Disseminated leishmaniasis (DL) is an emergent severe disease manifesting with multiple lesions. To determine the relationship between immune response and clinical and therapeutic outcomes, we studied 101 DL and 101 cutaneous leishmaniasis (CL) cases and determined cytokines and chemokines in supernatants of mononuclear cells stimulated with leishmania antigen. Patients were treated with meglumine antimoniate (20 mg/kg) for 20 days (CL) or 30 days (DL); 19 DL patients were instead treated with amphotericin B, miltefosine, or miltefosine and meglumine antimoniate. High levels of chemokine ligand 9 were associated with more severe DL. The cure rate for meglumine antimoniate was low for both DL (44%) and CL (60%), but healing time was longer in DL (p = 0.003). The lowest cure rate (22%) was found in DL patients with >100 lesions. However, meglumine antimoniate/miltefosine treatment cured all DL patients who received it; therefore, that combination should be considered as first choice therapy.

The CONVINCE trial demonstrates that high-dose hemodiafiltration offers a survival advantage for patients in the high-flux hemodiafiltration group compared to hemodialysis. We compared the outcomes of hemodialysis and hemodiafiltration using real-world data. We conducted an analysis on a cohort of patients who underwent hemodiafiltration therapy (HDF) at a single center, NefroStar Clinics. The results obtained were then compared with data from patients receiving hemodialysis (HD) therapy within the Brazilian Public Health System (SUS). The primary outcome was mortality from any cause. Results: A total of 85 patients undergoing hemodiafiltration were compared with 149,372 patients receiving hemodialysis through the Brazilian Public Health System (SUS). Using a 2:1 propensity score, we compared the 170 best-match HD patients with 85 HDF patients. In the Cox analysis, HDF therapy showed a reduced risk of mortality with an HR of 0.29 [0.11-0.77]. The propensity score analysis showed a HR of 0.32 [95% CI: 0.11-0.91]. This analysis was adjusted for age, type of access, KT/v, hemoglobin, and phosphorus. The Kaplan-Meier analysis showed respective survival rates for HDF and HD at the end of one year, 92.1% and 79.9%, p < 0.001. These results suggest high-flux hemodiafiltration has survival advantages over hemodialysis in a real-world scenario.

BACKGROUND: Vitiligo is the most common pigmentary disorder and is considered a chronic, cumulative, multifactorial disease. The crucial role of cytotoxic CD8+ T lymphocytes and the IFNγ/CXCL10 axis has been demonstrated in its pathogenesis. OBJECTIVE: To evaluate the clinical profile and immuno-inflammatory markers in patients with vitiligo in a reference medical center. METHODS: Cross-sectional study in which all patients with vitiligo seen at the medical center the from 2019 to 2022 were evaluated, to outline the clinical profile. Moreover, cardiovascular risk biomarkers (neutrophil/lymphocyte ratio and C-reactive protein levels) were measured, as well as cytokines and chemokines (TNFα, IFNγ, IL10, IL15 and CXCL10) in the serum of a subgroup of 30 patients. RESULTS: There was a predominance of females, with a mean age of 43 years. Most were phototypes IV or V (71.3%), without comorbidities (77.55%), and without a family history of vitiligo (70.41%). Higher levels of neutrophil/lymphocyte ratio, C-reactive protein, and inflammatory cytokines/chemokines were documented in vitiligo patients when compared to the control group (non-significant). As relevant data, the highest values of CXCL10 were detected in patients with vitiligo versus controls, as well as in patients with disease of shorter duration (p<0.05). STUDY LIMITATIONS: The number of assessed patients was small due to recruitment difficulties caused by the COVID-19 pandemic. CONCLUSION: The present data contribute to confirming the relevant role of the IFNγ/CXCL10 axis in the pathogenesis of vitiligo, highlighting CXCL10 as a possible activity marker.

