RESUMO

Objective: To explore the association between blood pressure control and risk of ischemic stroke (IS) in patients with hypertension. Methods: A total of 5 488 patients with hypertension from 60 communities were randomly selected from 101 communities in 8 streets of Nanshan District in Shenzhen City by using two-stage sampling method. The social demographic characteristics, behavior and life style, coronary heart disease and diabetes were collected and the physical condition, blood pressure and blood biochemical indexes were measured. From April 1, 2010 to August 31, 2017 as the follow-up period, the incidence of IS was annually collected by using telephone survey. Cox proportional hazard regression model was used to analyze the relationship between blood pressure control, systolic blood pressure (SBP), diastolic blood pressure (DBP) and the risk of IS. Results: The age of all patients was (58.50±12.14) years old, including 2 712 males (49.42%) and 3 112 patients with well-controlled blood pressure (56.71%). During the follow-up period, 358 new cases of IS were confirmed, and the incidence density was 1 346.27/100 000 person-years. Cox proportional hazard regression model analysis showed after adjusting for confounding factors, unstable blood pressure control, SBP≥150 mmHg (1 mmHg=0.133 kPa; compared with SBP<120 mmHg), and DBP≥95 mmHg (compared with DBP<80 mmHg) were associated with risk of IS. The HR (95%CI) was 1.29 (1.04, 1.59), 2.00 (1.26, 3.17) and 1.52 (1.01, 2.64), respectively. Subgroup analyses showed these associations only existed in female patients with hypertension. The HR (95%CI) was 1.39 (1.05, 1.85), 2.53 (1.41, 4.56) and 1.73 (1.00, 3.36), respectively. Conclusion: Unstable blood pressure control increases the risk of IS in female patients with hypertension.

Tuberculosis (TB) is generally considered a disease that principally afflicts the low-income segments of a population. In the Nanshan District of Shenzhen, China, with the economic transformation and a new Headquarters Economy (HE) emerging, there are now more cases in office workers than in manufacturing workers. To illustrate this trend, we describe a small TB outbreak in an office building located in the centre of the rapidly growing HE district. Two active pulmonary tuberculosis cases were found in workers who shared an office, and whole genome sequencing showed that the genetic distance between the strains of the two cases was just one single nucleotide polymorphism, consistent with intra-office transmission. Investigation of 30 other workers in the same or adjacent offices with interviews, interferon-gamma release assays (IGRAs) and chest X-rays, identified one new TB case and latent tuberculosis infection (LTBI) in 40.0% (12/30) of the contacts. The offices were under-ventilated. None of the IGRA positive, asymptomatic contacts agreed to receive treatment for LTBI, presumably due to TB stigma, and over the next 2 years 69.0% (20/29) of the contacts were lost to follow-up. Treatment for LTBI and stigma of TB remain challenges here. Office workers in the HE of rapidly economic developing areas should be targeted with increased vigilance by TB control programmes.

The aim of this research was to study the clinical significance and expression of CD4+CD25+ regulatory T cells (Tregs) and p3Forkhead transcription factor-3 (Foxp3) in peripheral blood of patients with gastric carcinoma (GC) and to investigate the effects in the occurrence and development process of GC, to further comprehend their clinical values and provide a theoretical basis for the early diagnosis and immunotherapy of GC. The expression levels of CD4+CD25+Foxp3+Tregs in GC patients, at TNM staging, differentiated degree, lymphatic metastasis, cancer sites and cancer diameter of GC, were analyzed within the groups. The comparison of the expression levels of CD4+CD25+Foxp3+Tregs in peripheral blood between the GC group and the healthy control group showed a statistically significant difference. At TNM staging within the groups, pairwise comparisons of the expression levels of CD4+CD25+Foxp3+Tregs indicated that differences among the stage I+II group, stage III group and stage IV group were statistically significant. The expression levels of CD4+CD25+Foxp3+Tregs are closely relative to the occurrence and development of GC, providing theoretical bases and evidence for the early diagnosis, prognosis evaluation and immunotherapy of GC.

