RESUMO

This cross-sectional survey examined the relationship of the competence of 529 nurses to their designated career structure levels. The extent of the relationships between nursing education and experience with clinical competence was determined, and attitudes of nurses toward their work environment were measured. Results indicated a wide dispersion in clinical nursing skills and that nurses' designation in the Western Australian Career Structure did not reflect their clinical competence. Clinical experience and education independently exerted a significant effect on competence, but there was no interaction between these variables on increasing competence. Attitudes of the nurses were found to be affected by their work environment.

The restrictions imposed on patients with asthma is problematic to them, in that their physical disability interferes with personal, interpersonal and/or professional goals. Twenty-three patients under the care of general practitioners were surveyed by interview in their homes, doctors' surgery or at work to determine their current control of selected variables related to their asthma: exercise, use of drugs and related factors. An instructional programme was offered to interested participants to assist in complying with their prescribed medication regimens and to tailor exercise to their tolerance levels. Findings showed that 18 patients experienced moderate to severe restrictions when troubled with asthma. Although over one-half of the patients exercised regularly for fitness, including seven with aerobic exercise, some chose sports they thought could provoke an asthmatic attack. Half took precautionary measures when exercising. All patients used bronchodilators to relieve their asthma, yet one-third did not keep their inhalers accessible. None of the most troubled 18 lived in a smoke-free environment, and one-third kept furry animals as pets. Recommendations are made for further study of the effects of instructional programme to improve compliance to a healthy lifestyle.

Communication difficulties between hospital doctors and nurses are well documented. A survey undertaken jointly by medical and nursing administration at Sir Charles Gairdner Hospital in Perth, Western Australia, verified difficulties in doctor-nurse communication as perceived by doctors and nurses, as well as by ward clerks as impartial observers. Questionnaire responses revealed some impediments in the flow of communication. Both nurses and doctors perceived less frequency of difficulties in communicating with members of their own professional group than with members of the other group. Nurses with university preparation and other special clinical qualifications perceived significantly fewer communication problems with doctors than nurses with less education. Interns perceived greater frequency in difficulty communicating with nurses than did more highly qualified doctors, and female doctors who were not interns claimed fewer problems than their male counterparts. Moreover, more highly qualified male doctors who had a previous occupation acknowledged fewer doctor-nurse communication problems.

The value of research to the practice of nursing has been well documented. The difficult question today is how to establish a formal research component into the nursing division when funding sources are scarce or nonexistent. The process that one large tertiary-care hospital used to accomplish this goal is analyzed. The authors discuss design, resources, implementation, financing, and program maintenance.

This communication describes the methods used and results obtained with a 21-year-old Standardbred sire bred, using artificial insemination techniques, to a book of 108 mares during the period 11th September to 16th March, 1979-80. A positive pregnancy test rate of 93.5% was achieved. Those mares becoming pregnant averaged 4.6 inseminations per pregnancy (range 1-13). Barren mares, maiden mares and wet mares, irrespective of age, all achieved highly satisfactory pregnancy rates and it was noted that breeding through the January/February period improved the ultimate success rate markedly. When the official returns for the breeding season under review were returned the sire had a live foal success rate of 84%.
