With new algorithms of pattern recognition three types of regulation of oxygen delivery and consumption were studied: at normal regulation, hypo- and hyperfunction of heart. It is shown that a surplus systemic blood flow results in increase of arterio-venous shunt flow. Insufficient blood flow results in increase of arterio-venous gradient of blood oxygen content due to the increase of gradient of oxygen content on both sides of capillary wall and to activating of vasomotor function of microcirculatory arterial bed. Quantitative estimations of shunt and capillary blood flow are obtained.

A study of 197 patients with uncomplicated duodenal ulcer revealed clinical and pH-metric particular features of the action of pariet (rabeprazole) and six secretolytics: cimetidine, ranitidine, ranisan, quamatel, omez. It was established that the main clinical criteria of treatment efficacy is duration of ulcer cicatrisation and percentage of cicatrized ulcers during 21 days, and additional criteria are duration of disappearance of clinical signs and stage of acid production suppression. Possibilities of applying methods of pharmacoeconomic study were examined: analysis of the treatment cost, cost minimization, and cost-efficacy with the definition of the increase of expense efficacy for drug selection. It was shown that the cost of the antisecretory therapy makes up from 0.45 to 9.14% of inpatient treatment. A cost-efficacy analysis showed that it is most expedient to use pariet.

Experience of the myocardium defense for correction of the heart failures in conditions of artificial blood circulation was analyzed in 55 patients. Antegrade pharmacool crystalloid cardioplegia, based on the potassium chloride solution was applied in 32 patients. In 23 patients perftoran was added to cardioplegic solution. Investigations of systemic hemodynamics in early postperfusion period data of electrocardiography and of the biopsies histological investigation as well confirm the efficacy of cardioplegic action of crystalloid cardioplegic solution raising while its oxygenation, after adding perftoran in its contents.

In 98.2% patients with infectious endocarditis, admitted to the hospital for surgical treatment, the moderate immune deficiency state is revealed with the cell link of immunity depression and phagocytic function of neutrophils insufficiency, which become aggravated after operative intervention performance, especially using artificial blood circulation. The cell link of immunity depression, the B-lymphocytes function inhibition are preserved during 6 months after the operation.

The possibility of early extubation of the first year of life children after surgical correction of congenital heart diseases while artificial blood circulation conduction was studied. For the 1996-1999 period surgical treatment was conducted in 381 patients aged at average (5.71 +/- 3.17) month, body mass at average (5.77 +/- 1.8) kg. 163 (42.7%) patients were converted to independent respiration in 6-12 hours after operation, satisfactory clinical state and indexes of gas exchange were in 122 (74.8%), 8 patients needed reintubation. Early extubation permit to reduce of respiratory complications frequency, duration of treatment in intensive therapy unit.

The content of some lipid fractions of the bile and blood serum in calculous and acalculous cholecystitis has been studied by thin-layer chromatography. The findings evidence that a significant reduction of the free fatty acid concentrations and of the ratio of these acids and bile cholesterol is a characteristic feature of cholelithiasis. It has been also revealed that the activity of the serum lipolytic processes does not answer the hypertriglyceridemia level. The detected shifts may be used to characterize the bile lithogenicity and to diagnose cholelithiasis.

The influence of isocaloric semi-synthetic rations with varying protein components (casein, wheat gluten, isolates of cotton, soybean and rice proteins) on lipid transport processes in the blood serum was studied in experiments on made August rats. It has been shown that the alimentary proteins studied affect blood serum lipids and lipoproteins by altering lypolysis activity in the serum, by modifying the hepatic production of high-density lipoproteins and their free cholesterol, by disturbance of low-density lipoprotein degradation. The rest mechanisms regulating lipid transport in the blood serum are changed insignificantly.

Balanced diet during 60-day hypokinesia leads to inhibition of lipoprotein lypase (LPLA) and liver triglyceride lypase (L-TGLA) activity of the rat blood serum. The level of very low density lipoproteins (VLDLP) grows, and suppression of lecithin-cholesteryl-acyltransferase (LCAT) activity is accompanied by reduction of the share of cholesterol derivatives with polyunsaturated fatty acids. Combined effects of protein-vitamin insufficiency and hypokinesia result in parversion of the lipolysis processes, that manifests in prevalence of L-TGLA over LPLA. The levels of VLDLP increase, and growth of LCAT activity is acompanied by the growth of cholesteryl linoleate share and level. Hypokinesia combined with the studied experimental diets was found to lead to increase of the free fatty acid level and to decrease of the blood serum levels of phospholipids and triglycerides.

Distinct alterations in the reactions responsible for development of lipoprotein hydrophobic nuclei was observed in rat of the August strain maintained for 60 days under conditions of hypokinesia on a ration, containing wheat gluten as a protein source and deficient in retinol, tocopherol and ascorbic acid. Under these conditions the ratio of activities of lipoprotein lipase and liver triglyceride lipase was altered; acylglycerols were accumulated in lipoproteins of low and very low density. Besides, cholesterol esters and their fractions were increased both in blood serum and in individual classes of lipoproteins. Increase in content of cholesterol bound with arachidonic and linolenic acids in lipoproteins of very low density was a typical pattern developing due to hypokinesia in animals maintained on various experimental diets.

Long-term hypokinesia of August and Wistar Albino Glasgow rats caused some alterations in composition of total lipids and cholesterol esters both in blood serum and in individual lipoprotein fractions. The most distinct alterations were the following ones: I. increase in content of very low density lipoproteins (VLDL) and of low density lipoproteins (LDL), 2. decrease in the level of cholesterol esters in blood serum with a simultaneous increase of their content in VLDL and LDL as well as with alteration of their composition, 3. decrease of triglyceride content in VLDL and increase--in LDL, exhibiting the alteration in activity of lipolytic enzymes. Functional activity of muscles was very important for maintaining of the normal level of blood serum lipids and lipoproteins.

The two-month feeding of male August rats with the wheat gluten-containing diet led to an increase in the blood level of low density lipoproteins (LP), free and esterified cholesterol, free fatty acids and acylglycerides. Meanwhile the content of very low density LP and high density LP declined. Cholesterol esters showed a rise in the content of cholesterol linolenate, cholesterol oleate and cholesterol palmitate. Liver microsomes demonstrated an increase in the amount of arachidonic and eucosotrienic acids. The shifts cited were largely determined by the changes in cholate formation and bile excretion, which were marked by the reduced levels of tauro- and glycocholic acids, increased amount of phospholipids and free fatty acids in the bile, and decreased content of cytochromes P-450 and b5 in hepatocyte microsomes.
