BACKGROUND: Feedback is an essential tool for learning and improving performance in any sphere of education, including training of resident physicians. The learner's perception of the feedback they receive is extremely relevant to their learning progress, which must aim at providing qualified care for patients. Studies pertinent to the matter differ substantially with respect to methodology, population, context, and objective, which makes it even more difficult to achieve a clear understanding of the topic. A scoping review on this theme will unequivocally enhance and organize what is already known. OBJECTIVE: The aim of this study is to identify and map out data from studies that report surgical residents' perception of the feedback received during their education. METHODS: The review will consider studies on the feedback perception of resident physicians of any surgical specialty and age group, attending any year of residency, regardless of the type of feedback given and the way the perceptions were measured. Primary studies published in English, Spanish, and Portuguese since 2017 will be considered. The search will be carried out in 6 databases and reference lists will also be searched for additional studies. Duplicates will be removed, and 2 independent reviewers will screen the selected studies' titles, abstracts, and full texts. Data extraction will be performed through a tool developed by the researchers. Descriptive statistics and qualitative analysis (content analysis) will be used to analyze the data. A summary of the results will be presented in the form of diagrams, narratives, and tables. RESULTS: The findings of this scoping review were submitted to an indexed journal in July 2024, currently awaiting reviewer approval. The search was executed on March 15, 2024, and resulted in 588 articles. After the exclusion of the duplicate articles and those that did not meet the eligibility criteria as well as the inclusion of articles through a manual search, 13 articles were included in the review. CONCLUSIONS: Conducting a scoping review is the best way to map what is known about a subject. By focusing on the feedback perception more than the feedback itself, the results of this study will surely contribute to gaining a deeper understanding of how to proceed to enhance internal feedback and surgical residents' learning progress. Motivation: The proliferation of genetic testing and consumer genomics represents a logistic challenge to the personalized use of GWAS data in VCF format. Specifically, the challenge of retrieving target genetic variation from large compressed files filled with unrelated variation information. Compounding the data traversal challenge, privacy-sensitive VCF files are typically managed as large stand-alone single files (no companion index file) composed of variable-sized compressed chunks, hosted in consumer-facing environments with no native support for hosted execution. Results: A portable JavaScript module was developed to support in-browser fetching of partial content using byte-range requests. This includes on-the-fly decompressing irregularly positioned compressed chunks, coupled with a binary search algorithm iteratively identifying chromosome-position ranges. The in-browser zero-footprint solution (no downloads, no installations) enables the interoperability, reusability, and user-facing governance advanced by the FAIR principles for stewardship of scientific data. BACKGROUND: Paracoccidioidomycosis (PCM) is the leading cause of death among systemic mycoses in Brazil. On the other hand, oral squamous cell carcinoma (OSCC) is the most prevalent malignant neoplasm of the mouth. Both lesions rarely affect the tongue dorsum and may share similar clinical characteristics. This study aimed to retrieve cases of single oral ulcers diagnosed as PCM or OSCC. MATERIAL AND METHODS: A cross-sectional retrospective study was conducted. All patients who had a single ulcer on dorsum of the tongue and confirmed diagnosis of PCM or OSCC were evaluated. RESULTS: A total of 9 patients (5 women and 4 men) were evaluated, 5 patients had OSCCs (mean age = 69,8 years old), and 4 patients PCM (mean age = 51 years old). Most of the lesions were infiltrated and indurated in the palpation exam. Duration ranged from 1 to 12 months (mean time of 5.2 months and 4.7 months for OSCC and PCM, respectively). OSCC was the main clinical diagnosis hypothesis. CONCLUSIONS: Although uncommon, PCM and OSCC should be considered as a diferential diagnosis hypothesis in infiltrated ulcers on the tongue dorsum. Iincisional biopsy is mandatory to confirm the diagnosis and indicate the appropriate treatment.

RESUMO

Antifungal infections are becoming a major concern to human health due to antimicrobial resistance. Echinocandins have been promising agents against resistant fungal infections, primarily caspofungin, which has a more effective mechanism of action than azoles and polyenes. However, fungi such as Cryptococcus neoformans appear to be inheritably resistant to these drugs, which is concerning due to the high clinical importance of C. neoformans. In this review, we review the history of C. neoformans and the treatments used to treat antifungals over the years, focusing on caspofungin, while highlighting the C. neoformans problem and possible explanations for its inherent resistance.


Caspofungin is a drug used to treat several types of fungal infections. Resistance to caspofungin is a huge problem, especially in those that are immunocompromised. It is important to understand the history of caspofungin discovery, its clinical applications and its mechanism of action, as well as if a new drug target could be used overcome resistance. This review may perform guide new studies combining caspofungin with other drugs and indicate new potential targets for caspofungin.

