RESUMO

Differential diagnosis between essential hypertension and secondary arterial hypertension (AH) that developed in the presence of chronic pyelonephritis was made in 12 patients with AH and the urinary syndrome (leukocyturia and bacteriuria). With that purpose in view for revealing functional renal asymmetry, a study was made of the effective renal plasma flow (ERPF) and glomerular filtration rate (GFR) in each kidney separately based on computation of 99Tc-DTPA and 131I-hippuran clearances. Some patients in whom AH was later regarded as essential manifested practically equal values of the ERPF and GFR from both sides, with the real characteristics of the latter ones being little different from the respective populational values calculated by computer bearing in mind the body surface of those patients. The other patients' group showed, along with a decrease of the total ERPF and GFR, a dramatic asymmetry of the intrarenal hemodynamics of both ERPF and GFR, which is fairly characteristic of chronic pyelonephritis. This allows the syndrome of AH to be regarded in those patients as secondary.

RESUMO

Methods of radionuclide diagnosis modified with regard to the clinical problems and combined with new mathematic approaches to the processing of the data obtained make it possible to substantially enlarge the diagnostic information on myocardial contractility, central hemodynamics, the status and perfusion of the myocardium and to use it in combination with the latest research methods applied in cardiological patients.

Eighteen patients aged 60 to 80 years suffering from chronic pyelonephritis (CPN) and nephrogenous arterial hypertension (AH) were examined for intrarenal hemodynamics: effective renal blood flow (ERBF), glomerular filtration rate (GFR), filtration fraction (FF), renal vascular resistance (R). Renal hemodynamics in patients with senile pyelonephritis and AH was found to be characterized by a decrease of ERBF, GFR, FF, an increase of R (p less than 0.01), with the alterations being related to the AH intensity, mass of the functioning renal parenchyma (for GFR) and partly (for R) to the pyelonephritis and AH standing. R which is dependent on the duration and course (number of exacerbations) of CPN is one of the factors that determine the development and gravity of AH in senile pyelonephritis.

RESUMO

Coronary flow reserve was assessed by means of the 133Xe clearance test making use of dipyridamole in the left coronary artery of 11 patients with angina of effort and coronary arterial stenoses involving also the basin of the left coronary artery (group 1), 6 patients with the anginal syndrome showing no angiographic evidence of coronary lesions (group 2A), and 6 patients with cardialgias and intact coronarograms (group 2B). Coronary reserve was shown to be decreased in groups 1 and 2A, as compared to the values shown by group 2B patients.

The authors present a mathematical model for determining the separated effective renal blood flow (ERB) and a computer program which was used to determine separate parameters of the ERB in 56 patients including 35 with a unilateral lesion of the kidneys (20 patients with renovascular hypertension and 15 with unilateral pyelonephritis) and 21 patients with symmetrical renal damage (12 patients with chronic diffuse glomerulonephritis and 9 with essential hypertension). The patients with a unilateral renal lesion exhibited a significant decrease in the ERB parameters on the affected side whilst the patients with the symmetrical damage showed no significant differences in the ERB values. Following the surgical correction of the stenosing renal artery, the patients with renovascular hypertension displayed in the presence of a pronounced hypotensive effect a redistribution of the ERB with its increase on the side of the interference. A similar although less evident tendency toward the predominant increase in the ERB on the affected side was also observed in the patients with unilateral pyelonephritis. The patients with the symmetrical damage to the kidneys presented no redistribution of the ERB after the treatment.

RESUMO

Labelled microspheres in combination with reactive myocardial hyperemia were used to assess the state of the expansion reserves of the myocardial microcirculation in patients with coronary heart disease (CHD). It was found that in the majority of CHD patients the expansion reserves were reduced whereas in patients with marked atherosclerosis of the coronary arteries and post-infarction cardiosclerosis they were absent altogether.

RESUMO

Scintigraphy with 99mTc -Sn-glucoheptonate permits the determination of the focus of transmural myocardial infarction at the earliest stages of the disease (from three hours to the fifth day of the disease) whereas scintigraphy with 99mTc -Sn-pyrophosphate makes it possible to follow-up the zone of transmural infarction from Day 2 through Day 7-10. Both methods fail to provide enough information to diagnose non-transmural myocardial infarction. A comparative evaluation of the nature of glucohepatonate and pyrophosphate incorporation in the myocardium of patients with unstable angina is suggestive of the severity of ischemic changes in the myocardium.