O presente estudo é um recorte de uma pesquisa mais ampla sobre a experiência da maternidade de mulheres brasileiras migrantes. Ele tem como objetivo investigar o apoio recebido por elas no processo de tornar-se mãe. Trata-se de uma pesquisa qualitativa na qual foram realizadas, em 2022, sete entrevistas com mulheres brasileiras que vivenciaram gestação, parto e primeiro ano de vida do filho (a) em Portugal, Reino Unido, Itália, Espanha e França. Os resultados apontam para a relevância da presença de pessoas que compartilham da mesma cultura de origem na construção da maternidade e na rede de apoio durante a perinatalidade. Conclui-se que o distanciamento da rede familiar, de amigos e de referenciais da cultura de origem da mulher migrante gerou o aumento das sobrecargas psíquica e física inerentes à maternidade. Ao mesmo tempo, de acordo com as participantes, o distanciamento da família e da cultura de origem proporcionou maior liberdade e abertura para a construção da maternidade, provendo novas representações culturais que contribuíram para a construção de sua forma de ser mãe.(AU)


This study is a segment of a broader research project on the maternity experience of Brazilian migrant women. It aims to investigate the support received by these women in the process of becoming mothers. It encompasses a qualitative research study involving seven interviews conducted in 2022 with Brazilian women who experienced pregnancy, childbirth, and the first year of their child's life in Portugal, the United Kingdom, Italy, Spain, and France. The findings highlight the importance of having individuals sharing the same cultural background in the construction of motherhood and in the support network during the perinatal period. The distance from the family network, friends, and references of the migrant woman's culture of origin generated an increase in the psychic and physical burdens inherent to motherhood. Simultaneously, according to the participants, distancing from family and their cultural origins provided greater freedom and openness in constructing motherhood, offering new cultural representations that contributed to shaping their way of being mothers. El presente estudio es parte de una investigación más amplia acerca de la experiencia de la maternidad de las mujeres migrantes brasileñas. Tiene como objetivo investigar el soporte recibido por ellas en el proceso de convertirse en madre. Se trata de una investigación cualitativa en la que, en 2022, se realizaron siete entrevistas con mujeres brasileñas que vivieron el embarazo, el parto y el primer año de vida de su hijo en Portugal, Reino Unido, Italia, España y Francia. Los resultados apuntan para la relevancia de la presencia de personas que comparten la misma cultura de origen en la construcción de la maternidad y en la red de apoyo durante la perinatalidad. Se concluye que el alejamiento de la red familiar, amigos y puntos de referencia de su cultura de origen de la mujer migrante ha generado un aumento de las sobrecargas psíquicas y físicas inherentes a la maternidad. Al mismo tiempo, según las participantes, el alejamiento de la familia y de la cultura de origen les ha proporcionado una mayor libertad y apertura para la construcción de la maternidad, brindando nuevas representaciones culturales que contribuyeron para la construcción de su forma de ser madre. INTRODUCTION: The pathogenesis of cutaneous and mucosal leishmaniasis is associated with different immune responses. Vitamin D may modulate the immune system. Here we evaluate the association of vitamin D levels with the severity of the clinical forms of cutaneous and mucosal leishmaniasis. METHODS: We conducted an observational study evaluating the association between vitamin D levels, disease severity and therapeutic response in patients with cutaneous and mucosal leishmaniasis. Additionally, we conducted a cross-sectional study to compare vitamin D levels in patients with leishmaniasis and healthy subjects. Hypovitaminosis D was defined as a serum level of 25 (OH) D < 30 ng/mL. RESULTS: In patients with leishmaniasis, vitamin D serum levels were 38.5 ± 11.54 ng/mL, and 37.5 ± 10.43 ng/mL in healthy subjects The prevalence of hypovitaminosis D was 23.3% and 20.0%, respectively (p = 0.72). There was no correlation between vitamin D serum levels, disease severity, and healing time in the mucosal leishmaniasis group. CONCLUSION: Vitamin D levels are not associated with neither susceptibility nor severity of tegumentary leishmaniasis.

Este estudo é parte de uma ampla investigação sobre a vivência do processo de adoção malsucedida de crianças e adolescentes sob a perspectiva dos adotantes. Foi realizada uma pesquisa qualitativa, com base em entrevistas semiestruturadas com 11 sujeitos independentes, nove mulheres e dois homens, moradores de diferentes estados do Brasil, que vivenciaram adoções malsucedidas. Buscamos analisar as percepções dos adotantes relacionadas à temporalidade no estabelecimento do vínculo parento-filial nessas adoções. A temporalidade da gestação simbólica foi vivenciada pelos participantes de diferentes formas, podendo ser afetada pela lentidão no processo administrativo e/ou por fantasias e idealizações referentes à origem da criança/adolescente. Tanto a demora quanto a tentativa de agilização do processo de adoção são fatores que podem gerar ansiedade na experiência da gestação simbólica e que não serão amparados no tempo cronológico, afetando o estabelecimento do vínculo parento-filial. Ressaltamos a relevância do cuidado nos períodos iniciais de construção do vínculo parento-filial, considerando a temporalidade particular de cada caso e a história pregressa da criança/adolescente, aspecto que influencia o sucesso do processo de adoção.(AU)