Apurinic/apyrimidinic endonuclease 1/redox factor-1 (APE1/REF-1) is a multifunctional protein acting on cellular signaling pathways, including DNA repair and redox activities. APE1/REF-1 has emerged as a target for cancer therapy, and its role in breast cancer models would reveal new strategies for cancer therapy. APX2009 is a specific APE1/REF-1 redox inhibitor whose anticancer properties have not been described in breast cancer cells. Here, we investigated the effect of the APX2009 treatment in the breast cancer cell lines MDA-MB-231 and MCF-7. Breast cancer cell lines were cultured, and WST1 and colony formation assays were performed to evaluate cell proliferation. Annexin V-FITC/7-AAD and LDH-Glo™ assays were performed to evaluate cell death. The wound healing assay and Matrigel transwell assay were performed after APX2009 treatment to evaluate the cellular migration and invasion processes, respectively. Our findings demonstrated that APX2009 treatment decreased breast cancer cell proliferative, migratory, and invasive properties. Furthermore, it induced apoptosis in both cell lines. Our study is the first to show the effects of APX2009 treatment on apoptosis in a breast cancer cell. Therefore, this study suggested that APX2009 treatment is a promising anticancer molecule for breast cancer.

Abstract Acute diarrhea is the second leading cause of morbidity and mortality attributed to infections in children under five years of age worldwide, with 1.7 million annual estimated cases and more than 500,000 deaths. Although hydroelectrolytic replacement is the gold standard in treating diarrhea, it does not interfere with the restoration of the intestinal microbiota. Several studies have searched for an adequate alternative in restructuring intestinal homeostasis, finding that treatments based on probiotics, prebiotics, and synbiotics are effective, which made such treatments increasingly present in clinical practice by reducing illness duration with minimal side effects. However, there are still controversies regarding some unwanted reactions in patients. The diversity of strains and the peculiarities of the pathogens that cause diarrhea require further studies to develop effective protocols for prevention and treatment. Here, we provide a descriptive review of childhood diarrhea, emphasizing treatment with probiotics, prebiotics, and synbiotics.

Objective: To investigate whether having a higher number of depressive symptoms is associated with negative self-rated health (SRH) even in the absence of illness. Methods: This is a secondary analysis of baseline data from the Brazilian Longitudinal Study of Aging (ELSI-Brazil), conducted in 2015-2016, using a national sample of 9,412 people aged 50 or over. SRH was dichotomized into poor or very poor and very good or excellent, good, or average. Depressive symptoms were assessed through the eight-item Center for Epidemiologic Studies Depression Scale (CES-D8). Sociodemographic variables, information about unhealthy behaviors, and the number of chronic conditions were also analyzed. Results: Having depressive symptoms was strongly associated with poor or very poor SRH both in the unadjusted and adjusted analyses. The magnitude of the association was reduced when the number of chronic illnesses was included in the multivariate analysis, along with the other sociodemographic variables and unhealthy behaviors (OR 1.35, 95%CI 1.31-1.39). Conclusion: Having depressive symptoms may contribute towards having a poorer perception of health, even in the absence of health conditions. SRH is a multidimensional construct that can accurately reflect a person's state of general mental health.

Foot and mouth disease is a highly contagious viral disease that poses a significant economic threat to cloven-hoofed animals, including cattle and sheep. The emergence of a novel foot and mouth disease virus-A isolate, FMDV-A-Egy-AHRI-RL385-Ven-2022, in Egypt in 2022 has raised concerns about its potential impact on existing vaccination programs. Given that vaccination is a key strategy for foot and mouth disease virus control, the present study was aimed to assess the cross-protective efficacy of both local and imported inactivated vaccines against this new threat. Through challenge experiments and serum neutralization tests, we observed limited effectiveness of both vaccine types. The calculated r1-values at 28 days post-vaccination indicated a minimal immune response to FMDV-A-Egy-AHRI-RL385-Ven-2022 (0.176 and 0.175 for local and imported vaccines, respectively). Challenge experiments further confirmed these findings, revealing 0percent protection from the local vaccine and only 20percent rotection from imported vaccines by day 7 post-challenge. These results underscore the urgent need to update existing foot and mouth disease virus vaccines in Egypt by incorporating the newly circulating FMDV-A-Egy-AHRI-RL385-Ven-2022 strain. This proactive measure is crucial to prevent future outbreaks and ensure effective disease control(AU)


