The authors studied the diagnostic possibilities of ultrasonic angiography and doppler sonography as compared to digital subtraction angiography. A total of 506 arterial segments was examined in 116 males suffering from various degree of atherosclerosis obliterans of the lower limb vessels. The coincidence of and disparity in the diagnosis were analysed on the aortoiliac and femoropopliteal levels according to three criteria: normal vessel, stenosis, occlusion. The diagnostic efficacy was appraised with the use of the Kapp index (K), the percentage of absolute coincidence of the diagnoses, sensitivity, and specificity. The precision of doppler sonography was found to be comparable with that of digital subtraction angiography. On the aortoiliac level K-0.68 +/- 0.06, on the femoropopliteal level K = 0.78 +/- 0.05. The diagnostic possibilities of ultrasonic angio-scanning were significantly lower. Thus, on the aortoiliac level K = 0.28 +/- 0.07, on the femoropopliteal level K = 0.4 +/- 0.05. Doppler sonography is therefore a reliable noninvasive method in the diagnosis of obliterating diseases of arteries of the lower limbs.

Reinfusion of the blood irradiated with the ultra-violet rays in patients with chronic insufficiency of the blood supply of the extremities leads to significant increase in the linear and volumetric blood flow rate in the damaged extremities, improvement in the microcirculation, reduction in the blood viscosity. The effect doesn't depend on the genesis of occlusion and degree of the impairement of circulation.

Examination of 38 patients with chronic postembolic pulmonary hypertension revealed gross hemorheological disorders: increased viscosity of blood, extreme aggregation activity of blood cells, diminished property or erythrocytes to undergo deformity when circulating through the capillaries. Small amounts of UV-irradiated autologous blood were infused repeatedly for the correction of these disorders. After a course of treatment the hemorheological indices were normalized in all the patients, which was attended by improvement of the general condition and positive hemodynamic shifts.

A correlation of the degree of increase of activity of lipid peroxidation with genesis and degree of the disease is considered. It was shown that a course of infusions of UV-irradiated autologous blood decreased the lipid peroxidation activity and inhibited thrombocyte aggregation.

Experience with ultrasonic investigation of venous abnormalities in the legs has suggested that combined use of dopplerography, doppler scanning and echolocation techniques can adequately replace conventional radiopaque phlebography in most cases of varicose veins and post-thrombotic disease. This combination has high diagnostic value.

Radiopaque and radio-isotope findings in the pulmonary arterial channel in 43 patients with postembolic lesions of the pulmonary trunk and its major branches are reported. Angiographic and scanographic symptoms are described. The significance of detected systolic-pressure gradient in the probing of pulmonary vessels is demonstrated. Possible sources of diagnostic errors are analysed, and ways of eliminating them are suggested. Total and selective multipositional angiopulmonography is believed to be the principal method of diagnosing this condition, while pulmonary scanning only allows topical diagnosis in cases of markedly stenosed major pulmonary arteries.

Good results were obtained in the treatment of 10 patients with massive embolism of the pulmonary artery by regional infusions of small doses of streptase and heparin. The efficiency of the method was dependent on duration of the disease and character of the embolic occlusion of branches of the pulmonary artery, a prolonged activation of the patient's fibrinolysis contributing to lysis of emboli and to recovery of pulmonary blood flow in late follow-up terms.

The examination of 132 patients with thromboembolism of the pulmonary artery has shown correlation between the clinical manifestations of the disease and the volume of lesions of the pulmonary vascular bed according to the data of scanning which evidences certain relationships between the clinical symptoms and perfusion deficiency in the lungs.

The article deals with chronic pulmonary hypertension consequent upon thromboembolism of the pulmonary artery. From experience gained in the treatment of 15 patients 4 of whom underwent operation, the authors discuss the pathogenesis, clinical picture, diagnosis, and treatment of chronic postembolic pulmonary hypertension.

The article is based on the analysis of 11 implantations of intravenous filters in 108 patients. The results of iliocavagraphy, perfision scanning of lungs and angiopulmonography were used in addition to clinical data. The authors arrive at a conclusion that the implantation of the intravenous filter is an effective and safe method for the surgical prevention of thromboembolism of the pulmonary artery. It was noted that in the postoperative period there appeared an occlusion of the iliocaval segment resulting from embolic closure of the filter or the development of distal occlusing thrombosis.