This study is part of a broad investigation about the experience of the unsuccessful adoption process of children and adolescents from the perspective of the adopters. Qualitative research was carried out, based on semi-structured interviews with 11 independent subjects, nine women and two men, living in different states of Brazil, who experienced unsuccessful adoptions. We seek to analyze the perceptions of adopters related to the temporality in establishing the parent-child bond in these adoptions. The temporality of the symbolic gestation was experienced by the participants in different ways, which can be affected by the slowness of the administrative process and/or by fantasies and idealizations regarding the origin of the child/adolescent. Both the delay and the attempt to speed up the adoption process are factors that can generate anxiety in the experience of symbolic gestation and that will not be supported in chronological time, affecting the establishment of the parent-child bond. The relevance of care stands out in the initial periods of parent-child bond construction considering the particular temporality of each case and the child's/adolescent's past history, aspect that influences the success of the adoption process.(AU)


Este estudio es parte de una extensa investigación sobre la experiencia del proceso fallido de adopción de niños y adolescentes desde la perspectiva de los adoptantes. Se realizó una investigación cualitativa a partir de entrevistas semiestructuradas con 11 sujetos independientes, nueve mujeres y dos hombres, residentes en diferentes estados de Brasil, que experimentaron adopciones fallidas. En este trabajo se analizan las percepciones de los adoptantes relacionadas con la temporalidad en el establecimiento del vínculo padre-hijo en adopciones fallidas. La temporalidad del embarazo simbólico fue vivida por los participantes de diferentes formas, las cuales pueden verse afectadas por la lentitud del proceso administrativo y por fantasías e idealizaciones sobre el origen del niño/adolescente. Tanto la demora como el intento de agilizar el proceso de adopción pueden generar ansiedad por la vivencia del embarazo simbólico y que no serán sustentados en el tiempo cronológico, lo que afecta establecer este vínculo. Se enfatiza la relevancia del cuidado en los períodos iniciales de construcción del vínculo considerando la temporalidad particular de cada caso y la historia pasada del niño/adolescente, un aspecto que influye en el éxito del proceso de adopción.(AU)

RESUMO

A transmissão psíquica trabalha no sentido de vida e de morte, por meio das alianças inconscientes, entre e através das gerações. O objetivo deste artigo é discutir o lugar da incestualidade e da sensorialidade na transmissão psíquica do negativo. Enfatizamos que a herança do negativo faz parte da pré-história dos sujeitos, e que os traumatismos não elaborados, não acessíveis à memória, são inscritos no psiquismo familiar e nos corpos singulares. A fim de ilustrar a discussão proposta, apresentamos uma vinheta clínica de psicoterapia psicanalítica de família. Concluímos que a transmissão do negativo e a qualidade incestual atuantes nos laços familiares se evidenciam por meio da sensorialidade, no atravessamento dos espaços psíquicos e na temporalidade da família, causando prejuízos no reconhecimento das diferenças e nos processos de subjetivação (AU).


Psychic transmission works in the sense of life and death, through unconscious alliances, between and across generations.The purpose of this article is to discuss the place of incestuality and sensoriality in the psychic transmission of the negative.We emphasize that the inheritance of the negative is part of the prehistory subjects and that the untreated traumas, not accessible to memory, are registered in the family psychism and in the singular bodies.In order to illustrate the proposed discussion, we present a clinical vignette of family psychoanalytic psychotherapy.We conclude that the transmission of the negative and the incestual quality acting in the family bonds are evidenced through sensoriality, in the crossing of psychic spaces and in the family's temporality, causing losses in the recognition of differences and in the subjectification (AU).


