RESUMO

Recurrent vulvovaginal candidiasis (RVVC) affects millions of women worldwide, severely impairing their quality of life. Despite the existence of multiple induction and maintenance therapeutic strategies, mainly based on antifungals, relapse rates are still high. Palomacare® is a vaginal gel containing, among others, hyaluronic acid and Centella asiatica, with repairing and moisturizing properties. A series of 5 clinical cases showed that symptoms improved and even disappeared in women with RVVC receiving Palomacare®. None of the patients experienced recurrence after the treatment, having their vaginal health restored, suggesting that the non-hormonal Centella asiatica, hyaluronic acid and prebiotic-based gel is effective for preventing RVVC relapse in women of reproductive age.

BACKGROUND: The occurrence of a discordant chromosomal abnormality in monozygotic twins is an extremely rare condition. CASE: We report the prenatal sonographic findings and cytogenetic studies in a monochorionic twin pregnancy discordant for severe fetal anomalies. Amniocentesis was normal for both twins. The pregnancy was managed conservatively, resulting in the delivery of discordant twins at 28 weeks. Cytogenetic analysis performed on cultured lymphocytes from peripheral blood revealed a mosaic 47XY+21 (in 2% of the cells)/46XY (in 98%) in the structurally normal twin, and a mosaic 47XY+21 (4%)/46XY (96%) for the abnormal twin. The abnormal neonate died shortly after delivery. The structurally normal twin survived without sequelae and had a normal karyotype 2 years later. CONCLUSION: This report adds to the literature a case of a monochorionic twin pregnancy with a mosaic fetus who gives his co-twin trisomic cells through placental vascular anastomoses, this twin being a chimera, highlighting the necessity of performing molecular genetics with polymorphic DNA markers to differentiate chimerism from mosaicism and define the origin of cell lines.

Ovarian metastatic breast cancer is infrequent and usually is originating from lobular carcinomas. It was found that the risk of developing an ovarian neoplasm is approximately doubled in women with a history of breast cancer. The finding of an adnexal mass in these patients involves a particular concern and requires a study. We report a case of a 67-year-old female diagnosed of an infiltrating lobular breast carcinoma. It is done lumpectomy and an axillary dissection of lymph nodes resulting 2 of 13 lymph nodes positives. She was treated with chemotherapy and hormone treatment staying the disease in remission for years. After she was admitted with malignant pleural effusion and pathological costal fracture. The ultrasound shows an increase of size of annexes and a CA125 and CA15.3 increased in the analysis. Bilateral oophorectomy was performed. The pathology was consistent with lobular breast carcinoma. Subsequently another income was required because of disease progression. Currently after almost two years since the adnexectomy, is in close monitoring by medical oncologists. Although the diagnosis of an adnexal mass in a woman who has had breast cancer is usually a benign finding, the risk in these women to develop a malignant ovarian pathology is increased compared to the general population. Therefore, although the patient remains asymptomatic, it is important an abdominopelvic exploration from time to time. If the ultrasound image of the adnexal mass is suspect, the CA125 is increased, or estrogen receptors are negatives in the original breast tumor, should be performed surgical evaluation.
