RESUMO

The article presents the analysis of data of "Study on Global AGEing and Adult Health, 2007-2010" (SAGE) concerning subjective judging of health, prevalence of ischemic heart disease under different levels of arterial pressure in similar age groups. An attempt is made to evaluate spectrum of pharmaceuticals applied in treatment of patients with arterial pressure in hospital at the level of oblast hospital. According official statistics of Minzdrav of Russia, in 20102 prevalence of diseases characterized by higher arterial pressure amounted to 10.5% in adult population and to 22.3% in population older than able-bodied age. At the same time, according SAGE data, prevalence of various forms of hypertension disease in population older than 50 years amounted to 52.8%. That is, this pathology is often missed in conditions of mass medical network. Thereafter, patients receive no necessary medicinal therapy. The one or another treatment was provided to almost 94% of patients with hypertension disease diagnosed by physician. At that, among patients received treatment the normal levels of both systolic and diastolic pressure were established in less than 20% of patients with hypertension. Therefore, the issue of effectiveness of applied methods of correction of arterial hypertension become a matter of interest. The appropriate treatment of hypertension is a mean to decrease complications and as a result to diminish mortality of diseases of circulatory system.

RESUMO

The comprehension of significance of health impacting of such unchangeable factors as climate and atmospheric phenomenons allows carrying out measures to decrease their negative effect on human health. The data of study implemented in Orel in 2009-2012 made it possible to distribute the annual totality of diagnoses registered in medical organizations on the groups of diseases of blood circulation system according ICD-10. The analysis established lower level of visits to physician because of these conditions both in ambulatory polyclinic institutions and hospital during summer period. The corresponding percentage came to 21.9% out of all diagnoses in polyclinic and 22% in hospital at highest levels of 27% in summer period. Therefore, in development of population contacts with medical organizations an expressed seasonality is detected. This occurrence is possibly related to characteristics of development of pathological conditions in different seasons.

RESUMO

The article deals with issue of physical development of children as an important summarizing health parameter and indicator of social well-being of society. The organism development in child age determines the main traits of health of this generation in older age including potential longevity and transferring of corresponding qualities to future generations. The analysis is presented concerning accumulation of factual data and development of methodical instruments to evaluate physical development of children in Russia and other countries. The universal application in Russia of corresponding WHO standards is proposed. The necessity to include the integrated indicators of physical development into official statistical reporting is emphasized.

RESUMO

The results of analysis of actual situation in the field of medical care provision to population by medical institutions of different form of property and departmental belonging demonstrated that the percentage of additionally identified cases of pathology of blood circulation system comes up to 20% and more in certain territories. This occurrence mainly depends on organization of medical care of population in the field and development of medical institutions network outside the system of Minzdrav of Russia. It is needed to further elaborate the process of getting the data specifying the statistics of pathology propagation.

RESUMO

The comparative analysis of mean duration of stay in hospital of patients with pathology of circulatory system in the Russian Federation (form No 14 "The information about hospital activities in 2011") and particular European countries (data about main characteristics of functioning of hospitals in European countries from the information system "The Hospital morbidity Database (HMDB) January 2011") established the characteristics of structure of hospitalized patients of the mentioned class including differences in duration of stay of patients in hospital. The significant differentiation between countries exists concerning the indicators of mean duration of stay in hospital of patients with various diseases of circulatory system. At that, in the Russian Federation certain indicators of particular nosology forms are lower than corresponding indicators in foreign countries. This peculiarity is to be taken into account in organization of medical care of these patients in Russia national conditions.

RESUMO

The analysis of data bases established that in the Russian Federation population mortality of diseases of blood circulation system in toto and of main diseases included in this class (100-199 of ICD-10) is significantly higher than in European countries. The population mortality is determined both by morbidity and quality of medical care in case of development of disease. In Russia, the increase of number of cases of hospitalization per one patient passed away due to these causes. This indicator was lower in comparison with corresponding indicators characterizing levels of hospitalization in EU countries. In Russia, to decrease mortality of this pathology it is needed to extend indications for hospitalization.

RESUMO

Despite showing that cardiovascular disease mortality in Russia is declining evaluation of medical care for cardiovascular patients, using different information sources, revealed a lot of problems in this field need it's solving. Together with modernization of medical services it is urgently necessary to transform the medical education, including post graduate, information support for medical professionals in field of modern medical technologies, using in countries with low mortality rates, creation the conditions for regular updating professional knowledge. It is necessary to reconsider formal criteria for medical care evaluation, especially taking into account co morbidity of cardiovascular diseases. Our data illustrate that social disparities influence on outcomes of diseases and so for further decreasing mortality rates and increasing the life expectancy adequate treatment must be provide for all patients independently of their material wellbeing.

