Periodontal diseases significantly impact about half of the global population, and their treatment often encompasses relieving symptoms as well as regenerating the destroyed tissues. Revolutionary research in the management of periodontal disease includes biomaterials, a boon to re-generative dentistry owing to their excellent biological properties: non-toxicity, anti-inflammatory, biocompatibility, biodegradability, and adhesion. This study aimed to fabricate an injectable fucoidan containing chitosan hydrogel and prove its effectiveness in periodontal bone regeneration. The injectable hydrogel was prepared using the sol-gel method and was subjected to various physical, chemical, and biological characterizations to understand its efficacy in formation of new bone. The effectiveness of the developed hydrogel was assessed in periodontal bony defects to study the soft and hard tissue changes. A total of 40 periodontitis patients with bony defects were recruited and randomized into two groups to receive fucoidan-chitosan hydrogel and concentrated growth factor, respectively. Customized acrylic stents were used to guide the hydrogel placement into the defect site. Post-surgical changes in clinical parameters were assessed at 3, 6, and 9 months to appreciate the soft and hard tissue changes using repeated measures analysis of variance and Bonferroni's post hoc test. Significance was kept at 5%. The porosity, water uptake of the prepared hydrogel showed good efficacy, with particle size of the fucoidan containing chitosan hydrogel of 6.000 nm. The MG-63 osteoblasts cell line revealed biocompatibility, biodegradability and showed slow and sustained drug release, increased cell proliferation, and enhanced alkaline phosphatase secretion. Mineralization assay was greatest in the fucoidan containing chitosan hydrogel. Clinically, it exhibited significantly lower probing depth values and a higher mean improvement in clinical attachment level as compared to the concentrated growth factor (CGF) group at the end of 3 and 6 months (p < 0.05). The mean of the defect fills in the fucoidan containing chitosan group was 1.20 at the end of 9 months (p < 0.001) as compared with defect fills observed in the CGF group. The presence of fucoidan in the hydrogel significantly contributed to bone regeneration in humans, thus strengthening its potential in tissue engineering. Fucoidan-chitosan will be a promising biomaterial for bone tissue regeneration.

Technological advances in the field of implantology have led to the concept of surface modifications to enhance implant stability by utilization of current concepts of tissue engineering and materials such as platelet concentrates and stem cells. The purpose of the present randomized controlled clinical trial was to evaluate and compare the effect of platelet rich fibrin matrix (PRFM) with and without peripheral blood mesenchymal stem cells (PBMSCs) on implant stability; by assessing the bone to implant contact (BIC) using resonance frequency analysis (RFA), insertion torque and also to establish and correlate the same with implant stability quotient (ISQ). A total of 15 patients with 30 sites ensuring a minimum of two dental implants adjacently placed in an edentulous area; with the age group of 25-50 years of both the sexes were categorized into Group 1 (dental implant with PRFM) and Group 2 (dental implant with PBMSCs embedded in PRFM). Insertion torque values at the time of dental implant placement and ISQ using RFA was recorded at 1 week, 1 month, and 3 months post operatively. There was no significant difference (p = 0.81) in Insertion torque values between both the groups (G1 and G2). Platelet rich fibrin matrix along with PBMSCs enhanced implant stability as higher and statistically significant ISQ values were noted at 1 week (p = 0.18), 1 month (p ≤ 0.001), and 3 months (p ≤ 0.001) intervals in the G2 group. Platelet rich fibrin matrix and PBMSCs showed promising results as a potential regenerative material for increasing and enhancing BIC and hence implant stability.

INTRODUCTION: Comparison of platform switched (PS), sloping shoulder, and regular implants on stress reduction in various bone densities with finite element analysis. MATERIALS AND METHODS: A total of 12 three-dimensional finite element models were built to analyze the stress distribution model. Nobel Biocare 4.3 × 8 mm regular platform replace select implant with matching diameter easy abutment, Nobel Biocare 4.3 × 8 mm replace select implant PS with 3.5 mm diameter easy abutment, Bicon 4 × 8 mm implant with 4 mm diameter sloping shoulder abutments were created virtually in compact bone density using software. The 130 N axial force and a 90 N oblique loading force were applied to the abutment to analyze the stress. RESULTS: Under horizontal and vertical loading, the sloping shoulder implant had lesser stresses in cancellous bone when compared with PS and regular implants. Sloping shoulder implant showed more stress distribution at implant-abutment interface and at crestal area, whereas with regular implants, the stresses were distributed at cortical area. CONCLUSION: Sloping shoulder implant in subcrestal position is much favorable for bone growth, stress distribution, and preservation of remaining bone. CLINICAL SIGNIFICANCE: Use of sloping shoulder implant design distributes the stress apically and creates lesser stresses when compared with PS implants.