La fiebre aftosa es una enfermedad vírica muy contagiosa que supone una importante amenaza económica para los animales biungulados, entre ellos el ganado vacuno y ovino. La aparición de un nuevo aislado del virus A de la fiebre aftosa, el FMDV-A-Egy-AHRI-RL385-Ven-2022, en Egipto en 2022 ha suscitado preocupación por su posible impacto en los programas de vacunación existentes. Dado que la vacunación es una estrategia clave para el control del virus de la fiebre aftosa, el objetivo del presente estudio fue evaluar la eficacia protectora cruzada de las vacunas inactivadas locales e importadas frente a esta nueva amenaza. Mediante experimentos de desafío y pruebas de seroneutralización, observamos una eficacia limitada de ambos tipos de vacuna. Los valores r1 calculados a los 28 días posvacunación indicaron una respuesta inmunitaria mínima frente a FMDV-A-Egy-AHRI-RL385-Ven-2022 (0,176 y 0,175 para las vacunas local e importada, respectivamente). Los experimentos de provocación confirmaron aún más estos resultados, revelando un 0 por ciento de protección de la vacuna local y sólo un 20 por ciento de protección de las vacunas importadas al séptimo día después de la provocación. Estos resultados subrayan la urgente necesidad de actualizar las vacunas existentes contra el virus de la fiebre aftosa en Egipto incorporando la nueva cepa circulante FMDV-A-Egy-AHRI-RL385-Ven-2022. Esta medida proactiva es crucial para prevenir futuros brotes y garantizar un control eficaz de la enfermedad(AU)

OBJECTIVE: The objective of this study was to assess the impact of internal medicine consultation on mortality, 30-day readmission, and length of stay in surgical patients. METHODS: This is a retrospective descriptive study developed in a public Brazilian teaching hospital with 850 beds. RESULTS: A total of 70,245 patients were admitted from 2010 to 2018 to the surgery departments. The main outcomes measured were patients' mortality, 30-day readmission, and length of stay. Mortality of high-risk patients was lower when followed by internal medicine consultation: patients with ASA≥3 (RR 0.89 [95% confidence interval (95%CI) 0.80­0.99], p=0.02), patients with ASA≥3 plus≥65 years (RR 0.88 [95%CI 0.78­0.99], p=0.04), patients with ASA≥3 plus high-risk surgery (RR 0.86 [95%CI 0.77­0.97], p=0.01), and patients with ASA≥4 plus age ≥65 years (RR 0.83 [95%CI 0.72­0.96], p=0.01). The 30-day readmission of high-risk patients was lower when followed by internal medicine consultation: patients with ≥65 years (RR 0.57 [95%CI 0.37­0.89], p=0.01) and patients with high-risk surgery (RR 0.63 [95%CI 0.46­0.57], p=0.005). The Poisson multivariate regression with adjustment in variances showed that all the variables (namely, age, ASA, morbidity index, surgery risk, and internal medicine consultation) were associated with higher mortality of patients; however, internal medicine consultation was associated with a reduction of mortality in high-risk patients (RR 0.72 [95%CI 0.65­0.84], p=0.02) and an increase of mortality in low-risk patients (RR 1.55 [95%CI 1.31­1.67], p=0.01). CONCLUSION: High-risk surgical patients may benefit from perioperative internal medicine consultations, which probably decrease hospital mortality and 30-day hospital readmission.

ABSTRACT In order to evaluate the effect of total parenteral nutrition associated with glutamine and enteral fluid therapy, associated or not with glutamine, on the lipidogram of horses subjected to starvation (phase 1) and refeeding (phase 2), 16 adult healthy horses were used, mixed breed, aged between four and fourteen years and average weight of 248.40 ± 2.28kg, divided into four experimental groups, with four animals per group: Group ENTGL: enteral fluid therapy with electrolytes associated with glutamine; Group PARGL: total parenteral nutrition associated with glutamine; Group ENTFL: enteral fluid therapy with electrolytes; and Group PARFL: parenteral fluid therapy. This study was divided into two phases: phase 1 and phase 2. Phase 1 consisted of performing exploratory laparotomy and beginning of starvation, in addition to the administration of treatments, according to the group, while phase 2 consisted of re-feeding the animals. Total cholesterol values remained within the normal range for the species throughout the experimental period, but there was a slight increase during phase 1, followed by a decrease in values in phase 2. Hypertriglyceridemia in the ENFL, ENTGL and PARFL groups during phase 1 occurred because of the negative energy balance. The provision of enteral fluid therapy with electrolytes and an energy source, with or without glutamine, or parenteral fluid therapy were not effective in reversing the effects of negative energy balance. Thus, therapeutic protocols that combine enteral or parenteral fluid therapy for prolonged periods and food deprivation need additional nutritional support to avoid the deleterious effects of negative energy balance.