La transmisión psíquica funciona en el sentido de vida y muerte, por las alianzas inconscientes, entre y a través generaciones. El objetivo de este artículo es discutir el lugar de la incestualidad y de la sensorialidad en la transmisión psíquica de lo negativo. Destacamos que la herencia de lo negativo es parte de la prehistoria de los sujetos, y que los traumas no elaborados, no accesibles a la memoria, están inscritos en el psiquismo familiar y en los cuerpos singulares.Para ilustrar la discusión propuesta, presentamos una viñeta clínica de psicoterapia psicoanalítica familiar.Concluimos que la transmisión de lo negativo y la calidad incestual en el trabajo en los lazos familiares se manifiestan a través de lasensorialidad, en el cruce de espacios psíquicos y en la temporalidad familiar, provocando pérdidas en el reconocimiento de las diferencias y en el proceso de subjetivación (AU).

Neste artigo, discutiremos a noção de matriz sensorial familiar, evidenciando a sua face opressora e o surgimento do sintoma psicossomático como resposta ao imperativo da sensorialidade durante o processo de análise. Por meio de uma vinheta clínica, apresentaremos o caso de uma jovem que sustenta o processo de separação da família e a conquista do espaço psíquico singular via sintomas alérgicos. Propomos considerar, portanto, a alergia tanto como expressão psíquica primária defensiva quanto como reveladora de um tipo de relação de objeto alérgica, o que sinaliza questões arcaicas e objetais atuantes no psiquismo.(AU)


In this article, we will discuss the notion of the familiar sensory matrix, highlighting its oppressive face and the emergence of the psychosomatic symptom as a response to the imperative of sensoriality during the process of analysis. Through a clinical vignette, we will report the case of a young woman who underpins the separation process from the family and the achievement of the singular psychic space via allergic symptoms. Therefore, we propose considering allergy as a defensive primary psychic expression that reveals a type of allergic object relation, which suggests archaic and object issues in the psyche.(AU)

Introduction: The high numbers of accidents involving venomous animals in tropical countries is a major public health problem and has prompted the World Health Organization to place them on its list of neglected tropical diseases. The Notifiable Diseases Information System shows that the number of notifications of accidents involving venomous animals increases every year. In Brazil, accidents involving venomous animals are the number one cause of human intoxications. The public health importance of accidents and their consequential importance for workers is clearly illustrated by the more than 100,000 accidents and almost 200 deaths that occur annually. Objectives: To observe and analyze the profile of accidents involving venomous animals in Brazil's Federal District. Methods: A retrospective descriptive case series study of data from four databases containing information on accidents involving venomous animals. In addition to analyzing each one, they were also compared to each other. Results: A total of 11,376 accidents involving venomous animals from 2009 to 2019 were registered in the Federal District. There were 363 occupational accidents and zero deaths in the period. There were discrepancies between each of the databases analyzed. Conclusions: Subnotification and discrepancies should be better evaluated and subsequently resolved by the managers of each database. Actively seeking information and better organization of the organs responsible for database management are possible solutions to the current problems.

O presente estudo parte de uma investigação mais ampla sobre a vergonha e o pudor na intimidade da família e tem por objetivo investigar a percepção do pudor em famílias com filhos adolescentes. Realizou-se uma pesquisa qualitativa na qual foram entrevistados oito sujeitos do segmento socioeconômico médio. Os resultados foram analisados de acordo com o método de análise de conteúdo, vertente temático-categorial e discutidos a partir do referencial teórico da psicanálise e das ciências sociais. Considerando o objetivo do presente estudo, apresentamos e discutimos as categorias: estranhamento do pudor; transmissão do pudor: um legado em questão; novos rumos da intimidade no lar e imagem no espaço público: protegendo a vergonha dos filhos. Os resultados apontam que os participantes atribuem ao pudor um significado de intimidade corporal e sexual transmitido pelas gerações anteriores e com pouca expressão na atual interação familiar. Concluímos que a transmissão geracional do pudor vem sofrendo uma descontinuidade, tendo em vista a maior proximidade afetiva com os filhos e seus reflexos na transformação da intimidade familiar.


ABSTRACT The present study is part of a broader investigation of shame and modesty in the intimacy of the family and aims to investigate the perception of modesty in families with adolescent children. Qualitative research was conducted in which eight 8 subjects from the middle socioeconomic segment were interviewed. According to the content analysis method, the results were analyzed, thematic-categorical, and discussed from the theoretical framework of psychoanalysis and social sciences. Considering the objective of the present study, we present and discuss the categories: the strangeness of modesty; transmission of modesty: a legacy in question; new directions of intimacy in the home and image in the public space: protecting the shame of children. The results show that the participants attribute to modesty a meaning of bodily and sexual intimacy transmitted by previous generations and little expression in the current family interaction. We conclude that the generational transmission of modesty has suffered a discontinuity, given the greater affective proximity to the children and their reflexes in the transformation of family intimacy.


