RESUMO
The efficiency of postoperative analgesia using the epidural infusion of bupivacaine (0.2 mg/kg/hr) in 42 children and with opioid analgesics (1 mg per year of life) was studied in 17 children after Paltia thoracoplasty for funnel chest deformity. Analysis of hemodynamic and external respiratory parameters and visual pain scores demonstrated the high postoperative efficiency of analgesia using the epidural infusion of bupivacaine. By causing respiratory depression, opioid analgesics did not deteriorate alveolar ventilation during active postoperative management.

The investigation of the composition and properties of blood in 20 children with small intestine fistulas has revealed hypovolemia, disorders in the electrolyte composition, increased hematocrit and viscosity, changes of tests of coagulogram and thromboelastogram pointing to disseminated intravascular coagulation. Similar changes were noted in experimental studies in 20 dogs with duodenal fistulas. Drugs improving rheological properties of blood and anticoagulants gave better results of the treatment.

The activity of natural immunity factors in appendicitis reached its maximum in the period from 24 till 48 hours and then decreased. In most patients with low level of natural immunity preoperative clinical manifestations were less pronounced, the postoperation period being inactive.

It is found that in appendicular peritonitis, as the inflammatory process progresses, the protective functions of the organism are lowered, and the intensity of inflammatory changes is increased. The determination of the natural resistance status of the body in peritonitis in children may be used for an objective estimation of the process course, the efficacy of therapeutic measures and prognostication.

Phosphatase activity of neutrophils was studied in 342 patients directed to the hospital with the diagnosis of acute appendicitis, and in 56 healthy children. The index of phosphatase activity of neutrophils (IPAN) was found to be considerably increased in all forms of appendicitis in childhood. An intensity of the reaction depends on the form and terms, elapsed since the disease onset. The determination of IPAN in the complex with clinical findings could be utilized for the differential diagnosis purposes in investigation of patients suspected of acute appendicitis, while in the postoperative period - for estimating a gravity of the course and prognosis of the disease.
