INTRODUCTION: Clozapine is an atypical antipsychotic drug eligible for treatment-resistant schizophrenia. It frequently represents the best and the only choice in resistant schizophrenia. However, its use is feared by many professionals due to its possible adverse effects, such as eosinophilia. CASE REPORT: We report a case of a young white male suffering from treatment-resistant schizophrenia who rapidly developed eosinophilia after starting clozapine. DISCUSSION: We present a case of a 26-year-old white man diagnosed with schizophrenia with poor clinical response to several antipsychotics owing to which clozapine was started. Psychotic symptoms improved dramatically but a progressively ascendant eosinophilia was reported during serial haematological analyses. The patient remained physically asymptomatic. An exhaustive assessment with ancillary diagnostic tests revealed no cause for eosinophilia. Thus, a diagnosis of clozapine-induced eosinophilia was made. The drug was discontinued and eosinophil count progressively returned to normal but psychotic symptoms worsened. CONCLUSIONS: Clozapine treatment is frequently feared due to its possible side effects and complications, delaying its use in refractory schizophrenia. Also, to our knowledge, there are no specific guidelines on how to manage haematological side effects such as eosinophilia. This is problematic as, in some cases, it may lead to an unnecessary withdrawal of clozapine with a worsening of psychotic symptoms. We present a brief discussion of the recent literature on the subject.

