BACKGROUND & AIMS: Different nutritional screening tools have been proposed in childhood cancer, but none has shown convincing predictive capacity so far. The "nutrition screening tool for childhood cancer (SCAN)" has been specifically designed for this population and provides an easy-to-use, promising approach to identify patients at risk of malnutrition. We aim to: 1. Validate the SCAN tool prospectively in identifying malnourished patients or those who required nutritional support 2. Validate the SCAN tool prospectively in predicting toxicities or outcome. 3. Compare performance of a pediatric screening tool (STRONGKIDS) with SCAN. METHODS: Children in our center with a new diagnosis of cancer from August 2018 to May 2019 were offered to participate in the study. Measurements (SCAN questionnaire, weight, height, body-mass index (BMI), and mid upper-arm circumference (MUAC)) were taken at diagnosis and at regular intervals throughout therapy. The last measurement was taken 6 months after finishing the intensive treatment phase. SCAN score at diagnosis was validated prospectively against variables of interest. RESULTS: A total of 49 patients were recruited. When considering malnutrition during therapy the SCAN tool showed a sensitivity of 37.5% and negative predictive value (NPV) of 81%. Patients who required nutritional support were identified with a sensitivity of 50% and NPV of 62%. The SCAN tool was not able to predict increased toxicities, risk of relapse or decreased survival. The pediatric screening tool STRONGKIDS was unable to discriminate nutritional risk and labeled all 49 patients (100%) as medium or high-risk. Applying SCAN periodically during therapy increased sensitivity for identifying malnutrition to 87.5%. CONCLUSIONS: In our study, applying the SCAN tool at diagnosis showed low sensitivity in identifying patients who go on to develop malnutrition during therapy. However, patients labeled as "not at risk" were unlikely to need nutritional support in the form of nasogastric tube or total parenteral nutrition. Using SCAN throughout therapy could be helpful in building awareness for malnutrition and successfully discriminates between patients who need further support and those who don't.

OBJECTIVES: Data regarding the palliative needs of pediatric patients with central nervous system (CNS) cancer are scarce. We aimed to describe the attention provided by a pediatric palliative care (PPC) team to patients with CNS cancer and the differences in care compared to patients who did not receive PPC. METHOD: This retrospective study was based on the clinical records of deceased patients with CNS cancer attended by a PPC team over 10 years, analyzing their trajectory and provision of PPC, including medical, psychological, social, and nursing interventions. Furthermore, we compared the last month of life care of deceased patients with CNS cancer in the same institution, based on whether they were attended by the PPC team. RESULTS: Of 71 patients, 59 received PPC, with a median of 1.6 months (Interquartile range: 0.6-5.2) from referral to death. Home hospitalization was provided to 84.8%, nursing interventions were registered in 89.8%, psychological characteristics in 84.7%, and social interventions in 88.1%. The most common symptoms were pain, dyspnea, and constipation. When comparing patients from the same hospital who received PPC (n = 36) with those who did not (n = 12), the former spent fewer days in the hospital in their last month and last week (p < 0.01) and were more likely to die at home (50% vs. 0%; p < 0.01). SIGNIFICANCE OF RESULTS: Patients with CNS cancer show various medical, social, and psychological needs during end-of-life care. Providing specific PPC interventions decreased the number of days spent at the hospital and increased the rate of death at home.

No standardized approach towards nutritional screening and assessment of pediatric oncology patients has been established. The nutrition screening tool for childhood cancer (SCAN) has been previously published as an effective screening method. This is an observational cross-sectional study to assess the validity and reliability of the SCAN tool, compare it to the detection of undernutrition using standard measures of assessment, and determine the overall prevalence of malnutrition and micronutrients alterations in our cohort. We included children newly diagnosed with cancer in a pediatric tertiary hospital in Madrid, Spain from August 2018 to May 2019. The following measurements were performed: SCAN questionnaire, anthropometric measurements, nutritional markers in blood, and micronutrient levels. A total of 49 patients were included. 22 patients (45%) were at risk of malnutrition according to the SCAN questionnaire. Four patients (8%) could be diagnosed with moderate undernutrition. These undernourished patients were distributed homogeneously among at-risk and not at-risk populations identified by the SCAN tool. Several micronutrient deficiencies were identified. We conclude that the SCAN questionnaire is an easy-to-use tool for everyday clinical practice. By not including anthropometric measurements it misses patients considered to be malnourished. Future data might help clarify if it is an effective tool in predicting a higher nutritional risk during the entire treatment course.