RESUMO Com o objetivo de avaliar o efeito da nutrição parenteral total associada à glutamina e da fluidoterapia enteral, associada ou não à glutamina, sobre o lipidograma de equinos submetidos à inanição (fase 1) e realimentação (fase 2), foram utilizados 16 equinos adultos hígidos, sem raça definida, com idade variando entre quatro e 14 anos e peso médio de 248,40 ± 2,28kg, divididos em quatro grupos experimentais, quatro animais por grupo: grupo ENTGL: fluidoterapia enteral com eletrólitos associada à glutamina; grupo II PARGL: nutrição parenteral total associada à glutamina; grupo ENTFL: fluidoterapia enteral com eletrólitos; e grupo PARFL: fluidoterapia parenteral. Este estudo foi dividido em duas fases: fase 1 e fase 2. A fase 1 consistiu na realização da laparotomia exploratória e no início da inanição, além da administração dos tratamentos, de acordo com o grupo; a fase 2 consistiu na realimentação dos animais. Os valores de colesterol total se mantiveram dentro da faixa de normalidade para a espécie durante todo o período experimental, porém houve discreta elevação ao longo da fase 1, seguida de diminuição dos valores na fase 2. Hipertrigliceridemia nos grupos ENTFL, ENTGL e PARFL durante a fase 1 ocorreu em decorrência do equilíbrio energético negativo. O fornecimento de fluidoterapia enteral com eletrólitos e uma fonte de energia, associada ou não à glutamina, ou o de fluidoterapia parenteral não foram efetivos em reverter os efeitos do equilíbrio energético negativo. Dessa forma, protocolos terapêuticos que associem fluidoterapia enteral ou parenteral por períodos prolongados e privação alimentar necessitam de suporte nutricional adicional para evitar os efeitos deletérios do equilíbrio energético negativo

ABSTRACT This study aimed to evaluate the addition of different concentrations of IGF-I and insulin to egg yolk-based extender to improve bovine semen cryopreservation. Two experiments were developed to evaluate the effects of the additives in two commercial extenders, Botubov® (Experiment 1) and Triladyl® (Experiment 2), both with the same design. Three ejaculates from four bulls (n = 12) were used. Each ejaculate was divided into seven equal fractions for dilution (60x106 spermatozoa/mL) in the following treatments: CON: extender only; IGF100: IGF-I 100ng/mL; IGF200: IGF-I 200ng/mL; INS150: insulin 150µUI/mL; INS200: insulin 200µUI/mL; ASS1: IGF-I 100ng/mL + insulin 150µUI/mL; ASS2: IGF-I 200ng/mL + insulin 200µUI/mL. Semen was cryopreserved by an automated system. Post-thawed sperm were evaluated regarding motility by CASA (Computer-assisted sperm analysis), and membranes by fluorescent probes (H342, PI, FITC-PSA and JC-1). For Botubov® extender, INS150 was more efficient in preserving total and progressive motility, VCL, BCF, plasma and mitochondrial membranes. A similar response was seen when insulin was added to the Triladyl® extender, INS150 was more efficient in preserving sperm motility, plasma membrane integrity and mitochondrial potential. Thus, the addition of insulin 150µUI/mL, regardless of the composition of the extender, contributes to better preserving bovine sperm from the cryopreservation effects.


RESUMO Este estudo teve o objetivo de avaliar a adição de diferentes concentrações de IGF-I e insulina a diluidores, à base de gema de ovo, para melhorar a criopreservação do sêmen bovino. Dois experimentos foram desenvolvidos para avaliar os efeitos dos aditivos em dois diluidores comerciais: Botubov® (Experimento 1) e Triladyl® (Experimento 2), ambos com o mesmo delineamento. Foram utilizados três ejaculados de quatro touros (n=12). Cada ejaculado foi dividido em sete frações para diluição (60x106espermatozoides/mL), nos seguintes tratamentos: CON: somente diluidor; IGF100: 100ng/mL de IGF-I; IGF200: 200ng/mL de IGF-I; INS150: 150µUI/mL de insulina; INS200: 200µUI/mL de insulina; ASS1: 100ng/mL de IGF-I + 150µUI/mL de insulina; ASS2: 200ng/mL de IGF-I + 200µUI/mL de insulina. O sêmen foi criopreservado por sistema automatizado. Após a criopreservação, o sêmen foi avaliado quanto à motilidade espermática por CASA e quanto às membranas espermáticas (plasmática, acrossomal e mitocondrial) por sondas fluorescentes (H342, PI, FITC-PSA e JC-1). Para o diluidor Botubov®, INS150 foi mais eficiente em preservar motilidades total e progressiva, VCL, BCF, integridade da membrana plasmática e potencial mitocondrial. Resposta semelhante foi observada quando a insulina foi adicionada ao diluidor Triladyl®, INS150 foi mais eficiente na preservação da motilidade, integridade das membranas e potencial mitocondrial quando comparado aos demais grupos. Assim, a adição de 150µUI/mL de insulina aos diluidores, independentemente da composição, contribui para melhor criopreservação dos espermatozoides bovinos.