Introduction: The spectrum of sexually motivated behaviors is extremely wide, including behaviors that are not biologically reprductive and hold exclusively ludic functions. We refer to the persistent and recurrent patterns of sexual arousal resulting from the exposure to non-normative sexual stimuli as paraphilias, and they are paradigmatic of the broad heterogeneity of human sexuality. Among these, paraphilic infantilism was first reported in the scientific literature by Tuchman and Lachman in 1964. It is characterized by the presence of sexual arousal or pleasure when playing childhood roles or using objects proper of childhood.Objective: We intend to describe a clinical case, with a focus on the atypical profile of sexual interests and behaviors comprising paraphilic infantilism and sadomasochism phenomena. We also seek to discuss the understanding of these phenomena in the context of the global clinical picture, their nosological framework, and their implication in the psychotherapeutic process.Methodology: In this study, we present the clinical case of a patient observed in the outpatient clinic of the first author. A targeted literature review was carried out through a bibliographic search in the PubMed database and a selection of reference works in the Sexology field. These data provided a basis from which we develop our clinical discussion of the case.Results: This article illustrates the case of a patient referred for psychotherapeutic intervention due to an anxious-depressive syndrome. During follow-up, a cluster of sexual behaviors was observed that involved preferential sexual interest for role-playing as a baby, and practices that involved physical suffering, dominance and submission. Introducción: El espectro de conductas con motivación sexual es muy amplio, abarcando conductas que no son biológicamente reproductivas y que sostienen funciones exclusivamente lúdicas. Se denominan parafilias a los patrones persistentes y recurrentes de excitación sexual resultante de la exposición a estímulos sexuales no normativos, y su existencia resulta paradigmática de la amplia heterogeneidad de la sexualidad humana. Entre ellas, el Infantilismo parafílico fue descrito por primera vez en la literatura científica Tuchman y Lachman en 1964 y se caracteriza por obtener excitación o placer sexual a través de la representación de roles infantiles o del uso de objetos propios de la infancia. Objetivo: Nos proponemos describir un caso clínico, enfocando el perfil atípico de intereses y conductas sexuales que comprenden fenómenos de Infantilismo parafílico y Sadomasoquismo. Pretendemos discutir la comprensión de estos fenómenos en el contexto del cuadro clínico global, su marco nosológico y su implicación en el proceso psicoterapéutico. Metodología: Se relata el caso clínico de una paciente observada en el ámbito de la consulta de la primera autora. Se realiza una revisión no sistemática de la literatura científica a través de una búsqueda bibliográfica en la base de datos Pubmed y de la consulta de obras de referencia en el área de la Sexología. Se procede a la discusión clínica, basada en los datos presentados. Resultados: Este artículo ilustra el caso de una paciente, derivada a intervención psicoterapéutica por síndrome ansioso-depresivo. Durante el acompañamiento, se identificó un patrón de respuesta sexual caracterizado por un interés sexual preferencial en representar el rol de un bebé y prácticas que implicaban sufrimiento físico, dominación y sumisión. Introducción: La clozapina es un fármaco antipsicótico atípico eligible para la esquizofrenia resistente al tratamiento. Con frecuencia representa la mejor y la única opción para la esquizofrenia resistente. Sin embargo, muchos profesionales temen utilizarla por sus posibles efectos adversos, como la eosinofilia. Reporte de caso: Se expone el caso de un joven blanco que sufre esquizofrenia resistente al tratamiento y desarrolló eosinofilia rápidamente tras comenzar el tratamiento con clozapina. Discusión: Varón de 26 años con diagnóstico de esquizofrenia y mala respuesta clínica a varios antipsicóticos, por lo que se inició clozapina. Los síntomas psicóticos mejoraron drásticamente, pero los análisis hematológicos seriados informaron una eosinofilia en ascenso progresivo. El paciente permaneció físicamente asintomático. Una evaluación exhaustiva con pruebas de diagnóstico complementarias no reveló ninguna causa de eosinofilia. Por lo tanto, se diagnosticó eosinofilia inducida por clozapina. Se suspendió el fármaco, el recuento de eosinófilos volvió progresivamente a la normalidad, pero los síntomas psicóticos empeoraron. Conclusiones: A menudo se teme tratar con clozapina por sus posibles efectos secundarios y sus complicaciones, lo cual retrasa su uso en la esquizofrenia refractaria. Además, hasta donde sabemos, no existen pautas específicas sobre cómo tratar los efectos secundarios hematológicos como la eosinofilia. Esto es problemático porque, en algunos casos, puede conducir a suspender innecesariamente la clozapina y que empeoren los síntomas psicóticos. Purpose: This case reports on anomic aphasia related to COVID-19. Increasing knowledge about rare symptoms and complications may aid in the characterization of the disease, understand its pathophysiology, identify more quickly possible infected people and break the transmission chain. Case description: This work reports on the case of a middle-aged man who presented to his assistant psychiatrist complaining about difficulty with naming objects in his daily routine surroundings, with ten weeks of duration and following a SARS-CoV-2 infection. The organic study, including brain magnetic resonance imaging, was unremarkable. The symptoms resolved spontaneously within fourteen weeks. Comment: Neurological manifestations of COVID-19 may be related to the dysfunction of the blood-brain barrier, resulting in immune cell infiltration and neuroinflammation that can persist for weeks or months after the resolution of the infection. Weakened health after overcoming the infection acute phase is being reported increasingly and called post-COVID-syndrome. Rare disorders such anomic aphasia can occur in this syndrome.

Os cães idosos são acometidos com frequência por enfermidades no sistema cardiovascular e respiratório, estando muita das vezes assintomáticos até terem indicação para algum procedimento cirúrgico que requer a avaliação pré-operatória, momento este que muitos achados clínicos poderão ser detectados, ao ponto da necessidade de tratamento prévio à intervenção cirúrgica. Diante disso, o trabalho tem por objetivo detectar as principais manifestações cardiorrespiratórias e sua correlação aos dados encontrados nos exames complementares e informações epidemiológicas de 138 cães atendidos em um serviço especializado em cardiologia e doenças respiratórias, no período de 2017 a 2021, para fins de risco cirúrgico. Foi possível observar maior predominância de animais idosos, de pequeno porte e do sexo feminino. Dentre as doenças cardíacas, a doença degenerativa crônica de mitral foi a mais prevalente, seguida da dirofilariose. Em relação as afecções respiratórias, a bronquite crônica e o colapso de traqueia foram mais diagnosticadas, seguidas da broncopneumonia e da síndrome braquicefálica. Quando da observação do ritmo cardíaco por meio do eletrocardiograma, a arritmia sinusal, ritmo sinusal e sinus arrest foram os principais achados. Quanto aos achados laboratoriais, foi possível observar aumento da média de hematimetria, redução da média do hematócrito e das plaquetas. Entretanto, alguns animais apresentaram azotemia, leucocitose e eosinofilia. Observou-se que pacientes com doenças cardíacas em estágios de congestão possuem redução da pressão arterial, aumento da hematimetria e taquiarritmias, como taquicardia sinusal e/ou complexos atriais prematuros. Pacientes com idade mais avançada tendem a ter redução dos valores de leucometria global e maior probabilidade de manifestação de aumento da pressão arterial. Portadores de dirofilariose cursam com aumento de eosinófilos.