RESUMEN El presente estudio, parte de una investigación más amplia sobre la vergüenza y la modestia en la intimidad de la familia y tiene como objetivo investigar la percepción de la modestia en familias con hijos adolescentes. Se realizó una investigación cualitativa en la que se entrevistó a 8 sujetos del segmento socioeconómico medio. Los resultados fueron analizados según el método de análisis de contenido, temático-categórico y discutidos desde el marco teórico del psicoanálisis y las ciencias sociales. Teniendo en cuenta el objetivo del presente estudio, presentamos y discutimos las categorías: extrañeza del pudor; transmisión del pudor: un legado en cuestión; nuevas direcciones de intimidad en el hogar e imagen en el espacio público: protegiendo la vergüenza de los niños. Los resultados muestran que los participantes atribuyen al pudor un significado de intimidad corporal y sexual transmitido por generaciones anteriores, y con escasa expresión en la interacción familiar actual. Concluimos que la transmisión generacional del pudor ha sufrido una discontinuidad, ante la mayor proximidad afectiva con los hijos y sus reflejos en la transformación de la intimidad familiar.

Background: Mucosal leishmaniasis (ML), the most inflammatory form of tegumentary leishmaniasis, is predominantly caused by Leishmania braziliensis. The disease is characterized by the development of lesions, mainly in the nasal mucosa. An exacerbated inflammatory response has been associated with the presence of destructive and disfiguring lesions, with stages of severity ranging from small nodulations to the complete destruction of the nasal pyramid architecture. As Leishmania is an intracellular parasite, most immunological studies have emphasized the cell-mediated immune response, while relatively few studies aimed to investigate the role antibodies in protection against, or the pathology of ML. Methods: Patients with a confirmed diagnosis of ML were classified according to clinical staging criteria. Serum levels of Leishmania-specific IgG, IgG1 and IgG2 antibodies were determined by ELISA before and after treatment with antimony or antimony plus pentoxifylline. Results: Patients in stages IV and V produced higher concentrations of IgG and IgG1 antibodies when compared to those in stage I and II. Significant reductions were seen in the concentrations of IgG and IgG2 antibodies in most patients who responded well to treatment. Conclusions: Our data demonstrate an association between IgG antibody titers and the severity of mucosal disease. The observed reduction in antibody production after successful treatment in most patients preliminarily indicates that these tests can be used to aid in the assessment of therapeutic response.

This research is part of a broader study concerning siblings and the family network. Its objective was to investigate care among siblings and the repercussions of generational transmission on the sibling group. It employed clinical-qualitative methodology, centered on preliminary clinical interviews with families during the family assessment period. Clinical data were gathered at the Applied Psychology Service of a private university in Rio de Janeiro. The 14 families who took part in the study had the following configurations: four married-couple families, four stepfamilies, three single-parent families and three separated-parent families. We observed that both reduced parental investment and excessive family-care workload made it difficult to establish a more health-promoting parent-child relationship, often leading to the parentification of one or more of the children, with the assignment of parental functions to them. Flaws in the psychic assimilation of elements of generational transmission also influence in the creation of sibling bonds, inhibiting the development of family ties of solidarity and the establishment of healthy care practices among siblings.


Este estudo faz parte de uma pesquisa mais ampla sobre fratrias e rede familiar. Teve-se como objetivo investigar o cuidado entre irmãos e repercussões da transmissão geracional na fratria. Utilizou-se metodologia clínico-qualitativa, centrada em entrevistas clínicas preliminares com famílias no período de avaliação familiar. Os dados clínicos foram coletados no Serviço de Psicologia Aplicada de uma universidade privada carioca. Participaram da pesquisa 14 famílias, com as seguintes configurações: quatro casadas, quatro recasadas, três monoparentais e três separadas. Constatou-se que tanto o reduzido investimento parental quanto a sobrecarga nos cuidados familiares dificultam o estabelecimento de uma relação parento-filial mais promotora de saúde, muitas vezes levando à parentalização de um ou mais filhos, com atribuição de funções parentais a eles. Falhas na apropriação psíquica de elementos da transmissão geracional também repercutem na constituição do vínculo fraterno, dificultando a construção de laços familiares solidários e o estabelecimento de práticas salutares de cuidado entre os irmãos.