Objetivo: analisar o perfil epidemiológico da violência física e sexual contra crianças menores de cinco anos, no Piauí, entre 2014 e 2018. Métodos e Materiais: estudo descritivo, transversal e retrospectivo, que utilizou dados secundários do Sistema de Informação de Agravos de Notificação, no período de 2014 a 2018, extraídos do site do Departamento de Informática do Sistema Único de Saúde (DATASUS). Resultados: foram registrados 406 casos de violência física e 381 de violência sexual. Crianças com idade entre um e quatro anos predominaram entre os casos de violência física (64,29%) e sexual (85,30%). Quanto ao perfil, observou-se predomínio da raça parda (70,78%) e do sexo feminino (69,89%). Os principais agressores foram a mãe, na violência física (50,0%), e amigo/conhecido da família, na violência sexual (40,51%). A residência configurou o local predominante nos casos de violência física (71,92%) e sexual (70,08%). Conclusão: a compreensão sobre a epidemiologia dos tipos de violência em crianças torna-se importante na avaliação dos programas e políticas já estabelecidas.


Objective: to analyze the epidemiological profile of physical and sexual violence against children under five years of age, in Piauí, between 2014 and 2018. Methods and Materials: descriptive, cross-sectional and retrospective study, which used secondary data from the Notifiable Diseases Information System, in the period from 2014 to 2018, extracted from the website of the Department of Informatics of the Unified Health System (DATASUS). Results: 406 cases of physical violence and 381 of sexual violence were registered. Children aged between one and four years predominated among cases of physical (64.29%) and sexual (85.30%) violence. As for the profile, there was a predominance of brown race (70.78%) and females (69.89%). The main aggressors were the mother, in physical violence (50.0%), and a friend/acquaintance of the family, in sexual violence (40.51%). Home was the predominant location in cases of physical (71.92%) and sexual (70.08%) violence. Conclusion: understanding the epidemiology of types of violence against children becomes important in evaluating established programs and policies.

We compared the virulence profile and REP-PCR genotypes of Escherichia coli strains isolated from subclinical and clinical mastitis cases and dairy farm environments in Minas Gerais State, Brazil, to determine virulence factors and genotypes potentially associated with subclinical persistence in the udder. The virulence profile was obtained by the search for three virulence genes: lpfA (long polar fimbriae), fliC (flagella), and escN (type III secretion system). Subclinical isolates exhibited mainly the fliC gene (33.33%) and fliC + escN genes (30.30%). Clinical isolates exhibited mainly fliC + escN genes (50%) and environmental isolates the lpfA + escN genes (58.04%). Strains isolated from subclinical mastitis showed 6.75 times more positivity to fliC than environmental isolates. Thirty-four genotypes were observed in the REP-PCR analysis, and clinical mastitis isolates indicated more genetic proximity to dairy farm environment isolates than subclinical mastitis isolates. In conclusion, the results suggested that flagella may be an important virulence factor for mammary persistent E. coli infection in cattle, however, none of the E. coli REP-PCR genotypes were associated with subclinical infection.

Los riesgos asociados a la neumonía por (SARS-CoV-2) es la generación de insuficiencia respiratoria secundaria que en algunos casos desencadenara al tan temido síndrome de distres respiratorio (SDRA); Informes sobre atención clínica, indican que tiene una incidencia (SDRA) de 3-10 % con necesidad de Asistencia Respiratoria Mecánica (ARM) en pacientes hospitalizados; por lo que dispositivos de oxigenación no invasivos siguen siendo una opción atractiva, de forma inicial. Caso clínico: mujer de 47 años con insuficiencia respiratoria secundario a neumonía por COVID-19, por la gravedad se indica su ingreso a terapia intensiva, pero por razones de falta de unidad es manejada en unidad respiratoria, con el uso de dispositivos de oxigenación de armado ARTESAL, de manera exitosa, con la utilización de CNAF-artesanal, se pretende mejorar el trabajo respiratorio, índices de oxigenación, mientras se da tratamiento a la infección por el COVID-19; el objetivo del presente caso es reportar el presente caso con evolución favorable a la literatura disponible. Discusión: El uso de terapia de oxigenación con dispositivo de Cánula Nasal de Alto Flujo, aún no ha sido normatizado en pacientes con COVID-19, pero existe evidencia clínica sobre los efectos beneficiosos en la insuficiencia respiratoria en neonatos mas no en adultos. Conclusión: El uso temprano de la CNAF-artesanal en la insuficiencia respiratoria resulta muy atractivo, más aún con dispositivo de confección artesanal, da una opción más al paciente fuera de UTI, pudiendo apoyar en evitar la intubación y su ingreso a ventilación mecánica.


