RESUMO

This article summarizes the experience of managing patients with COVID-19-associated pneumonia treated in one of the intensive care units of a multidisciplinary hospital with and without a heated oxygen-helium mixture. It has been shown that the use of a heated oxygen-helium mixture in the complex therapy of pneumonia caused by SARS-CoV-2 is effective. A comparative analysis of the clinical manifestations of the disease and the results of laboratory tests in the main and control groups of patients confirm with a high degree of reliability the improvement of blood oxygenation, the normalization of the acid-base balance and the positive dynamics of the main parameters, which ultimately increases the efficacy and reduces the time of treatment.

RESUMO

Acute surgical diseases were and remain one of the most important problems of the organization of medical care and treatment of patients in the conditions of long distant sea voyage, when there is no possibility for medical evacuation. We analyzed the positive experience of surgical care in the sea, gained by Soviet and then and by Russian Navy physicians. As we haven't registered significant changes in morbidity of Navy crewmembers, we think that studying and creative application of this experience will have the positive effect.

RESUMO

The article is devoted to the history of formation and development of the Central Navy Clinical Hospital (since 2010--3rd branch of Burdenko General Clinical Hospital) for the last 30 years. Nowadays normal bed capacity totals 600 beds, 30 medical and preventive units function in the hospital. Annually 11,000 patients receive treatment in the hospital.

The structure of accidents and injuries in victims of major maritime disasters involving fires and explosions on warships from 1976-2011. It is established that the main damage of the injured are acute inhalation of carbon monoxide poisoning is mild, 33.3%, combined blast and mechanical trauma 25%, the combined mechanical and burn trauma-cold 21.2%, the combined mechanical-burn injury 13.4%, combined blast and cold injuries 5.3%, burns and chemical damage a 1.8%. The main causes of mortality were acute inhalation poisoning with carbon monoxide, severe burns of the upper respiratory tract bums, III and IV 12.4%. Bums I and II degree against carbon monoxide poisoning is mild with drowning as a major cause of death recorded 11.8%; combined mechanical-burn were travmy 19.2%, the combined mechanical-burn-cold injury was 9.4%, combined blast and mechanical trauma 26.3%, combined explosive, mechanical, and cold injuries 14.7%, and chemical burn of 6.2%.

RESUMO

The traumas received by victims in the largest sea catastrophes and accidents, connected with case (hull) damage, overturning and flooding of courts from 1983 to 2010 are considered. It is established that principal views of damages at rescued and victims are sharp hypothermia of light and moderate severity, combined mechanic-cold and mechanic-burn-chemical traumas, drowning. The analysis of the received combined traumas reveals prevalence of heavy damages among the rescued victims in the sea catastrophes and accidents. The size and structure of sanitary losses depends on weight of accident, remoteness from coast, weather conditions, speed of flooding of a vessel, readiness of forces of rescue and the beginning of carrying out of a rescue operation, training of crew and passengers.

RESUMO

For last few years in Russia and in the Armed Forces of RF is marked a growth of number of acute intoxications by GABA-agonists and by muscle relaxants, particularly by baclofen. Manifestations of intoxication by this specimen include cerebral and somatic-vegetal disorders, in grave cases of illness could evolve psychic excitement, delusions and coma. Treatment of baclofen intoxication compares of gastric lavage using a thick pathfinder, accompanied by insertion of adsorbent and apocathartic means, maintaining of impaired vital functions of body. Hereafter these injured patients should be hospitalized in a treatment institute, where exists a possibility of effectuating of reanimation and intensive care as well as extracorporal detoxication (hemosorption, hemodialysis).

RESUMO

Outhospital pneumonia and bronchitis by service men by call-up in Northern fleet is still high. In 2004 for prophylaxis of these diseases in calendar of prophylactic immunization for service men was used vaccine "Pneumo-23" of the firm "Aventis Paster" (France). In the autumn period 900 call-up persons were vaccined. There was analyzed the dynamic of morbidity of bronchitis and outhospital pneumonia among the call-up persons during following 4 months. Veridical reduction of morbidity in analyzed groups of vaccined and not vaccined wasn't found. It was proposed to realize vaccination by "Pneumo-23" for 1-3 months earlier the call-up.

RESUMO

The investigation of 198 servicemen suffered from peptic ulcer has revealed the clinical specific features of disease course and specific character of relapse season. The multiple correlation between such parameters of psychological status as personal anxiety, neurotism, emotional non-stability and frustration tension were established. The leading type of attitude to one's own disease (ergopatic) conditioned by high level of realizing the social needs and by the aspiration for maintaining the professional status.