Introducción: El tumor de Wilms es el tumor renal más frecuente en la edad pediátrica. Su tratamiento es multimodal: incluye quimioterapia y cirugía, con o sin radioterapia. La supervivencia de estos pacientes es excelente, superando el 90%. Presentamos la experiencia de nuestro centro en el tratamiento del tumor de Wilms durante los últimos 15 años. Pacientes y métodos: Se ha realizado un estudio retrospectivo de 40 pacientes pediátricos diagnosticados de forma consecutiva de nefroblastoma entre 2002 y 2016 en el Servicio de Hemato-Oncología pediátrica del Hospital Niño Jesús de Madrid. Se analizaron las características clínicas, los métodos diagnósticos, el tratamiento realizado y la evolución posterior. Resultados: De los 40 pacientes, 23 eran niños con una mediana de edad al diagnóstico de 2,5 años (rango, 4 meses-15 años). A 3 pacientes se les realizó nefrectomía inicial, 3 recibieron una punción aspiración con agua fina, seguida de quimioterapia y 34 pacientes recibieron quimioterapia preoperatoria directamente. La mediana de seguimiento de los pacientes fue de 6,75 años (rango, 10 meses-13,92 años). Dos pacientes fallecieron de progresión de su enfermedad. Ningún paciente falleció de toxicidad en relación con el tratamiento. La supervivencia global y la supervivencia libre de evento a los 5 años fue del 94,6 ± 3,7% y 89,4 ± 5%, respectivamente. Conclusión: El tratamiento del tumor de Wilms es un éxito de la medicina moderna, consiguiendo en la actualidad una supervivencia que en nuestra serie alcanza el 95% (AU)

Conclusion: Wilms' tumour treatment is a success of modern medicine, currently achieving a survival rate of 95% in our series (AU)

INTRODUCCIÓN: Los tumores de células germinales intracraneales son un grupo poco frecuente de tumores en niños. Comprenden un grupo heterogéneo de neoplasias, que aunque comparten un origen común, presentan comportamientos clínicos muy diferentes. PACIENTES Y MÉTODOS: Análisis retrospectivo de las características epidemiológicas e histológicas, las manifestaciones clínicas y la evolución de 20 pacientes diagnosticados de tumor de células germinales intracraneal en el Hospital Infantil Universitario Niño Jesús de Madrid durante los años 1994-2014. RESULTADOS: Se obtuvieron 20 pacientes: 14 niños y 6 niñas. La edad media fue de 11,1 años (rango 2-18 años). Se realizó confirmación histológica en el 95% de los pacientes. De los 20 pacientes, 14 fueron germinomas puros (70%) y 6 tumores de células germinales no germinomas (30%). Las localizaciones más frecuentes fueron pineal (45%) y supraselar (45%). Los síntomas más frecuentes en el momento del diagnóstico en los tumores de localización pineal fueron cefalea y vómitos (77,77%), seguido de alteraciones visuales (44,4%), y en los tumores de localización supraselar, polidipsia y poliuria (100%). En el momento del diagnóstico recibieron radioterapia el 90% de los pacientes y quimioterapia asociada a la radioterapia el 55%. Presentaron recaída tumoral 4 pacientes (20%), de los cuales 3 fallecieron. La supervivencia global fue del 80%, siendo un 85,7% para los germinomas y un 60% para los no germinomas. CONCLUSIÓN: El tipo histológico más frecuente fue el germinoma. Los tumores de células germinales son un grupo heterogéneo de tumores que conllevan un pronóstico diferente, por lo que un adecuado diagnóstico y estadificación es fundamental para planear el tratamiento