The risks associated with pneumonia (SARS-CoV-2) is the generation of secondary respiratory failure that in some cases will trigger the much feared respiratory distress syndrome (ARDS); Reports on clinical care indicate that it has an incidence (ARDS) of 3-10% with the need for Mechanical Respiratory Assistance (ARM) in hospitalized patients; so non-invasive oxygenation devices remain an attractive option, initially. Clinical case: a 47-year-old woman with respiratory failure secondary to covid-19 pneumonia. Due to the severity, her admission to intensive care is indicated, but for reasons of lack of unity, she is managed in a common room, with the use of high-pressure oxygenation devices. ARTISAL assembly, successfully, with the use of CNAF-artisanal, is intended to improve the work of breathing, and oxygenation indices, while treating the infection by COVID-19; The objective of this case is to report the present case with a favorable evolution based on the available literature. Discussion: The use of oxygenation therapy with a High Flow Nasal Cannula device has not yet been standardized in patients with COVID-19, but there is clinical evidence on the beneficial effects in respiratory failure in neonates but not in adults. Conclusion: The early use of the artisan HFNC in respiratory failure is very attractive, even more so with an artisanal device, it gives the patient another option outside the ICU, being able to help avoid intubation and admission to mechanical ventilation.

Aim: The dose response in growth inhibition of Staphylococcus aureus treated with colloidal copper oxide nanoparticles (CuO-NP) was evaluated. Methods: An in vitro microbial viability assay was conducted with CuO-NP concentrations spreading over the 0.4-848.0 µg/ml range. The dose-response curve was modeled with a double Hill equation. UV-Visible absorption and photoluminescence spectroscopies allowed tracking concentration-dependent modifications in CuO-NP. Results: Two specific phases separated by the critical concentration of 26.5 µg/ml were observed in the dose-response curve, with each exhibiting proper IC50 parameters, Hill coefficients and relative amplitudes. Spectroscopy techniques reveal the occurrence of a concentration-triggered aggregation of CuO-NP starting from this critical concentration. Conclusion: The findings demonstrate a dose-related change in S. aureus sensitivity to CuO-NP, which probably arises from the aggregation of this agent.


Antibacterial agents are often used to stop the growth of bacteria such as Staphylococcus aureus (S. aureus). Copper oxide nanoparticles (CuO-NP) stand as a promising candidate for this purpose. Generally, the agent´s effectiveness is inspected following a dose-response curve in which de agent´s antibacterial response is plotted against its dose (concentration). In this work, employing an extended mathematical interpretation we were capable of discerning experimentally the existence of two stages of dose-response (biphasic dose-response) in the treatment of S. aureus with CuO-NP. These results in combination with insights from spectroscopic techniques lead to the understanding that the biphasic behavior arises from the aggregation of CuO-NP at high concentrations. Therefore, according to the adopted concentration to treat S. aureus, the agent can behave as a dispersed nanoparticle or as an aggregated nanoparticle. In summary, understanding whether antibacterial agents transform as a function of concentration is important in determining their practical applications.

Aim: We examined whether ADIPOQ (rs266729 and rs1501299) and NOS3 (rs3918226 and rs1799983) SNPs or the haplotypes formed by them, affect blood pressure (BP) control in 196 patients with adherence to antihypertensive therapy grouped into controlled (BP <140/90 mmHg) and uncontrolled (BP ≥140/90 mmHg) hypertension. Materials & methods: The average of the three most recent BP measurements was retrieved from the patients' electronic medical records. Adherence to antihypertensive therapy was evaluated using the Morisky-Green test. Haplotype frequencies were estimated using Haplo.stats. Multiple logistic/linear regression analyses were adjusted for the covariates ethnicity, dyslipidemia, obesity, cardiovascular disease and uric acid. Results: ADIPOQ rs266729 genotypes CG (additive model) and CG+GG (dominant model) were associated with uncontrolled hypertension and CG was associated with higher systolic BP and mean arterial pressure (p < 0.05). ADIPOQ haplotypes 'GT' and 'GG' were associated with uncontrolled hypertension and 'GT' was associated with higher diastolic BP and mean arterial pressure (p < 0.05). Conclusion: ADIPOQ SNPs and haplotypes affect BP control in hypertensive patients undergoing treatment.

