[Basis of the algorithm of patient conducting using removable dentures in case of partial teeth loss].

Paper is devoted to the actual problem in stomatology - removable dentures influence upon the intensity of prosthetic bed relief changes. Study of prosthetic bed relief in patients who for 2 years had used plate removable and clasp dental prostheses was done. The most pronounced prosthetic bed relief changes were seen under plate type of prostheses with prolonged defects the least - in patients with plate type of prostheses with short defects. In cases of end defects under clasp dental prostheses changes of prosthetic bed relief exceeded relief changes under plate type of prostheses during all terms of use. Based upon the received data algorithm of patient conducting during different terms of use by plate removable and clasp dental prostheses.

[Evidence based medicine as background for development of stomatological patient report conducting].

Urgent need in methodology of information estimation for true clinical decision taking gave powerful impulse for evidence based medicine concept development that appeared at the end of 80th years. Proved efficacy and safety, optimal correlation of the indices were the stages of the methods and means selection for rationing. In our country such approach found its reflection in developing clinical protocol conducting (standards for doctors), in vitaly important medicines listing and lists of medicines that could be refunded in the system of additional medicine provision.

[Clinical-epidemiological analysis of the results of treatment by non-removable constructions in patients with partial absence of teeth].

In representative clinical-epidemiological study of the results of treatment of the patients with embraced defects of dentitions with non-removable bridge-like constructions having intermediate part of 1 to 3 artificial crowns has revealed risk profiles of complications and defects. It is shown that cumulative portion of complications by remote results for 10-year period reaches 33% for punched-soldered dentures and 10%--for cast dentures. Models of the type of "tree of decisions" for prognosis of non-removable bridge-like dentures are proposed.

[Analysis of conflict situations in orthodontics by Moscow Municipal Committee for Expert Evaluation of Orthodontic Treatment and Denture Making]. / Analiz konfliktnykh situatsii v ortopedicheskoi stomatologii po dannym gorodskoi komissii Moskvy po ékspertize ortopedicheskogo lecheniia i izgotovleniia zubnykh protezov.

The activities of Municipal Committee for Expert Evaluation of Orthodontic Treatment and Fitting with Dentures promote resolution of conflicts: only 5-7% conflict affairs are investigated in the court after investigation in the Committee. A total of 686 patients, mainly with combined maxillofacial diseases, were consulted for 3.5 years of its work. Each patient had 2 diseases and/or complications, on the average. The most frequent complaints were associated with common crowns. The patients complained of pain, poor cement fixation of bridge dentures, and difficult chewing. The most common deviations from routine regulations of orthodontic treatment, which led to development of complications, are enumerated.

[Forensic medical expert evaluation in connection with civil actions of patients complaining of improper dental care]. / Sudebno-meditsinskaia ékspertiza po grazhdanskim iskam patsientov na nekachestvennoe okazanie stomatologicheskoi pomoshchi.

The authors discuss questions facing forensic medical experts in connection with civil "medical" actions. The questions are classified as common and private. Expert evaluations of correspondence of the scope and quality of dental care are offered as the base for analysis of these problems within the framework of civil legislation. The data evidence efficiency of forensic medical expert evaluations in civil legal procedure and the significance of forensic medical service in the solution of problems of quality and efficiency of medical (including dental) care of the population.

[The orthodontic treatment of traumatic occlusion in sagittal bite anomalies]. / Ortopedicheskoe lechenie travmaticheskoi okkliuzii pri sagittal'nykh anomaliiakh prikusa.

A total of 288 patients (100 females and 188 males) aged 25 to 60 with prognathic and progenic occlusion were examined and treated. Dentition deformations were revealed in 182 (63.1%) patients. Clinical and x-ray signs of traumatic occlusion were observed in 219 (76.04%) cases. Abnormal mobility of teeth was detected in 181 (79%) subjects with prognathic occlusion, dentogingival and osseous pouches in 164 (71.6%) subjects. Reduced height of occlusion was found in 68 (25.3%), dysfunction of the temporomandibular joint in 19 (8.2%) cases. Traumatic occlusion was observed in 38 of the 59 patients with progenic occlusion. In sagittal abnormalities of occlusion, traumatic occlusion was associated with dysfunctions of the masticatory muscles and of local circulation in the periodontium, which were recorded by electromyography and rheoparodontography. The treatment was carried out by therapists, surgeons, and orthodontists. Orthodontic treatment consisted in levelling of the occlusion surface of dentitions, orthodontic preparation of the maxillodental system, fitting with splints and prostheses. Remote results in periods from 1 to 3 years were good in 94.8% of patients.

Positive follow-up results achieved in patients with metal-glass ceramic dentures coated with Simet permit the authors to recommend glass ceramic Simet for wide clinical introduction. In reconstructive dental prosthetics with metal glass ceramic and cermet materials the dentures should be modelled in the articulator. Preventive examinations are recommended once in half a year.

Good results of clinical follow-up of patients with cermet dentures coated with cimet (glass ceramic) permit recommending this type of coating for wide practical use. Reconstructive prosthetics with glass ceramic and metal and cermet dentures should involve denture modeling in the articulator and a prophylactic check-up at least once a year with oral hygiene and functional occlusion control.