The protozoan parasite Perkinsus marinus is the etiological agent of "dermo disease". This pathogen is considered by the World Organization for Animal Health (OIE) as reportable due to the high mortalities that it produces in the eastern oyster Crassostrea virginica in the US. In 2006, this parasite was detected in the pleasure oyster Crassostrea corteziensis in Nayarit on the Pacific coast of Mexico, indicating a new host and an extension of its known distribution. Epizootiological data of P. marinus in the pleasure oyster are unknown. With the objective of determining the prevalence and intensity in relation with temperature and salinity throughout time, as well as for studying interactions of host size and sex with the parasite, a monthly sampling was carried out in two aquaculture sites of Nayarit from 2007 to 2014. A total of 7700 oysters were analyzed. In both localities, prevalence was low in winter (<6%) when temperature and salinity fluctuated around 24°C and 33, respectively; and the highest prevalence values occurred during summer (37%) when temperature and salinity were around 30°C and 20, respectively. Infection intensity increased in summer, but severe cases remained on average <10%. Larger oysters showed the highest prevalence and intensity, and higher prevalence were generally observed in females. No unusual mortalities directly related with P. marinus were observed.

Acute inflammation of a single cerebellar hemisphere (hemicerebellitis) is a rare disorder of unknown origin. The clinical presentation is mainly characterized by headache, ataxia, dysmetria, and vomiting. In addition, some children may develop severe intracranial hypertension. The neuroimaging of hemicerebellitis raises a challenging differential diagnosis, particularly with posterior fossa tumours. Although there is no standard treatment for hemicerebellitis, its outcome is usually favourable. However, ipsilateral hemicerebellar atrophy develops in up to half of cases, and a minority of children may show persisting fine motor and/or neurocognitive sequelae. In this article, we contribute with three new reports and review a total of 35 cases of hemicerebellitis.

RESUMO

Aneurysm of the inferior vena cava is a rare finding in the pediatric population. We report the case of a 5-month-old infant presenting with anemia, hypertension, and dehydration in the emergency room. A renal mass was found with ultrasound and MRI and a renal tumor was first considered. Histopathologic review of the surgical specimen led to the diagnosis of aneurysmal dilatation of the vena cava.

Micafungin is one of three echinocandins, a novel class of antifungal agents active against 1,3-beta-D glucan in the fungal cell wall. It is a favorable safety profile have made it an attractive option in the treatment of invasive Candida and Aspergillus infections. Available studies have shown that younger children have lower C(max), shorter t(1/2) and faster clearance than adults.

AIM: It has been suggested that there is an inverse association between breastfeeding and the risk of childhood cancer. We investigated the association between full breastfeeding and paediatric cancer (PC) in a case control study in Spain. METHODS: Maternal reports of full breastfeeding, collected through personal interviews using the Paediatric Environmental History, were compared among 187 children 6 months of age or older who had PC and 187 age-matched control siblings. RESULTS: The mean duration of full breastfeeding for cases were 8.43 and 11.25 weeks for controls. Cases had been significantly more often bottle-fed than controls (odds ratio (OR) 1.8; 95% confidence interval (CI) 1.1-2.8). Cases were significantly less breastfed for at least 2 months (OR 0.5; 95% CI 0.3-0.8), for at least 4 months (OR 0.5; 95% CI 0.3-0.8), and for 24 weeks or more (OR 0.5; 95% CI 0.2-0.9). CONCLUSIONS: Breastfeeding was inversely associated with PC, the protection increasing with the duration of full breastfeeding. Additional research on possible mechanisms of this association may be warranted. Meanwhile, breastfeeding should be encouraged among mothers.