Elderly dogs are frequently affected by diseases in the cardiovascular and respiratory system, being often asymptomatic until they are indicated for a surgical procedure that requires preoperative evaluation, at which point many clinical findings can be detected, to the point of need for treatment prior to surgical intervention. Therefore, the work aims to detect the main cardiorespiratory manifestations and their correlation to data found in complementary exams and demographic information of 138 animals treated at a service specialized in cardiology and respiratory diseases, from 2017 to 2021, for risk purposes surgical. It was possible to observe a greater predominance of elderly, small and female animals. Among heart diseases, chronic degenerative mitral disease was the most prevalent, followed by heartworm disease. Regarding respiratory affections, chronic bronchitis and tracheal collapse were more commonly diagnosed, followed by bronchopneumonia and brachycephalic syndrome. When observing the cardiac rhythm through the electrocardiogram, sinus arrhythmia, sinus rhythm and sinus arrest were the main findings. As for laboratory findings, it was possible to observe an increase in mean hematimetry, a reduction in mean hematocrit and platelets. However, some animals showed azotemia, leukocytosis and eosinophilia. It was observed that patients with cardiac diseases in stages of congestion have reduced blood pressure, increased hematimetry and tachyarrhythmias, such as sinus tachycardia and/or premature atrial complexes. Older patients tend to have reduced global white blood cell values and more likely to manifest increased blood pressure. Heartworm patients have an increase in eosinophils.

Few studies investigated the biological effects of American grape cultivars. We investigated the metabolic response after acute consumption of grape juice or wine from Bordo grapes (Vitis labrusca) in a placebo-controlled crossover study with fifteen healthy volunteers. Blood samples were collected 1 hour after the intake of 100 mL of water, juice, or wine to measure TBARS, ABTS, FRAP, glucose, and uric acid levels. To evaluate differences in cellular response, intracellular reactive species production (DCFH-DA) and metabolic mitochondrial viability (MTT) were assessed after exposure of human neuron-like cells (SH-SY5Y) to juice or wine. Glycemia was reduced after juice or wine consumption, whereas blood levels of uric acid were reduced after juice consumption but increased after wine consumption. Juice and wine consumption reduced plasma lipid peroxidation and increased plasma antioxidant capacity (ABTS and FRAP assays). Furthermore, juice inhibited H2O2-induced intracellular production of reactive species (RS) and increased the viability of SH-SY5Y cells. In contrast, wine (dealcoholized) exhibited a per se effect by inducing the production of RS and reducing cell viability. These results indicate a positive impact of acute consumption of Bordo juice and wine on human oxidative status, whereas only juice had protective effects against oxidative stress-induced cytotoxicity.

Patients with Beckwith-Wiedemann syndrome (BWS) are known to be at an increased risk for childhood malignancies, particularly Wilms tumor and hepatoblastoma. We report a case of genetically confirmed BWS in a 5-month-old girl who presented with a 9.5-cm abdominal mass associated with elevated α-fetoprotein levels. The clinical impression was strongly suggestive of hepatoblastoma. Histologic examination of the surgically excised mass revealed mesenchymal hamartoma of the liver (MHL), a benign hepatic neoplasm.