Resumen La hidradenitis supurativa (HS) es una enfermedad dolorosa y crónica, que afecta en especial la unidad folículo-pilosebácea de la piel ubicada en ingle, axilas, región perianal, perineo, genitales y submamaria,regiones anatómicas donde se encuentran glándulas sudoríparas apocrinas. El curso clínico de la HS es heterogéneo pues varía desde formas muy graves con abscesos fluctuantes profundos y drenajes eventuales; concicatricesresiduales graves, hasta otra forma de enfermedad comparativamente leve caracterizada por la aparición de algunos nódulos inflamatorios,pústulas ypápulas,de manera recidivante. Comunicamos el caso de un niño de 12 añosde edad, con diagnóstico de hidradenitis supurativa,quien fue tratado con Adalimumab. Realizamos la revisión del estado de arte de esta patología, describimos sus características clínicas, criterios diagnósticos, diagnósticos diferenciales y los posibles tratamientos. Nuestra presentación, tiene por objeto relatar nuestra experiencia en el seguimiento del caso del paciente, y las vicisitudes diagnósticas al respecto, que variaron desde acné inflamatorio grave hasta finalmente arribar al diagnóstico de certeza de hidradenitis supurativa, basándonos en criterios clínicos y ecográficos.Consideramos de interés haber podido emplear el "agente biológico" inhibidordel factor de necrosis tumoral (FNT) que hemos mencionado, con excelente respuesta.


Abstract Hidradenitis suppurativa (HS) is a painful and chronic disease that particularlyaffects the follicle-pilosebaceous unit of the skin located in the groins, armpits, perianal region, perineum, genitals, and submammary glands, which areanatomical regions whereapocrine sweat glands are found. The clinical course of HS is heterogeneous as it varies from very severe forms with deep fluctuating abscesses and eventual drainage; with severe residual scars, to another form ofacomparatively mild condition characterized by the presence of some recurrent inflammatory nodules, pustules and papules as well. We report the case of a 12-year-old boy, diagnosed with hidradenitis suppurativa, who was treated with Adalimumab. We reviewed the state of the art of this pathology, described its clinical characteristics, diagnostic criteria, differential diagnoses and carried out possible treatments. The purpose of our presentation is to report our experience in monitoring the patient's case, along with the diagnostic vicissitudes in this regard, which ranged from severe inflammatory acne to finally arriving at a certain diagnosis of hidradenitis suppurativa, based on clinical and ultrasound criteria. We consider of interest the possibility of having been able to use the "biological agent" inhibitor of the tumor necrosis factor (TNF) that we have mentioned, with an excellent response.

Communicative and social functioning in old age involves cognitive and emotional resources. According to the Socioemotional Selectivity Theory (tss), older adults have an affinity for positive events and cultivate their emotional skills. Objective: To explore the relationship between communicative and social functioning and the processing of emotional stimuli in the elderly. Materials and methods: Older adults and young adults performed a computerized lexical decision task (ldt) and were assessed on their com-municative functioning. Results: There were statistically significant differences in the valence factor, with faster processing of stimuli with emotional versus non-emotional content (2196 = 36.39, p <.001); Regarding communicative and social functioning, when comparing it with performance in the tdl, it was shown that older adults obtained longer reaction times and higher error rates in the processing of stimuli, as well as lower scores in the asha-facs. Conclusion: Even when there is no evidence of an interaction between the processing of emotional stimuli and communicative and social functioning, emotional valence influences word processing in older adults, with a preference and selectivity for positive stimuli.


el funcionamiento comunicativo y social en la vejez implica recursos cognitivos y emocionales. Según la teoría de la selectividad socioemocional (tss), los adultos mayores tienen afinidad por eventos positivos y cultivan sus habilidades emocionales. Objetivo: explorar la relación entre el fun-cionamiento comunicativo y social y el procesamiento de estímulos emocionales en el adulto mayor. Materiales y métodos: adultos mayores y adultos jóvenes realizaron una tarea computarizada de decisión léxico (tdl) y fueron evaluados en su funcionamiento comunicativo. Resultados: hubo diferencias estadísticamente significativas en el factor de valencia, con un procesamiento más rápido de estímulos con contenido emocional versus no emocional (2196 = 36.39; p < 0.001). En cuanto al funcionamiento comunicativo y social, al compararlo con el desempeño en la tdl, se evidenció que los adultos mayores obtuvieron tiempos de reacción más prolongados y mayores tasas de error en el procesamiento de estímulos, al igual que menores puntajes en el ashafacs. Conclusión: aun cuando no se evidencia una interacción entre el procesamiento de estímulos emocionales y el funcionamiento comunicativo y social, la valencia emocional influye en el procesamiento de palabras en los adultos mayores, con una preferencia y selectividad por los estímulos positivos.