Este estudio es parte de una investigación más amplia sobre fratrías y redes familiares. El objetivo fue investigar el cuidado entre hermanos y las repercusiones de la transmisión generacional en la fratría. Se utilizó metodología clínico-cualitativa, centrada en entrevistas clínicas preliminares a las familias durante el período de evaluación familiar. Los datos clínicos se recolectaron en el Servicio de Psicología Aplicada de una universidad privada en Rio de Janeiro. Catorce familias participaron en la investigación, con las siguientes configuraciones: cuatro casadas, cuatro vueltas a casar, tres monoparentales y tres separados. Se encontró que tanto la menor inversión de los padres como la sobrecarga por los cuidados con la familia dificultan el establecimiento de una relación entre padres e hijos que promueva la salud, lo que a menudo conduce a la crianza de uno o más hijos con atribución de las funciones parentales a ellos. Las fallas en la apropiación psíquica de elementos de transmisión generacional también inciden en la constitución de un vínculo fraterno, dificultando la construcción de lazos familiares solidarios y el establecimiento de prácticas de cuidado saludable entre los hermanos.

ABSTRACT Introduction: The radiosynovectomy (RS) is one treatment option for recurrent hemarthrosis in patients with hemophilia (PWH). A prospective cohort study was designed to evaluate the effects of the RS on the synovial membrane volume in the ankles and knees of PWH and patient characteristics related to the RS outcome. Methods: In a one-year follow-up, 25 joints of 22 PWH who presented 3 bleeds or more in the same joint over the last 6 months (target joints) were subjected to the RS. Two groups were compared: those who retained target joints following the RS and those who did not (less than 3 bleeds/6 months after the RS). The groups were analyzed according to age, hemophilia type/severity, joint, body mass index (BMI), inhibitor and Hemophilia Joint Health Score 2.1 (HJHS). The magnetic resonance images (MRI) of six ankles and six knees were acquired prior to, and 6 months after, the RS. The synovial membrane volume and arthropathy MRI scale were accessed and volumes were compared and correlated with the Yttrium-90 dose injected. Results: Patients with a mean age of 12 years and a mean HJHS of 6.7 (p < 0.05) retained target joints after the RS. The inhibitor, joint, type/severity of disease and BMI showed no significant differences between groups. The synovial membrane volume had a significant reduction after the RS (p = 0.03), but no correlation with the Yttrium-90 dose. In proportion to the synovial membrane volume, doses injected to the ankles were larger than those injected to the knees. Conclusion: The synovial membrane volume is reduced after the RS, regardless of the effective 90Y dose.

BACKGROUND: In this study, we determined the accuracy of anti-Leishmania IgG and IgG subclasses to distinguish clinical forms of American tegumentary leishmaniasis (ATL) and and determined the relationship between antibodies levels with cytokine production and severity of ATL. METHODS: Participants were 40 patients with cutaneous leishmaniasis (CL), 20 patients with mucosal leishmaniasis (ML), 20 patients with disseminated leishmaniasis (DL), and 20 individuals with subclinical Leishmania braziliensis infection (SC). Diagnosis was performed by DNA of L. braziliensis or IFN-γ production in SC. IgG and subclasses of IgG to soluble Leishmania antigen and cytokine levels in supernatants of mononuclear cells were detected by ELISA. RESULTS: IgG was detected in 95%, 95%, and 100% of patients with CL, ML, and DL, respectively. Higher levels of anti-Leishmania IgG and IgG2 were seen in DL compared to CL, ML, and SC. ROC analysis confirmed the ability of IgG to distinguish DL from the other clinical forms. A direct correlation was observed between IgG titers and levels of IFN-γ and CXCL10 in CL and DL, and IgG2 antibodies were correlated with the number of lesions in DL. CONCLUSIONS: High anti-Leishmania IgG and IgG2 levels are characteristic of DL, and while IgG was correlated with pro-inflammatory cytokines, IgG2 was direct correlated with the number of lesions.
