RESUMO

UNLABELLED: The aim of the study was the analysis of epizootic and epidemiological situation of rabies in the region of Pomerania in 1986-1991 and the evaluation of correctness in determining indications for antirabies vaccination. In analysed period of time 441 cases of rabies in domestic and wild animals were noted. Altogether 2941 subjects were admitted to the Rabies Prophylaxis Outpatient Center (Fig. 1, Tab 1-7). Indications for vaccination were analysed in each case as well as the possibility to reduce the number of vaccinated patients in respect of economical benefits was considered. Vaccinated patients were inquired about side effects of Human Diploid Cell Vaccine (HDCV) (Merieux France). Among 2941 patients admitted to the out-patient clinic indications for rabies vaccination were given in 933 cases (31.7%). Most often they were determined due to dogs bite or another contact with dog's saliva. Postvaccinal reactions were observed in 61 cases (6.5%), mostly topical ones. Neurological complications were not noticed. CONCLUSIONS: 1. Despite the fact that the epizootic situation in domestic and wild animals has changed for worse, the number of indications for prophylactic vaccination decreased in last six years. 2. Proper evaluation of the indications for prophylactic vaccinations against rabies is the most important factor influencing the reduction in the number of vaccinated people. 3. Rationalisation of indications for vaccination against rabies justified claims to be economical one. 4. HDCV (Merieux) is a safe vaccine and its usage reduced the side effects to minimum degree.

The focus of trichinellosis was presented comprising 28 patients and resulting from consumption of the wild boar meat. Early confirmation of trichinellosis diagnosis in the first case (index case) and an accurate epidemiological analysis established that the patients became infected with Trichinella spiralis strain originating from natural environment. A severe clinical course was disclosed in the index case, moderate course of trichinellosis in 11 patients, a mild course in 15 cases and an abortive course in one patient. The most frequent trichinellosis symptoms included muscular pain (92.3% cases), fever above 38 degrees C (62.2% cases), conjunctivitis (53.3%), periorbital and facial oedema (42.9% cases); headaches and excessive sweating were less frequent (35.8%), while diarrhoea, hemorrhages to the fingernail beds and skin rush were noted in single cases only. No leukocytosis was detected in 15 patients (53.5%) and number of acidophilic granulocytes was normal in 8 patients (28.5%) including 5 patients with moderate course of the disease. Also, no full correlation was detected between severity of the clinical course and anti-Trichinella antibody titres. Increased activity of a muscular enzymes creatine kinase (CPK) could be detected in 27 patients and increased activity of lactic acid dehydrogenase (LDH) in 9 patients. The increase in muscle enzyme activity (CPK in particular) in some patients failed to correlate with the severity of the clinical course. In 10 patients parasitological and histological study of muscle tissue biopsies was performed to determine intensity of the invasion and the character of pathomorphological lesions.

RESUMO

The observations are reported of 289 patients receiving rabies diploid vaccine Merieux. The infrequent postvaccination reactions (14%) included mainly local reactions with reddening, oedema and painfulness at the injection site. These changes were short-lasting and regressed spontaneously in most cases. Systemic reactions included mainly fever with malaise (2%), headaches and low mood (1.7%). These reactions were also short-lasting and left no sequelae. Allergic reactions of the type of hyperergic purpura and urticaria were found in isolated cases only (0.3%).