O handebol é um esporte que demanda resistência associada a ações rápidas e potentes, como saltos, bloqueios, sprints e arremessos. O objetivo deste estudo foi avaliar os efeitos de treinamento físico sistematizado de 38 semanas aplicado em uma equipe de handebol feminino sub-21 sobre a potência de membros superiores e inferiores, velocidade e resistência de sprints de 30m. A periodização consistiu de adaptação da teoria de cargas concentradas e objetivou dois picos de performance durante a temporada, com seis coletas de dados. Os valores de mediana e amplitude de variação dos dados (entre parênteses) para o teste de arremesso de medicine ball de 3kg foram: 2,98m (2,15-3,50), 2,84m (2,43-3,20), 2,90m (2,60-3,38), 3,10 (2,83-3,81), 2,84 (2,55-3,57) e 3,34 (2,93-3,83). Para o teste de salto triplo horizontal alternado: 5,60m (4,93-6,58), 5,37m (5,04-6,38), 5,36m (4,93-6,12), 5,65m (4,80-6,78), 5,63m (5,00-6,40) e 5,83m (5,14-6,05). Para o teste de velocidade de sprint de 30m: 5,8m/s (5,45-6,44), 6,64m/s (6,24-7,09), 5,65m/s (5,17-5,95) (não houve coleta IV para esta capacidade), 6,19m/s (5,57-6,26) e 5,83m/s (5,14-6,05). Para o número de sprints até queda de 10 por cento velocidade de 30m: 4 (4-6), 5 (4-9), 4,5 (4-16) (não houve coleta IV para esta capacidade), 6 (4-12) e 5 (4-5). Observamos diferenças estatisticamente significativas nos testes de arremesso de medicine ball de 3kg e salto triplo horizontal em relação aos testes do início da temporada (p < 0,05) em pelo menos um dos períodos planejados para obtenção do pico de performance, sem melhoras significativas na velocidade média e resistência de sprints de 30m. O treinamento aplicado foi eficiente para a melhoria do condicionamento físico específico nos picos de performance, propiciando também melhor ajuste no treinamento para a próxima temporada.


Handball is a sport that demands endurance associated with fast and powerful actions such as jumps, blocks, sprints and throws. The aim of this study was to evaluate the effects of a 38-week systematic physical training applied to a women's under 21 handball team on upper and lower limb power, 30m sprints speed and endurance. The periodization applied was an adaptation of the Verkhoshansky theory, and aimed at two performance peaks during the season with six data collections. The median and range values for three kg medicine ball throwing was: 2.98m (2.15-3.50); 2.84m (2.43-3.20); 2.90m (2.60-3.38); 3.10 (2.83-3.81); 2.84 (2.55-3.57) and 3.34 (2.93-3.83). Regarding the three-pass running test: 5.60m (4.93-6.58); 5.37m (5.04-6.38); 5.36m (4.93-6.12); 5.65m (4.80-6.78); 5.63m (5.00-6.40) and 5.83m (5.14-6.05). Regarding the 30-m sprint test: 5.8m/s (5.45-6.44); 6,64 m/s (6,24-7,09); 5.65m/s (5.17-5.95); (there was not IV moment for this test); 6.19 m/s (5.57-6.26) and 5.83 (5.14-6.05).Regarding the 30-m sprint endurance test until 10 percent decrease: 4 sprints (4-6); 5 sprints (4-9); 4,5 sprints (4-16); (there was not IV moment for this test); 6 sprints (4-12) and 5 sprints (4-5). Significant differences (p<0.05) were observed in three kg medicine ball throwing and three-pass running tests at least in one of the performance peak planned, with no significant differences in 30-m sprint speed or endurance tests. The applied physical training was efficient at improving the specific physical fitness in the performance peaks, as well as giving support for better physical training adjustment for the upcoming season.