The article deals with the study of actual rate of abortions in Russia. It is established that multi-aspect statistical information is needed to compare with the data of international organizations. The regular monitoring is important to control the different characteristics of population reproductive behavior abortion prevalence and the all-inclusive participation in the reporting activity of all medical institutions involved independently of property forms and sectorial membership.

RESUMO

The analysis of report forms No 12 "The information about number of diseases registered in patients residing in the area of medical institution supports" and No14 "The information about functioning of hospital" demonstrated that in conditions of ambulatory policlinic institutions there is no active monitoring of patients with diseases of blood circulation system. These diseases are the major causes of population mortality.

The standardized indicators of mortality of population in Russia exceed the corresponding indicators in Europe in three groups of pathology: external causes (4.7 times), cardiovascular diseases (3.1 times), symptoms, signs and deviations (3.2 times). In 2010, the morbidity of children aged 0-14 years and 15-17 years included 2.2% of symptoms, among adult population 0.3% in totumn; in 2007-2.6% in children and 0.4% in adults; in various territories in children 6% (Stavropolsky kray Nenetsky natsionalny okrug), 11% (Voroniejskaya oblast) and even 14% (Nijegorodskaya oblast). The symptoms, signs and deviations are to be taken into account and to be monitored because they can be first signals about disease in process of formation and final diagnosis is yet to be done. According to the data of international study "The global ageing an adult health" the role of symptoms as a cause of turning to medical care will only to increase paralleling the ageing of population.

RESUMO

Comparative analysis founded on WHO European database was carried out concerning mortality in progress as compared between Russia and CIS countries. It is testified that the Russian Federation is distinguished by excelling rate of reduction of able-bodied population of middle age group. It is suggested that in this process significant role is played by health care system defects along with life-style factors. Motivation for enhancing activity and attention of medical workers as related to this age group, especially males is needed.

RESUMO

An analysis of the results of a sociological study showed that both men and women aged around 50 and noting subjectively no chronic pathology regard health as a life value. Social hardships reliably worsen the preventive activity and limit its variability. An essential share of population (above 30%), while taking care of its health, does not give up such harmful habits as smoking and abuse of alcohol. The described data denote the need in not so much propagating the healthy mode of life as in motivating it. The disturbing trend towards ignoring the motivations targeted at health promotion deserves special attention; the above trend is observed in absence of age-related chronic pathologies and against the background of an improving social status especially in men.

Comparative description of two groups of population--seeking and not seeking emergency medical care--is presented. To demonstrate dynamics of appealability for particular types of medical care official statistics data from 1999 to 2003 was used. To explore characteristics of health and self-care behavior in population seeking medical care data of survey carried out during 1999-2000 on various national territories was applied. In respondents seeking out-patient and in-patient care during a year demand in emergency medical care was higher in men than in women. In both genders this demand increases successively with age whereas contingent of persons who evaded extreme deterioration of health during a year reaches its maximum at the age of 30-49. More direct involvement of patient care institutions is needed with such patients as males, older people, indigent and least educated persons who are much less than others inclined and capable to obtain completeness and effectiveness of one's diagnostic and treatment.

Objective evaluations of population's health belong to the most important tasks of the public-health science. Such evaluations made in respect to children are of extra importance. One must comprehensively understand the potentialities and abilities of the methods used for the purpose. A comparative evaluation of 387 children, aged 10, was based on the findings of medical check-ups and on questionnaires of their parents. 37.7% of children, who were evaluated by their mothers as having good health, were diagnosed by doctors as having a chronic disease, i.e. the diagnosis did not entail any life limitations for such children. 40% of those children, who were referred to by their parents as sick (poor health), were classified by doctors as belonging not to category 3 (chronic pathology) but to a category of healthier children (category 2). Below 10% of children, who were describe by their parents as having poor health, were among those who were put by doctors on the list of group 3 on the basis of medical check-ups. As for those children who had an actively displayed chronic disease with a pronounced clinical course, like bronchial asthma, ulcer etc., the opinions of doctors and of their parents coincided in a majority of cases. The comparative evaluations of children' health based on questionnaires of their parents and on medical check-ups exposed a certain coincidence and specificity of such evaluations since they are based on different approaches.