BACKGROUND: Typing of human platelet antigens (HPA) has proven to be useful in some clinical situations related to platelet alloimmunization. OBJECTIVE: The objective of this study was to investigate HPA 1-16 and to determine genotype and allele frequencies by polymerase chain reaction-sequence specific primer (PCR-SSP) in apheresis platelet donors in Guangzhou Blood Center. METHODS: A total of 200 random samples from donors were involved in the study. Genotype and allele frequencies of HPA 1 to 16 were detected by PCR-SSP method. RESULTS: The frequencies obtained from these donors were 99·50 and 0·50% for HPA-1a and -1b; 96·25 and 3·75% for HPA-2a and -2b; 54·25 and 45·75% for HPA-3a and -3b; 99·50 and 0·50% for HPA-4a and -4b; 99·00 and 1·00% for HPA-5a and -5b; 97·00 and 3·00% for HPA-6a and -6b and 42·25 and 57·75% for HPA-15a and -15b. There is only a/a homozygosis detected in HPA-7, -8, -9, -10, -11, -12, -13, -14 and -16. In this study, none of HPA-1b/ 1b, -2b/2b, -5b/5b homozygosis were detected which were found in other racial groups. One homozygosis of HPA-6b/6b in 200 individuals was detected which was not found in a study involving 1000 Chinese (Feng et al., 2006). CONCLUSION: The HPA-3 and -15 appear to the highest priority and HPA-2, -6, -5 -1 and -4 to be the second priority in Chinese Cantonese when it comes to the diagnosis of neonatal alloimmune thrombocytopenia and to provide the HPA-matched platelet for patients with platelet transfusion refractoriness. The PCR-SSP method makes it possible to detect genotype of HPA-1 to -16 in less than 4 h and to establish a donor database for HPA genotype in a blood bank.

AIM: To analyze the role of P-selectin in intestinal ischemia and reperfusion injury (IRI) using murine models. METHODS: A model of warm IRI wherein the SMA was occluded for 100 minutes was undertaken in the following groups (10 mice per group): Group 1 (control) wild-type (WT) C57BL6, no treatment; Group 2: 0.4 mg/kg of r-PSGL1-lg 10 minutes before and after clamping; Group 3: PSGL KO mice. Survival was assessed at 7 days; the intestine was assayed for histopathology, apoptosis, myeloperoxidase (MPO), IL1, and TNF. A second model of cold IRI followed by intestinal transplantation (IT) was undertaken in the following groups (two mice per group): Group A WT --> WT: Group B PSGL KO --> WT (1-hour ischemia); Group C: PSGL KO --> WT (2 hour ischemia). Survival only was assessed. RESULTS: Survival was 50% in group 1, 90% in group 2, and 100% in group 3. Graded histopathology and crypt apoptosis demonstrated significantly less injury in groups B and C. MPO was not different between groups. IL1 and TNF were significantly reduce in groups 2 and 3. Following IT, survival was <12 hours in group A, >7 days in group B, and <72 hours in group C. CONCLUSION: This study clearly demonstrates the importance of P-selectin in warm and cold IRI in that the blockade of P-selectin using rPSGL1-lg or the absence of P-selectin using KO mice confers a survival advantage and reduction in tissue injury. The mechanism is unclear but appears to be independent of neutrophil infiltration.

Tetramethylpyrazine is one of the alkaloids contained in Ligusticum wallichii Franch. Ferulic acid is a phenolic compound contained in Ligusticum wallichii Franch and Angelica sinensis (Oliv.) Diels. The present study was carried out to examine the effect of tetramethylpyrazine and ferulic acid and the combined effect of both compounds on spontaneous uterine contractions in rats in situ. Tetramethylpyrazine and ferulic acid showed an inhibitory effect on uterine movement when given perorally and intravenously, respectively. The combination of both compounds, at doses individually insufficient to inhibit, synergistically inhibited uterine contraction. It was found that tetramethylpyrazine and ferulic acid inhibited uterine contractions and the inhibitory effect induced by the combination of both was due to the potentiation.
