RESUMO

The data on etiology, clinical signs, diagnosis and variety of approaches to the treatment of gastrointestinal bezoars were presented, basing on the worldwide literature analysis. The results of surgical treatment of patients for bezoars, using miniinvasive methods, are presented.

RESUMO

Experience of treatment in the clinic of 162 patients with pancreatic pseudocyst was analyzed. Modern principles of diagnosis and treatment of patients were adduced. Operation before the cyst capsule formation is compulsory, caused by the complications occurrence and the pancreonecrosis progression. External drainage of an acute pancreatic pseudocyst was performed in 61 patients, internal drainage--in 72. After external drainage the cyst recurrence was noted in 17, the pancreatic fistula formation--in 7 patients. The poorest results were noted in patients with infected pancreonecrosis and in pseudocyst formation.

RESUMO

The results of surgical treatment of 320 patients with an acute biliary pancreatitis are analysed. The therapeutic-tactical algorythm, using miniinvasive endoscopical interventions, is elaborated. Mortality for pancreatitis in systemic-toxical phase have constituted 10.1%, and in septical one--33.3%.

RESUMO

The increased risk factors--gender, age of the patient, the cholelithic disease existence duration, the acuteness of inflammation, the operations performed earlier on the abdominal cavity organs, influencing the laparoscopic cholecystectomy conduction for Courvoisier's gallbladder--were analyzed. The increase of intraoperative complications occurrence (hemorrhage, perforation of gallbladder, the outhepatic biliary ducts) in patients of elder age with durable disease was noted.

RESUMO

At present the endoscopic methods are the most acceptable in the treatment of gall-stone disease and its complications. Obstructions of terminal part of common bile duct and bile duct stones are removed by means of endoscopic sphincterotomy. Cholecystectomy is performed by means of laparoscope. A decrease in mortality and post-operative complications has been noted. At the same time negative affects of endoscopic operations in patients with concomitant somatic diseases were revealed. These patients require individual methods of approach to guarantee safe operation.

RESUMO

The results of surgical treatment of 853 patients with obstructive jaundice of non-tumor genesis and initial bilirubin level in the blood of 100 mumol and higher are analysed. Individualization of therapeutic-tactical approach with the use of preoperative diagnosis of the level of obstruction of the bile ducts and severity of the state of a patient, and as well their preoperative decompression with the use of endoscopic methods permit to accomplish a number of measures aimed at reduction of the incidence of purulent-necrotic complications and lethality, improve the long-term results of treatment of the patients with obstructive jaundice.

RESUMO

Course of the different forms of acute pancreatitis was studied in 234 patients. The therapeutico-tactical approach was individualized on the basis of considering the form of pancreatitis, its etiology, and four groups of the patients were distinguished. Conservative therapy with the wide use of therapeutic endoscopy permits in the majority of cases to achieve an abortive course of the disease. The developed programme for diagnosis and treatment of acute pancreatitis contributed to decrease in the incidence of purulent-necrotic complications in generalized forms of pancreatitis.
