the case of psycho-physiological parameters left-side without filter, which indicates less disorder after meditation, but these changes were not significant. The study showed a reversed change in integral entropy in the right side without filter; however, the values on both sides with filter increased, which indicates disorder. CONCLUSION: The study suggests that EPI can be used in the recording functional physiological and psychophysiological status of meditators at a subtle level.