INTRODUCTION: Mouthrinses have been in use for centuries as breath fresheners, medicaments, and antiseptics. Dill is said to be a good source of calcium, manganese and iron. It contains flavonoids known for their antioxidant, anti-inflammatory, and antiviral properties. Dill can help with microbial infections in the mouth; and its anti-oxidants minimize damage caused by free radicals to the gums and teeth. Being a good source of calcium, dill also helps with bone and dental health. AIMS AND OBJECTIVES: To compare the effectiveness of commercially available 0.2% chlorhexidine gluconate mouthrinse and dill seed oil mouthrinse on plaque levels and gingivitis. MATERIAL AND METHODS: A randomized controlled, double blind parallel arm study was conducted over 90 days on 90 subjects. The subjects were randomly divided into 2 groups and baseline data was collected using Loe and Silness gingival index and Quigley Hein plaque index and oral prophylaxis was performed on all the subjects. The mouthrinses included in the present study were dill seed oil and Hexodent (0.2% chlorhexidine gluconate). Intervention regarding the mouthrinsing was given to the subjects and were followed up for 45 days and 90 days, after this post intervention changes were assessed using the respective indices. RESULTS: It was observed that there is no significant difference in gingival & plaque scores among two mouthrinses from baseline to 45 days and 90 days. It was observed that there is statistical difference in gingival and plaque scores when compared with baseline to 45 days (p<0.001), baseline to 90 days (p<0.001) and 45 days to 90 days (p<0.001) when intergroup comparisons were done. CONCLUSION: It was concluded that dill seed oil and Hexodent (0.2% chlorhexidine gluconate) mouthrinse have similar antiplaque and antigingival effectiveness.

BACKGROUND: Studies regarding the effect of various methods to increase the surface hardness of Type IV dental stone are not conclusive. Therefore, this study was carried out to evaluate the effect of air drying, micro oven drying and die hardener on surface hardness of Type IV dental stone. MATERIALS AND METHODS: A standard metal die was fabricated; polyvinyl siloxane impression material was used to make the molds of metal die. A total of 120 specimens were obtained from two different die stones and were grouped as Group A (kalrock) and Group B (pearl stone), and were subjected to air drying for 24 h, micro oven drying and application of die hardener. These models were then subjected to surface hardness testing using the knoop hardness instrument. The obtained data were subjected to statistical analysis. RESULTS: The hardness of Group A specimens was 64 ± 0.54 Knoop hardness number (KHN) after application of die hardener, 60.47 ± 0.41 KHN after 24 h air drying, 58.2 ± 0.88 after microwave oven drying and 24.6 ± 0.4 after 1 h air drying. The hardness of Group B specimens was 45.59 ± 0.63 KHN after application of die hardener, 40.2 ± 0.63 KHN after 24 h air drying, 38.28 ± 0.55 KHN after microwave oven drying and 19.91 ± 0.64 KHN after 1 h air drying. CONCLUSION: Group A showed better results than Group B at all times. Application of the die hardener showed highest hardness values followed in the order by 24 h air drying, microwave oven drying and 1 h air drying in both groups. The study showed that air drying the dies for 24 h followed by application of a single layer of the die hardener produced the best surface hardness and is recommended to be followed in practice.

BACKGROUND: This study was conducted to compare the linear dimensional accuracy of die materials before and after disinfection of the impression. Type IV and V conventional dental stone, type IV-resin impregnated and copper-plated die materials were studied. MATERIALS AND METHODS: A stainless-steel master die analogs to a complete veneer crown preparation with three scribed lines (I: vertical, II and III: horizontal) was machined and measurements were made from these scribed reference lines. Impressions were made with monophase addition silicone impression material for each of the specimens. 2% glutaraldehyde was used as a disinfectant. The fabricated dies were measured to the nearest 0.0001 mm. ANOVA and post hoc was carried out using Scheffe multiple comparison test at significance level of 0.05. RESULTS: Type IV resin-impregnated dental stone and copperplated dies approximated the dimensions of the master die. Type IV and V conventional dental stone dies showed greater variation in measurements. Statistically significant differences were observed for type IV resin-impregnated and copper-plated dies in dimension I and III. For dimension II no significant differences were found for dies fabricated from four die materials. A one-way analysis of variance indicated no statistical significant differences among the two groups of dies fabricated from disinfectant treated impressions and those fabricated from nondisinfectant treated impressions. CONCLUSION: Type IV resin-impregnated dental stone and copper-plated dies are dimensionally more accurate than type IV and V conventional dental stone die materials. No significant linear distortion in the dies fabricated from the disinfected impressions was observed.

Proper occlusal plane is an essential consideration when multiple long span posterior restorations are designed. The determination of the occlusal plane can have a profound effect on the short and long term success of a restorative case. Purpose of Study. (1) To determine the appropriate occlusal curve for individual patients. (2) To compare the deviation of the clinical occlusal curve with the ideal ones. Materials and Methods. A total of 20 subjects were examined and study models were made of their maxillary and mandibular dentition. Inter-occlusal records were made and the casts were articulated in semiadjustable articulator. An ideal occlusal plane was created. The distance of the farthest cusp tip from the Broadrick curve was measured along the long axis of the tooth for each individual. Paired t-tests were used to compare the findings between subjects and controls. Results. A statistically significant difference P < 0.05 was found in the deviation from the Broadrick curve between patients who have lost posterior teeth and the control group who had a full dentition with no missing teeth. Conclusion. Proper utilization of the broadrick flag on a semi-adjustable articulator will allow for a correct determination of the occlusal plane.