Resultados: houve diferenças estatisticamente significativas no fator de valência, com processamento mais rápido de estímulos com conteúdo emocional versus não emocio-nal (2196 = 36,39; p < 0,001). Em relação ao funcionamento comunicativo e social, ao compará-lo com o desempenho no tdl, evidenciou-se que os idosos obtiveram maiores tempos de reação e maiores taxas de erro no processamento dos estímulos, além de menores escores no asha-facs. Conclusão: não há evidência de interação entre o processamento de estímulos emocionais e o funcionamento comunicativo e social, a valência emocional influencia o processamento de palavras em idosos, com preferência e seletividade por estímulos positivos.

El objetivo fue analizar la calidad de vida relacionada con la salud oral en pacientes con diferentes niveles de severidad de maloclusión que acudieron a consultorios odontológicos públicos de Paraguay durante el 2017. Materiales y métodos: estudio transversal. Se midieron variables sociodemográficas (como sexo, edad, residencia y nivel de estudios) y variables clínicas (como diastema, máxima irregularidad anterior maxilar y mandibular, resalte incisal y mordida cruzada anterior). Se utilizó el cuestionario Perfil de Impacto de Salud Oral en su versión paraguaya (ohip-14Py) y el Índice de Estética Dental (IED). Resultados: formaron parte del estudio 269 pacientes, la mayoría mujeres (75.1 %). El puntaje IED fue de 31.6 ± 11.6. Se observó resalte incisal aumentado (>2 mm) en poco más de la mitad (52.4 %) y una baja frecuencia de mordida cruzada anterior (5.9 %) y mordida abierta (10.0 %). Al evaluar por dimensiones, la incapacidad psicológica (p = 0.028), social (p = 0.034) y la minusvalía (p = 0.552) aumentaron conforme el nivel de severidad de maloclusión, por lo que fueron estadísticamente significativas las dos primeras. Conclusión: conforme aumenta la gravedad de la maloclusión, disminuye significativamente la calidad de vida oral para las dimensiones incapacidad psicológica e incapacidad social.


Patients requiring orthodontic treatment reported negative impacts on oral health-related qual-ity of life compared to patients with normal occlusion. Objective: To analyze the oral health-related quality of life in patients with different levels of severity of malocclusion attending public dental offices in Paraguay in 2017. Methodology: This was a cross-sectional study. Both, the sociodemographic variables, like sex, age, res-idence, and educational level, and the clinical variables, like diastema, maximum anterior maxillary, and mandibular irregularity, incisal overhang, and anterior crossbite were measured. The Oral Health Impact Profile questionnaire was used in its Paraguayan version (ohip-14Py) and the Dental Aesthetic Index (ied) scale. Results: A total of 269 patients were a part of the study. Moreover, 75.1 % of them were women. The ied score was 31.4 ± 11.6. An increase in the incisal protrusion (>2 mm) was seen in 52.4 % of the patients. Whereas a low frequency of anterior crossbite and open bite were seen in 5.9 % and 10.0 % of the population, respec-tively. While conducting an evaluation based on the dimensions, the psychological disability (p = 0.028), social disability (p = 0.034), and handicap (p = 0.552) increased according to the level of severity of malocclusion, being statistically significant in the first two. Conclusion: As the severity of the level of malocclusion increases, the oral quality of life decreases for the psychological disability and social disability dimensions.

Metodologia: estudo transversal. Foram mensuradas variáveis sociodemográficas como sexo, idade, residência e escolaridade e variáveis clínicas como diastema, irregularidade anterior maxilar e mandibular máxima, sobressaliência incisal e mordida cruzada anterior. Foram utilizados o questioná-rio Perfil de Impacto em Saúde Bucal em sua versão paraguaia (ohip-14Py) e o Índice de Estética Dental (ied). Resultados: fizeram parte do estudo 269 pacientes, sendo a maioria mulheres (75,1 %). A pontuação do ied foi de 31,6 ± 11,6. Observou-se aumento da saliência incisal (>2 mm) em pouco mais da metade (52,4 %) dos pacientes, além de baixa frequência de mordida cruzada anterior (5,9 %) e mordida aberta (10,0 %). Ao avaliar por dimensões, a incapacidade psicológica (p = 0,028), incapacidade social (p = 0,034) e desvantagem (p = 0,552) aumentaram de acordo com o grau de severidade da má oclusão, sendo as duas primeiras estatisticamente significativas. Conclusão: à medida que a gravidade da má oclusão aumenta, a qualidade de vida oral diminui significativamente para as dimensões de incapacidade psicológica e incapacidade social.
