RESUMO

Fifty-six patients with ulcerative colitis of mild to moderate severity were entered into a randomized, double-dummy comparison of sulphasalazine, 3 g/day, with olsalazine, 3 g/day. Patients were assessed clinically, and by sigmoidoscopy and biopsy, on entry and at 5 weeks. Both agents produced a similar reduction in stool frequency and in the passage of blood and mucus. Improvements in sigmoidoscopic and histological appearances of the rectal mucosa were observed to a similar extent in both groups of patients. Two patients treated with olsalazine were withdrawn because of increased diarrhoea attributable to the medication. Two patients given sulphasalazine for the first time developed a skin rash. Other side-effects seen during the trial were mild. In this small short-term study, oral olsalazine appeared to be as effective as sulphasalazine in the treatment of mild to moderate ulcerative colitis.

RESUMO

A case of Zollinger-Ellison syndrome complicated by a gastroduodenal fistula is described in a 42-year-old man. So far as the authors know, this is the first report of this complication in gastrinoma.

Sixty patients with gastric ulcers were treated for four weeks with either 1 g cimetidine per day or with identical tablets containing lactose. The healing rate, assessed by endoscopy, was 23 out of 35 (66%) in the patients given cimetidine and 13 out of 25 (52%) in those given placebo. The difference between the groups is not significant. During each of the four weeks of the study the cimetidine group experienced significantly fewer attacks of pain and consumed less antacids than the placebo treated patients.

Twenty-five patients with healed gastric ulcers completed an 11-month double-blind controlled trial comparing cimetidine 1 g daily with placebo in the prevention of ulcer relapse. Ulcers recurred in 14 patients, 12 on placebo, 2 on cimetidine. In 11 patients, 9 on cimetidine, 2 on placebo, the ulcers remained healed at the end of the trial (P less than 0.002). Cimetidine 1 g daily significantly reduces the relapse rate in gastric ulceration.
